Nepal Aircraft Accident Investigation Commission: Yeti Airlines 691 (YT691) ATR72-500 9N-ANC - Final Report
On Mar 28th 2024 Nepal's Aircraft Accident Investigation Commission (AAIC) released their final report via the French BEA concluding the probable causes of the crash were:
The most probable cause of the accident is determined to be the inadvertent movement of both condition levers to the feathered position in flight, which resulted in feathering of both propellers and subsequent loss of thrust, leading to an aerodynamic stall and collision with terrain.
The contributing factors to the accident are:
- High workload due to operating into a new airport with surrounding terrain and the crew missing the associated flight deck and engine indications that both propellers had been feathered.
- Human factor issues such as high workload and stress that appears to have resulted in the misidentification and selection of the propellers to the feathered position.
- The proximity of terrain requiring a tight circuit to land on runway
- This tight circuit was not the usual visual circuit pattern and contributed to the high workload. This tight pattern also meant that the approach did not meet the stabilised visual approach criteria.
- Use of visual approach circuit for RWY 12 without any evaluation, validation and resolution of its threats which were highlighted by the SRM team of CAAN and advices proposed in flight procedures design report conducted by the consultant and without the development and approval of the chart by the operator and regulator respectively.
- Lack of appropriate technical and skill based training (including simulator) to the crew and proper classroom briefings (for that flight) for the safe operation of flight at new airport for visual approach to runway 12.
- Non-compliance with SOPs, ineffective CRM and lack of sterile cockpit discipline

Final Report - Interstate Aviation Committee (MAK)
The cause of the An-26B-100 RA-26085 aircraft crash was the crew's violation of the established instrument approach procedure to Palana aerodrome, which was manifested in flying with significant deviation from the set route and descent well below the established minimum descent height (MDH) under weather conditions that excluded stable visual contact with ground landmarks, leading to the collision of the aircraft with a coastal cliff in controlled flight, its destruction, and the death of the crew and passengers.
Contributing factors to the accident may have included:
- The crew's failure to execute a missed approach with the acquisition of the established minimum safety altitude (MSA) when information about the bearing indicated a significant deviation of the aircraft from the established approach procedure;
- The absence in the Palana aerodrome dispatcher's work technology of actions in the presence of information about the bearing indicating a significant deviation of the aircraft from the established approach scheme, as well as the dispatcher's passivity when such information was available;
- The lack of warning signals from the early ground proximity warning system under conditions that should have triggered it. It is not possible to determine the reason for the absence of the warning signals;
- The overestimation of the barometric altimeter readings in the final phase of the flight due to the specific airflow around the steep coastline creating a low-pressure zone and the overestimation of the variometer readings, the cause of which cannot be determined.
NOTE: This investigation report does not meet international language standards stipulated in ICAO Annex 13 - Accident and Incident Investigation. The report was published in Russian only, which required translation by 3rd parties. Due to the translation issue, the accuracy of specific details in this report cannot be guaranteed.

Aviation is so expensive, the only billionaires that don't hate spending money on it are aviators who fly and maintain their own planes. Anybody else who pays for a 13min flight in a jet, whether they own it or not, probably has a good reason for it. Jet A-1 isn't cheap, and neither are the pilots and hourly maintenance...
...Oh, and there's those pesky landing fee's, parking fee's, airport improvement fee's, ground handling fee's, air navigation service charges, and the Feds will want their cut as well.

The goat's fate has largely been accepted by the people of Gävle, who were amused to see the birds eat the straw, even before they were finished setting it up. The general consensus amongst the town was to leave the goat alone and let nature take it's course.
So once again, tradition has been vindicated. May the Gävle Goat rest in peace, and may the odds of 2024 be ever in our favour.

Can't stress this enough. I come from a family that has meticulously documented and archived our family history. One thing I've noticed, as they get older, my family will look at a picture and have a completely different recollection than the 100's of times telling the same story behind it or pointing out the same people in the picture over the course of 30+ years. Sometimes it takes a few, now elderly, family members to kind piece together information about 1 picture, that wasn't properly documented, that they all could 30+ years ago.
Basically it comes down to this: If it isn't documented, information will be lost, forgotten, or get jumbled up.

No. You won’t. Write down who is in that photo. Note the date and event. Put a year on everything.

1) WRONG. Years get murky, relationships fade, people change, family members look alike (especially when very old or very young)
2) even if you somehow do, the next generation has no idea who what where when or why.
