I saw a comment on your blog that says 'the way you eat does not cause diabetes'...are you able to expand on that or provide a source I could read? I've been told by doctors that my pre-diabetes was due to weight gain because I get more hungry on my anti psychotics and I'd like to fact check what they've told me! Thank you so much!
Pre-diabetes was rejected as a diagnosis by the World Health Organization (although it is used by the US and UK) - the correct term for the condition is impaired glucose tolerance. Approximately 2% of people with "pre-diabetes" go on to develop diabetes per year. You heard that right - TWO PERCENT. Most diabetics actually skip the pre-diabetic phase.
There are currently no treatments for pre-diabetes besides intentional weight loss. (Hmm, that's convenient, right?) There has yet to be evidence that losing weight prevents progression from pre-diabetes to T2DM beyond a year. Interestingly, drug companies are trying to persuade the medical world to start treating patients earlier and earlier. They are using the term “pre-diabetes” to sell their drugs (including Wegovy, a weight-loss drug). Surgeons are using it to sell weight loss surgery. Everyone’s a winner, right? Not patients. Especially fat patients.

1️⃣ You can't prevent insulin resistance. It's coded in your DNA. It may be impacted by your environment. Studies have shown it has nothing to do with your BMI.
2️⃣ The term "pre-diabetes" is a PR stunt. The correct term is impaired glucose tolerance (or impaired fasting glucose) which is sometimes referred to as intermittent hyperglycemia. It does not predict T2DM. It is best ignored and tested for every 3-5yrs.
3️⃣ there is no evidence that losing weight prevents diabetes. That's because you can't reverse insulin resistance. You can possibly postpone it by 2yrs? Furthermore there is evidence that those who are fat at the time of diagnosis fair much better than those who are thin.
4️⃣ Weight loss does not reverse diabetes in the VAST majority of people. Those that do reverse it are usually thinner with recent onset T2DM and a low A1c. Only a tiny minority can sustain that over 2yrs. Weight loss does not improve A1c levels beyond 2 yrs either.
5️⃣ Weight loss in T2DM does not improve macrovascular or microvascular health outcomes beyond 2 years. In fact, weight loss in diabetics is associated with increased mortality and morbidity (although it is not clear why). Weight cycling is known to impacts A1c levels.
6️⃣ Weight GAIN does NOT increase the risk of cardiovascular OR all causes mortality in diabetics. In fact, one might even go so far as to say that it's better to be fat and diabetic than to be thin and diabetic.

Tl;dr: nonpharmaceutical intervention is still the best way to stop covid. Mask up. Filter your air. Distance when possible.

The following information is from Sickening, by John Abramson:
Not to sound like a conspiracy theorist on main but when I say Big Pharma is a racket I mean...
Doctors work via particular guidelines. Many of them are punished by their hospital systems if they fail to follow those guidelines, which are considered "best practice". The guidelines are written largely by pharmaceutical companies or people paid by pharmaceutical companies.
Just like Disney likes to keep it's copyrights fresh, pharmaceutical companies like to keep their patents. And they do this by churning out new drugs. Only about 1 in every 8 of these new drugs actually represents an actual advantage over existing drugs or therapies. They, of course, market the absolute shit out of all of them as though they did.
In theory, newer insulin analogs (lispro, glargine, etc...) work better and have fewer side effects than older recombinant human insulins (regular, NPH). In the actual trials the only difference between the two is a single average non-fatal hypoglycemic event over the course of 5 years of therapy. There was no difference in effectiveness. Oh. And the newer ones are 10 times the out of pocket price (an average of $468/year vs $5,224/year). Which is considerable considering 1 in 4 insulin-users report "rationing" their insulin for cost reasons. 90% of people on insulin take the newer insulin analogs because that's what's on the guidelines.
Pharmaceutical companies have all the data on their drugs, which they don't share and which they alone interpret. It took 4 years of near-daily pestering for Cochrane Reviews (a major independent reviewer) to get a copy of the data for the drug tamflu. When they got it, they found that in 77 trials, the only thing it consistently did was decrease the symptomatic time from an average of 7 days to an average of 6.3 days, even though the company was marketing it under claims that it reduced complications and hospitalizations- something none of the trials showed.
You have to treat 140 people who have not had a heart attack or stroke with statins (cholesterol-lowering medicines) for 5 years in order to prevent 1 single non-fatal heart attack or stroke. There is no difference in death rates from cardiovascular causes between statin-users and non-statin-users who have not had a heart attack or stroke. You have to treat 30 people who have had a heart attack or stroke to prevent one heart attack or stroke. You have to treat 80 to prevent a death.
According to a very large, independent (non-pharmaceutical industry) study called the ACCORD study, people with type 2 diabetes actually had significantly worse cardiovascular outcomes if their average blood sugar was kept in a "normal" (non-diabetic range) (under 125) vs a somewhat higher range (150-180). So significant were these findings that they ended the study early because too many people in the "normal" range were dying.
Omeprazole and Esomeprazole are technically the same drug that work exactly the same way and exactly as well when given at the same dose. The only thing that makes esomeprazole any better is that it is given at a higher dose. And it's way, way more expensive.

