I’m tempted to start going by the name my mother almost gave me because the story behind it is too funny. The tl;dr is that when pronounced a little differently/the emphasis is changed, the name sounds exactly like a slur that means gay (as in homosexual) in Arabic.

My mother explained to me, as I stared at her in disbelief and irony, that the reason she finally decided against it was that she didn’t want people calling her daughter gay. Talk about tempting fate lmao

Shaza is a 25 year old male Skeleton currently working as a Psychologist.

This anxious bean stands at a whoppin’ 6′7.

His cracks are the result of one of his clients attacking him mid session due to unintentionally triggering them. Despite still recovering from the traumatic situation, he is still working relentlessly in his field. Likes: Tea, cats, succulents, instruments (Especially the piano), and praise. Dislikes: Humid/Hot weather, most sweets, and being touched without permission. Fears: Dogs, the dark, small spaces, and deep bodies of water. Personality: Anxious, curious, mannerly, has a tendency to get absorbed in things.

Some Trivia  1. Depending on Shaza's emotions, his eyes and mouth may become 'drippy'  2. Under extreme stress, Shaza tends to fade out, but he can't turn completely invisible.  3. Shaza favors his left (connected) hand, but he's ambidextrous.  4. Despite his insecurities he works well with other's issues; just not his own. 5. Can get overly enthusiastic/absorbed in things he enjoys or finds interesting. (This could be something as silly as investigating his friends to see who’s capable of licking their elbows or not.) 6. Shaza’s rib cage is completely fused.

Finding More about Elagabalus/Illah Al-Jabal 1

So Elagabalus or Illah Al-Jabal is a God I am very interested in so I figured I would share what I find on here. (I usually call him Illah Al-Jabal, which is the Arabic translation of the name, cuz I know more Arabic than I do Greek so I find that more easier personally) This is also my first type of this kind of post, so please forgive me if it’s all over the place

Molefi Kete Asante and Shaza Ismail, Interrogating the African Roman Emperor Caracalla: Claiming and Reclaiming an African Leader:

“Severus's wife, Julia Domna, came from a famous city in the ancient world called Emesa. She was well regarded when married to Septimius Severus due to the temples from her home which were dedicated to the God Elagabalus (the Sun God), represented by a black stone which is said to have fallen from the sky as a meteorite (Haynes, 1993). When Septimius Severus died in 211 a.d. while on campaign in Britain, he left the Roman treasury with a secure surplus and the military at a height of loyalty to the emperor that it had not seen in many years. Unlike his father, unfortunately, Caracalla was just not likable (Boatwright et al., 2004).”

This was interesting mainly in that it was said that Illah Al-Jabal was represented by a black stone that was a meteorite. I am sure for everyone a certain black stone came to mind, which was also said to be dedicated to a different God in the area. But with this I wanted to look up what this source was so I decided to find this Haynes source. And well....... it wasn’t exacty what I was looking for but it was still a great treasure.

