Because my most popular post is about weight loss and how it's a crock, I get a lot of questions about various things, including bariatric surgery--just posted the link to the post I did about that--but also Ozempic/Wegovy, the once-weekly injectable semaglutide medication that was developed for diabetes but was found to have independent benefits on weight loss.
I always said that weight loss was like Viagra: when a medication came along that actually worked, it would explode. We'd all hear about it. Fen-phen in the 90s worked, but it was bad for your heart. Stimulants, like meth, may cause weight loss, but they do it at the cost of heart health, and raise your likelihood of dying young. Over the counter weight loss supplements often contain illegal and unlisted thyroid hormone, which is also dangerous for the heart if taken in the absence of a real deficiency. Orlistat, or "Alli," works the same way as the Olestra chips Lays made in the 1990s--it shuts off your ability to digest fats, and the problem with that is that fats irritate the gut, so then you end up with fatty diarrhea and probably sharts. Plus Alli only leads to 8-10lbs of weight loss in the best case scenario, and most people are not willing to endure sharts for the sake of 8lbs.
And then came the GLP-1 agonists. GLP stands for glucagon-like peptide. Your body uses insulin to make cells uptake sugar. You can't just have free-floating sugar and use it, it has to go into the cells to be used. So if your body sucks at moving sugar into the cells, you end up with a bunch of glucose hanging out in places where it shouldn't be, depositing on small vessels, damaging nerves and your retinas and kidneys and everywhere else that has a whole lot of sensitive small blood vessels, like your brain.
Glucagon makes your liver break down stored sugars and release them. You can think of it as part of insulin's supporting cast. If your body needs sugar and you aren't eating it, you aren't going to die of hypoglycemia, unless you've got some rare genetic conditions--your liver is going to go, whoops, here you go! and cough it up.
But glucagon-like peptide doesn't act quite the same way. What glucagon-like peptide does is actually stimulating your body to release insulin. It inhibits glucagon secretion. It says, we're okay, we're full, we just ate, we don't need more glucagon right now.
This has been enough for many people to both improve blood sugar and cause weight loss. Some patients find they think about food less, which can be a blessing if you have an abnormally active hunger drive, or if you have or had an eating disorder.
However, every patient I've started on semaglutide in any form (Ozempic, Wegovy, or Rybelsus) has had nausea to start with, probably because it slows the rate of stomach emptying. And that nausea sometimes improves, and sometimes it doesn't. There's some reports out now of possible gastroparesis associated with it, which is where the stomach just stops contracting in a way that lets it empty normally into the small intestine. That may not sound like a big deal, but it's a lifelong ticket to abdominal pain and nausea and vomiting, and we are not good at treating it. We're talking Reglan, a sedating anti-nausea but pro-motility agent, which makes many of my patients too sleepy to function, or a gastric pacemaker, which is a relatively new surgery. You can also try a macrolide antibiotic, like erythromycin, but I have had almost no success in getting insurance to cover those and also they have their own significant side effects.
Rapid weight loss from any cause, whether illness, medication, or surgery, comes with problems. Your skin is not able to contract quickly. It probably will, over long periods of time, but "Ozempic face" and "Ozempic butt" are not what people who want to lose weight are looking for. Your vision of your ideal body does not include loose, excess skin.
The data are also pretty clear that you can't "kick start" weight loss with Ozempic and then maintain it with behavioral mechanisms. If you want to maintain the weight loss, you need to stay on the medication. A dose that is high enough to cause weight loss is significantly higher than the minimum dose where we see improvements in blood sugar, and with a higher dose comes higher risk of side effects.
I would wait on semaglutide. I would wait because it's been out for a couple of years now but with the current explosion in popularity we're going to see more nuanced data on side effects emerging. When you go from Phase III human trials to actual use in the world, you get thousands or millions more data points, and rare side effects that weren't seen in the small human trials become apparent. It's why I always say my favorite things for a drug to be are old, safe, and cheap.
I also suspect the oral form, Rybelsus, is going to get more popular and be refined in some way. It's currently prohibitively expensive--all of these are; we're talking 1200 or so bucks a month before insurance, and insurance coverage varies widely. I have patients who pay anything from zero to thirty to three hundred bucks a month for injectable semaglutide. I don't think I currently have anyone whose insurance covers Rybelsus who could also tolerate the nausea. My panel right now is about a thousand patients.
There are also other GLP-1 agonists. Victoza, a twice-daily injection, and Trulicity, and anything else that ends in "-aglutide". But those aren't as popular, despite being cheaper, and they aren't specifically approved for weight loss.
Mounjaro is a newer one, tirzepatide, that acts on two receptors rather than one. In addition to stimulating GLP-1 receptors, it also stimulates glucose-dependent insulinotropic polypeptide (GIP) receptors. It may work better; I'm not sure whether that's going to come with a concomitantly increased risk of side effects. It's still only approved for diabetes treatment, but I suspect that will change soon and I suspect we'll see a lot of cross-over in terms of using it to treat obesity.
I don't think these medications are going away. I also don't think they're right for everyone. They can reactivate medullary thyroid carcinoma; they can fuck up digestion; they may lead to decreased quality of life. So while there may be people who do well with them, it is okay if those people are not you. You do not owe being thin to anyone. You most certainly do not owe being thin to the extent that you should risk your health for it. Being thin makes navigating a deeply fat-hating world easier, in many ways, so I never blame anyone for wanting to be thin; I just want to emphasize that it is okay if you stay fat forever.

"Come on, it's not that bad!" You smiled, pushing at Spencer's shoulder as he took a sip of your coffee, face screwing in disgust. "You're such a baby."
"No," He disagreed, voice hoarse as he handed you your coffee back. "That is disgusting."
"Just because it doesn't have about 3 pounds of sugar," You taunted, looking down at his mug with disapproval. "All that sugars gonna' catch up to you one day you know."
"Actually," He started and you felt yourself beginning to groan. "White sugar is a type of carbohydrate that provides energy to the body so what happens is the body breaks down sugar into glucose that can be readily used for energy and to carry out various functions. It improves brain functioning—"
"Spence, it's eight in the morning," You said, resting your forehead on his arm. "As much as I care about you, you are not allowed to go wikipedia on me at this time."
