I imagine him lounging on the throne of Hell, belly swollen and gurgling from him glutting himself on blood and sugar equally, imperiously demanding more and more and more from infernal attendants...and completely oblivious to how fast he's plumping up. Using psychic powers doesn't burn a ton of calories. Imagine one of those he's demanding food from - maybe even the main one - being Dean.
Speaking of infernal attendants, even if Sam's not a brat (to begin with), maybe Hell's just a little too eager to please and care for its new, long-prophesied ruler. Or maybe a bigger king means more power, or various demons and factions are desperate to curry favor with Sam...or all of the above. It all leads to the same place: an increasingly-chubby Sam slumped in the throne, in a glutted stupor, at least one demon rubbing his groaning belly while another hand-feeds him, pampered constantly and attentively whether he wants it or not. Again: any of these attendants could be Dean.
Then you've got feeding as control. I am a weak-kneed sucker for any storyline that involves that. Human!Dean keeping his half-feral, yellow-eyed younger brother locked into a constant calorie overload, too full and too focused on the discomfort in his overfed stomach to use any kind of psychic power...and then eventually too fat for it. Another demon, most likely Crowley, feeding Sam into docile uselessness to become the power behind the throne. Demon!Dean fattening boyking!Sam until he's pretty much just a pet - and delighting in the fact a mere Knight now has this much power over the born-and-bred king of Hell.
Reverse that: King!Sam fattening human or demon!Dean so that he won't be a problem. Or just because that's what he wants to do; he's king of fucking Hell now, and if he wants to indulge his weird kinks, who on earth is gonna stop him? Or the fattening happens accidentally. He just wants to make Dean happy, gives him whatever he wants...and turns out that's an insane amount of food. Turns out Sam likes him like this.
Just gonna leave everyone with one final image: Sam, in all his boyking glory, horns and yellow eyes, maybe wings, using the stomach of a massively, impossibly fat Dean as his throne as he rules over Hell.
