Something I read about on Twitter and something that isn't talked about is how much early transition can be difficult. So I've been on 5 weeks of 2mg Estradiol and I am now on 1 week of 100mg Spironolactone. I also started Finasteride and minoxydil back in December of last year to reverse hair loss. My skin feels softer, terminal hair growth has slowed, and I've noticed sexual changes. However for me, dysphoria has not decreased. In fact it's increased.

Also TW regarding sensitive stuff after this, also some TMI sexual stuff.

The reason why, is that dysphoria has been there for about a decade, but I've been able to sort of run from it, and feelings of being foreign in my body and gender, I eventually became somewhat habituated to it. I also was greatly in denial about many things partially because I discounted many things I was thinking and feeling with as "you're being irrational because you are mentally ill". Now that I'm socially transitioning more and now that I'm undergoing medical transition, I can't hide from my feelings anymore. And the fact that I'm not instantly transformed into a perfectly cute and androgynous person and I still have many masculine characteristics is distressing to me.

From what I understand, this is not unusual for early transition to be difficult. In fact, my route of socially transitioning before I started medical transition is not what everyone does. Many girls simply boymode for quite a long duration of time while undergoing HRT. However for me, something that really hurt is how much I've been in denial about my appearance. My upper body is quite large. I have trouble hiding stubble and my face breaks out too much if I shave every day. I was in the early stages of Norwood Hamilton. And when I looked in the mirror and saw these things, I denied it to myself, and everyone else told me I looked fine. I've even had people hugbox me on the internet and tell me they can't tell whether I'm AMAB or AFAB in some pictures. Especially before starting hairloss treatment, I did NOT look okay. I'm still a long way from how I wish to look. I feel so hurt that people just let me be that ugly without saying anything so that I could do something about it. Now I'm forced to confront it.

Another factor is that someone hurt me shortly before I started medical transition. A former friend basically went behind my back and was snickering about how I'm an ugly hon. She also behind my back attacked the legitimacy of me calling myself bisexual because I told her that I didn't enjoy anal sex with a male. When I was given screenshots from a friend of this happening and I confronted her, she justified what she said by saying that I'm delusional, she herself feels betrayed by my appropriating being trans and homosexual, my transition is illegitimate and I'm driven by autogynephelia, and that I was deserving of her going behind my back like that. I shortly after cried my eyes out and relapsed on self harm.

I have since been hyper fixated on everything wrong with me much more so than before. I think I've demonstrated both wrong approaches on how to talk to someone with dysphoria. The dishonest hugboxing approach is wrong, and the TTTT brained attacking someone else's appearance to make them feel bad so you can feel better. I think there are constructive ways of addressing someone's body issues in a way that's honest.

For example, people could have told me "hey, you are right that your hair seems to be receding. Whether or not you go full out with medical transition, you can see a dermatologist and be prescribed Finasteride for that. Or people could tell me how transition would help with the things I'm insecure about. Another complicating factor for me is being nonbinary.

Most medical transition is for binary trans people. Also, nonbinary people who have unassailable gender issues, feel dysphoria, and may need medical transition are largely invisible in favor of teenagers going through a they/them phase. If I was a binary trans woman, I suspect I would have had to come to terms with it much earlier in life and I would be less fearful of medical transition.

I've been fearful that HRT would overfeminize me and I would feel dysphoria from being too feminine instead of from being too masculine. However at my doses, these are fairly conservative. I shouldn't be excessively feminized, and the process is gradual. If breast growth is too much for me, I can simply stop taking the 2mg Estradiol and switch to SERMs and maintain the Spironolactone.

However breasts especially at my conservative doses should not be prominent. While the size of breasts when going through medical transition is largely genetically influenced, it takes the medication to activate these genes in the first place. If you don't take strong doses, theses pathways will not be walked down very far so to speak.

Another thing that I was very afraid of was negative sexual side effects. I was afraid that I would be at risk of erectile dysfunction, my testicles would shrink, and that I'd have nothing to ejaculate IF I could reach orgasm. I've heard of even binary trans girls detransitioning because the sexual side effects were too unbearable. Apparently testicles can appear slightly smaller from being inactive during medical transition since they're not filling up with and generating high amounts of sperm, however the physical structure of them goes unchanged. Any amount they grow during male puberty is permanent.

