This item, H.R. 7152, the Civil Rights Act of 1964, faced strong opposition in the House Rules Committee. Howard Smith, Chairman of the committee, refused to schedule hearings for the bill. Emanuel Celler, Chairman of the Judiciary Committee, attempted to use this discharge petition to move the bill out of committee without holding hearings. The petition failed to gain the required majority of Congress (218 signatures), but forced Chairman Smith to schedule hearings.

Anticoagulation Lecture 6/21/23

Factor Xa inhibitors = rivaroxaban, apixaban (Eliquis)

Direct thrombin inhibitor = dabigatran

Vitamin K inhibitor = warfarin

Apixaban has the lowest bleeding risk compared to the other Direct Oral Anticoagulants (DOACs); avoid in pts with BMI greater than or equal to 40 or weight more than 120 kg. Avoid dual inhibitors or inducers of CYP3A4 and P-gp.

VTE/PE tx = 10mg bid x7 days, then 5 mg bid

AFib = 5 mg bid (reduce to 2.5 mg bid if Cr greater than or equal to 1.5 or weight less than 60 kg or age greater than 80

Rivaroxaban (Xarelto) = once daily dosing compared to Eliquis

VTE/PE: 15 mg bid x21 days; then 20 mg qd

AFib: 20 mg qd with food to increase absorption

PAD/CAD: 2.5 mg bid (+antiplatelets if at increased risk)

Avoid with CYP3A4 inhibitors or inducers and P-gp

Edoxaban (Savaysa) – not used in pts with CrCl greater than 95

VTE/PE = parenteral anticoagulation for 5-10 days, then PO

VTE/PE/AFib = 60 mg qd

Does not have reversal agent like other DOACs do (which are reversed with Kcentra or andexanet)

Dabigatran (Pradaxa)

AFib: 150 mg bid; consider 110 mg bid if bleeding risk

VTE/PE: 5 days IV, then 150 mg bid

VTE ppx after THA/TKA: initial 110 mg once, then 220 mg qd x10-35 days

5-10 day bridge required for PE/VTE

Caution in pts greater than 75 years due to increased bleeding risk

Has own reversal agent (praxbind; idarucizumab)

C/I in pts with prosthetic heart valve

Store in original bottle and discard if unused after 4 months

Eliquis and Xarelto can be crushed and given via feeding tubes.

Warfarin (Coumadin)

Dosing is pt specific. Goal INR is 2-3. (2.5-3.5 if mechanical mitral valve).

Slow onset; slow time to steady state means dose taken today may not be reflected in INR for several days (2-4 days).

So many drug-drug interactions!

Metabolized by CYP pathways

Vitamin K antagonist

Dosing: recommend taking at night because INR is taken during the day, makes it easier to adjust dose.

Warfarin’s therapeutic steady state is based on half-lives of clotting factors. At least 5 days of consecutive warfarin needed for pt to be fully anticoagulated (this is why you use heparin until they get to this point).

Usually start with 5 mg qd. If bleeding risk, start with 2.5 mg qd. For obese pts or otherwise healthy/young can start with 7.5 mg qd. You go to maintenance protocol on 7th day.

Pts on VTE/PE tx are not anticoagulated the first 5 days, so use heparin IV or LMWH outpt. Once anticoagulated x24 hours, d/c heparin.

Start with 5 mg warfarin for first 3 days, then follow up with warfarin clinic on day 4. Typically, pt goes for weekly INRs. Then when at goal INR for 2 weeks, can spread out how often they f/u on their INR levels. Eventually they can go q6-8 weeks to check INR level.

VTE/PE – always bridge

AFib – risk assess to see whether heparin bridging is needed (assess bleeding risk [HASBLED]; CHA2DS2VASc for thrombotic risk).

Every 10 years, the dosing needed will decrease by 10%, so as pts age, the dose of warfarin needed will decrease.

