I am screeching over the Starfleet Academy news. This series has been in rumors since I was in university, I swear. Yesterday it was finally ordered for Discovery+. I am an utter nerd so pardon me while I celebrate by shaking my fandoms together like a champagne bottle and seeing what comes out.
Billy always wanted to go to the stars. After his parents divorced and his mom moved off planet she would send him holocards and vids from distant planets where she helped stabilize and revitalize federation planets effected by war and natural disasters. Whenever he needed to get away from Neil, Billy would drive up from San Diego just to park outside the Starfleet campus and pretend for a little bit that he was one of those cadets, on his way to discover the universe and the best that life had to offer. But eventually he always has to turn back around and head home to the cramped apartment he shares with his dad and his new wife. Billy has been preparing his whole life for the star fleet entrance exam with an eye on several science and engineering degrees, and the year he turns sixteen he takes the exam and passes on the first try.
He’s on the young side for a cadet, and he expects to be placed in a room with another protege type but he gets Steve instead, the infamous son of two celebrated Starfleet officers who is returning from a year of probation and spends all his time mooning over his ex-girlfriend and rescuing stray creatures. It’s obvious to Billy that Steve is only here because his father is an admiral, and worse - Steve knows it too. He knows he can fall asleep in class and forget his portion of the group assignment because the commanders will bend over backwards to make special accommodations for him. Everyone else knows that they have to receive at least an ‘ME’ (Meets Expectations) in all of their classes the first year or risk probation, and once you’re on probation the academy disciplinary council can decide to send you home whenever they feel like it. If Steve were anyone else, Billy knows he would have been expelled ages ago.
Billy’s double majoring as well as completing premedical studies and he somehow manages to get a BE in his command elective Diplomacy: Hostile Species. It’s so stupid because that class is basically a joke and the instructor has no sense of humor. Billy wouldn’t actually tell a hostile alien to eat his ass.
His track advisor offers to speak to the instructor for him about raising his term score with additional credits - the catch, his advisor is Dr. Harrington, Steve’s mother. And the assignment she has in mind is tutoring Steve. Billy has to make sure that Harrington receives an ME in all of his classes this term or risk academic probation - shameless blackmail! But Dr. Harrington seems to believe he can do it, and it’s not like Billy can complain to anyone. So Billy spends spring term up Steve’s ass, figuratively as he tries to cure his depression and glues him to chairs until he finishes his assignments. And then also, quite literally as they get closer and he figures out that Steve is just fucking lonely, and can be easily motivated with praise and touch.
 It’s like having a big bitchy cat who keeps coming home knocked up (their dormitory is filling up with one eyed cats and three legged dogs, and whatever that grey thing is with the fucking teeth) but it works. They work. They bitch, and they scrap, but they also laugh more and they’re able to say the real shit they can’t tell anyone else. They grow. They find their rhythm and they find themselves, realizing their value is not in what they achieve. Steve passes the term and shocks his father by switching from the command school to the Division of Arts & Sciences to study Sociology &  Zoology. He eventually learns from his new classmate Dustin that his sometimes cat eating pet alien (Doug) is called a demodog and hails from a hostile planet in a distant star system. No one knows how he ended up on earth but Steve dreams about one day helping return him to his family.
Billy gets his extra credit and eventually graduates with a boyfriend, a bunch of new pets, and for the first time, he doesn’t have to look up at the stars to find a reason to live. All he has to do is turn over in bed and all the stars are right there in Steve’s eyes.
Post school, Billy has years of field residency ahead of him and so does Steve and the chances that they’ll be placed on the same planet let alone in the same area is slim, but exceptions are made for married individuals or whatever the cultural equivalent of their species. They’re both committed to making the distance work and Billy could always call in a favor with Dr. Harrington (she loves him and would pull strings to get him stationed wherever he asked) but his thoughts keep straying back to the idea of marriage. He knows 20 is young to partner, but he can’t think of a more perfect life than exploring the final frontier with Steve by his side.