He stifled a laugh, glancing down at you with a soft smile. He took no offense to what you were saying since he knew your favorite thing was to listen to him. You pulled away, taking a sip of your coffee. You looked up at him, opening your mouth to speak but being abruptly interrupted as the kitchen door burst open, causing you to jump and stain your white blouse with coffee.
You looked down at the shirt, hissing in complaint. "Are you fucking—"
You turned, face twisted with anger. Spencer shrank at your demeanor looking over at the door to see who was about to receive the latter end of your anger. He took a sip of his own coffee when he saw  Derek standing by the door. The anger in you began bubbling even closer against the surface. "Really Derek?!"
Just as you were about to scold him even more for being such a brute at eight am, you noticed the features in his face laced with an all too familiar tensity. Realization dawned upon you. He started "Looks like we've got a visitor,"
You watched him, glancing briefly back at Spencer before setting your mug down. "And they're asking for you."
The case currently at hand, although pinching a little too close to your past then comfortable, wasn’t overwhelming you. It made you fidgety and a little more anxious than usual, sure, but you weren't overwhelmed by it. All it did was irritate you, how you always tried so hard to keep your past out of your present, and the present away form the past.
All you wanted, was to find the UnSub and move on. Still, you sighed heavily, popping out of the nice bubble Spencer and you had isolated yourselves in. You left your coffee mug long forgotten as you followed Morgan out of the room and across the hall, Spencer following very closely behind.
"A girl who barged in here, asking for you," Derek explained. You continued your stride across the hall, wondering who could possibly be asking for you. "She said she doesn't wanna' answer any questions until she's spoken to you,"
"Does it have to do with our recent case?" You asked, knowing otherwise but still hoping the answer was no. When Derek's lips pulled into a tight line, you sighed in disappointment letting your shoulders slump. Just as you rounded the main hall, you saw a familiar mop of red hair ranting at Hotch in a blazing fury. You froze in your tracks, almost causing Spencer to knock into you. You watched as she pointed her finger at Hotch, stress, and anxiety written all across her face.
"Claire..?" You said, voice barely above a whisper. All the anger in her face untwisted as she turned to you, her features suddenly soft and all too familiar. She hadn't changed a bit in the ten years you haven't seen her.
"Oh my god," She said, relief flooding her voice as she rushed over to you, pulling you into a tight hug. You remained still, not reciprocating the affectionate gesture as Hotch eyed you from behind her with a questioning look. You shrugged slightly at him, remaining completely neutral.
"I can't believe it," She said, voice soft as she pulled away, grabbing you by your shoulders. You noticed the eye bags and the panicky look in her eyes and suddenly things started clicking for you.
Back in high school, you, Easton, and Claire had been each other's rocks, always going everywhere together and relying on each other greatly. The three of you were best friends and you had been for all four years of high school. After what happened during and after your senior year, you fell out with the two of them and continued your advanced studies, not necessarily making any time to ever see both of them again.
Claire had always been the mediator in the trio. You always felt closer to her than you ever did to Easton. Claire always listened, and she gave the right advice when you needed it. You would've loved staying friends with her after highschool, but since she was so close to Easton, it remained impossible. You'd be lying if you said that seeing her again after all this time didn't tug at the strings of your heart in the slightest.
"You haven't changed a bit," You said, giving her a small smile. She did look the same, all of her sharp features still in tact the same way they were back then. There was a silence. She looked behind you momentarily and you turned, realization hitting you. "Shit, uhm—“
You turned, causing her hands to fall from your shoulders. "This is SSA Derek Morgan, SSA Aaron Hotchner and Dr. Spencer Reid,"
You introduced everyone, pointing at each of your co-workers referringly, to which each responded with a small nod or smile. Her gaze lingered on Spencer, who stood closely beside you. She smiled politely at them all, apprehension still laced in her gaze.
"It sucks that our reunion has to be under such shitty circumstances," She laughed, hugging herself protectively. The humor hadn't quite reached her eyes which caused you to shoot her an empathetic look.
"How are you holding up?" You asked. She opened her mouth to speak, but nothing came out. She sighed heavily, reaching up to cover her face with her hands. She shook her head.
"I cant believe any of it," She sighed, looking like she hasn't had a single ounce of sleep in days. "It seems like just yesterday she was coming back from her honey moon and now, she just—“
You felt for her, you really did. Even if you and Easton had your own history, you knew her and Claire remained great friends after high school. You didn't know they had still been friends after all of these years though. "Were you at the wedding?"
"I was her maid of honor,"
Oh,
"We're very sorry for your loss," Spencer added. You looked over at him, smiling sadly and appreciating his efforts to be empathetic with her. You reached out, rubbing the side of her arm. She smiled at the two of you weakly. "I'm sure this is all extremely hard on you."
She reached up, rubbing her eyes rapidly, as if trying not to get sensitive in front of everyone. It was understandable, suddenly being surrounded by people she had no clue of, and losing someone close to you is not easy and it could be very overwhelming.
You looked over at Hotch before starting. "We're trying our hardest to figure out who could have done this to them,"
"Do you mind coming in for some questioning?" You asked. Claire looked over at Hotch apprehensively, clearly not approving of him.
"We think you can help us determine if there was anyone that may have wanted to intentionally hurt Easton and Michael—" You explained, trying to ease her up. "—And Sarah and Adam.."
She sighed heavily, nodding her head with acceptance "Yeah, that's fine.."
"I promise, we'll catch up afterward," You smiled as Hotch and Morgan guided her towards one of the interrogation rooms. You watched her intently as she walked away. With a beat, you looked down at your watch, huffing quickly and furrowing your brows.
"Must be hard for her," You stated. Spencer took notice of how unbothered you seemed, and he didn't skip out on calling you out.
"And what about you..?" Spencer asked curiously, shoving his hands into his pocket and staring you down from his spot next to you. You turned to him, tilting your head.
"What?" You asked.
He shrugged, looking to the side momentarily. "For a case hitting so close to home, it would be normal for this to affect you,"
You pursed your lips and responded truthfully. "It doesn't,"
Spencer looked you in the eye and did what he always tried doing when he suspected something was up with you. He tried profiling you, something that with time, you knew how to fight against, especially when it came from Spencer. His brows crinkled with a familiarity that was only present when he was worried about you. You reached up, rubbing your thumb against the furrowed skin of his brows and smoothing it out gently. "You worry too much."