DOACs = no INR monitoring, no dietary interactions, lower rates of bleeding, limited availability of reversal agent; contraindicated in pts with mechanical heart valves

AFib – AC (anticoagulate) indefinitely

1st VTE or PE that is provoked – AC for 3 months

1st episode of VTE/PE in setting of cancer – 3 months of AC (LMWH is better than DOAC or warfarin--new research shows DOAC may actually be more effective, so can use DOAC now; LMWH not preferred)

1st episode of VTE/PE unprovoked (no idea what caused it) – AC more than or equal to 3 months

2nd VTE – AC indefinitely

Heparin reversal – protamine (max dose is 50 mg), to reverse enoxaparin, give 1 mg protamine for each 1 mg of Lovenox

Warfarin reversal – vit K; 4 factor prothrombin compex (KCentra); FFP (if KCentra not available); KCentra is for life-threatening bleeding or if pt needs surgery for life-threatening condition

Apixaban/Rivaroxaban reversal – Kcentra (life threatening bleed or need emergency surgery; has thrombotic risk); FFP (Fresh Frozen Plasma); Andexanet Alfa. KCentra can cause clotting in 5% of pts; don’t give if you don’t need to.

Dabigatran reversal – idarucizumab (Praxbind); dosed as 5 g IV given 2.5 mg no more than 15 minutes apart)

Heparin lasts 2 hours

LMWH is preferred in pts who are pregnant (avoid warfarin). ESRD on dialysis – warfarin or Eliquis (avoid Xarelto). Obese pts – DOACs or warfarin. Cancer – use LMWH; recent study shows Eliquis is equal to or superior to LMWH in cancer pts who need anticoagulation.

Pregnancy = lovenox better, doesn’t cross placenta; UFH (unfractionated heparin) is an alternative in pts with poor renal function.

Stop heparin gtt and give Eliquis now or give 2 hours after stopping heparin gtt. Give loading dose Eliquis even if the pt was on heparin gtt.

You have probably noticed that I do love all kinds of seasonal fruit cakes. Pumpkin has a special place in my heart, as it's nutritious, healthy and so versatile that it can be used for many sweet and savory dishes!

This pumpkin Bread is one of my most favorites, because it's easy to prepare, perfectly spiced, moist and tangy with the subtle undertones from Greek Yoghurt.

I can definitely vouch that it's a must-try. Perfect for a family Thanksgiving or a Halloween party.

Great for breakfast, coffee, (high) tea or as a school/work snack! Make it with the indisputable Queen of Autumn! Let's begin!

Suitable for lacto-ovo vegetarians.

INGREDIENTS

Ingredients for boiling the pumpkin:

· 300 gr. / 10.5 oz fresh Pumpkin, cubed

· 1/3 cup / 80 ml Water

Ingredients for the Bread:

· 1 & 3/4 cups / 220 gr. / 7.8 oz All-purpose Flour

· 1/2 cup / 110 gr. / 3.9 oz Butter, semi-melted

· 2 medium Eggs @ room temperature

· 1 cup mashed Pumpkin (puree from what we boiled earlier)

· 1/4 cup / 70 gr. / 2.5 oz Greek Yoghurt 0% fat

· 3/4 cup / 160 gr. / 5.6 oz light brown Sugar

· 1/2 cup / 110 gr. / 3.9 oz Sugar

· 1 tbsp. Mixed-Spices or Pumpkin spices

· 1 tsp. ground Cinnamon

· 1 tsp. baking Soda

· Pinch of Salt

· For topping: 3 tbsp. / 25 gr. / 0.9 oz Pumpkin Seeds and

· 1.5 tbsp. / 14 gr. / 0.5 oz Sunflower Seeds

METHOD:

A. Boil the pumpkin the evening before (the quantities make 1 cup mash-puree): 1. Place water in a small pot over medium heat, adding also the pumpkin cubes. 2. When water starts to boil, reduce heat to very low. Place lid on top and let pumpkin cubes simmer for 18' or until all water has evaporated and pumpkin cubes are soft when pierced. 3. Remove from heat and let them cool down. 4. Press them with a small fork or special utensil to mash them. 5. Let the pumpkin mash (puree) cool down completely by placing it in a cup, covered, in the fridge for some hours, before continuing with the recipe.