So, uh... Three and a half months working as a doctor. This time last year I was out of my mind stressing about my general surgery state exam and now I’m trying to survive intensive care. It’s a lot of fun, but also a lot of stress. Thankfully not too serious for me, yet. Like when I brought a patient to the ICU after surgery and their labs came back looking horrific (acidosis, high pCO2, ions all over the place) and I literally ran to a senior doc thinking the poor patient was dying and it was probably me who killed them during surgery, but no... I just had to hyperventilate them for a bit. Or when I told a nurse at a different ward to premedicate the patient with Dormicum (midazolam) and they told me that they were out of Dormicum and whether they could use midazolam instead and I was like... what? Sure, it’s the same thing... And then I saw them walking with a syringe full of unknown liquid and heard them talking about having to put an iv in for that patient so, once again, I ran across the entire ward to make absolutely sure that they wouldn’t just give 7,5mg of midazolam to an unmonitored patient intravenously. Thankfully it turned out to be a syrup 😅 and they also told me that of course they wouldn’t do that.
I haven’t done anything incredibly stupid yet, but I have a feeling my time will come. I can’t get cocky. To be fair, I got comfortable in the OR after spending two months there, I was okay with spinals, epidurals, GA, whatever... But then the head consultant started putting me to the ward more often so my coresidents would be in the ORs and today when I was supposed to do an epidural I had a pulse of 150 at least. I questioned every step, questioned every proof that the epi catheter was in the right position and then probably didn’t wait long enough and when the surgeons wanted to start prepping for the surgery and the patient still felt pain, I almost had a heart attack. Thankfully the anesthesia worked in the end, but oh my god 😅 Epidurals are always extremely nerve-wrecking for me.
Anyway, I just wanted to give a little update. This job is a lot, but I also enjoy it a lot. The anesthesia part also involves doing cardioversions and cathlab anesthesia and the ICU involves some pretty hard cases that I still need to study a lot for. If you have questions, here’s my ask box. Though given the nature and place of my job, I don’t think that many people can relate 😁

Mental Woes before Medical School
Applying to medical school, like the process of becoming a doctor, is arduous and often stressful. In a survey of more than 550 medical school applicants, spanning 180 colleges and universities in the United States and Canada, UC San Diego School of Medicine researchers say the Covid-19 pandemic has only exacerbated the situation and, in fact, revealed that significant numbers of applicants report symptoms of major depressive disorder (MDD) or generalized anxiety disorder (GAD).
The findings, published September 30, 2021 in the journal The Primary Care Companion for CNS Disorders, indicated that 31 percent of medical school applicants reported MDD symptoms and 70 percent reported GAD symptoms. Just under 30 percent reported symptoms of both disorders. MDD is a mood disorder that causes a persistent feeling of sadness and loss of interest. GAD is characterized by excessive, ongoing anxiety and worry that are difficult to control and interfere with day-to-day activities.
In both cases, the reported percentages were significantly higher than previous mental health surveys on the topic. Notably, 11 percent of the responding applicants said they had experienced suicidal symptoms on several days and 5 percent reported them more than half or nearly every day of the week.
The findings suggest worsening mental health among medical school applicants. For example, a 2010 study by UC San Diego researchers reported an MDD prevalence of 14.7 percent among surveyed premedical students, less than half of the latest data. In the general population, the prevalence of MDD is estimated at 6.7 percent among persons 18 years and older.
For generalized anxiety, a 2018 survey of undergraduate students reported roughly 30 percent said they had experienced overwhelming anxiety within the past two weeks and 19 percent over the past year. In the U.S., GAD affects 3.1 percent of the population.

“Applying to U.S. medical schools is highly competitive,” said Kamdar. “The pandemic introduced new stressors, such as exam delays and lack of campus access, which may have unearthed or exacerbated mental health symptoms or impacted application decisions.”
Kamdar said the findings highlight a population of highly motivated pre-medical students who may be “suffering in silence” and whose mental health impairments may have short- and/or long-term implications. He said a potential remedy might be for undergraduate counseling offices to adopt a more personalized approach to premedical advising, acknowledging the prevalence of mental health impairments and providing tools for coping.
Kamdar also noted advice from the Association of American Medical Colleges and many universities that advocate “gap years” between undergraduate studies and medical school. That may be good guidance for many applicants, he said, but the approach can also introduce unnecessary pressure and doubt for applicants who intended to enter medical school without interruption for a variety of reasons, including financial.
“A better understanding of these issues, including early identification of at-risk individuals, may help guide interventions to address mental health impairments early and prevent qualified applicants from delaying medical school or abandoning their plans altogether,” Kamdar said.

Was wondering if you could give me any tips on how to make my cat less of a gremlin for the vet! She’s sweet with being picked up and held by me but I can’t tell you how many times I’ve said sorry to the vet because she’s a turd for medical exams etc (even flea treatment - but worming somehow is okay?) - shes about 5 if it’s relevant
Hey! I promise you aren’t alone in having a cat that doesn’t like the vets. Most cats don’t, it’s uncommon to have a cat come in and not be stressed to some degree.
Even I have apologised to my colleagues when I brought my cats in. If they’re scared or reactive there isn’t too much you can do except prewarn the vet, reduce the time spent out of the house/out of her travel cage, and assist in holding her for the vet’s examination. Bring a towel and spray the towel as well as her travel cage in products such as Feliway which contain pheromones that may calm some cats. 
Personally, I appreciate when owners are willing to hold their cat and even scruff them briefly when required. It helps the consult go much quicker than if I am trying to protect myself, focus on their vitals, and stop them from jumping off the table all at once. 
If she is starting to get quite difficult, one thing you can chat to your vet about is premedicating her prior to appointments. There are a few medications available that can help her cope better with the stress and be more amendable to examination and procedures.