"Haha." He shoved your hand away as you snickered to yourself. You were glad that with everything going on, you could still find the time to laugh. Spencer made that very easy.
"You," You reached up to link your arms with his as you began dragging him down the hall, all the way to Garcia's office. "Are going to accompany me to visit our lovely technical analyst,”

You watched as both Easton and Claire downed whatever drink it was they had in their hands, music blaring loud and people waltzing around intoxicated at every turn you made. Not completely sober, but not all that drunk either, you found yourself feeling as if yet another Friday night had gone to waste.
"Can you believe we're already seniors?" Easton cheered, throwing her hands around both you and Claire. You stifled a laugh at her slurred voice, knowing the hangover she was bound to have tomorrow was going to be brutal.
"This year is going to be absolutely insane," Claire said, excitement laced in every syllable of her voice. You rolled your eyes and cringed internally, swirling your drink in your hands and debating whether or not you should throw it out since you weren't really in the mood to drink anymore.
You knew you had to get a lot of studying done tomorrow because you were taking some advanced courses that would get you ahead of certain subjects, effectively allowing you to finish the school year earlier and begin your college studies ahead of time.
You had always been very different from Claire and Easton, but you had never seen a problem with it. Claire cheered you on whenever you decided to focus on school work rather than go out on a Friday night, claiming that she could never have your willpower, but Easton always teased you and pushed you to 'let loose'. Of course, you went out with them sometimes, but you also knew when to stay in and focus on yourself, and you never saw that as a crime. It was like enjoying the best of both worlds.
Although lately, going out with Easton felt more like a chore than something you genuinely enjoyed doing.
"Are we finally going to attempt to loosen Y/n up?" Easton shouted over the blasting music, pulling you into a side hug. You laughed awkwardly, a very small feeling of annoyance bubbling in your stomach.
"Oh shush Easton," Claire slapped the girl's arm, rolling her eyes indefinitely. "We know Y/n is going to rock it this year in her own special way."
"Shoving her face into a book isn't my definition of rocking your senior year," Easton added.
"Well shoving my face into someone else's twice a week isn't my definition of rocking my senior year, but you don't see me shaming you, huh?" You bit back, deciding to defend yourself rather than let her step on your toes for what felt like the fifth time this week. You and Easton worked like that. Sometimes it was complicated though, since it was hard to draw the line between teasing and passive-aggressive comments.
"Not my fault you're a prude," You sighed, crossing your arms as she let you go. That's the complicated line you were talking about. Claire shot a disapproving look towards Easton.
"I'm kidding!" She shrugged innocently, holding her hands up in defense. "But like seriously, isn't there anyone that sparks your attention? Not even a little bit?"
You sighed once again, knowing this conversation was popping up. Easton was a very 'out there' person, to say the least. She had a very bad habit of agreeing to hook up with any 'hot man' that breathed. And being part of a private school, everyone seemed to be connected to everyone, somehow. If someone didn't know your business, it was someone else's personal goal that they did, and so the story goes on. And for some peculiar reason, Easton was very vocal and passionate about the fact that you didn't throw yourself at anything that breathed.
"No Easton,"  You set your drink down on the small stool that was available right beside you, not in the mood to drink at all anymore. "Just like I said last time you asked me."
"What about Henry? He seems cool," She pursed her lip in thought. Claire's face twisted with disapproval.
"Henry's a total jerk," Claire warned. Easton sighed in defeat and began scanning the room attentively. You watched her, unamused and annoyed, up until her face popped with excitement.
"Wait a second," She said, reaching out for your arm and pulling you in closer to whisper. "What about the guy in white, over by the kitchen aisle that's practically drooling at the sight of you?"
You deadpanned at Easton but looked in the direction she pointed to anyway. As you looked over, your eyes suddenly fell on a man in a white-clad t-shirt, who was in fact, looking over at you with deep curiosity. You suddenly, for the first time in who knows how long, felt nervous under the gaze of a man. He smiled at you softly from the other point of the room, to which you uncontrollably returned and your heart flipped inside your chest. Just as you opened your mouth to protest, the man began making his way over to the three of you.

You had spent all afternoon skipping through file after file and picture after picture with Garcia and Spencer, while Hotch and Morgan interviewed Claire. Rossi, JJ, and Prentiss called you every few hours to dump more newly acquired information on the victimology report based on what they were seeing over at the crime scene. Your brain was practically about to melt out of your ears. The comfort of the conference room chair seemed overwhelming once you sat down, finally giving your back a break.
Claire had left headquarters just only a few hours ago which now left you all to lay out and create a new profile. Everyone sat at the table until Hotch and Morgan walked into the room.
"We've interviewed Claire Thomspon for more accurate victimology reports and further insight on our Unsub's possible motives for killing both pairs of victims," Morgan stated, rubbing his chin absentmindedly. "Claire declared that Easton had various romantic and sexual relations during high school and every relationship she started was as soon as she ended a previous one,"
"So enraged ex that most probably wanted revenge?" Prentiss said.
"But then what connection would Sarah and Adam have to the murders?" You asked, furrowing your brows in contemplation.
"His murdering methods reflect his psychological state and motivations. For example, if he meticulously plans and execute his killings with precision and calculation, which clearly isn't the case here, it could suggest a need for control and dominance, but since his methods are more frenzied and impulsive, it could indicate overwhelming emotions, and a lack of impulse control." Spencer explained, looking over at you. You nodded, glancing down at the file report and scribbling something across the side of it.
"Meaning Sarah and Adam could've just been collateral damage," You finished, looking back up at him from your seat across him. He nodded.
"If this is revenge, the Unsub may experience, intense feelings of powerlessness, and a need to exert control over others as a way to cope with his emotional pain. Slaughtering married couples could serve as a twisted form of retribution symbolizing the perceived betrayal and abandonment he experienced." Spencer continued, hands dancing around freely, something that always happened when Spencer talked on like this, he was very expressive with his hands.
"By targeting couples, he may be projecting his own feelings of envy and resentment towards those who have found happiness and stability in their relationships." You completed, giving Spencer a proud smile.
