Do you know how our understanding and treatment of diabetes has changed through history?
Oooh good question, anon!
As you may guess, diabetes mellitus is not new.

We've known about it since at least the Ebers Papyrus (1550 BCE) when the disease and a treatment was first described. This treatment was: "a liquid extract of bones, grain, grit, wheat, green lead and earth." I did not look these up, but I would guess they did not do a whole lot for the treatment of diabetes.
Later during the 6th century BCE it was first given a name when it was described by Hindu physician Sushruta as madhumeh or "honey urine."
Honey urine is a very apt descriptor for diabetes. In any type, one of the most measurable symptoms is that the person urinates a lot, and the urine tastes sweet (or, if one didn't feel like tasting, that it ferments, or that it attracts ants). This was also the first test for diabetes.
The reason for the sweetness of the urine (as well as a lot of other general info about diabetes) is spelled out more clearly in my "Don't Be That Guy Who Wrote Hansel and Gretel: Witch Hunters" post.
A Greek physician Apolonius of Memphis named it Diabetes, meaning "to siphon" (referring to the large amount of urine lost).
Roman physician Aretaeus later made the first precise description of diabetes. This included the classic symptoms of incessant thirst, copious urination, and constant hunger leading to emaciation and death. He also notes that if deprived of water, the patient will continue to urinate until they become so dehydrated that they die.
The term "Mellitus" was not added until the 1600s by an English physician Thomas Willis. This was again due to the sweetness of the expressed urine. Willis prescribed a diet of "slimy vegetables, rice, and white starch. He also suggested a milk drink which was distilled with cypress tops and egg whites, two powders (a mixture of gum arabic and gum dragant), rhubarb and cinnamon". Supposedly his patients improved if they kept to this diet, though few managed it long term. I honestly don't know how it would have worked, even temporarily.
A major breakthrough came in 1889 when it was discovered that if you removed the pancreas from a dog, the dog would become diabetic (particularly, that it would urinate large quantities of sweet urine). Up until this point it was thought that diabetes stemmed from the kidneys and bladder, or perhaps the lungs. This was the first time it had been shown experimentally that the pancreas was the problem.
Speaking of this, this was also part of a series of experiments where an English physician named Merkowski implanted a small amount of pancreas in the pancreas-less dog's fat, which reversed the diabetes temporarily. This proved that the pancreas was making something that helped regulate blood (and thus urine) sugar.
What this was wasn't figured out until 1921, when Canadian scientists Banting and Best (with help from McLeod and Collip) isolated something they called insletin (after the islets of langerhans, where the substance was being produced). It's important to note that all of these scientists hated each other so much they almost refused a Nobel Prize over it. Later, Collip would refine the substance and McLeod would rename it insulin.
Prior to insulin existing there was basically 1 vaguely useful treatment for diabetes. Unfortunately, that was starvation. So you could either die a slow and painful death by diabetes or you could die a slightly less slow but still painful death due to eating about 500 calories per day. Either way, diabetes was fatal, usually within a couple of years of diagnosis.
By 1923, the first commercial insulin product, Iletin, had been developed. Iletin was a U10 insulin (10 units per 1 milliliter- less potent than today's U100 and U500 insulins) and was made from pork pancreases. It took nearly a ton of pork pancreas to make 1oz of insulin. Fortunately, as a byproduct of the meat industry, pancreases were readily available.

Now, you might be thinking- no one has mentioned type 1 or type 2 yet in this entire post!
Well, you would be right, because diabetes wouldn't be split into 2 forms (insulin-dependent and non-insulin dependent) until 1979, and wouldn't be classified as types 1 and 2 until 1995. That's right- some of you were alive when there was only one kind of diabetes out there.
Now, there's more about the types in the Hansel and Gretel post, but essentially type 1 diabetes occurs when the pancreas itself stops producing insulin, usually in childhood. When this happens, the body stops being able to use sugar (insulin, a hormone, acts as a "key" to let sugar into cells for use). Without replacing that insulin, the person dies because their cells starve.
Type 2 diabetes occurs when the pancreas still produces insulin, but the cells stop responding to it correctly. This causes high sugar levels in the blood, which causes longer-term complications (infections, ulcers, blindness, neuropathy, heart and kidney disease, hyperosmolar syndrome, etc..) which eventually lead to death.
We started discovering oral drugs that worked on what would later become type 2 in the 1950s. Particularly those that worked by increasing the insulin output of the pancreas, but only when the pancreas was still producing some insulin.
