Introduction

Coronavirus disease 2019 (COVID-19) is a highly contagious respiratory illness caused by the severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2). The virus was first identified in Wuhan, China in December 2019 and has since spread globally, leading to a worldwide pandemic. The majority of individuals who contract COVID-19 experience mild to moderate symptoms and recover within a few weeks. However, a significant proportion of individuals continue to experience symptoms long after the acute phase of the illness has resolved, a condition known as Long-COVID or post-acute sequelae of SARS-CoV-2 infection (PASC). Long-COVID symptoms can be diverse and can range from mild to severe, and can include fatigue, depression, anxiety, joint pain, headaches, and cognitive impairment.

There are currently no effective or broad treatment options for Long COVID, but some symptom-related management has been trialed. Traditional treatment options, including rehabilitation, physical therapy, and medications, may not be fully effective for all individuals. The pathophysiology of Long-COVID is still being elucidated and there is a need for alternative treatment options. A recent review has suggested that psilocybin and Methylenedioxymethamphetamine (MDMA) may be an effective treatment option for the mental health challenges associated with COVID. Psychedelics have been traditionally used in spiritual and religious practices, but in recent years, there has been a resurgence of research on the therapeutic potential of psychedelics. Psychedelics have shown promise in the treatment of various medical conditions such as depression, anxiety, PTSD, and addiction. This case report describes the experience of a 44-year-old female who developed Long-COVID symptoms after contracting COVID-19 and her experience managing her symptoms and using psychedelics as a treatment. To our knowledge, this is the first case report describing the potential efficacy of psychedelics for Long-COVID symptoms.

Day 2 Post-Op:

Team Sadie PSA: Proof exists that prayer and intentional well wishes are overwhelmingly powerful. So powerful, we might even need to direct this group toward a new cause. She’s made so much progress, so quickly that it’s surprising everyone. She had open heart surgery 2 days ago.

Oxygen, gone

Chest tube (only one!), gone

Pacing wires, gone

Cerebral NIRS, gone

Central line, gone

Arterial line, gone

Morphine pump, gone

1/3 peripheral IV lines, gone

Though she’s made remarkable progress in 24 hours, the path to healing is never straightforward. Sadie’s cough has returned (remained?) and though it doesn’t seem to bother her, it has been unrelenting. Constant coughing kept her from sleeping much again last night. Pulm and Respiratory Therapy are convinced her lungs aren’t the culprit - it’s likely irritation from intubation. And though this is good news, it makes finding a solution more challenging. She needs to rest.

Sadie started the day NPO, low on sleep and in pain as her chest tube was extremely uncomfortable. Our friend, Jill from child life returned to help with the removal most lines - huge blessing.

Then Nora came to visit for the first time. My heart was made whole again to have our girls together. Nora was a little nervous about all of the equipment and it was scary seeing Sadie with so many bandaids on, but it was a ray of sunshine for all of us. Plus, she got to wear a super cool badge. Matt and I were brought to tears to watch as Sadie took the lead in making Nora feel comfortable and less intimated. We could not be more proud.

After lunch, Jill returned for a special surprise. Sadie was allowed to unplug from her tree and walk to the playroom. She LOVED it so much that she refused to return to her bed after she walked back. She was invigorated by the exercise and the “fresh air” (her words) down the hall. She was excited to explore the rest of her room and spent the afternoon snuggling/puzzling/reading on the couch. She was also tickled by a drive-by visit from our wonderful neighbors. When the physical therapist arrived, she said it’s been a long time since she’s seen a kid this mobile and motivated this quickly post-op.

Our critical care team this weekend has been fantastic. They’ve been providing excellent care for Sadie and have included my opinions in the treatment plan each day.

Sadie’s been running a low-grade fever since she was admitted to the CICU and while the cardiology fellows aren’t yet concerned, I feel differently. I had amazing support and advocacy from our nurse, Natalie, and the CV critical care attending. Both were eager to follow my instincts. Waiting on cultures to determine next steps.

Overall, we’re in awe and these are hopefully small bumps in the road. A good reminder that her body is still adjusting to a major shock. Regardless, we’re already on the waitlist for the step-down unit and know we’re still getting the best care for our girl. Still taking one day at a time.

DYNE-251 for the treatment of Duchenne Muscular Dystrophy Received FDA Fast Track Designation

HK-Magicure -- On October 31, Dyne Therapeutics announced that the U.S. FDA has granted Fast Track designation for DYNE-251 for the treatment of Duchenne muscular dystrophy (DMD) mutations amenable to exon 51 skipping. DYNE-251 is being evaluated in the Phase 1/2 DELIVER global clinical trial.

DYNE-251 is Dyne’s product candidate being developed for people living with Duchenne muscular dystrophy (DMD) who are amenable to exon 51 skipping. DYNE-251 consists of a phosphorodiamidate morpholino oligomer (PMO) conjugated to a fragment antibody (Fab) that binds to the transferrin receptor 1 (TfR1) which is highly expressed on muscle. It is designed to enable targeted muscle tissue delivery and promote exon skipping in the nucleus, allowing muscle cells to create a truncated, functional dystrophin protein, with the goal of stopping or reversing disease progression.

In preclinical studies, robust and durable exon skipping and dystrophin expression were observed in the mdx mouse model in skeletal and cardiac muscle as well as reduced muscle damage and increased muscle function. In non-human primates, DYNE-251 demonstrated a favorable safety profile.

DELIVER is a Phase 1/2 global clinical trial evaluating DYNE-251, consisting of a 24-week multiple ascending dose (MAD) randomized placebo-controlled period, a 24-week open-label extension and a 96-week long-term extension. The trial, which is designed to be registrational, is expected to enroll approximately 46 ambulant and non-ambulant males with DMD who are ages 4 to 16 and have mutations amenable to exon 51 skipping therapy. The primary endpoints are safety, tolerability and change from baseline in dystrophin levels as measured by Western blot. Secondary endpoints include measures of muscle function, exon skipping and pharmacokinetics. Dyne anticipates reporting data from the MAD placebo-controlled portion of the DELIVER trial on safety, tolerability and dystrophin in the second half of 2023.

About Duchenne Muscular Dystrophy (DMD)

Duchenne Muscular Dystrophy (DMD) is a rare disease caused by mutations in the gene that encodes for dystrophin, a protein critical for the normal function of muscle cells. These mutations, the majority of which are deletions, result in the lack of dystrophin protein and progressive loss of muscle function.

DMD occurs primarily in males and affects an estimated 12,000 to 15,000 individuals in the U.S. and 25,000 in Europe. Loss of strength and function typically first appears in pre-school age boys and worsens as they age. As the disease progresses, the severity of damage to skeletal and cardiac muscle often results in patients experiencing total loss of ambulation by their early teenage years and includes worsening cardiac and respiratory symptoms and loss of upper body function by the later teens. Abstract

Since the declaration of coronavirus disease 2019 (COVID-19) as a pandemic, it remains a widespread infection with a major impact on global resources and health infrastructure. The hallmark of COVID-19 continues to be the well-documented effects it has on the respiratory system. With the passage of time, the involvement of the severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) virus in other systems has become more apparent, with the increased incidence of thromboembolic events, cardiac involvement as well as gastrointestinal and neurological symptoms secondary to the infection. Our case report demonstrates a presentation of vertigo, hearing loss, tinnitus, and aural fullness. Our patient was diagnosed as positive for COVID-19 by reverse transcription-polymerase chain reaction (RT-PCR) nine days prior to developing these symptoms. Her COVID-19 infection was otherwise relatively mild, for which she did not seek any medical intervention. A careful assessment ruled out cerebrovascular causes and led us to the diagnosis of SARS-CoV-2-induced labyrinthitis. Our patient was successfully treated as an outpatient without unnecessary investigations and responded well to standard therapy for viral labyrinthitis as per National Health Service (NHS) guidelines. She eventually reported having made a full recovery within three weeks of the initial encounter. Audio-vestibular consequences of COVID-19 are less reported compared to other symptoms of neurological involvement, such as gustatory or olfactory dysfunction, which have become key indicators aiding in the diagnosis of the infection. Among these disorders, the commonly reported presentation is that of vestibular neuronitis. Our case report demonstrates that labyrinthitis is also among the neurological manifestations to be considered as a result of COVID-19, which can be safely managed in the community with the same strategies as those employed for other viral triggers. It also reveals the need for further research into the effects that COVID-19 may have on the audio-vestibular system.

It is recognized as an accredited program by the National Board for Respiratory Care (NBRC), which provides enrolled students who complete the program with eligibility for the Respiratory Care Credentialing Examination(s). The program will remain on Provisional Accreditation until it achieves Continuing Accreditation.