And this is exactly why Hotch thinks the two of you work best together, it’s as if your minds were synchronized and every blossoming thought was finished by the other.
"Do we have any leads?" JJ asked.
"We've asked Garcia to pull up some names of people who have a past with Easton." Morgan explained with ease, you looked down once again.
"That's gonna be a long list," You mumbled, low enough for anyone to hear.
It was true though. You weren't shaming her for sleeping around. It was simply a reality that she broke a lot of egotistical men and saw having sex with them as a way of feeling good about herself. You even heard her say once that 'men were just trophy's waiting to be collected by her'. It worked, she felt amazing about herself, and she truly had any man she ever wanted.
"So what would you say our profile is?" Rossi finally spoke up, being awfully quiet as he silently drew his own conclusions.
Hotch began. " We're looking for a 27-year-old male with an unremarkable, physical appearance, something that allows him to blend in with the general population."
You allowed yourself to think back on your past, dwelling if you remembered anyone that might fit the Unsub report.
"He maintains a fit or athletic build which helps him overpower his victims and he leads a relatively solitary existence, avoiding social interactions." He continued in a serious demeanor. "He exhibits narcissistic tendencies with a sense of entitlement, believing that he deserves attention or admiration. He's adept at manipulating others and his actions may be driven by impulsive, completely disregarding consequences, leading to reckless behavior."
"He also harbors a severe paranoia, showing irrational fears and suspicion leading to a heightened sense of vigilance and a tendency to see threats where non-existent." Morgan said, closing off as he sat back down into a chair. Rossi pursed his lips in thoughts.
That described about half of your class though— unfortunately. You rubbed your temples, lowering your head and processing the information, trying to rack your brain on anyone viable who gave you 'serial killer vibes' back in the day. You looked back up, and turned your gaze towards Hotch, opening your mouth to speak
"So wait, if our UnSub presents a lack of impulse control, couldn't that mean that he could strike again even if Easton is dead?" You asked. Hotch nodded and you felt your hands begin to grow clammy.
"We asked Garcia to pull out the names of the couples that have been married in the last year or so from the class of North Virginia High School,"
"How many married high school sweethearts are there in one graduating class?" Emily asked, looking over at you in disbelief. You leaned back in your chair and scoffed.
"Our class was compared to an orgy," You stated, an unamused stance settling your gaze. "That says enough,"
You couldn't even count all the people in your class that had either hooked up, had sex, got together or got married, with your two hands. You never understood what the desperation was at the time, apart from the raging hormones— and its not like your class was full of A-list celebrity lookalikes.
Easton had always wanted you to have your fair share of hookups with the guys in your class, but you had fortunately settled your low scale of bodies on a whooping number one. 
"So, we just—" Emily's brows were furrowed deeply. "—wait until our Unsub shows signs of attacking one of our couples?"
Everyone waited for an order. You didnt believe that was the smartest idea, because yet another two people could possibly end up killed if you didnt play your cards right. You felt uneasy as you waited for Hotch to put together a plan that you could all follow, but he even he was struggling to decide how to love on. Suddenly, Rossi's voice cuted through the air.
"No, that would be stupid." He hummed, narrowing his eyes in deep contemplation. "We need someone from the inside."
You stopped, peering intently at Rossi as you gave Morgan a quizzical look, slightly stupefied by Rossi's sudden intervention. He shrugged his shoulders at you, clearly just as lost as you were. Sometimes, it was hard to follow along with Rossi's thought process, which was surprising to say the least, since you completely understood Spencers.
"We could use you, as a decoy," He said, cutting his gaze over to you and pointing a finger. You froze in your seat, shrinking at the sudden call out. You choked on your own breath, being completely taken off-guard by him. Not because you didn't have it in you to go undercover but because—
"Uh—" You cleared your throat, coughing once or twice. "Sir, I'm not married..."
"Yet." Was all Rossi said.
You tilted your head, truly not getting a single word coming from this man's mouth "Come again?"
Everyone shared dumbstruck looks. When you looked over at Spencer, he was just as confused as you were. You shared a silent conversation but all you could answer with were confused shrugs. Hotch looked ahead, suddenly seeming to grasp onto Rossis implications.
"You mentioned Claire was still in contact with many of your ex-classmates, right?" Hotch asked. You pursed your lips, blinking slowly and turning to him.
"Uh, yeah, she mentioned something like that earlier—"
"So then you lure the Unsub in," He stated, explaining what Rossi's brilliant mind was getting at. You closed your mouth and narrowed your eyes just ever so slightly. "You and Reid go undercover as a newly wed couple, and you lure the Unsub in, making yourself the next posible target."
Your mind screeched to a halt as you straightened up in your chair. Suddenly, Spencer was heard choking on his own breath in his seat in front of you. You felt your minds racing hit a brick wall, silence and stillness hitting every fiber of your body. "You want me to—Huh..?"
You weren't even quite sure how to process anything that had just been said. Questions began dawning at you, because one, why you? And why Spencer? And why together— as a married couple?
"It'd be a complex operation, that would include meticulous planning but," Rossi suddenly pulled out papers and began scribbling down. "Comprehensive security measures would be implicated, and with Claire in the mix, you two can find ways to get yourselves to interact with the high schools graduate community and it's very probable that the Unsub, is baited in, especially with Y/n being an ex-classmate."
"Actually," Emily started, looking between you and Spencer, who both shared the exact, red painted look on both your faces. "Thats not a bad idea."
"Wait, wait— You want me, to go undercover as a married couple, with Spencer..?" You asked again. It all started slowly falling, like dominos. You finally allowed your minds gears to rear back into movement as you shook your head slightly.
"Yes,"
Pretending to be a married couple implicated everything being a married couple was. And Rossi was implying, that you pretend to be a married, romantic, madly in-love, head-over heels about each other, couple with— Spencer? Which meant living in the same house, sharing the same things, going everywhere with each other, hugging, touching, kissing, sharing the same bed?—
Okay, now you were overthinking it.
You felt a sudden patter in your chest, that traveled all the way to your skull and through your ears, pressing against your throat. You felt heat rush to your cheeks at just the thought of all of that, and if you weren't mistaken, you could've sworn you felt your stomach flip in circles.