Predicting which diabetics would benefit from oral therapies was challenging, but it was recognized that when the onset of diabetes was slow and came on in adulthood, the oral agents would work, while if it came on suddenly in childhood, the oral agents wouldn't. Terms like "adult onset" and "maturity onset" were common:

(Side note: if you have ever read Alas, Babylon (1955) there is a diabetic character who by today's standards clearly has type 1 diabetes, but wants to switch to the "new oral pill" (called "orinase" in the book, though they are likely referring to diabinese pictured above).)
From 1923 into the 1980s, insulin was given once or twice per day, and not particularly titrated to blood sugar. This was probably just because we didn't have a great way to measure blood sugar in real time. Pre-1970s, there was no way to test blood sugar outside of a lab setting.

Urine testing was common starting in the 1940s, but was cumbersome as it required a flame for heating the urine. By the 1950s, a test had been developed that didn't require a flame, but was still not practical for home use. In the 1960s, paper strips were developed that changed color for different amounts of sugar in the urine. The problem with this was that the strips couldn't change color until there was sugar in the urine- a blood sugar level of over 200 by today's measurements. Low blood sugar readings were impossible at this time, and had to be treated based on symptoms.
In the 1970s, blood sugar could finally be measured by putting a drop of blood on a test strip, wiping it off, and matching the color of the test strip to a chart. While less cumbersome than urine tests, this was still something that would generally only be done at a doctor's office.

In 1983, the first home blood glucometer is developed. Finally, it was practical to take one's sugar multiple times per day, and it becomes possible to experiment with "sliding scale" insulin injections that keep tighter control of blood sugar. By the late 90s, continuous glucose monitors became available- though unlike today's CGMs that allow readings in real time on a smartphone or monitor, these had to be downloaded to a computer at regular intervals.
The 1980s were the first decade where insulin pumps become widely available. The very first pump was large and had to be carried in a backpack, but it represented a huge step forward in glucose control, as it more closely mimicked the function of a working pancreas than once-daily injections.
For the next 30 or so years you really had to work to qualify for an insulin pump, but recently it's been found that pumps greatly improve compliance with blood glucose control whether or not the person had good compliance before getting the pumps, and insurance has gotten better about covering them (though CGMs are still a pain to get insurance to cover).
The 1980s was also the decade that recombinant human insulin (insulin made by genetically modified bacteria) was first used. Up until that point the only insulins were pork and beef insulins, which some people had allergic reactions to. Recombinant insulin was closer to regular human insulin than beef or pork, and represented a big change in how insulin was made.
Today for people who take insulin to manage their diabetes, insulin is usually given as a single injection of a long-acting basal insulin, coupled with smaller doses of ultra-short-acting insulins with meals or snacks. This is the closest we've gotten to mimicking the way a pancreas would work in the wild, and keeps very tight control of blood sugar. This can be done by fingerstick blood sugar tests and individual injections of insulin, or it can be done with a CGM and pump- it just depends on the resources available to the person and their personal preference.

This person was enormous, alleged to have 27" biceps and admitted that he was "not exactly" a natural bodybuilder. When asked how he achieved such gargantuan proportions, he answered "protein, plenty of protein." It was obvious this individual needed more protein than the average bodybuilder; however, I am convinced that some very top grade pharmaceuticals were responsible for his physique. His arms would hang to their sides and his biceps would not elongate, but instead, they had the appearance of softballs stuck under their skins. Mike Mentzer had stated in his writings not to take his word for something just because he won a few physique contests. He wanted bodybuilders to think and understand the concepts of exercise science and nutrition. Unfortunately, a lot of the competitors were seeking the guest poser's advice about which supplement or exercise is best to increase their size, strength, and definition. After he gave them some friendly information, they would each walk away as though they had found the Holy Grail to a championship physique. I found this analogous to asking a Gorilla as to what makes him so strong or quizzing the Cheetah on how he became the fastest land animal. The former would give me a few grunts, and the latter would have me for a snack. It is obvious that these two noble beasts (and many athletes) could not articulate in any fashion as to how they became either strong or fast; their DNA determined that for them. Many bodybuilders with little or no knowledge of biology, anatomy or nutrition, put themselves out there as experts on both nutrition and training, when the truth behind their results is good genetics (good metabolisms and adaptability to exercise stress). I