Only a few months old at the time, shiny GO Transit F40PH 511 heads up a westbound 9-car train of new bilevels and an APCU on the tail end, passing by Spadina Avenue just west of Toronto's Union Station. GO 511 was part of a six-unit order of F40PH units built by GMD at their London ON plant in April-May 1978, a change from GO's previous orders of freight GP40-2W units. New F59PH deliveries a decade later would result in GO flipping the six orphan units to Amtrak in 1990. 511 would see another decade of service on Amtrak as their 411, and would go on to work AMT's commuter operations out of Montreal. It last saw service on the the Saratoga & North Creek Railway (via LTEX) with another ex-Amtrak F40. The bilevel cars trailing are also new, part of the first order built by Hawker Siddeley and in service for a little over a year since introduction in March 1977. Since the first bilevel order included no cab cars, another locomotive, APCU, or old Hawker Siddeley single-level cab car had to be used on the other end in the early years. This scene off Spadina Avenue is notable in that it's under transition: the old CN steam-era freight platforms, team tracks and yard trackage east of Bathurst Street yard are still present, but truncated for the new CN Tower's parking lot (note the yellow bridge crossing the rail corridor near TTR's John St. interlocking tower). In later years the RBC's new data center (325 Front St. W), the Metro Toronto Convention Centre and CN's L'Hotel would fill much of the vacant land along Front Street in the 80's. The usual downtown office towers of the day including the TD Centre, Royal Bank Plaza, CP Hotel's Royal York, and the CNCP Telecommunications Building (151 Front Street) are quite unobscured compared to today. Another study has documented how opioid prescribing guidelines over the past decade have led to fewer opioid prescriptions for cancer patients --- even though the treatment guidelines typically state they are not intended for patients suffering from cancer pain.

In findings recently published in PLOS ONE, researchers at Johns Hopkins Bloomberg School of Public Health and the University of Michigan analyzed pharmacy claims for up to 53 million privately insured U.S. adults from 2012 to 2019. During that period, opioid prescribing fell from 49.7% to 30.5% for people with chronic non-cancer pain (CNCP) and from 86% to 78.7% for people with cancer.

While the drop in prescribing appears minor for cancer patients, a closer look at the numbers shows a significant decline in high dose prescriptions and in the number of days opioids were prescribed.

In 2012, for example, the average daily dose for a cancer patient was 62.4 morphine milligram equivalents (MME). By 2019, that had fallen to 44.7 MME – a decline of over 28 percent.

The percentage of cancer patients getting over 90 MME, a ceiling recommended by the CDC for chronic non-cancer pain, fell from 26.2% to just 7.6% -- a 71 percent decrease.

The average number of days per year a cancer patient received opioids also fell, from 34.8 days to 22.3 days – a 36 percent decline. (Read more at link)

Direção-Executiva aprova serviços integrados de cuidados paliativos no SNS

A Direção-Executiva do Serviço Nacional de Saúde (DE-SNS) aprovou a criação de serviços integrados de cuidados paliativos nas unidades do SNS e o plano de ação para garantir o acesso a estes cuidados.

“A DE-SNS aprovou a criação de Serviços Integrados de Cuidados Paliativos, que resultou de uma proposta da Comissão Nacional de Cuidados Paliativos (CNCP), que foi alvo de ampla discussão pública, finda a qual se integrou muitas das sugestões no documento”, lê-se no comunicado da estrutura executiva do SNS hoje divulgado.

Segundo a DE-SNS, a proposta “servirá de guia para a concretização da Rede Nacional de Cuidados Paliativos (RNCP) a nível das ULS [Unidades Locais de Saúde] e em todo o SNS”.

Segundo o organismo, foi também aprovado o Plano Estratégico para o Desenvolvimento dos Cuidados Paliativos em Portugal Continental 2023-2024, que contém orientações e o plano de ação para garantir o acesso a cuidados paliativos no SNS.

“Na área dos Cuidados Paliativos Pediátricos foram aprovados os Critérios de Referenciação para Cuidados Paliativos Pediátricos que vêm clarificar e facilitar o acesso aos utentes pediátricos e suas famílias”, adianta ainda o comunicado.

A DE-SNS, que integra na sua estrutura a CNCP, autorizou ainda a criação de um grupo de trabalho “para desenvolver o sistema de informação utilizado pelas equipas de cuidados paliativos” e a constituição de uma equipa técnica de suporte à CNCP.

“O acesso a cuidados paliativos de qualidade em todos os níveis de cuidados do SNS, sem esquecer o necessário desenvolvimento dos cuidados domiciliários, continuará a ser uma das nossas prioridades”, refere a DE-SNS.