6. For longer storage, keep in freezer for 1 month. Defrost before using.

B. Prepare the bread batter:

7. NOTE: Make your own "mixed spices" that are used to flavor the Pumpkin Bread, if you can not find them ready in the supermarkets. You can also add them to your Christmas Turkey Stuffing or hot Cappuccino or to other sweets or foods, to make them slightly spicy.

The recipe is to mix together and homogenize:

✔︎ 9 parts ground Cinnamon

✔︎ 2 parts ground Nutmeg

✔︎ 2 parts ground Ginger

✔︎ 1.5 parts ground All-Spice

✔︎ 1.5 parts ground Cloves

8. In a small bowl, mix together the two nuts for topping. Set aside

9. Brush with butter the bottom and sides of a bread (loaf) tin of dimensions (WxLxH) 14 x 27.5 x 7.5 cm / 5.5 x 10.8 x 3 in.

10. Place butter in a large mixer bowl and start beating it in high speed.

11. Add both sugars and beat together with the butter.

12. Next pour in the whisked eggs, beating well to incorporate them.

13. Add both spices, salt and baking soda.

14. Add the 1 cup pumpkin puree and continue beating until it's combined.

15. Lower the speed, and slowly add flour, spoon by spoon.

16. Halfway, stop the mixer and add the yoghurt in the batter.

17. Start the mixer again and continue with rest of flour, beating just enough to incorporate it.

18. Stop the mixer.

19. Empty the batter in the bread tin, leveling its surface.

20. Top it by spreading the nuts mix. Ready for the oven!

C. Baking instructions:

21. Bake the tin with Pumpkin Bread, on the bottom shelf of a preheated oven-both top and bottom heating elements on = @180℃ / 350℉ for 50'-1 hour (depends on the oven).

22. After about 1 h, open the oven and check if bread is baked through by inserting a knife in its heart. It should come out clean.

23. Remove bread from the oven and place it on the kitchen bench to cool down completely.

D. Serving Suggestion:

To serve, turn the tin twice upside down on a wooden, serving board, remove it and cut the bread in thick slices. Now it's ready to be devoured; who can wait with these tantalizing smells in my kitchen!?

A delicious Autumnal treat, beautifully moist, deliciously spicy, tangy and nutty! What else could you ask for? Come for coffee!

E. Storage info:

Pumpkin Bread can be stored in an airtight plastic bag and be placed in the fridge for up to one week. In the freezer, you can store it for up to 1 month.

Θα πρέπει να έχετε παρατηρήσει ότι λατρεύω όλα τα είδη κέικ με φρούτα εποχής. Η κολοκύθα έχει ιδιαίτερη θέση στην καρδιά μου, καθώς είναι θρεπτική, υγιεινή και τόσο ευέλικτη που μπορεί να χρησιμοποιηθεί σε πολλά γλυκά και αλμυρά πιάτα!

Αυτό το ψωμί κολοκύθας είναι ένα από τα πιο αγαπημένα μου, γιατί είναι εύκολο στην παρασκευή του, τέλεια καρυκευμένο, ζουμερό και πικάντικο με ελαφριά ξινή χροιά από το γιαούρτι.

Μπορώ με σιγουριά να σας εγγυηθώ ότι πρέπει να το δοκιμάσετε. Ιδανικό για οικογενειακό γεύμα την Ημέρα των Ευχαριστιών ή για το Χάλογουιν.

Εξαιρετικό για πρωινό, καφέ, τσάι ή σα σνακ στο σχολείο/δουλειά! Φτιάξτε το με την αδιαμφισβήτητη Βασίλισσα του Φθινοπώρου! Ας ξεκινήσουμε!

Κατάλληλο για χορτοφάγους.