have engaged many bodybuilders who endorse certain training and nutrition methods but receive no "scientific explanation," and when I pressed further, they would simply say "they have been around the sport for "x" number of years" - end of story. Many of the muscle magazines would have you believe you need to consume a "horn of plenty" filled with protein supplements every day to build muscle. Mike Mentzer has spoken about the fact that bodybuilders require about 25% of their calories from protein, and some nutrition scientists are now purporting we may need between 10% to 35%, depending on our activity and lifestyle. Both of these recommendations are higher than the Food Guide Pyramid's 10-15%. I had the privilege to speak with Donald Layman, Professor of Nutrition Division of Nutritional Sciences, Department of Medicine at the University of Illinois Urbana-Champaign per telephone interview. He stated that protein foods high in the amino acid leucine (dairy and meats) help maintain muscle mass while promoting fat loss. Protein has a satiating affect and leucine regulates insulin, which helps better control blood glucose (sugar) leading to a more successful weight management program. According to Dr. Layman, eating protein rich foods consumed soon after exercise may speedup muscle recovery. Leucine appears to have an impact on skeletal muscle and is a direct regulator of protein synthesis. Examples of complete protein foods that contain leucine are beef, chicken, fish, dairy, and eggs. These foods are balanced with the other amino acids; therefore, it would be unwise at this point to take a leucine amino acid supplement. Layman recommends a protein range of 1.4 to 2.0 grams per kilogram of bodyweight. This range happens to fall within the safe and adequate guidelines of the Dietary Reference Intakes (DRIs). So a liberal intake for a 200 pound (90 kilogram) bodybuilder of 2.0 grams per kilo would come to about 182 grams of protein. Dr. Layman explains other points worth noting about protein. First, Marathon runners actually may need more protein secondary to depletion of glycogen. During a long race, they burn triglycerides as fuel, but when they turn up the intensity, thus increasing their pace, they begin to use glycogen (stored glucose) in the muscles for fuel. Higher protein intake helps the body develop and stabilize blood glucose by increasing the ability of the liver to make glucose from the amino acids, and also allowing the muscles to re-cycle the glucose carbons from the muscle to the liver to be re-made into blood glucose." Secondly, Dr. Layman asserts that protein certainly does not cause kidney failure in normal functioning kidneys. However, in cases of pre-existing renal failure (i.e. diabetes) individuals may get benefit from lower solute loads, such as salts or urea. It is also questionable at what point a "low protein" diet is beneficial, because low protein intake causes kidney atrophy. So the trade off is lower solutes and lower rates of protein turnover (normal repair processes). Finally, Dr. Layman claims that gout is never caused by high protein intakes. Gout is a metabolic disorder, an issue of purines (orotic acid) and is only related to protein in the potential that some red meats are high in purines, but so are some plants. If one has gout, they may gain relief by a diet lower in these types of foods. As a registered dietitian, working with hospital patients, athletes and people wanting to get back in shape, I am typically asked the following questions about protein: Q. How many grams of protein do I require daily? A. In my liberal example, a person engaging in high intensity training or Dr. Layman's long distance running example may need between 1.6 to 2.0 grams per kilogram (2.2 pounds) of body weight, while a couch potato may only require 1.4 grams per kilogram of body weight. Q. Shouldn't protein intake vary per person depending on body size? A. The more you weigh, as a result of training, the more protein you will require. For example, using 2.0 grams per kilogram of bodyweight for an individual weighing 175 pounds (80 kilograms) would require they consume about 160 grams of protein daily. However, after gaining ten pounds of muscle, resulting in a weight of 185 pounds (84 kilograms), that same person would need 168 grams of protein daily. Q. Should I be eating every 2 hours (6 meals a day) with each meal having protein? A. Let me answer the last part of this question first. Each meal should have some protein, because protein helps regulate blood glucose and provides satiety (fullness or satisfaction). I would recommend 3 meals and a snack daily and not to let more than 4-5 hours go by without eating. This will help you have control over your Calorie intake and meet requirements from other food groups. Q. How do I measure protein in grams? A. 1. One ounce of meat, fish, poultry, or lean pork contains 7 grams of Protein. 2. One egg, two tablespoons of natural peanut butter, or ½ cup of dried beans, peas, or lentils each provide 7 grams of protein. 3. ½ cup of cooked vegetables or 1 cup of leafy greens provides 5 grams of protein. 4. 1 cup of milk or yogurt, or 1 ounce of natural cheese typically provides 8 grams of protein. 5. Typically, one serving of grains (either 1 slice of bread, or ½ cup of rice, or ½ cup of pasta, or ½ cup of cereal provides 3 grams of protein-amount of protein varies on brand of cereal). 