新冠與痛風：疫情之中，痛風的發病率增加8倍

墨西哥研究報告：疫情中痛風發作率增加8倍

2021年11月，《臨床風濕病》雜志發表壹份墨西哥的研究報告：在2020年4月至7月之間，也就是墨西哥的第壹波新冠疫情期，在接受隨訪的101個痛風患者中，有36人痛風發作，複發率達到了35% [1]。

相比之下，在疫情之前的2019年10月至2020年3月間，痛風患者的複發率只有4.4%。

對比疫情前後，痛風複發率從4.4%增加到35%，有8倍之多！

IL-17A與痛風

壹般來說，痛風是因爲體內尿酸水平過高所致。食物中的票呤在人體內會代謝成尿酸，通常情況下，人體將尿酸通過腎髒排出體外。在特殊的情況下，當人體産生過多的尿酸，或者腎髒無法有效地將尿酸排出體外時，尿酸就會積累在關節和周圍組織中，形成尿酸晶體，引起疼痛和炎症，導致痛風的發作。

這裏有壹個關鍵詞： 炎症。

而與炎症有關的壹個重要的細胞因子，是IL-17A。

在痛風的動物模型中，給小鼠注射尿酸鹽晶體，可以誘發痛風， 但是如果給小鼠使用IL-17A的中和抗體治療，可以減少關節的炎症[2]。

這說明IL-17A直接導致了痛風者的炎症。

新冠病毒與IL-17A

2022年發表的壹項研究表明，新冠病毒感染後表達的壹個 病毒蛋白ORF8，功能類似人體的IL-17，能夠結合IL-17 的受體 A 和C，並且能比人體的IL-17A引發更強、更明顯的炎症反應[3]。

在新冠蛋白ORF8中，Y42 和 E106這兩個氨基酸對 IL-17 受體A的結合很關鍵， I71 和 I76則對IL-17 受體C的結合很關鍵。這些位點發生突變，ORF8結合IL17受體的能力就會有變化。

面對新冠，痛風患者該幹什麽已經很清楚了

新冠病毒、ORF8、IL-17A、痛風，已經形成了壹個閉環，完美解釋了疫情之中痛風的高發。

根據2017年數據，中國高尿酸血症患者人數高達1.7億，其中痛風患者超過8000萬。目前的痛風患者，應該有1億。

如果可以套用墨西哥的數據，在1億人中，4個月之內正常情況下會有400萬人痛風複發；在疫情中，會有3500萬人痛風複發。

在去年年底，中國在壹個月時間內，陽過率達到了90%，遠遠高于2020年墨西哥的感染率。

多少人會痛風複發？或者說首次爆發痛風？這是答案尚未揭秘。

因爲中國基本陽過壹遍了，該陽的都陽了，該痛風的也痛風了，似乎沒有必要過多憂慮。

但是，痛風總會複發，新冠病毒也還在，對于痛風患者，可能還是需要���別注意壹下。