ΥΛΙΚΑ

Υλικά για το βράσιμο της κολοκύθας:

• 300 γρ. / 10,5 oz φρέσκια Κολοκύθα, σε κύβους

• 1/3 φλ. / 80 ml Νερό

Υλικά για το ψωμί:

• 1 & 3/4 φλ. / 220 γρ. / 7,8 oz Αλεύρι για όλες τις χρήσεις

• 1/2 φλ. / 110 γρ. / 3,9 oz Βούτυρο, μισολιωμένο

• 2 μέτρια Αυγά σε θερμοκρασία δωματίου

• 1 φλ. πολτοποιημένη Κολοκύθα πουρές (αυτή που έχουμε βράσει νωρίτερα)

• 1/4 φλ. / 70 γρ. / 2,5 oz Γιαούρτι στραγγιστό 0% λιπαρά

• 3/4 φλ. / 160 γρ. / 5,6 oz Ξανθή Ζάχαρη

• 1/2 φλ. / 110 γρ. / 3,9 oz Ζάχαρη

• 1 κ.σ. Ανάμεικτα Μπαχαρικά ή μπαχαρικά για κολοκύθα

• 1 κ.γ. Κανέλα σκόνη

• 1 κ.γ. μαγειρική Σόδα

• Πρέζα Αλάτι

• Για επικάλυψη: 3 κ.σ. / 25 γρ. / 0,9 oz Σπόροι κολοκύθας (Πασατέμπο) και

• 1,5 κ.σ. / 14 γρ. / 0,5 oz Ηλιόσποροι

ΜΕΘΟΔΟΣ:

Α. Βράστε την κολοκύθα το προηγούμενο βράδυ (οι ποσότητες φτιάχνουν 1 φλιτζάνι πουρέ):

1. Βάλτε το νερό σε μια μικρή κατσαρόλα πάνω από μέτρια φωτιά, προσθέτοντας και τους κύβους κολοκύθας.

2. Όταν το νερό αρχίσει να βράζει, χαμηλώστε τη φωτιά στο πολύ χαμηλό. Τοποθετήστε το καπάκι από πάνω και αφήστε τους κύβους κολοκύθας να σιγοβράσουν για 18' ή μέχρι ��α εξατμιστεί όλο το νερό και οι κύβοι κολοκύθας να είναι πολύ μαλακοί όταν τρυπηθούν.

3. Αποσύρετε από τη φωτιά και αφήστε να κρυώσουν.

4. Πιέστε τους με ένα μικρό πιρούνι ή ειδικό εργαλείο για να πολτοποιηθούν.

5. Αφήστε την κολοκύθα πουρέ να κρυώσει εντελώς βάζοντας την σε ένα φλιτζάνι σκεπασμένη στο ψυγείο για μερικές ώρες, πριν συνεχίσετε με τη συνταγή.

6. Για μεγαλύτερο χρόνο φύλαξης, διατηρήστε τη στην κατάψυξη για 1 μήνα. Ξεπαγώστε πριν τη χρήση.

Β. Ετοιμάστε τη ζύμη για το ψωμί:

7. ΣΗΜΕΙΩΣΗ: Φτιάξτε τα δικά σας «ανάμεικτα μπαχαρικά» που χρησιμοποιούνται για να αρωματίσουν το ψωμί κολοκύθας, αν δεν τα βρείτε έτοιμα στα σούπερ μάρκετ. Μπορείτε επίσης να τα προσθέσετε στη χριστουγεννιάτικη γέμιση γαλοπούλας ή στον ζεστό καπουτσίνο σας ή σε άλλα γλυκά ή φαγητά, για να γίνουν ελαφρώς πικάντικα.

Η συνταγή είναι να ανακατέψετε μαζί για να ομογενοποιηθούν:

✔︎ 9 μέρη Κανέλα σκόνη

✔︎ 2 μέρη Μοσχοκάρυδο σκόνη

✔︎ 2 μέρη Τζίντζερ (πιπερόριζα) σκόνη

✔︎ 1.5 μέρη Μπαχάρι σκόνη

✔︎ 1.5 μέρη Γαρύφαλο σκόνη

8. Σε ένα μικρό μπολ, ανακατέψτε μαζί τους δύο ξηρούς καρπούς για την επικάλυψη. Αφήστε στην άκρη.

9. Αλείψτε με λίγο βούτυρο τον πάτο και τις πλευρές μιας φόρμας για ψωμί διαστάσεων (ΦxΜxΥ) 14 x 27,5 x 7,5 εκ. / 5,5 x 10,8 x 3 ίντσες.

