Every Pokemon has an organ (I'll be calling it simply Energy Organ) that is supposed to create the energy needed for a Mega Evolution but it's not developed enough in most Pokemon. Rayquaza has the most developed Energy Organ leading to not needing a Megastone and instead using the energy it gets from consuming meteors. The Energy Organ is strong enough in every Pokemon to use Z-moves though.

Megastones, Z-Crystals and the Eviolite are made out of very similar material that is able to react with the Energy Organ to create energy. The Eviolite only works for unevolved Pokemon and greatly increases their defenses without the need of a Key stone or Z-Ring. Both the Key stone and the Z-Ring focus the energy created by the Energy Organ inside the Pokemon Trainer wielding it to synchronize with the Megastone/Z-Crystal of the Pokemon.

While it's unknown how all three of these rocks are initially created, there are multiple theories.

Megastone, Z-Crystals, Key stones, Sparklingstones and Eviolites were created by Arceus and other Legendary Pokemon together with the universe.

Megastones and Eviolites slowly grow inside of a Pokemon's Energy Organ, which can be removed from it but will prevent that one Pokemon from using any of the stones due to the Energy Organ being damaged and unable to produce energy even when fully healed. There have been reports of Pokemon like Audino having their Megastone inside the body revealed through X-Rays or found when cut open for meat like Magikarp and Gyarados. Only very, very rarely have there been records of wild megaevolved Pokemon and those records are many hundreds of years old, some preceeding the discovery of Mega and Key stones.

Z-Stones, Megastones and Eviolite grow in areas where a large amount of a certain Pokemon meet for long periods of time. Eviolites with non-fully evolved Pokemon, Z-Stones with Pokemon of a certain type, Megastones with one certain Pokemon and Pokemon-specific Z-Stones with that Pokemon knowing the corresponding move.

Whatever way the stones might be created, only humans are able to unlock the full potential of them by giving them the shape they are known for today and working together with Pokemon. The shape each of the stones has has been perfected over hundreds of years to awake a Pokemon's fullest potential.