“Emesa was famous in the ancient world for its magnificent temple to the god Elah-Gebel (Elagabalus). The city was ruled by the Sohemais family of priest-kings for many generations and it was the daughter of one of these, Julia Domna, that the ambitious Septimius Severus married. It was not until the reign of the emperor Heliogabalus (June A.D. 218-March A.D. 222) that the city's god became widely known in the Roman empire. Dedications from Intercisa indicate that the god was venerated by the Hemeseni based in the fort. A number can be dated reasonably accurately. The most interesting is a unit dedication datable to the 23 August A.D. 214.71 Even if it is accepted that the regiment could have been receiving Semitic recruits as late as A.D. 183, as Kennedy argues, such soldiers would have retired at least six years before this inscription was erected. The inscription cannot be explained as the result of imperial dictates as Domaszewski argued, because Heliogabalus was still four years away from ascending the throne. Its date shows that even after the ethnic composition of the regiment may have changed the cult was sufficiently popular to attract such dedications. The naming of Elagabalus on two other datable inscriptions, both recording work on a temple,73 reinforces the impression of strong links between the regiment and the god. Indeed, it is quite likely that cohors I Hemesenorum was responsible for the initial construction of the temple at Gorsium and that later soldiers simply followed the practices of their predecessors. Three further undated inscriptions from the area attest to the worship of the god, but, with the exception of one which records a Semitic soldier from an unknown unit, they are of little interest. The veneration of other gods was less common among the followers of Elagabalus than those of other deities. It is not therefore surprising that Dea Syria, for example, does not receive the attention at Intercisa that she does elsewhere. The area around Intercisa has produced a number of altars and dedications to gods other than Elagabalus. Significantly almost all of these are to gods of the Roman pantheon or the Imperial cult. The only possible exceptions to this are dedications by an eques (AE 1956, 16) and a veteranus (Alba Regia ii (1970), No. 463) to a probably localised form of Jupiter, IOM Culminari. The inscriptions left by cohors I Hemesenorum indicate that where a wide range of datable epigraphic material is available it is possible to show that beliefs originating in regimental homelands could survive even after a unit's ethnic composition had changed.”

While I was unable to find any information about the black stone mentioned, I found a lot more information, maybe even more interesting! Even when this auxiliary unit that it is mentioning no longer has Syrian or Semitic recruits, the worship of Illah Al-Jabal stayed with the auxiliaries, even to the exception of other Gods. While I am certainly just starting with my research with this, this definitely tells me that there is more to Illah Al-Jabal than can be seen at first glance

"Listen, I get it. I ain’t considered “the underdog”. I’m a light skinned, skinny, middle class raised, opinionated, smart, smartass, black woman with long hair. So, no matter the merit of wut I say I’m the PERFECT person for mean minded folks 2 shit on without shame. LOL #goawf," she tweeted.

She followed up with tweeting:

That tweet must have struck a nerve in singer-songwriter Frenchie Davis' body because she hopped on Facebook to blast Amanda for her "skinny, light skinned mean girl superiority complex." The former "American Idol" contestant said she's had enough of Amanda projecting "the sh*t" on people and would love for her to just SHUT UP.

“I’m probably gon delete this later but I’m getting my period soon and this is exhausting me. This sister SO CLEARLY has a skinned light skinned mean girl superiority complex… and works super hard at projecting the sh*t on to other people. It’s exhausting. Like, sis…we all love Tracy Ellis Ross…Michaela Angela Davis…Zendaya…Amandla Stenberg…and countless other outspoken opinionated thing light skinned educated women," she wrote.

Horner Syndrome Secondary to Central Venous Catheter by Shaza Aouthmany MD and Alexandra Lu MD in Open Access Journal of Biogeneric Science and Research

Abstract

64 years old female admitted to the hospital for GI bleed, developed hypotension and required a central venous catheter. After 4 unsuccessful attempts of placing a central line, the patient immediately developed ptosis of the right eye, with associated anisocoria. Patient as transferred to the ICU due to hemodynamic instability and for further work up. Neurology was consulted and CT brain non contrast as well as CTA carotids were ordered. CT brain was unremarkable and CTA carotids showed all neck vasculature to be widely patent with no hematomas or other acute abnormalities noted on the imaging. A diagnosis of horner syndrome secondary to right IJ central venous access was made.

Background

Horner Syndrome secondary to central venous catheter (CVC) is a relatively uncommon complication of the procedure, however it is a complication that should be considered when placing CVC. Care should be taken during the procedure to avoid injury to the carotid sheath. If injury is suspected, further workup is needed to determine severity. Horner's syndrome associated with CVC of the IJ is caused by the damage to the nerve plexus that lies adjacent to the carotid artery sheath.