10. Βάλτε το βούτυρο στο μεγάλο μπολ του μίξερ και αρχίστε να το χτυπάτε με δυνατή ταχύτητα.

11. Προσθέστε και τις δύο ζάχαρες και χτυπήστε τις μαζί με το βούτυρο.

12. Στη συνέχεια ρίξτε μέσα τα χτυπημένα αυγά, ανακατεύοντας καλά να ενσωματωθούν.

13. Προσθέστε και τα δύο μπαχαρικά, το αλάτι και τη μαγειρική σόδα.

14. Προσθέστε το 1 φλ. πουρέ κολοκύθας και συνεχίστε το χτύπημα μέχρι να ενωθεί.

15. Χαμηλώστε την ταχύτητα, και προσθέστε σιγά σιγά το αλεύρι, κουταλιά-κουταλιά.

16. Στα μισά, σταματήστε το μίξερ και προσθέστε το γιαούρτι στη ζύμη.

17. Ξεκινήστε ξανά το μίξερ και συνεχίστε με το υπόλοιπο αλεύρι, χτυπώντας όσο χρειάζεται για να ενσωματωθεί.

18. Σταματήστε το μίξερ.

19. Αδειάστε το μείγμα στη φόρμα ψωμιού, ισιώνοντας την επιφάνειά του.

20. Από πάνω απλώστε το μείγμα των 2 ξηρών καρπών. Έτοιμο για το φούρνο!

Γ. Οδηγίες ψησίματος:

21. Ψήστε τη φόρμα με το ψωμί κολοκύθας, στο κάτω ράφι ενός προθερμασμένου φούρνου - στις πάνω και κάτω αντιστάσεις στους = @180℃ / 350℉ για 50'-1 ώρα (εξαρτάται από το φούρνο).

22. Μετά από περίπου 1 ώρα, ανοίξτε το φούρνο και ελέγξτε αν έχει ψηθεί το ψωμί, τρυπώντας με ένα μαχαίρι την καρδιά του. Θα πρέπει να βγει καθαρό.

23. Βγάλτε το ψωμί από το φούρνο και τοποθετήστε το στον πάγκο της κουζίνας για να κρυώσει εντελώς.

Δ. Πρόταση σερβιρίσματος:

Για να σερβίρετε, αναποδογυρίστε τη φόρμα δύο φορές πάνω σε μια ξύλινη σανίδα σερβιρίσματος, αφαιρέστε τη και κόψτε το ψωμί σε χοντρές φέτες. Τώρα είναι έτοιμο να καταβροχθιστεί. Άλλωστε ποιος μπορεί να περιμένει με αυτές τις δελεαστικές μυρωδιές στην κουζίνα μου!;

Μια πεντανόστιμη φθινοπωρινή απόλαυση, ένα υπέροχα ζουμερό ψωμί, πικάντικο και με τη φανταστική γεύση ξηρών καρπών! Τι άλλο θα μπορούσατε να ζητήσετε; Ελάτε για καφέ!

Το ψωμί κολοκύθας μπορεί να φυλαχτεί σε αεροστεγή πλαστική σακούλα στο ψυγείο για έως και μία εβδομάδα. Στην κατάψυξη, μπορείτε να το φυλάξετε για έως και 1 μήνα.

All the elements of the periodic table in the Kichwa language

Jose E. Andino-Enríquez, Manuel A. Andino-Enríquez, Francis E. Hidalgo-Báez, Sisa P. Chalán-Gualán, Santiago D. Gualapuro-Gualapuro, Simone Belli and Michelle B. Chicaiza-Lema recently published in the Journal of Chemical Education a proposal to adapt the names of the 118 elements of the periodic table to Kichwa (Adaptation of the Periodic Table to Kichwa: An Ecuadorian Native Language; J. Chem. Ed., 2022 99 (1), 211-218, DOI: 10.1021/acs.jchemed.1c00383).

The authors explain that the Kichwa lacks scientific tools to respond to educational needs, triggering the gradual loss of intercultural diversity. To adapt the periodic table to Kichwa, they took into account the different linguistic variations of the language and the opinion of the speakers. This is the adaptation compared to the Spanish version, the language most widely spoken in Ecuador:
