#I want to stress that this is (probably) very minor #I do not live in the US. Hospital visits are free and my doctor told me I should go to the ER so. YOU SINK INTO THE steaming water, your muscles immediately relaxing. A small moan escapes your lips at the instant relief of your loosening muscles. Spencer's lips quirk upward. You roll your hips in rhythm with his as he guides you in and out of him with his rough hands gripping your waist. He removes his lips from yours and attacks your neck, sucking on your delicate skin and leaving love marks in his wake.

He helped you dress in your own clothes this time and lead you back to bed as he laid with you in his arms for a while, breathing in the scents of each other.

"I don't think it would be healthy for either of us to stay inside my apartment the rest of the month."

You groan and lift your head from his chest. "Depends what you have in mind. I plan on being a pain in the ass if I don't want to do it."

He rolls his eyes but you know his annoyance is faked with the way the corners of his mouth lift into a smile. "Well, with the holidays soon, you know exactly how Penelope gets. She texted me to ask if we wanted to come to her place tonight for a friends-giving. There is a requirement though."

"Hmm?"

"Everyone has to make something and bring it."

"I hope you're a good cook."

"Too bad you're gonna help me."

"I'm so tired."

"Come on, we better get started."

Spencer drags you out of bed by your hand and leads you into the kitchen. "I'll let you choose what we make."

You dramatically tap your chin as you think, you run through all the possible recipes you could know but you've never been much of a good cook. "One of the few memories I have from my childhood is my mom's baked mac and cheese. I've made it for myself countless times, it's always a hit...with myself."

"Baked mac and cheese it is, then."

He pulls you into an embrace one more time, unable to get enough of you. Too quickly, he releases you and beings to gather all the necessary ingredients, spices, and kitchen equipment you guys will need to make dinner.

As he leaves you to start the pasta, he grabs a speaker from his bedroom and connects his phone to it. You only notice when Fly Me to the Moon begins to play and he ceases your waist. He whips you towards him as he takes your hands and begins dancing with you. You can't help but giggle as you both badly bellow the words together.

You begin to wonder what kind of playlist he could possibly have on when the songs change from Footloose to Enter Sandman to Little Lion Man to The Way You Look Tonight. You sing loudly and dance goofily in between mixing and seasoning and cooking. When you finally pop the mac and cheese in the oven to bake, one song ends and another turns into Can't Help Falling in Love. Spencer takes your hand, snaking the other around your waist to pull you close. You both softly sing to each other as you slow dance around the kitchen, soaking in the love floating around the entire apartment. You move in perfect cadence, as if your bodies were perfectly sculpted to match the other.

"I'm sorry for not telling you sooner that I'm madly in love with you," Spencer smiles, his face mere centimeters away from yours.

"I think you made up for it."

"Good, because I'd get on my knees for you if you asked me."

"I find that doubtful."

"I don't beg for anything or for anyone. But, I would absolutely get down on my hands and knees to beg for you. A hundred percent."

"I might just have to put that to the test."

"Careful," he purrs, pulling you into a tender kiss.

Cold open with a case report:

60M with a past history of previous wrist fracture presents with worsening pain and swelling, erythema and reduced range of motion, after suffering an abrasion from changing a tire.

the previous injury resulted from an MVA 9 yrs prior with a closed rist fracture

there was also a history of ETOH use and possible malnutrition, which can result in immunosuppression

Xray showed extensive changes in the wrist bones compared to past years with near loss of several, suggestive of pyogenic arthritis

Aspiration of the wrist joint grew Finegoldia magna

What is it

not actually uncommon

but is more commonly associated with post op infections or prosthetic joint infections (so case reports are harder to find in this category)

previously known as peptostreptococcus (sounds like peptobismol) but was renamed following 16S rna sequence availability, so in older texts/articles you'll still see it's old name

it's a commensal, so normal part of our microflora, but is opportunistic like so many other pathogens we usually live with and covers our body

It is a Gram Positive Anaerobe, of the same flavour of clostridium. Remember there's fewer gram positive anaerobes than there are aerobes. So consider it in cultures if there's only growth in the anaerobe bottle and it's gram positive (purple). Usually it likes mucosal surfaces, such as the GI or GU tracts, but it doesn't mind skin either. It's considered a "gram positive anaerobic cocci" (GPAC) and has increasing prevalence and antibiotic resistance in the group.