So you know how when you orgasm there's a lot of muscle tensing and pulsing? How does that feel with your dick and balls? Okay well this one definitely goes under a cut lol
So this comes (hurr hurr) in two stages.
The first was testosterone, of course. It made orgasm MUCH easier for me to achieve, but also made them a bit shorter and sharper—at least clitoral and anal orgasm. The vanishingly-rare g-spot orgasm was still what you’d expect, the whole full-body thing, but for me that was never worth the gross feeling of psychological wrongness.
This was amplified by bottom surgery. It’s kind of like for a few seconds every muscle in your body tenses up and something goes POP but in a really, really good way, centered on the dick. I feel my orgasms mostly at the base and tip of my dick with a hot (as in really warm) feeling of—like an ache but pleasant, if that makes sense, in what’s now my scrotum (formerly my labia majora), and in the afterglow everything down there feels pretty great and quite warm.
And then I have to take a few minutes to catch my breath and let my heartbeat stop being all weird, but I’ve always had the heart thing and I think it might be just me, idk. (Is it normal to have arrhythmia after orgasm? I’ve never asked a doctor or looked it up. But man after an orgasm my heartbeat sounds & feels like a drunk trying to tapdance, it’s really weird)
[ETA: I looked it up and a doctor with 53 years of experience says it’s not necessarily a bad thing if your cardiovascular health is already fine, and my last EKG was completely normal, so I think I’m in the clear]
Strangely, though I had a vaginectomy as part of my bottom surgery, I can still feel orgasm in whatever is left of my Skene’s glands, though blessedly there isn’t enough left of my front hole to feel anything else in that vicinity, thank fuck.
(For readers who actually like the front hole but still want bottom surgery: never fear, there are MANY options. You can even have phalloplasty without a vaginectomy—the catch being you won’t be able to have urethral lengthening so you’ll still have to sit to pee. Which plenty of cis men do as a habit or for religious reasons anyway, so.) [ETA: a kind Anon pointed out that you can, in fact, have urethral lengthening without a vaginectomy, just with higher risk of a minor complication that is more annoying than dangerous]

The Weather
Twenty-seven US states remain at High to Very High levels of SARS-CoV-2 detected in wastewater since February 28, 2024, with five states not reporting.

Wastewater levels are decreasing throughout the country except in the Midwest. The South is still experiencing extremely high transmission. As a reminder, the last two weeks, shown in gray, are provisional data. These values may change as additional wastewater sites report data.

Despite these ongoing high levels of transmission in most of the country, and in defiance of pleas from civil society to protect the most vulnerable among us, the CDC has once again decided to walk back already insufficient protections – this time in the form of isolation guidance. It announced on Friday at 1 pm that it will repeal the 5-day isolation period for COVID-19 and instead treat COVID-19 like it does other respiratory viruses like flu and RSV, linking isolation length with symptomatology and fever. This policy, of course, is not based on the best evidence–just vibes and a lack of care for those of us still dying (1000s weekly) and still becoming disabled by Long COVID. 
Last week, the CDC recommended that people 65 years and older should receive an updated booster. It did not approve a booster for other people in high-risk categories because they didn’t bother studying the benefits and risks in these groups.

Long Covid
In an article published in Science, Drs Ziyad Al-Aly and Eric Topol reviewed the perplexing case of Long COVID. They discuss the lack of consensus and systematization when it comes to the classification of the disease, partly due to its different mechanisms and manifestations. For example, younger adult and female Long COVID patients are more likely to be afflicted by fatigue, dysautonomia, brain fog, and post-exertional malaise, while older patients are more likely to deal with cardiovascular and metabolic complications. They also discuss the current potential treatments for Long COVID, but express that since nonpharmaceutical interventions have been neglected, vaccines are the only available therapy. However, vaccines are unable to prevent Long COVID. Ultimately, they state that the only way to prevent Long COVID is by ending COVID transmission, such as by administering pan-variant neutralizing intranasal vaccines.
On March 15, 2024, the Long COVID March will be taking place in Washington D.C. at the Lincoln Memorial. The march is intended to mobilize Long COVID patients and allies to demand acknowledgment and policy changes from the government. You can read the march’s mission statement, which includes a list of objectives. 
Take Action
Join us in demanding the CDC reinstate the 5-day COVID isolation policy. Despite the guidelines having already been dropped, we must still fight for adequate protections from COVID to safeguard our communities from harm. summary: it’s time to work with kento, the man you had just rejected after a one-time unlikely experience in his car. he’s keeping things professional, but can you?

