i think that Dean Winchester used to smoke in his teenage years(or when Sam left 'em and went to Stanford). he quit it when john saw him smoking and beat the crap outta him. since then he has a big trigger on smoking(especially on its smell). Sam once came with the smell of cigarettes which made Dean furious so he just left their hotel room for a whole night, cause of this strong, astringent smell