Increased risk and when to think of it

chronic ulcers, diabetic ulcers

associated with biofilms (always bad, requires prolonged duration of antibiotics)

prosthetic joints

prosthetic valves

as it is opportunistic, also consider it in the immunocompromised

In the case above, the patient had septic arthritis, for which anaerobes account for 20% of cases. And Finegoldia is often the culprit in the case of post op and prosthetic joint infections. Rarely affects normal joints or healed joints from previous closed trauma years prior.

In chronic wounds it can impair healing via Protein L, which bindings to antibodies, causing immunomodulatory effect.

Bit of history

named for Sydney Finegold, an American physician who was a founding member of IDSA (infectious diseases society of American) and one of its early former presidents

but the name did make me think of 'fine gold'

Treatment

luckily susceptible to penicillin - but there is growing resistance, hence relevance of always chasing susceptibility

alternatives: metronidazole (typical class that covers anaerobes), tazocin and carbapenems

also has increasing resistance to clindamycin (a consideration in penicillin allergy or during empirical therapy for wounds)

While the "exam" is free, the xray will cost 30$. So I can't even get an estimate until his exam..

So, I'm sleepless at 1am and I'm tired and I'm upset so I'm going to get out the shit that has been going on for me the past month and a half, because it keeps leaking out in tags and I guess maybe I should not do that. This is going to be long and it's a huge bummer. I just don't feel like I can come out and talk about it directly to anyone because it is indeed a huge bummer and I just. can't deal with like. the feeling of unloading this on someone and then what. but throwing it into the tumblr void and hiding it behind cuts feels less like I'm throwing a bunch of gross stuff at someone's feet so like, yeah. Content warnings for uh, facing the mortality of aging parents, the American healthcare system being a fucking nightmare, mentions of strokes, infections, and other huge medical problems, and basically stuff being awful.

I have lived with my mom as her caregiver for the past three years, since losing my job during covid. She is six years in remission from breast cancer, diabetic, and in end stage renal failure, meaning her kidneys don't work and she needs hemodialysis 3 days a week. I came in mostly to do things like shopping, the majority of cooking and cleaning, be present in case of falls, things like that. It's worked out, mostly. It sucks that she had to move to AZ to live near her brother simply due to the fact that her cozy little house in MT that she got remodeled to her needs has no dialysis centers within an hour's drive and the closest ones had no open seats anyway, not even for a summer stay, because of the nationwide nursing and technician shortage in healthcare. So she lives in AZ and goes to dialysis 10 minutes away. It works out, sorta. But. Since August 1st:

she had a stroke, which affected the language centers of her brain and made speech, writing, and complex communication difficult

she spent a week in neuro ICU for follow up; this was at a hospital 20 miles away in Phoenix. yes I drove there every day to see her.

she was transferred to a rehabilitation center after that week, where she had several good days but then started to experience severe lower back pain.

the back pain got so severe that she was crying and even screaming when being helped into/out of bed, or having the head of the bed elevated so she could eat.

myself and her other present family were very upset about this, because she was in pain! and also it was preventing progress in her PT/OT after the stroke! we wanted them to do something and find out what was going on!

an xray was done. "no significant findings."

we asked for a CT scan or MRI if an xray found nothing.

it didn't happen. maybe insurance said no, maybe assumptions were made that it wasn't necessary.

but mom spent the rest of her 15 days at that rehab facility on tylenol and then oxycodone.

she was then discharged, and yet in no state to come home.

we found a short-term care facility for her that she'd actually stayed at after her initial ER stay that discovered her renal failure. we thought that she'd do okay there.

she didn't. she declined. she was still in severe pain. MWF for her dialysis, she was basically just, done. and they had scheduled her dialysis at 6am. so. the day was just over, no ability for therapy.

they still just gave her oxycodone so even on good days she was sleeping a lot and very out of it.

we asked to speak to the physician and her case worker. we were not able to.

we asked for her to get further testing instead of just this pattern of throwing narcotics at her and watching her decline. it didn't happen.

at the end of her second week there, she was becoming less responsive and more inclined to just sleep.

she wasn't eating. I tried so hard to encourage her to eat, I even fed her. she still would eat a few bites and then be uninterested in more.

one day she only ate a cup of yogurt.

then she was so badly out of it and barely responsive that I demanded that they do something. so they gave her narcan. because maybe she was too lost in the oxycodone sauce. didn't help, they gave her more. didn't help. they called EMTs.

she ended up in the ER, where they did a CT, MRI, and bloodwork.