Tomorrow came too fast for your liking. It was time for you to work with Kento, the man you had just rejected after a one-time unlikely experience in his car.  You’re sure this is karma for all the hearts you’ve broken of the young men you dated.
You were never the one for long term, serious relationships. The guys you would meet thought that’s what they wanted too, a fun time with no strings attached. But they all ended after a short amount of time when you reject their offer for something more serious. They’d call you a slut and a whore, but you never really cared. 
The only person it didn’t happen with was Choso. He let you come and go as you pleased and because of that you started coming around more often. You didn’t realize yourself doing that and when you did, Yuji had already offered you this great opportunity.
You expect it to be awkward with Kento or expect him to be upset with you for rejecting him the day prior. You’re pleasantly surprised when Kento is completely professional with you. He does the same as Satoru, shows you and Megumi CT scans, MRIs, and pathology reports to educate and quiz you both throughout the morning. It’s like nothing ever happened. You’re happy about that, right?
You learn Kento is a cardiovascular surgeon. He works with one of the most vital and delicate organs of the body. 
Kento’s preferred way of documentation is pretty simple. He likes full sentences and likes to document any social history that may cause complications before or after surgery.
He’s a lot more thorough than Satoru, who would just glance at the chart note and say good enough, save it, let’s move on. No, Kento stops after every patient to read your and Megumi’s chart notes from the screen of the laptops you held before approving them or asking for something to be changed.
When Kento checked your notes, his sense of personal space didn’t exist. You’d feel the firmness of his chest pressing against the back of your shoulder as he towered over you and leaned in to read your chart note. His voice was almost as close as it was that night in his car, so you had to focus even more to hear the correction he’d suggest for you to do. 
It didn’t help that Kento didn’t like wearing his white coat. He wore light blue scrubs that contracted nicely to his blonde hair and light brown eyes. His arms were big, you hadn’t noticed that before. They contoured and flexed under the bright lights of the clinic. They looked strong and muscular, veins sculpted on his hands and up his forearms. 
You don’t notice you’re staring until Megumi has the nerve to nudge your side with his pointy elbow. You grunt at him and direct your eyes to your own laptop screen.
Lunch comes around faster than yesterday. Yuji and Nobara are paired with Dr. Gojo today and their moods reflect it. They’re happier than the day before, though Nobara complains that Dr. Gojo talks too much nonsense with the patients and gets them get behind schedule.
Then, they lean in and ask how it’s going with “Dr. No Pump Chump.” You roll your eyes and have to press your lips together to not give them the satisfaction of smiling. 
“She’s been drooling over him all morning.” Megumi answers for you, then takes a bite of his salad. 
You scoff and hit his shoulder. “I have not!”

Kento didn’t waste time chatting unrelated topics with his patients like Satoru does. Sometimes an elderly patient would trap him in a conversation and he’d feel obligated to entertain them for a couple of minutes, but he was quick to say his goodbyes and open the exam room door for you and Megumi to step out first. 
One particular patient, a 73-year-old woman, came in with her granddaughter who seemed to be a couple years older than you.
“Thank you so much, Dr. Nanami.” The young woman says, and she reaches over and places a hand on his upper arm. His body constricts just for a moment, but you notice it. Her smile is alluring, beautiful, painted with red lipstick. “You’ve basically made my grandmother’s heart brand new.” 
Kento smiles small, like he did when he said I look forward to working with you yesterday. His response is simple and straight to the point, “of course. Thank you for joining her today.” 
He’s about to wrap the visit up when the young woman jumps in and continues trying to converse with him. She leans in a little, her long hair falling to frame her face so perfectly. “Can I ask, what made you get into heart surgery? Got tired of breaking hearts so you decided to start fixing them?” 
Her eyes trail from his large arms to his collarbone visible from his scrub top, up his neck that you’ve bitten and kissed, until they get to his face. She’s eyeing him like a piece of candy.
He deals with this a lot. Patients or family of patients are always so interested in him. He’s attractive, tall, with a good career and with no wedding ring in sight. He just never likes how inappropriate the timing is.
He chuckles at her joke, but it’s not genuine. You can tell it isn’t. He laughed at everything you said at the bar, you know how deep and rumbling his true laugh is. 
“No, nothing like that.” He then gets up from the medical stool he’s seated on and opens the exam room door before the young woman could continue. He lets you step out first, then Megumi, then the patient, and he walks out chatting with the young woman together. 
You’re not sure why you’re so irritated, but you are. Megumi steps a couple of inches away from you as he can sense the tension in your face build up. You look at him and glare, like he’s acting unreasonable because you’re fine. Megumi’s face just says sure you are. 
“Don’t forget to look for signs of infection on her incisions. I’ll check the stitches on her next post-op visit.” You heard Kento explain to the young woman. She looks… disinterested, almost disappointed Kento isn’t flirting back. Your mood picks up slightly when hearing how strictly professional the private conversation they’re having is. 
Kento is obviously a no nonsense man. He doesn’t relish in the unneeded attention from patients like Satoru does. He doesn’t provoke, engaged, or instigate any of it. You’re almost positive if you hadn’t told him you weren’t looking for something serious yesterday, he would want something serious with you now. He’s not the type of man you usually go for, least of all at this time in your life when you have your future planned out ahead of you. 
He comes back to the nurse station and takes a seat an open chair in front of a computer. You watch as his arms flex when he brings up his hand to run through his blonde hair. You watch his fingers type away at the keyboard, they’re long and thick, you never got the chance to feel them. 
You see yourself back in his car, picking up where you both left off. Your bottoms are discarded to the backseat and your legs are on either side of his thighs. His fingers are rubbing tiny circles on your clit through your underwear. You can feel his hot mouth on your skin again, his perfect teeth scraping along your neck until he’s nibbling near your collarbone.