6. Always read the Nutrition Facts panel on food packaging that tells you the serving size and how many macronutrients are in each serving. If you do not have a calorie booklet that breaks down the food into Carbs, Protein, and Fats, I recommend you purchase one. Make sure that about 70% of your protein comes from meat, fish, eggs (or egg whites), or poultry. The complete protein provided by these foods works with the incomplete protein that comes from other food sources. Thus, your body makes the best of all the protein you consume. If you consume too much protein, and therefore too many Calories beyond your growth and maintenance needs, you will simply get fat. If you follow a restricted carbohydrate diet and replace the majority of carbohydrate foods with protein, the body will convert the protein to glucose for energy. The excess protein will not cause any further muscle growth beyond what has been stimulated from the previous workout. In regards to HIT training, I personally have a moderate amount of protein with some complex carbohydrates before a workout and make sure that I am well hydrated before and after. This way my workouts are productive, and I have gained 8 pounds of muscle in the past two months. My intensity during the workout is related to the fact that my protein intake in combination with carbohydrate and fat stabilizes my blood glucose for a steady supply of energy and allows for intense contractions. I want to point out that going out of one's way to consume large amounts of protein daily is a laborious task because, as described above, they satisfy your appetite for an extended period of time and aid in reducing the intake of too many Calories. Also, it becomes an exercise in futility to consume protein supplements, especially when they are unnecessary. It is simple and less expensive to meet your protein requirements with food. Also answering the anon who asked, “….how bad actually is it to go on that particular low carb diet?”
The short answer is not likely to kill you, nor will the bad effects hit you right away, but is counterproductive if used as recommended, and could lead to heart damage and liver damage if used for too long. Also, evidence points to a moderate-carb, plant based (not necessarily vegetarian) diet as the one most likely to be associated with overall health, and long life.
The long answer is… complex. This diet is the one that, as the first anon said is “no carbs hella protein.” But the differentiating factor here as opposed to other low carb-high protein diets is that you also eat a huge amount of fats. Like, basically all your calories are coming from protein and fat, with a few “low carb vegetables” thrown in. That is where this particular diet can cause harm, as opposed to just being a silly, unsustainable idea.
It’s honestly not extremely well documented yet, and studies are still ongoing, because this particular, super-strict diet was originally developed as a way to help treatment-resistant pediatric epilepsy. Which it does very well! But the studies in efficacy there weren’t looking at long-term coronary or liver effects, because the issue in question was “will it help reduce seizures.” And for many forms of pediatric epilepsy, it does. But then adults started getting hold of it many years later and touting it as a fat burning super diet. The idea being that you give your body none of the stuff it wants to burn (carbs), and instead force it to burn fat; as a result, you often also burn your own body’s fat stores. This does, for most people, work. But the “k-e-t-o” in this diet comes from the word “k-e-t-o-s-i-s”, which proponents call the “fat burning stage” and many medical folks tend to view as mild non-diabetic ketoacidosis. You know, the bad thing diabetics can get where your body releases high levels ketones that can damage your organs.
Generally speaking, this particular diet aims for “low levels” of ketone bodies, not the high ones that can damage your liver and kidneys in ketoacidosis. But we don’t actually yet know for certain the long-term effect of low-level sustained ketone production. We have a pretty good idea, tho, with a growing body of evidence to back it up: slow liver damage. That’s why people who have liver disease are told not to do k-e-t-o.
As for heart disease, the issue comes in the fats and high quantities of sodium, red meat, and ldl cholesterol-increasing saturated and trans fats, all of which we know are dangerous. The LDL is what makes coconut oil bad for you, too. Think of LDL and HDL cholesterol this way: your arteries are a vertical pipe. LDL cholesterol, or low-density lipoprotein, is like a cotton ball. It’s light, expansive, and not very heavy. Shove a bunch of cotton balls into the top of the pipe, and they’re unlikely to fall out of the bottom. They’re too light, and they’re bunching together. Now, HDL, high-density lipoprotein, is the “good” cholesterol, and it’s like a marble or a small rock. Heavy, compact, and dense. Drop it into the top of the pipe, it’ll fall right out the bottom, bringing a lot of cotton balls with it. Saturated fats and trans fats increase your LDL, and many plant foods high in fiber increase your HDL. While too much of either is bad, high LDL is a predictor for heart disease because it clogs up your arteries. This diet is chock full of saturated and trans fats, which combined with the high levels of sodium found in many fat sources and meats, makes it very likely that long term use of the diet will cause heart disease. The first round of longitudinal studies found just that.