Spencer was your best friend yet why did this feel so incredibly exposing..?
"If you think about it, Reid and Y/l/n would be the only ones able to convincingly portray a married couple..." JJ shrugged. You glared over at JJ wondering what the hell she meant by that.
Suddenly, things were moving all too quickly and all too suddenly. You dared yourself to look over at Spencer who was surprisingly, in the same shaken up state that you were. His cheeks were glowing red, and something in his gaze was silently speaking to you. You looked back at him, feeling an unfamiliar uncertainty begin to nip at you. You were too scared to speak up, and for the first time, you truly couldn't read a single thought behind those hazel eyes you had grown to care about so deeply.
"I—uhm," You said, voice hoarse and weak. "You really think this is a good idea?"
"I think it's our closest shot at catching this guy without anyone else getting hurt." Rossi stated.
"But we only do it if the two of you are a hundred percent willing to do so," Hotch reminded. You looked back at Spencer, not knowing what to think, or what to say, or what to do. His gaze relaxed, in constrast to your panicky one, and he gave you a small nod of encouragement. You swallowed thickly.
He looked over at Rossi, nodding slowly. "Okay, we'll do it."

Health Update
First, I want to say thank you to everyone who reached out or commented on the latest installment of Where's Mommy? to wish me good health. I am so grateful for all of you 💚
However, the health issue I'm currently dealing with isn't due to a virus or bacteria, and there's a possibility it won't get better.
A little history.
Back in 2022, there was a two week period where I felt like my blood sugar was dropping, and I was very symptomatic. There was a moment where I slumped down a wall at work because of it, and they had to dump sugar packets from the break room into my mouth to rouse me. It was a very scary time.
After those two weeks, I went to my Primary Care Physician who ordered blood tests and had me purchase a glucometer to test my blood sugar several times a day. However, during the two weeks she had me do this, I never got a reading below 70, and the same symptoms did not develop as they did prior. My blood work came back clean, and without a reading lower than 70, my PCP dismissed it and told me I was having anxiety attacks, lol. She told me to come back if the symptoms came back, and they never did.
Backing up a couple more years.
Without revealing too much of my medical history, I have a chronic illness called POTS (postural orthostatic tachycardia syndrome). It's a dysautonomia or a dysfunction of the autonomic nervous system, the system that controls all of the automatic functions of the body. It was caused by my battle with Lyme Disease in 2010 and is currently incurable. I was diagnosed with POTS in 2018, after being told for 6 years that I had anxiety, lol.
POTS is not a very well-known illness, but it's getting more attention these days. It garners a host of different symptoms, including tachycardia, chronic fatigue, brain fog, orthostatic intolerance, migraines, gut issues, syncope, dehydration, blood pooling, etc. Everyone's POTS presents differently, and most people with a POTS diagnosis live on disability. I made the choice not to.
Fast forward to 2024.
Fast forward again to this past Wednesday.
Well, I finally was able to get a POTS specialist in my state this year. A huge win! When I had my initial consultation, I had mentioned the low blood sugar episodes in 2022 and asked if it could be related to POTS. The doctor told me that they don't see POTS patients having low blood sugar issues, but we're concerned enough to refer me to an endocrinologist. Another big win!
I had my consultation with the endocrinologist, and he ordered more blood tests, some of the same tests as before, and some different (y'all, they took like 20 vials from me). He also gave me a CGM (continuous glucose monitor) to wear for 14 days so they can track my highs and lows to see if they can catch anything.
Well, the next night, my blood sugar dropped below 70, 20 times, and 55, 9 times. Which means I was woken up 9 times throughout the night. I got only about 2 hours of sleep, and still had to go to work the next morning. But, once again, it went back up by itself without any intervention from me.
Y'all, it caught A LOT in just the first day, actually night. My blood sugar dropped below 70, 11 times, and below 55, 4 times while I was sleeping. Now, because anything below 55 is considered critical and could be fatal, there is an alarm that cannot be overridden and will sound. It sounds like a smoke alarm. So, I was awoken 4 times.
The odd thing is that my blood sugar dropped, then went back up on its own. I didn't eat or drink anything. Blood sugar doesn't really do that, so I thought it was odd. This also begs the question: If I'm asymptomatic at 53, then what level was I at in 2022 when I had symptoms? Honestly, I don't want to know.

All of the red is considered low blood sugar, below 70, and anything close to that 50 line is considered critical low blood sugar. And again, I did not eat anything during the night.
There are four major anomalies with my low blood sugar occurences:
Most cases of hypoglycemia are seen in diabetics, I am not diabetic
Most cases of hypoglycemia seen in non-diabetics are sporadic, mine are consistent
Hypoglycemia is normally corrected by consuming sugar, mine auto-corrects
When blood sugar drops, it creates symptoms, I do not get symptoms
There are only a handful of things that can cause hypoglycemia in a non-diabetic and even less consistently at night time. The doctor has already ruled out insulinoma (insulin producing tumors in the pancreas), so that leaves even less, and also the good old "we don't know what's wrong with you".
I'm not going to lie. This whole thing terrifies me. There's no telling how long my blood sugar has been doing this, and it only takes one dip below 50 for me to slip into a coma and die in my sleep. Luckily, my blood sugar does this crazy autocorrect thing, and I haven't died yet! Humor makes this easier.
Right now, I'm emotionally, mentally, and physically exhausted. Adding this on top of my already difficult life with POTS has been hard to cope with, and I'm crying a lot.
Hopefully, I'll get results soon, and my endocrinologist can figure out why this is happening and how to manage/fix it if it can be managed/fixed. Maybe I've got a completely new illness, and you'll find me in a medical journal! Wouldn't that be something.
Anyway, thanks for the continued support. I have a lot of IRL support from friends and family, but while I go through this process, I may seem distant, my posting might be sporadic, I may not keep my fic posting schedule, etc. And when I have an update, I promise to let y'all know!

I can't sleep and I'm just sort of stewing over how unprepared I am to be entirely on my own managing my own healthcare. blech.
Did I mention, Farmsister was suffering from hip pain and went to her doctor and was diagnosed with the exact same problem I have?