There’s a lull now, waiting for his next patient to be ready. You peek over to see what Kento’s looking at on his screen, and see a diagnosis you’re familiar with.
“You’ve treated patients with Cordis Aneurisma?” You ask him.
Kento seems to maintain really good eye contact when speaking, but only with you. He doesn’t look at Satoru when he talks to him, he didn’t look at the young woman when she flirted with him, but with you his eyes never seem to waver. They didn’t at the bar and they don’t waver now.
“I treated them as much as I could. Surgery can only go so far.” He replies. “You know of it?” 
Cordis Aneurisma is incredibly rare. It’s genetic and eventually becomes terminal. Though, it takes years and years to get really severe. It starts off with simplest symptoms, then broad cardiovascular symptoms, then the telling symptoms start all too late.
“My mom had it.” You say it so easily. 
You haven’t spoken of your mother since moving. You told Yuji about her passing during the drive, and though he has been your childhood best friend, he didn’t know your mother well. He knew she was kind and worked two jobs to support three kids and was never home. You were always at Yuji’s house too, hanging with him and his brothers. 
A look of concern flashes on Kento's face as he watches you, but you don't notice it as your eyes read the chart note on screen. The patient is older than your mother and she's still alive, but her symptoms listed on the chart are similar to the ones your mom had recently. You seem sad for a moment, you miss your mom and you feel bad for this patient.
Now's not the time to call attention to this. Megumi doesn't know of your mother and you're at work right now. You brush it off quickly, and when you look back at Kento, he's still staring at you. You're not sure what the expression on his face is, but you don't pay any mind to it.
“Now, can I ask what really made you get into heart surgery, Dr. Nanami?” You're slightly mocking the young woman he spoke to just now, the telling smirk on your lips makes Kento laugh. It sounds light and real, like it did at the bar.
He likes hearing Dr. Nanami from your mouth. He momentarily thinks back at the pretty moans you gave him in his car, when you were on top of him and grinding against his erection. He looks down at his hands at the keyboard in front of him, having to readjust himself on the uncomfortable rolling chair he's sitting on before looking back at you. He's still smiling, though it's smaller.
“I like a challenge. Heart surgery is intense, hardcore, there's nothing's like it.” He explains. He sounds passionate. You can tell he likes what he does for a living.
“Except neurosurgery~” Dr. Gojo chimes in, in a sing-song tune. Kento's smile and light posture go away too soon as the younger physician comes out of nowhere. “I'd like to see the heart do anything without the brain telling it.”
“I assumed you were familiar with being brain dead.” Kento's comeback is quick without skipping a beat. You giggle at it, and Kento's smile returns for a minute.
Dr. Gojo isn't so amused like you are. His lips are in a straight line and his eyes are dull. Kento isn't looking at him, like he never does with anyone but you, and is instead back focus on the computer screen.
“Anyways,” Satoru moves on, his tone changing to upbeat once again. He calls for your attention. “How would you like to work with Dr. Nanami on your own tomorrow?”
“Alone?” You question. Your eyes flicker to Kento. “I mean, it's pretty soon.”
“I believe in you.” Satoru smiles at you. The simple but encouraging words from him make you feel a little more confident. You reluctantly nod in agreement. “Don't be too hard on her, Kento.” Satoru says, his voice fading as he walks away.
Kento finally drags his eyes away from the computer once Satoru is out of sight and looks at you. You give him a smile, the awkward tension filling you again. Tomorrow will be interesting.
