Awhile ago I made a post talking about how Aegon only views Helaena as his sister and not wife, and I think this really reinforces it!
We know that Aegon mostly ignores Helaena except when he's drunk (as Helaena says), and it's implied that the two of them already have their children (at the very least the twins).
A normal person being forced to sleep with their sibling would definitely need to be partly wasted to be able to go through with it.
The fact that Aegon not only gave her a present, but also made sure that it was something that she would genuinely like (as seen by the fact that she spends a lot of the dinner playing with it and even shows it off to Otto), kinda of shows that he does care about her, he just seems to be incapable of loving his sister the way a husband should.
(this isn't meant to excuse the shitty things Aegon has done, just an analysis I thought of)

People should make more doomed by narratives siblings relationship.
Like with lovers you can just sever it and not have it related to you ever again but with siblings how could you?
You grow up with them you raise them or they raised you you both know how unforgiving the world is to both of you? You would die for them but will hate them for doing the same and yet none of you would regret it and both of you know it. They could be the person you loath the most and miss the most cause you still remember how they sneaked a candy into your hands. You can sever the tie but you can never look away at what you've lost, at whom you've lost because fate doesn't allow you to be together, eating dinners in quiet peace, if only there's another life, another time, where i can make you another plate of pancakes i would im sorry im sorry im sorry —