Diagnosed, I said. Yes! Her doctor actually investigated the cause of her pain, diagnosed her, referred her to a physical therapist, but also came up with a plan of treatment. Told her physical therapy often can't resolve this issue, so after a set amount of PT, if there wasn't enough improvement she'd refer her to an orthopedist instead.
Imagine that. My doctor was like "you've got intermittently debilitating pain? You should go see a doctor about that." and that was that. I went to a physical therapist because that's what she recommended, but I don't have a plan, I'm just spending $150/wk to work out in a room full of other people. I guess I'll ask my PT if there's a plan or like timeline or like, idk, something we should look for, or what. IDK what a realistic goal is. Pain-free seems out of reach. I'd settle for largely functional, I guess? But I don't know, and I guess I'm on my own to figure it out.
And the same with the ADHD! She was like "oh, your insurance isn't going to cover it and it's probably going to take months of waiting, but you had better go see someone about that", and refused to engage any further. So I messaged the psychiatrist today and he doesn't check the messages on that platform so I texted the admin who was like oh usually medication is adjusted at follow-ups, and I'm like well in the three minutes he talked to me it didn't come up I guess, so then they texted back that he says to try taking two pills a day for a couple days and then schedule a follow-up.
I've asked around, and usually I guess the regular adderall pills, you take in two doses at separate times. But if the point is that I'm trying to see if a higher dose helps, I'd probably better take them at the same time??
It's just that when the small ineffective dose wears off four or five hours after I take it, without my ever having had a good phase, I get horribly drowsy and also get this kind of gross formless yearning that I think might be a dopamine crash, where I roam the house in itchy horrible discontent trying to think of something that might help me, but it's not candy and it's not reading a book and it's not napping, and I guess I understand why people turn to drugs or self-harm because the feeling is awful, spacy and wrung-out and aimlessly needy.
But I guess it's up to me to research what that is and what to do about it, and then at my $250 three-minute follow-up appointment in three days or whatever I'll tell the psych what I want prescribed to me, because it's sure as shit not like he's going to have any fucking advice for me.
And like. Laugh out loud at the notion that my primary care physician would give a single shit about this. Maybe I didn't mention this on here either but literally the only thing she has looked into about me is that my blood tests came back with a fasting blood glucose level of 5.7 (idk what units, just that she's fixated on that number) and it is exactly entirely that post circulating about """"pre-diabetes"""? She has put in my chart that she wants to start me on Metformin!! Christ all fucking mighty, it could not be more obvious that she took one look at my fat ass and was like "this bitch eats only candy! I'd better scare her straight!"
Ma'am fuck off. She wants me to get my blood retested in July and I am figuring I'll take advantage of having to have a visit then anyway to get the ADHD stuff entered into my main chart, and I'm also going to tell her that since she was so disinterested in literally any of the conditions actually debilitating me (my hip pain and my ADHD) I had to research those so I could treat myself, and in the course of researching that I found out about the fake "epidemic" of "pre-diabetes" which isn't a thing, it's a fucking PR gambit to sell drugs, and so if she prescribes me diabetes drugs when I do not fucking have diabetes I will not be taking them, and I will also be looking for a new doctor, because I do not appreciate her fixation on treating a condition I do not have while ignoring things that are literally preventing me from leading the life I want to, wherein I can do things like, stand/walk as much as I like and can also like, perform tasks.
So there's my timeline.
(Yeah my insurance won't cover blood testing a second time in a year so that's gonna cost me $200ish, and the phone doctor visit she insisted on to discuss the results last time was $45 and it'll be that again for this one, but I mean, eventually I'll hit my deductible maybe.)
I don't know, people tell me that they have medical professionals that actually listen to and treat them and give them like actual good actionable advice on how to improve their various health conditions but as far as I can tell that all sounds fake and isn't a thing.
Unfortunately, I am too fucking debilitated by my Can't Think Good disease to do a competent job at caring for myself, so I guess I'm just going to have to fucking muddle through somehow, or something.
Probably I should put together my citations on how Pre-Diabetes Is Fake so that when I unload that on my doctor I can do so with fucking footnotes at least. Lord knows I can't sleep at the moment so I might as well do something productive.

Headcanon time!
Jason & cooking
I'll preface this with these words: I love the fanon of Jason loving to cook and being madly skilled at it! I just look at it in a different light.
Firstly, food & hoarding behaviors
Coming from a childhood where food being on the table when it was needed was never a sure thing, especially after losing his already paltry housing, Jason definitely grew up with fixations and neuroses surrounding food and it being available to him.
He hoarded food when he first came to the manor, which Bruce and Alfred did not scold him for, but did spark lengthy conversations about getting out of the habit of taking and learning to ask and request, as well as outlining what was okay for him to keep in his room without refrigeration (individually wrapped bars, unopened cans, other nonperishables or fruits and other snacks he was planning to eat immediately).
While he now had access to regular (not to mention lavish) meals at the manor, he never fully grew out of the need to keep a stash. As Robin, he kept meal bars in his Robin costume, so that he would have something to eat if he felt a nibble of hunger during a long stake-out.
This compulsion to ensure he'd always have food within immediate reach was never fully beaten; even as Red Hood, he carries nutritious items all over his person. To account for his sheer body mass and above-average metabolism (a Lazarus Pit side-effect I HC for him) he carries compact high-calorie and protein-jammed bars in pockets and little hideaway spots that he made all over his gear.
(If he ever gives you a bar without prompting, that's him saying I like you. He doesn't share his food willy-nilly.)
Food & cooking/preparing his own
Very soon after becoming a resident in the manor, Jason showed interest in learning to prepare parts of the meals he was now being given. It offered him a place to bond with Alfred, as well as a productive activity other than school work or Robin duties. For the first few months, Jason was very focused on trying to be useful to his caretakers, because he still had the underlying fear that he could be at risk of expulsion from the house, or physical violence for not meeting expectations.
Bruce and Alfred of course frequently assured him that he did not have to labour for their approval, but they saw that learning to cook was something he actively enjoyed doing, so he wasn't discouraged.
In adulthood, Jason prepares food with whatever means he is given. Just a crappy stovetop with only one working pit? He'll steam veggies above some diced potatoes and fill in the gaps with anything else he can scrounge up. A big oven? He'll make a huge oven bake to portion up to share around or keep as leftovers.