Also, transition reduces or eliminates spontaneous erections and nocturnal erections, which has been my experience. However, I am still able to get erect when sexually aroused. Erectile function is basically a use it or lose it kind of deal. That's why many bottom trans girls are crazy about chastity cages I've learned. So long as I masturbate a few times a week, I should be safe. The size of my erection is also unchanged when physically measured. In addition, apparently no ejaculation to speak of is specific to taking Bicalutimade for androgyn suppression. Spiro, especially at my doses won't do that. In fact if anything, I've noticed MORE cum when I get off, not less. It's thinner and more translucent, but, it is substantial. In fact, I now have to be even more careful than before to not make a mess when getting off.

So much of what's been holding me back was misinformation, fear, and having an impoverished understanding of myself, and what are my needs. I feel like my perspective on transness is unique. I do not fall firmly in the transmedicalist camp, or in the self ID camp. I consider my positions sort of sensible and moderate. I feel like understanding of nonbinary people and how to best treat them is of great importance. Also to have workable definitions of gender and give people tools on how to better self evaluate themselves for who they are.

the energetic truth about beauty ☯️

this might be a hot take in the community but i feel like it needs to be said… beauty is actually a yang (masculine) trait. there’s this twitter thread, i’m gonna find it, that says that men naturally possess beauty bc of their yang energy: strong bone structure, thicker/naturally even-toned skin, thick hair, long eyelashes, full/thick eyebrows…. think about it, all of these traits are desired for women but men naturally have them.

think of the average woman without makeup: hormonal acne, redness, rounder faces, softer/more delicate skin (usually takes longer to recover from injury & can be prone to scarring). moreover, think of what women do to their hair to make it look thicker: extensions, growth treatments, & wigs. men don’t use these tools but naturally have thick hair. and they don’t get hormonal acne the way women do…

now this should go without saying but this is NOT a place for misogyny, nor and i’m trying to generalize/stereotype men and women. in fact, this post is not about men or woman as social constructs at all, i’m talking about ENERGY (yin/yang). and with that there are men who have a stronger yin energy and women who have a stronger yang energy naturally. (we also need a word for these ppl bc i’m one of them and i’ve never been able to find a term for us lol). i’m merely making an observation.

i would even argue that being able to recognize one’s own beauty is a yang trait. men, no matter what they look like, see themselves as beautiful. and yes misogyny & the patriarchy is at play, but think abt what that means… unconditional self love/confidence comes naturally to yang energy. even if it’s toxic, arrogance is still the shadow side to yang energy.

for myself, i realized that when i came into my yang energy for the first time was right around the time that my 7-year long battle with acne was ending, my confidence was up, my lashes grew longer, & my features started to look more pronounced… i don’t think that’s coincidental.

and a part of me laughs a bit when i see these “divine feminine glow up” videos/posts trending bc it’s really masculine energy that you’re channeling lol. 💌

You can also do a search for "years on hormones" on Twitter or something and you will find pictures that trans people have publicly shared of themselves before and after they started hormone treatment, which will not only provide you with a wide array of faces but can give you as an artist a better idea of how transition can make a character look.

You should also familiarize yourself with how HRT effects the whole body in general, not only so you can understand what it does do but what it doesn't do. However, I think it's important to understand the unique attributes of a trans body—speaking purely for myself, my broad shoulders and relatively narrow hips and strong jawline as a trans woman, for example—and the ways in which these, as trans features, are worth recognizing and appreciating and respecting in art and representation. I am irked when all of the trans representation in media that I see are of people who are perfectly cis-passing because these features, these "tells," are treated as inherently bad to have or even to acknowledge, and I am irked by any assumptions that all but state outright that the ideal trans person emerges fully formed from the ground in a perfectly cis-passing body with nothing to possibly suggest that they are trans on a corporeal level. It's not impossible for trans people to be cis-passing or very pretty, of course, but as I've said before I do not want pretty to be the "default" of character design and I appreciate it when a character's appearance, either pretty or not, actually matters.