(this was a week ago)

turns out she has an infection in her spine. which is osteomyelitis, an infection in the bone and bone marrow that just constantly hurts. oh also numerous pressure sores, including a bad one on her heel, and a bad one on her lower back. either could have introduced the infection into her spine.

that infection has spread to her blood. yeah she's got sepsis.

she also has had at least one additional stroke and her speech has degraded even further.

she spent several days in the ICU and it was very touch and go.

it is still very touch-and-go but she's now in progressive care (pcu) which is a downgrade from ICU in terms of how serious.

but yeah. sepsis is very dangerous for young and relatively healthy people. she is 74. her diabetes means her healing is very slow. her renal failure means that her system can't handle a heavy load of antibiotics so they have to be given more spaced out and in lower doses.

it. sucks.

I still go to see her every day and talk to her even if she's not responding. I sing songs she knows. I put on videos of the church services in her hometown for her to listen to. - it. sucks. so. much.

oh yeah also her place here in AZ is in a retirement park and I can only be here because she resides here. so if something happens even in terms of her moving her permanent residence elsewhere I can't stay here.

I have no income I basically just lived here with her and she covered my expenses as my pay for being her caregiver. so like. looks at the potential need to get independent housing, looks at bank account with $30 in it, laughs darkly and decides I can't think about that.

besides I need to think about how this could be the last time I have left with my mom.

This is very adulting. I felt very adult.

My father was rushed to the emergency room... My oldest sister asked me if I can leave (bcos I have work) to attend my father's needs and told me what happened. No blinking and thinking twice, I said yes. Nagsabi agad ako sa boss ko. Hindi na nga ako nakapagpalit ng damit e, nagsuot lang ako ng underwear XD, kinuha bag ko tas lumipad na ako. Nagsinungaling pa kami kay mama na yung kapatid kong sumunod ang pupuntahan ko kasi nagpapasundo.

Pagdating ko, na-ECG na raw si papa and waiting nalang yun result. Kinukwento nya nga ano nangyari tas nagpakwento sya kung ano ang reaction sa bahay. Time to time din nagmemessage ako sa mga kapatid ko at kina ate at kuya M (parents ni Mhigz) sila kasi nagcontact sa amin sa sitwasyon ni papa.

Normal naman ang ECG ni papa kaya para lang masigurado, nagparun ng xray at lab tests.

Nagsama nang matagal ang dalawang hindi mapakali kapag walang ginagawa. Lalo kapag may work, work lang nang work... Parang gusto ko nga yayain si papa maglakad lakad na lang sa loob ng ospital kaso ang sungit ng nurse delikado raw kasi lalo at chest pain. Nagkwentuhan saglit, minsan biruan. Kasi tinanong ko kung masakit ba yun injection XD tas tawa lang kami. tas tahimik lang kami pero parehas pala kami nakikinig sa paligid at ganap sa ER.

Dumating din yun kapatid kong bunso galing sa work. Nag undertime pala kasi nag-aalala raw sya kay papa saka ewan ko, feeling ko pinasunod sya ni ate kasi wala tiwala sakin na kakayanin ko. Kasi iniissue nila na takot daw ako sa tao ni hindi ko nga raw kaya makipag usap sa konduktor ng bus para magbayad ng fare at bumili sa counter ng fastfood -_-" Tho inaamin ko naman na wala ako alam sa process kapag ganun pero pwede naman magtanong.

Sinabihan nalang si papa na magpahinga for 3 days tapos magpa over all check para malaman talaga kung ano cause nung pananakit at paninikip ng dibdib nya.

Tho sabi ni papa naexperience nya na yun nung grabeng sama ng loob dulot ni ate sa kanya pero ang kaibihan lang dati raw naninikip batok at ulo nya. Pero ngayon dibdib lang talaga tas biglaan lang. Hindi naman daw sya galit or what.

Pero thank you Ama pa rin kasi wala naman kung anong masama ang nangyari. Plano rin namin ipa over all check up sina mama at papa e.