Why is your blog called the 'Lazy Yogi'? Are you lazy?
What the name “Lazy Yogi” means to me has evolved over the years. 
When I first started this blog, I was 22 years old and still in college. My father had died the year before I went to college and I was wondering what it all meant–Life, the Universe, and Everything. This eventually led me to the practice of meditation and the spiritual path. 
At that time, I was still getting the lay of the land. I would see other spiritual people traveling to exotic places all over the world in order to have unique experiences. I saw people going on extended retreats and getting yoga certifications. And I saw people going to the Amazon to take ayahuasca or shrooming in the desert. 
As I started to find my own path and practice, I realized that all of that wasn’t strictly necessary. It can be fun and it can be what we need at that time but really all you need is a little space and a little time to mediate. You need receptivity but also discipline. You need to be able to relax and smile but also open your heart and be present. 
And so I felt I was a lazy yogi. 
I graduated college in 2011 and then I was living at home without any prospects for career or direction in life. All my friends had moved on to high-powered jobs living in New York City. My girlfriend at that time had also broken up with me and she too was getting her life started in NYC. I felt lonely, confused, and aimless. 
But my spiritual practice kept me oriented. At this time, I was reflecting a lot on society and my place in it. I realized how society only really “sees” you if you add some sort of value to it. In the US, that value is mostly defined as productivity. Yet here I was, leeching off my family and just being a lazy ass. 
This was actually one of the most valuable periods of my life. A quote by Gangaji sums it up well, “To be truly happy you must recognize who you are with nothing.”
At that time, I was doing an unpaid internship in the movie industry, meditating an hour a day, and veritably consuming every book on spirituality that I could find. My suffering was immense but it was the perfect amount of suffering. It was strong enough to push me inward and yet not so strong that I gave into despair and hopelessness. 
Between the books The Power of Now by Eckhart Tolle, The Places That Scare You by Pema Chodron, and some other goodies, as well as the grace and guidance of my guru and my meditation practice, I eventually found a droplet of peace amidst all that confusion. 
Then the name the “Lazy Yogi” took on a more ironic meaning. We are not here because we need to be productive–although productivity in society is one particularly reliable way to survive. Put simply, we are here because the Earth allowed it–or more poetically, because the Earth wanted us here.
“When we walk like we are rushing, we print anxiety and sorrow on the earth. We have to walk in a way that we only print peace and serenity on the earth. Be aware of the contact between your feet and the earth. Walk as if you are kissing the earth with your feet.” ~ Thich Nhat Hanh
This was the new meaning of laziness to me. I didn’t need a job to know myself, I didn’t need a social scene in order to find a sense of interconnectivity, and I didn’t need a future or a past to orient my present. Just like someone lazing about on the beach. That was my way of contributing, by not printing my own fear and confusion onto the Earth around me.
As some of you might know, there came a point in which I reconsidered my life and the direction I wanted to take. I then decided to go back to school to become a doctor. It’s funny to write about that now because I will be graduating medical school this coming May. It has been a long journey! I made that decision 7 years ago. And this journey is really only just beginning. 
Throughout the hardships and insecurities of premedical education and then medical education, the “Lazy Yogi” took on another meaning. I was busy. Busier than I had ever been at any point in my life. And as a result, I learned my limits and how to gently–or sometimes not so gently–grow beyond those limits. 
At the same time, I continued my meditation practice and my spiritual way. The “Lazy” took on an inner meaning. That no matter how busy we might be outwardly in the world, it is important–essential, even–to remain inwardly at ease. I don’t always manage this but I do my best. For example, I have a nine hour board exam this Thursday. I have been waiting to hear back about interviews for residencies, which will decide where I live for the next 5 years. And I’ve been struggling a bit in my personal life as well. 
So while that inner “Lazy” might not feel so relaxed right now, I know not to give up and leave it behind. I know “this too shall pass” and I just do what I can. 
I wish I could write on this blog more, that I had more time and energy and inspiration for it. I know that so long as Tumblr exists, I will keep doing so as often as I can. I really love sharing meditation and spirituality with everyone, I love the people I get to meet and speak to on here, and the new influences I come across along the way. We’ll see how the future plays out! But somewhere, I’ll always be trying to share this stuff because I can’t think of anything more important or relevant to the human condition.
