Leaving her family isn't the worst thing she has done. After Rumple literally mutilated himself to be there for his son, (and then walked for days with that broken leg,) she told him she wished he had died in the war. Do you realize how horrible that is? Do you realize what it does to someone to hear a loved one say that? And that wasn't the only time she said something like that. Milah constantly made him feel worthless and unloved. She was only worried about her reputation, and apparently felt the need to essentially verbally abuse her husband just because he made a sacrifice for his family.