I headcanon that he does not particularly care for sweet snacks or chips; they're not terribly nutritious, mostly filled with sugars that'll provide a quick glucose shot and little more. If he needs a pick-me-up because he's feeling faint, he'll pop a dextrose tab and then get to work on a more substantial meal at the soonest good opportunity. His blood sugars already tend to fluctuate more than the median due to his Lazarus-tweaked metabolism burning through his food intake faster, so he tries not to overindulge on sugary things because let's not throw fuel on the fire and go into hyperglycemia. Hyperglycemia is a bitch.
(There will always be room for Alfred's cookies, though)
Unlike most headcanons I've seen, I don't picture Jason as being (or even wanting to be) skilled like a michelin-star chef. For me, his thing is making functional meals that'll fuel his body and satisfy him. He is a master of meeting all of the body's nutritional needs for a meal on one plate. You've got your proteins, your fiber, your fats, your carbs and minerals taken care of when he's cooking. It won't look like a meal a world-class cook would serve in a famous restaurant, but it's good for you and will taste great.
When free time permits it, he cooks big batches of food to hand out to locals who don't have the same food security he now cherishes. He cans veggies after he buys them in bulk (because he doesn't think any veggies he could try to grow in Gotham would be safe to eat for humans), and has a stockpile of rice ready to go in any safehouse. He donates both food and cash to food banks all over the state, and tells the poor kids in the area which foods are best for survival.
(He swears he isn't teaching them what to shoplift. He'd never give them pointers on how to do the necessary evil right if things are desperate enough. Okay fine, but if they were gonna steal anyway because the system's fucked, what's so bad about him giving them pointers on how to pick something worth the risk? He knows the tools of the trade, and he's not turning his back on the people whose lives are like his once was.)

How Te and Ti works

I was to stop making new ones for functions but I guess my interest came back for these. I haven’t fully grasped what the other functions are, but these are my initial thoughts for them. It would have been better to parallel Fi and Ti since they are technically similar but my previous posts paired both extroverted and introverted of one function, so I’ll just stick to it. I can’t find a mirror between both on how they work similar to what happens with Se and Si so I just drew it as how I understood it. Terms: OBJECT - anything that is real/present or can be perceived, event SUBJECT - you, the person doing the perception SUBJECTIVE FACTOR - past/present/future, experiences, etc. FACTS - definitions that are tangible or are true
INTROVERTED THINKING (Ti Dominant) - ITJ
First of all, I don’t see Accuracy as the main word for the function which is described by MBTI. It may be the end product though. It can be, in its core, correlated with PERSPECTIVE or just to be safe we can use CONCEPTS/THEORIES
- What Ti does is it usually sees an object or event which then an abstract thought pops up (sometimes it does this spontaneously) usually leading to a concept/theory which is still blurred. It then takes different perspectives of an object or objects breaking it down finding things that can justify on the concept/theory until it is finally formed/concluded. It feels like you’re trying to build something by looking on different things in different ways. When it is finally concluded, then this concept can be used on different objects/events.  An example would be this theory I had when I was still in college about “smart people being generally skinny”. It started with the thought of thinking/reading books/and using your brain needs a lot of glucose -> glucose is depleted thus fat is not stored (Yes fat comes from unused glucose). I then started observing the top students and true enough they are usually skinny or average in weight and included in different factors as well. But I never did conclude this theory because there are other factors that can still come into play.
MBTI or psychology in itself is full of concepts and theories that the Ti dom / Ti function would like to play and enjoy with since most of it are intangible.
Some Statements (not mine) about Ti which I found in a blog explaining Chapter X:
- I personally have a sort of Ti standard that if you cannot put it into simple terms then you do not understand it yourself. I feel negative when someone says, “It’s extremely complex, too hard to explain” Sure it takes time to make things simple and concise but don’t blame others for our own lack of Fe work and then belittle them, “You wouldn’t get it.” If you can’t Te and Fe it, then nobody can get it or trust that you yourself got it.
- That is because to attack his most function is to attack him. That is like a direct attack not to mention he does not trust the Fe to sympathize with the attacker. Not that he will attempt to press anyone with his convictions, but he will break out with retorts against every criticism, however just.
EXTROVERTED THINKING (Te dominant) - ETJ
Same with Ti, Te is not in its nature about Effectiveness. It can lead to everything being effective but the function itself is more of STANDARDIZATION. It’s not about having organizational skills or about scheduling as what MBTI describes.
- What Te does is that it collects a lot of facts and produces this what I believe is a filter. This filter is what we call a STANDARD/ SYSTEM. I’m not really sure how it is made but it’s there. Then with this standard it judges an object with facts about it and when all of the facts presented are true which then the object can be valued or is considered true.
An example would be an ENTJ friend of mine who shares about this “list of things to look for to consider her perfect boyfriend for her” which includes a basketball player, taller than her, smarter than her, more dominant, etc. And everyone who does not fit that criteria are all rejected and not given a chance at all. You may call them having high standards but that is what they perceive is true. Another instance would be having coworkers that are not up to her standards, she usually despises them, and she usually just does the work because she believes she can only trust herself.
Usually, they are skeptical to things that are not tangible to them or out of this world (ghosts).
Some Statements (not mine) about Te which I found in a blog explaining Chapter X:
- Te “produces” new facts that lead to mainstream truth. Concepts are “synthetic” and “predicative”. I think he (Carl Jung) is getting at the concept of gather vs organize here and noticing that Te is gathering for the mainstream good. It is going out and obtaining new data for them.

7th Diaversary!!!
Today, 3rd July 2023, I’m celebrating my 7th diaversary!!! 7 years since my life changed forever ♥ . Although the idea of "celebrating" being diagnosed with diabetes may seem puzzling at first, I realize that incredible efforts, unwavering perseverance, and extraordinary bravery throughout this journey are truly deserving of celebration and self-appreciation.
With every heartfelt letter I wrote to myself, I embark on a journey of reflection, tracing the footsteps of my past, present, and future dreams. It is a tender exploration of the hurdles I have overcome and the milestones I have accomplished. Each word on the page becomes a testament to my resilience, reminding me of the extraordinary strength within. As I pen down my thoughts, I am filled with a profound sense of gratitude for the endless possibilities that have unfolded and the dreams that have become realities. With each letter, I embrace the beauty of my journey, cherishing every step taken and every aspiration fulfilled; and this year I decided to share 7 lessons I’ve learned in these past years.