Case Presentation

A 64-year-old female, with past medical history of Multiple Sclerosis, presented to an emergency department for hematemesis and weakness. Upon presentation to the emergency department, the patient had one episode of hematemesis as well as a positive stool guiac. Patient's initial labs showed a stable hemoglobin of 9.0, normal coagulation profile and a CT abdomen that showed nonspecific wall thickening of the gastroduodenal junction. The patient remained hemodynamically stable within the emergency department, received intravenous (IV) fluids and IV pepcid and was admitted to the hospital for further workup of the gastrointestinal (GI) bleed.

While in the hospital, the patient had a bloody bowel movement which caused her to become hypotensive with systolic pressure in the 50s and was minimally responsive. The decision was made to place a central line for volume resuscitation/ possible pressor support. A right IJ triple lumen CVC was attempted using ultrasound to guide venous access. 4 attempts to thread the guidewire in the internal jugular (IJ) were unsuccessful and the procedure was terminated. The patient states that immediately following the failed IJ central line procedure, she started experiencing ptosis of the right eye, with associated anisocoria (minimal--1 mm difference between the eyes).

Following the failed attempt at right IJ CVC, the decision was made to transfer the patient to a hospital with higher capabilities due to her hemodynamic instability. Upon presentation to the hospital, the patient was transferred to the ICU. She was evaluated by neurology and received CT brain without contrast as well as CTA of the carotids. CT brain was unremarkable and CTA carotids showed all neck vasculature to be widely patent with no hematomas or other acute abnormalities noted on the imaging. MS exacerbation was considered as a source of the patient's presentation but the timing of the symptoms in relation to the procedure made this an unlikely cause of the patient's pathology. A diagnosis of horner syndrome secondary to right IJ central venous access was made.

Discussion and Conclusion

Horner's syndrome is a classic neurological syndrome characterized by ipsilateral blepharoptosis, pupillary miosis, and facial anhidrosis [1]. It should be noted that the diagnosis of Horner's syndrome can be made without the existence of all three diagnostic criteria. Depending on where the level of injury to the nerves occurs, the presentation of Horner syndrome will differ. The sympathetic nerves that innervate the areas involved in Horner syndrome arise in the hypothalamus, run down the cervical spine, along the brachial plexus, around the internal carotid sheath and finally run-in close relation to the respective cranial nerves [2]. The lengthy course of the sympathetic nerve fibers makes them particularly susceptible to injury.

In the instance of CVC in the IJ, injury to nerve fibers occurs in the post-ganglionic region. The post-ganglionic region contains nerves responsible for innervating the pupil and palpebral muscles. Injury in this area will present with anisocoria and ptosis [3]. Injury at the level of the postganglionic nerves is marked by lack of facial anhidrosis. The nerves responsible for facial sweating arise earlier in the pathway and are relatively sparse at the level of the carotid sheath around the IJ [3].

Horner's syndrome is an uncommon complication of central venous catheter insertion (CVC) occurring in around 2% of CVCs [4]. Horner's syndrome associated with CVC of the IJ is caused by the damage to the nerve plexus that lies adjacent to the carotid artery sheath [4]. Because of the nerve’s location in the carotid sheath, they are prone to damage if trauma to the area surrounding the carotid artery occurs. In the instance of IJ CVC, this damage occurs via expanding hematoma or direct injury by misguided CVC insertion (particularly if there are multiple attempts at insertion-- as was the case with this patient) [5]. If Horner syndrome secondary to CVC insertion is suspected, workup should be performed to ensure the carotid artery has not been critically damaged. If this is the case, further intervention may be required. Without significant carotid injury, Horner syndrome related to CVC can be expected to improve or resolve over the following weeks/months.

Learning Points

Horner’s syndrome secondary to central venous catheter is rare.

Further workup is critical if Horner’s syndrome secondary to CVC develops to ensure carotid artery is not damaged.

Without significant injury to the carotid artery, Horner syndrome secondary to CVC, is expected to improve or resolve within weeks to months.

Ultrasound guidance can be a valuable tool to assist the clinician in preventing unnecessary trauma to the carotid artery precipitating central venous catheter related Horner syndrome.