Note that I said “long term use.” As long as you don’t get your ketone levels too high, don’t have any preexisting complications, aren’t already have liver or heart disease, aren’t pregnant or breastfeeding, don’t have any mineral or vitamin deficiencies, are moderately active, still keep on top of your calorie count, aren’t old, and also can make it past the phase where your body literally freaks out, thinks its starving, and goes into withdrawal from the carbs it’s designed to burn, you should be good to do it for about a year. That’s the cutoff that long-term obesity studies use, because after that, it’s generally considered not the best idea. And it’s those studies that proponents love to tout as proof that it’s safe for your heart, and actually good for your heart. But the truth is, those studies are limited to clinically obese patients, they follow the diet under close doctor supervision, and only for a year at max. They stop after that. These patients are also usually transitioning to this diet from one that is TOO high in carbs and sugars, as well as processed foods. This might actually be the real reason that the diet works so well for rapid weight loss, but again, the research hasn’t got there yet. Also, most studies note that the positive effects on heart disease markers in these patients are limited in time, even before the study concluded.
The main issue is that, while it can induce rapid weight loss, cutting whole nutritional groups is not good or sustainable. Your body was built to burn carbs, that’s why you have a pancreas. Some people like to tout that the “k*to flu,” the carb withdrawal period, is proof that carbs are bad for you because you’re going through “detox.” But you can also describe dehydration as a withdrawal from water, or what happens in starvation as a “calorie withdrawal.” You can get over the withdrawal period, but eventually, you have to “relapse” to carbs because it just isn’t sustainable. To limit yourself to a truly tiny, not very nutritionally dense section of the food on earth makes it more likely that you’ll crash and burn in the diet later on. This is the problem with all restriction diets; they’re hard to follow either because your body is craving the things you cut out (and rightfully so when you can’t eat fruit!), or because it’s time, money, and labor intensive. When you inevitably come off the diet, your weight will almost certainly rebound, because your body goes “OMG we have to stock up, look what happened last time we can’t starve like that again PACK IT ON BOYS.” That’s not to say that everyone rebounds, but it’s very difficult to control. (It can be controlled, tho, don’t let me scare anyone who’s trying to transition.) But you have to come off it eventually. If you do stay on a high-fat carb free diet indefinitely, we go right back to the issue with heart disease, cholesterol levels, and potentially the long-term ketone exposure.
And then there’s a problem with nutrition. To do this diet, you basically can’t have any sugars. Including the ones in fruit. Most veggies are off-limits, too. Vegetables and fruits are extremely important in getting you your daily requirement of vitamins, minerals, nutrients, and fibers, as well as helping hydrate your body. The body craves plants. Sure, you can supplement with vitamins, but it’s really not the same; there’s something science still hasn’t quite figured out about the way your body uses food as opposed to supplements, tho it could have to do with the fact that enjoyment factors into nutrient absorption. It’s always better to get your nutrients from whole foods than a pill.
And for decades, a moderate-carb plant-based diet (not necessarily vegetarian diet, but mostly plants with meats as an afterthought) has been the number one recommended diet by everyone from the FDA to the CDC to increase life expectancy and both improve and maintain overall health.
In general, diets are a bad idea. Changing your diet, meaning your eating habits, is one thing, but going on a diet, meaning a prescribed set of dos and don’ts, is a recipe for unsustainable, yo-yo-ing changes that stress your body, lower your self efficacy, make you feel lousy, and just make your life harder in general. Get-thin-quick schemes and diets prey on our insecurities and fears, but there is no substitute for a healthful, whole, rounded diet and some moderate exercise. I say this having watched my own mother struggle with morbid obesity and try everything from weight watchers to atkins to starvation. I say this as a person who struggles constantly with their own weight, had an eating disorder in high school, and can’t seem to get down into a “normal” BMI range despite trying really, really hard. I know exactly how hard it is, and how much it looks like the Holy Grail. I was tempted for a really long time by the ultra-low-carb diets, not just this one. I did the research, talked with health science and nutrition professors at my university, talked with multiple doctors, and the conclusion I (we) came to was: it’s just not worth it.
