showed SSX to my friend and we noticed that often times harlock does his classic massive spin of the wheel and then they show the ship but it's not turning 💀💀

obviously animation issue / budget but we formed a headcanon that tochiro saw the state of the deathshadow and was scared what harlock could do to the arcadia

so he installed a system that can override whatever the wheel does and just navigates the ship with the computer. so harlock can go spin spin to his heart's content and the ship ignores it

he's like "omg tochiro idk how you did it but this ship is super easy to steer!! though it doesn't do big turns easily"

Veronica: Loves classical literature, John Hughs movies, and Disney movies. Has a thing for musicals like The Phantom of the Opera, Starlight Express and Cats.

Gold Heather: Despite her sweet demeanor and appearance, she LOVES horror and slasher movies like Hellraiser, Friday the 13th, Halloween, Child’s Play, Army of Darkness, Creepers, and Fright Night. The bloodier and more chaotic, the better. Red and Green are too disturbed to really judge or comment and just let Gold enjoy herself. She also dabbles in anime because of Specs, owning a Doraemon keychain he gifted her clipped to her purse. When asked by others, she denies knowing what it is and just found it cute in a crude attempt to protect her reputation.

Specs: HUGE Star Wars fan. Had he survived, he would also be a fan of the Prequels and Sequels, mostly happy there’s more movies to enjoy and analyze, despite agreeing the sequels aren’t that great. Big anime fan, enjoying stuff like Space Adventure Cobra, Demon City Shinjuku, Arcadia of My Youth: Endless Orbit SSX, Doraemon, Saint Seiya, Space Warrior Baldos, Robot Carnival, Giant Gorg, Ronin Warriors and Sherlock Hound, but particularly loves Tatsunoko Productions like the various Time Bokan series and Tekkaman.

Kurt: Enjoys stuff like Conan the Barbarian, TMNT, He-Man, Thundercats, Thundarr the Barbarian, Blackstar and Bravestarr, but hides it initially. He-Man is his jam, and is both his “gay awakening” and the reason he started working out and getting into sports. Once admitted to Dan that Prince Adam/He-Man is his idol and who he strived to be, but failed and instead became an Evil Warrior like Two-Bad (in conjunction with Ram). Likes to collect He-Man toys but is often in a disguise when going to places like KB Toys to find them. Similarly likes collecting He-Man and TMNT comics but tries to hide them In between workout and swimsuit magazines. Also reads the gay model magazine Stud Puppy, but isn’t as good at hiding it… Actually got to model for Stud Puppy in his early twenties as part of a personal milestone.

Ram: A big TMNT fan, especially the movies and video games, but initially hides it. Is actually a pretty big gamer when he’s not playing football, and frequents the arcades to play the games albeit in disguise. An expert BurgerTime, Pole Position, Contra, Pac-Man, Ms. Pac-Man, and Donkey Kong player, and is very fond of Mario and The Legend of Zelda. Isn’t particularly great at sports games like ExciteBike and Track & Field ironically. A casual fan of Star Wars, and unsurprisingly his favorite part of Return of the Jedi is the slave Leia stuff. Enjoys testosterone charged movies like Top Gun, The Running Man, Commando, Bloodsport, Big Trouble in Little China and The Warriors.

Betty: Similar tastes to Veronica and Martha. Seems particularly into Care Bears, ironic given her cruelty later on. Likes sitcoms like Family Ties, The Golden Girls and Nightcourt.

Jamie: Shares Dan, Kurt, Ram and Spec’s interests. Big Dungeons & Dragons kid and loves the cartoon. Big movie guy like Martha. Enjoys stuff like V, Quantum Leap, TerraHawks, Poltergeist, Batteries Not Included and Flight of the Navigator.

Disseminated Strongyloides and Hyperinfection

If you've ever rotated through any type of rotation that involves an immunocompromised host, your boss will have obsessed over the same few bugs. Including the parasite strongyloides. Aka threadworm.

They will never forget it, nor will they let you. They all seem to have that one case of disseminated disease that got missed and ended badly. Of all the bugs in the world to screen prior to starting significant immunosuppression, this is one of them, along side HIV, viral hepatitis and TB.

It's endemic to tropical environments in the developing world.

The concern is hyperinfection and disseminated disease in the case of the immunocompromised, in which case the mortality is 90%.

Hence the universal acceptance of screening for it in serology (I.e. IgG) in patients about to be immunocompromised on long term or high dose steroids. I.e. solid organ transplants, BMTs, rheumatology patients needing strong long term immunosuppression etc.

The how: As part of it's infective lifestyle, the larva penetrate the skin, and via skin & the mucosal layers, it enters the blood stream, travels to the right heart and into the lungs. As the host coughs, they swallow the larvae and it enters the GI. Does that sound completely skin crawling and eerie?

Clinical manifestations: Pulmonary - eosinophilic pneumonitis (the WCCs directed at parasites increase and are the sources of inflammation in the lungs), alveolar bleeding from larval damage (yikes). On CT imaging, essentially you just see ground glass changes - non specific findings of inflammation (which can be caused by a huge array of things, of which, disseminated strongyloides would actually be the last on your list if you're not in an endemic area...). classic presentation in any GG inflammation is SOB, sometimes cough.

GI: larvae mature, embed into the intestinal mucosa and produce eggs. The eggs perpetuate the lifecycle and the host gets diarrhoea, abdominal pain etc.