1. You are so much stronger than any crap life throws at you; it is the power of our mindset and the resilience of our attitude that ultimately shapes the outcome. Just believe in yourself and conquer the life challenges like a Boss.😎
2. Shittiest situations can provide you with the greatest of opportunities.😆
3. Through the wild ride of diabetes, I've discovered more about myself and the world than any textbook or lecture could ever teach me. Who needs school when you've got a mischievous pancreas as your professor? 😜
4. Surround yourself with the kind of people who not only motivate you but also celebrate your victories and have the uncanny ability to construct a Lego tower of awesomeness with you! 🙀
5. Explore experiences outside your comfort zone; feel the fear and do it anyway. Prepare to be astonished by the sheer magnitude of your own strength. And maybe some stories along the way! 🥴
6. Crying is acceptable; Tired is acceptable; Stressed is acceptable; Anger is acceptable; But “Quitting IS NOT”!😎
7. Be who you are; speak what you feel; do what you like; because those who mind don't matter and those who matter don’t mind. ❤️
So, on this diaversary, let's raise a sarcastic toast to two vital lessons I've learned:
The never-give-up attitude, because hey, who needs a functioning pancreas when you've got determination that could move mountains? No matter how many glucose rollercoasters I've ridden, I've always shown up, like a superhero.
The beauty of not giving a single drop of insulin about what others think. I've embraced the art of doing what's best for me, regardless of societal norms or raised eyebrows. Who needs approval when I've got my own glucose graph as proof that I'm living my best T1D life?💉💉
Cheers to being the unstoppable force that I am, boldly defying diabetes with a sarcastic smirk and a healthy dose of self-love. Let the world wonder while I keep thriving, showing them what it truly means to be a T1D badass.
Diaversary or not, I'll keep conquering this T1D adventure, laughing in the face of glucose fluctuations, and reminding myself that no matter what, I am stronger, braver, and funnier than this pesky pancreas could ever be.
Here's to many more years of rocking the T1D journey with sass, resilience, and a wicked sense of humor!

i feel like shouting into the void about an adhd discovery i have recently made.
i’ve mostly always been pretty dismissive of adhd “productivity” self help guides and tips, because of the way they seemed to pathologize my mind and enshrine neurotypical behaviour as the only reasonable way to behave. BUT the past few months, I’ve been very fortunate to live on an artist grant that funds all the essentials for living & studio costs for a year to work on a project, which is a dream right? BUT I COULD NOT DO IT. It was so upsetting and frustrating, no matter how hard I tried, I could not do the work that is all I’ve wanted to have the time and freedom to do for so long. All the obstacles I blamed before had been removed and I still could not see any progress.
I started learning about executive function & adhd, and that basically adhd people have a limited capacity for executive function and you can only do a task that requires executive function for so long before your energy store is depleted and then you have to do something that DOES NOT require executive functioning to let it restore itself. You can even do things to help restore faster, like eating or drinking things that keep your blood glucose levels up, or meditation (although I tried to do that for 2 minutes and I felt like my brain was dying. shen qiao is so powerful.) or SCROLL TUMBLR. Basically anything that fully rests your mind!
What I’d been doing was trying to do some task, that even though it’s art and what I want to do, still required executive functioning, and I would find myself spacing out, pacing, idk just anything but what I WANTED to do, and then keep trying to go back to the task I was supposed to do again and again, and fail again and again. Hours would go by, days would go by, and I’d maybe do 1-2 hours of work a day. That was the exact same amount I could manage when I had to go to job all day to pay my bills. So the last few days I’ve been taking a short break about every 30 mins or so, eating a lot of fruit/sweet drinks when I do, or walking around, going into my bedroom to rub my face on my cat for a few minutes, and oh my god. I’m able to work, I’ve been able to do so much!
What I realized I’ve done is internalize these neurotypical expectations of how we are supposed to work and force myself to keep at it despite my body screaming no. All my life I’ve had to work like that, everything took me a lot longer than everyone else but at least I could make up for it with working voraciously. A lot of the time I spent working would be spacing out, getting distracted, going on social media, so I had to remain “at work” longer to compensate. Now I realize I had maybe depleted all my executive function in the first hour, and then when I drifted off because my brain needed a break, I would punish myself by then dragging my limp, resistant body forward agonzingly for the next 11 hours. Now, when I find I’ve stopped doing whatever task I meant to do and have started scrolling tumblr AGAIN, I recognize that my brain has tapped out and I need to stop and rest for a few minutes.
Sometimes, before, when I would be doing something that wasn’t “on-task” that I knew I wasn’t supposed to be doing and I’d keep telling myself, in 10 minutes I’m going to go back to what I’m supposed to be doing (and usually plan to work even longer to make up for it), then the 10 minutes would pass, and sometimes hours would go by when I never got back to the task, just keep on wasting time, feeling horribly, really wanting to work on my art project and knowing that I needed to, but I could not make myself. There’s some subtle shift that happens when instead I notice I’m not doing what I am meant to, so I stop trying to do it and accept it is now time for a break, I’ve found if I do that, it’s relatively easy to go back to my work 5-20 minutes later.
I feel like a toddler, haha. maybe this is all stuff everyone knows. but when before I would spend hours trying to drag myself along trying to do something i really wanted to do but could NOT make myself do, now if I take ten minutes to eat some strawberries and pet my cat I actually find myself being able to be interested and motivated to do that thing I wanted to do. It’s amazing. I mean seriously the absolute WONDERS of snacks! And sparkling water with a little bit of lime! I had so many periods of time when I needed to finish a project and I would just be so infuriated by the inconvenience of having to eat and sleep. It’s been mind blowing to find that what I need is the exact opposite (exact opposite of trying to overpower my body's need for eating and sleeping i mean!). I have been really really hard on myself I realize, but it was always like, I didn’t have an option to be otherwise.

Liver is the largest gland in our body, and an important metabolic organ which carries out many essential biological functions. The liver is a wedge-shaped, reddish-brown in color with two lobes of different size and shape. The approximate weight of a human liver is 1.5 kilograms.