For some it might be, and if used properly– with a good reason (usually obesity), for a limited time (months to at most a year), with low amounts of saturated and trans fats, under a doctor’s supervision, and with a solid plan in place for transitioning off into a healthful eating pattern that includes carbs– it could be a good tool. But as preached by fad diet sites, online “health gurus,” and public understanding, it is not safe, or a good idea. I don’t like the literature that’s starting to show that it could hurt your heart and liver. I don’t like the way most people and informational sites treat it as an indefinite lifestyle change. I don’t like the way people preach it as a cure-all. It might have its place for some people (generally those who are obese, not someone looking for a 10 or 20 lb loss), but it has too many risks for my liking, and it goes against everything I learned in my degree program about how to eat well to optimally take care of and fuel your body.

A Nutritionists Answers Your Top 10 Questions About Autoimmunity
Chances are that you know someone with an autoimmune condition, or perhaps you’re one of the estimated 20 percent of the population affected by autoimmunity. (1) By the young age of 30, I had been diagnosed with rheumatoid arthritis, lupus, alopecia areata, autoimmune hypothyroidism, and eczema…all autoimmune diseases. I was told I’d need lifelong medications to control my symptoms, but I wasn’t willing to accept drugs as a solution, so I started doing some research.
I went back to school and obtained my Master of Science in Human Nutrition, writing my master’s thesis on the connection between diet and autoimmune disease, citing research that’s based in antiquity, yet unknown by most doctors today. What I learned is that all autoimmune disorders are intimately connected to gut health. Moreover, the gut/disease connection has been known for over 2,000 years! As the father of modern medicine (Hippocrates) said all those years ago: “All disease begins in the gut.”
Using what I learned, I changed my diet (abandoning nearly 25 years of vegetarianism) and went Paleo. The results were undeniable, and nearly a decade later I’ve been able to keep my autoimmune conditions in remission by using the wisdom of Hippocrates: “Let food be thy medicine, and medicine be thy food.”
Top 10 Questions About Autoimmune Disease
As a clinical nutritionist at PaleoPlan, I have the pleasure of helping thousands of people across the globe to also reverse their autoimmune conditions by modifying diet and lifestyle. Here are the most common questions I’m asked about autoimmune disease.
1. What is autoimmunity?
Stated simply, autoimmunity happens when the immune system gets confused and turns against us. Normally, the immune system keeps us healthy by attacking anything foreign that gains access into the body. When you catch a cold, your immune system produces antibodies and inflammation to destroy the infection. In autoimmunity, this immune response won’t turn off, which results in the production of chronic (long-term) inflammation and the destruction of healthy tissues. If left unchecked, it can cause serious damage, and even death.
2. How common is autoimmunity?
Autoimmune diseases are extremely common these days. When considered collectively, they are the most common diseases in the United States—and possibly worldwide. (2, 3) Some clinicians speculate that nearly everyone has some degree of autoimmune activity occurring in their body. Women are affected much more than men (likely due to hormones) and make up more than 75 percent of all autoimmune cases. (4)
3. How many different autoimmune diseases are there?
To date, nearly 100 different autoimmune diseases have been identified and theoretically, an endless number could exist because the immune system can attack virtually any part of the body. All autoimmune diseases involve an overactive immune system attacking healthy tissue, and what distinguishes them is the part of the body currently under attack.
For example, rheumatoid arthritis happens when the immune system attacks the joints. Hashimoto’s and other autoimmune thyroid conditions are the result of damage to the thyroid gland. Lupus is when your body’s organs are the victim. Psoriasis and eczema occur when your immune system attacks your skin. Multiple sclerosis is when the myelin sheath surrounding nerves is the target, and type 1 diabetes results from an autoimmune attack of the pancreas. While these conditions are very different in their outward manifestations, they share a common root cause: an immune system that won’t turn off. The resulting chronic inflammation is a key characteristic of all autoimmune disorders.
4. Can you have more than one autoimmune disease?
It’s very common to have more than one autoimmune disease, and they tend to occur in clusters and also run in families. (5) When the immune system is revved up and producing inflammation and antibodies, virtually any and all healthy tissues can fall under attack. In many cases, the first sign of autoimmunity is a malfunctioning thyroid. (6)
5. What causes autoimmune disease? Is there a cure?
The medical community is in conflict about what causes autoimmunity to develop. A number of different theories exist, and while researchers don’t agree on the exact details, they all share the belief that something from the environment is triggering these conditions. (7) This external trigger could be a virus, bacteria, another pathogen, a toxin, or something we ingest that gains access to our body by leaking through the gut wall.