Skin - rash, from dissemination of the larvae but also at the penetration site (may go unnoticed).

So then what's hyperinfection? T cell mediated immunity is suppressed in immunosuppression either by broadly suppresive steroids (dexamethasone, prednisolone etc.) Or transplant patients on therapy directed at the T cells, in order to prevent them rejecting their organs (maybe I'm still doing this, I'll make a blog on rejection).

Hyperinflation...wait.. hyperinflation..freudian slip but equally bad.

I mean hyperinfection results from the immune system leaving the worms to proliferate unchecked. "Excessive" worms are seen in usual affected organs - skin, lungs and gut. So expect an excess in clinical presentation, bloody diarrhoea, weight loss, malabsorption etc.

Disseminated disease occurs when it spreads outside the 'usual' routes of skin, lungs, gut and the numbers lead to catastrophic damage. Respiratory failure, AKI, shock, DIC, meningitis. Can spread widely to liver, heart, etc.

As added flavour, it can also lead to gut bugs causing bacteraemia. E.g. Ecoli, strep bovis (so look for malignancy as well as Strongyloides).

Given how devastating severe disease is, it's common practice for any immunocompromised host with eosinophilia and who's traveled to the subtropics/tropics to be screened. I've seen some really paranoid ones will send off serology on seeing eosinophilia.

As the bug can autoinfect a host, expect it to be there for a lifetime once infected.

Chronic/mild forms of the disease present as well, chronic GI symptoms (heart burn, anorexia, reflux, abdo pain, diarrhoea) and is often mistaken for IBD. Hence the importance of scopes and biopsies given therapy for IBD is immunosuppression.

investigations: easy pick up on biopsy if scoped or bronch'd. otherwise, serology. Suspect it on seeing eosinophilia. Stool mCS also an option. in chronic disease - mildly elevated eosinophilia and elevated IgE

Treatment of strongyloides vs hyperinfection: ivermectin or albendazole. Ivermectin is so infamous now post pandemic.

Issues: serology can take time to return, if in doubt, I've see some consultants/attendings empirically treat with ivermectin because the risk of mortality in hyperinfection is so high. But only in cases where patients are from or travelled to endemic areas. It's considered low risk if they have done neither.

So Vampire Academy premiered today on Peacock and, as someone who loves the books and recently reread them for this day, I'm pissed.

They really threw the books out the window and it was a very bad decision.

1st, what is with the wannabe classic vampire style? Why do the royals dress like 19th century vampires when the girls literally talk about their clothes being for Target, Forever 21, and GAP. I think the movie did better with balancing the gothic building and modern clothes.

Then, Rose gets moved to the dhampir dorms that look like the barracks from Divergent. No walls. No dividers between beds. Nothing!

And the moroi dorms are huge and the windows aren't tinted which is ridiculous considering they decided the moroi can't be in the sunlight.

Mia and Lissa share a room, which sets it up for Mia to be planting the dead animals even though Sonya will definitely be the one turning strigoi to help Victor.

If they're roommate and Mia's dad ks Victor, there's no way they get the severe rivalry of the books.

And wtf is with the Dhampir rankings thing? Is Meridith going to be important now? She was mentioned, like, once in the books.

Holy fuck! I almost forgot the weird feeder thing! The feeders aren't in private cubicles by the dining hall, they're in the basement in one big room with red velvet couches and, once again, no dividers between people. And the feeding scenes are fucking weird.

Y'all, I haven't even finished the pilot. It's so much different from the books that it could almost be it's own thing.

Biology is Dustin's favorite subject, especially when the focus is more on plants and animals.

English. He hates all the essays they're expected to write. He's always been marked down for his writing sounding too conversational. He also hate that they always have summer reading.

I don't see Dustin attending any university. If he were to attend, he'd go to Sim State University.

I think he'd take a while to declare a major, but he'd eventually settle on economics, political science, or biology. He'd be really drawn to polisci and bio, but I think econ would ultimately be his pick. It would feel more practical.

🔮: Something Random

Dustin is a Fortune sim more in the sense that he wants to be able to live comfortably, without fearing that the repoman will come. If he had the resources, he’d happily work from home to take care of the kids he eventually has. Skip wasn’t really the best dad (or husband) in my headcanon, but Dustin remembers fondly being able to play with his father, and he wants his kids to have similar memories with him.

"It's beautiful out here, isn't it?" she sighed happily. "Still a little too slow for my liking but it's interesting to hear what others are planning to do for the town."

"I mean we are all still deciding our roles. I know that I want to do a small business - maybe two - on the side of whatever career I choose. I'm tempted by the Adventurer or Gamer careers."

"Adventurer would be more up my street but Gamer is easier because I am brilliant at gaming, if I do say so myself. I miss playing online with my friends."

"What's your favourite game to play?" Jed asked, curiously.

"SSX 3 is a classic" Emily said. "But Spore and FIFA 08 are also very good."

"I play FIFA" Jed said, grinning. "It was what inspired me to play football, admittedly I'm not as good as I want to be on the game."

Emily sighed "I miss connecting with people. The travel aspect I cannot replicate anywhere I go. I desperately miss experiencing new cultures and seeing what else there is in this world."

"Sounds like you're going to be the first to fly out of the new airport connection we set up!"

Emily sighed heavily again. "And it depresses me how long that'll likely take!"
