For pts who smoke more than 10 cigarettes per day, this is the dosing for the nicotine patch: 21 mg qd x6 weeks, then 14 mg patch qd x2 weeks, then 7 mg patch qd x2 weeks.
For pts who smoke 6-10 cigarettes/day, the nicotine patch dosing is 14 mg qd x6 weeks, then 7 mg x2 weeks.

Proper way to use nicotine patches/lozenges:
This provider recommends a prescription of 21mg nicotine patches (Nicoderm CQ), once a day, for 4 weeks, for tobacco cessation. Pt stated understanding that the patches need to be replaced on upon awakening, the application sites need to be rotated daily, and they can be taken off when going to bed if it disrupts sleep. This provider recommends keeping the patch on overnight due to pt smoking within 5 minutes of waking.
This provider recommends a prescription of 2mg nicorette lozenges for tobacco cessation. Lozenges are recommended once an hour if needed, up to 24 in 24 hours, for 6 weeks. Lozenges can be used for up to 14 weeks if needed.
Pt stated understanding that the lozenges need to be sucked until mint or pepper is tasted. Pt understands that the lozenges need to be "parked" between cheek and gum line for absorption, repeat once the taste is gone, then cont. sucking and parking for about 30 minutes.