Many people are told autoimmunity is genetic and other than medications, there isn’t much that can be done about it. A more integrative approach to medicine would argue that while autoimmunity is indeed influenced by our genes, it’s possible to turn these genes “off” so they are not actively expressing disease.
A new theory set forth by Dr. Alessio Fasano suggests a trio of triggers must be simultaneously present in order for autoimmune disease to be active, and if any one of these triggers is removed, it will go into remission. (8) This trio of triggers includes:
a genetic predisposition to autoimmune disease
a leaky gut
one or more triggers causing the leaky gut
While we can’t remove genetic predisposition, we can turn these genes off by addressing the other two factors of the trio, which boil down to healing leaky gut. While there is technically no “cure” for autoimmune disease, many people have achieved remission simply by restoring their gut health.
6. How does a leaky gut trigger autoimmunity?
The gut lining is only ONE single cell layer thick, designed to allow the passage of nutrients while preventing the entrance of toxins. Nature put 80 percent of our immune system beneath the gut wall to protect us from any “bad guys” that manage to sneak through the gut and into the body.
During leaky gut syndrome, tiny holes develop in the gut lining and stuff enters your body that shouldn’t be there (like bad bacteria, undigested food particles, toxins, etc.) In response, the immune system produces inflammation to combat these foreign invaders. If the leaky gut never heals, inflammation can become chronic (long term) and cause widespread tissue destruction. This chronic inflammation also flips on the gene switch for autoimmunity, causing the disease to express itself in a vicious cycle. To achieve remission, we must turn off the genes and the inflammatory response by healing the leaky gut.
7. What causes leaky gut (and subsequently, autoimmunity)?
Two main triggers have been shown to produce leaky gut in everyone (not just people with autoimmunity): (9)
Gluten and similar proteins found in grains and beans (and nuts and seeds to a lesser extent)
Dysbiosis (imbalanced gut microflora)
Our collective gut health has been on a steady decline since the mid-1900s due to the increased consumption of refined and processed foods, grains, polyunsaturated vegetable oils, and carbohydrates, coupled with a decrease in saturated fats, fermented foods, food variety, and nutrient density. (10) By returning to an ancestral diet (how people ate only a few generations ago), the gut can heal, which disengages the immune system and stops the autoimmune attack.
8. Can Paleo reverse autoimmunity?
The Paleo diet naturally excludes the main triggers of leaky gut (grains, beans, common food allergens like dairy, refined and processed foods, excess carbohydrates, etc.), while including a higher percentage of healing fats and fibers. Paleo is beneficial for many autoimmune patients who often start experiencing relief from their symptoms within days or weeks. Depending on the severity, it can take longer, and some people need to take things a step further to fully heal their gut and reverse autoimmunity.
9. What is the AIP (Autoimmune Protocol)?
The Autoimmune Protocol (AIP) is a stricter form of Paleo that removes other potential gut irritants like nightshades, egg whites, nuts, seeds, and more. The AIP also encourages the consumption of bone broth, collagen, fermented foods, and other gut restorative measures.
People often wonder if they have to follow the AIP forever to keep autoimmunity in remission. Because the AIP restricts so many foods, it’s not ideal (and often unnecessary) to remain on it long term. When someone is new to Paleo, I recommend starting with strict Paleo for at least six weeks to allow for more variety and a gradual detox period. Paleo alone can do wonders to reverse autoimmunity and several other chronic conditions. If you’re still having symptoms after six weeks, then consider following the AIP for up to a year. Some people benefit more by following a GAPS/SCD approach to healing the gut, whereas others end up needing herbal antibiotics or probiotic support to treat underlying infections, or nutrient supplementation to correct underlying deficiencies.
10. Does anything other than diet trigger autoimmunity?
Other factors, such as stress, alcohol, medications including NSAIDs, infections, and other stressors can also influence gut permeability and instigate autoimmunity (although diet seems to play the biggest role for most people). We call Paleo a “lifestyle” rather than a “diet” because of its holistic approach addressing all of the essential lifestyle factors necessary for achieving optimal health.

If you suffer from an autoimmune disease, please know that there is hope! Many people achieve and maintain remission from their autoimmune (and other) chronic conditions simply by following an ancestral way of living and eating, and tending to the health of their gut.
