Natural disaster will always impact women more than men.
NEW DELHI/DERA ISMAIL KHAN, Pakistan -- The catastrophic floods that have upended life in Pakistan are posing unique threats to women, including well over half a million expectant mothers, aid officials have warned. 
A combination of unprecedented rainfall and melting glaciers has inundated a huge swath of the country in recent weeks, claiming over 1,300 lives while destroying millions of homes and crops. The government has said that more than 30 million people have been affected in some way. The severity has been widely blamed on climate change, with United Nations Secretary-General Antonio Guterres saying on a visit to Pakistan over the weekend that he had "never seen climate carnage on this scale."
But while the disaster is national in scope, the United Nations Population Fund (UNFPA) has raised alarm over the outsize impact on women and children. The UNFPA has estimated that more than 650,000 pregnant women in flood-hit areas require immediate health care services, including 73,000 women expected to go into labor next month. It also said that the damage to homes puts many women and girls "at an increased risk of gender-based violence."
Zohra, a 25-year-old woman in the village of Fateh in heavily flooded Khyber Pakhtunkhwa province, is seven months pregnant with her fourth child. After losing her house to the waters, she now fears losing her baby due to a lack of hospital access. The nearest medical facility is damaged -- one of at least 900 crippled facilities across the country, according to a World Health Organization estimate.
"First we lost our house, now we don't even have enough food to eat," said Zohra, who wanted to be identified by only one name. "I already have three children and I am unable to feed them. How do I feed the one who is inside me?"
Health professionals say the disruptions to women's care threaten to unleash a host of problems. 
Ayesha Bibi, a doctor with the Pakistan Red Crescent Society -- a humanitarian organization responsible for providing emergency medical relief -- said that even missing routine check-ups can expose pregnant women to serious maternal and fetal health issues. "They must visit for regular check-ups to ensure a healthy fetus and pregnancy," she said.
The destruction of roads and bridges has made that impossible for many. Another issue is a lack of medicines. This has made the last phase of 38-year-old Fatima's pregnancy unbearable. "Sometimes, the pain is so severe that I just lay down and pray for my child to be delivered on its own," she said.
Fatima, who is due next month, recounted her traumatic experience as the waters rose. "The floods were a nightmare for me," she said. "People tried to move me to safety but I didn't have the capacity to move. The room was filled with water and I just sat there, praying. I was fully prepared to die that night."
Fatima, whose husband is a daily wage laborer, complained that the response from the government and relief teams has been insufficient. "No one cares about poor people," she said. 
The provincial disaster management agency in Khyber Pakhtunkhwa insists it is doing what it can to mitigate the crisis women are facing.
"As per the government's policy, our first priority is to rescue people and save their lives," said Taimur Ali, a spokesperson for the PDMA. "However, we understand that women and children are most vulnerable during emergencies. Therefore, we have made provisions for pregnant and lactating women in our contingency plans."
Ali's plan involves deploying health workers door to door to ensure the nutritional needs of pregnant women are met. Citizens displaced due to the floods would be fed in the numerous relief camps that have been set up.
Still, local volunteers say they are waiting for government intervention in far-flung areas where people are desperately poor and helpless. At the same time, the floods have created a monumental challenge for a cash-strapped Pakistani government that is grappling with an economic crisis.
Ehtesham Hassan, a 25-year-old climate activist from the Dera Ismail Khan district of Khyber Pakhtunkhwa, decided to take matters into his own hands when no help reached the area.
"At first, we gave them food, water and basic aid," he said. "In the second phase, we are planning to give them tents and shelter. In the final phase, we will work toward rebuilding and rehabilitating the displaced people."
Hassan noted, "The nearest hospital in Dera Ismail Khan is located 8 km away. On heavy rain days, it took hours of wading in water for one to reach the hospital. The health facilities here were already poor, now things are just worse." And pregnancies will not wait for infrastructure to be rebuilt.
"Our people are poor and desperate," Hassan said. "They don't have time to wait for resources, we needed to act immediately."
Besides the danger to pregnant women, there are also concerns about the health of newborns delivered in these appalling conditions.
A woman from the village of Adil Siprah, who lost her house in the floods, now lives with her four children in a tent she received from the government. But continued rains have damaged her new makeshift home and she is not sure it will survive any more bad weather.
"Our tent is flooded with water every day. My children are sick because of the contaminated water. They have viral infections and stomach aches," said the woman, who declined to be named. Echoing a common refrain, she urged the authorities to do more.
"Everyone who lives here is poor. How do we buy medicines? The hospitals are flooded and the roads are broken. How do we reach the hospitals? The government should do something for us."