It's not even about trans features being "beautiful," because even though I do think many of these features are beautiful, beauty isn't something we all should be obliged to owe others. It's about authenticity, shamelessness, compassion, and basic respect for the myriad appearances of trans people.

Hi! I turned 21 earlier this year and have been learning so much about myself this year. One thing i’ve always known but never done much research on is the possibility of being intersex. As a newborn, doctors took me away from my mom for hours, running all these tests and trying to determine my sex. When they finally gave me back to my mom, they told her I was a girl, and that she should consider going through with surgery to make my genitalia look more “normal” I guess? They couldn’t tell if I was male or female because my clitoris was/is enlarged. My mom basically told them off with a quick little “fuck you, they can make that decision for themselves when they are of age, i’m not changing my kids body without their consent.” Which I am so thankful for!! However that’s all she’s ever told me about the instance. Growing up however was really strange. I got my period at 13 and have had many many issues with it since. Constant ovarian cysts, irregular periods, and lots of pain. I started to develop breasts at a pretty young age, but they just kind of stopped growing at a pretty young age. They’ve been the same very small size since I was probably 9, while all the other women in my family have DD and up size breasts. I also started to explore my sexual and gender identity at a young age, and have identified as non-binary since I was just 16 years old. I’ve been researching endlessly on google, reddit, twitter, etc. and i’ve gotten the general consensus that I am most likely intersex. What do I do/where do I go to determine if I am intersex or not? It makes so much sense to me, but I really want to talk to a doctor or therapist that specializes in these issues and can help me, I just don’t know where to start. Any advice is very helpful and I thank you in advance for taking the time to read my story!! :)

So I’d say that it’s absolutely very very likely that you are intersex. What you’ve described about your experiences at birth is absolutely an intersex experience, with the doctors being confused about ambigious genitalia and wanting to surgically “fix” it even though there’s absolutely nothing wrong. I’m so glad that your mom decided to wait and not have surgery performed on you without your consent, and it makes me so mad that doctors always push that on parents and tell them that they need to make that choice. Everything else you’ve said sounds like an intersex experience, too. Ovarian cysts and irregular periods are also signs of intersex variations, and especially when considered in context with your experience at birth, I really think that you are intersex. 

One possible condition that you might want to explore is Congenital Adrenal Hyperplasia (CAH). The things you’ve described are all symptoms of CAH or NCAH, especially having ambigious genitalia at birth and having irregular periods and cysts later in puberty. If you have other symptoms like “excessive” body hair and acne, those would also be signs, as well as rapid growth during childhood but a short stature later as an adult. There are several tests that doctors can do to diagnose CAH. They can check your  17-OHP levels, other hormones like testosterone and androgens, and the main test for CAH is something called ACTH stimulation. All these tests could lead to a diagnosis, if that’s something you want. Of course, I can’t diagnose you over the internet and it could be something entirely different, but it’s a possibility! 

If you’re interested in seeking a diagnosis, you could reach out to a doctor or a gynecologist and explain that you had ambigious genitalia at birth and are interested in seeking a diagnosis. If there’s certain traits that you want to seek treatment for, like painful periods, you could bring that up as well. Explaining your symptoms and saying that you’d like to look into diagnosis is a good way to start conversations with doctors, and if you think it’s something like CAH or another intersex condition, you could bring that up. Your regular doctor or gynecologist can probably refer you to an endochronlogist who is more of a specalist when it comes to intersex conditions, and they could do tests and help you through the process of diagnosis. 

I don’t know of any intersex specific therapists, but it might be a start to reach out to therapists who specalize in LGBTQ issues. They might be more informed and be able to help you through this process of discovery and self identity. And I just want to validate that whatever you’re feeling about this discovery that you’re intersex is really valid, and that you’re not alone. I know when I found out I was intersex it was really a shock to me, and I had a lot of conflicting feeling about it, and felt like everything I had ever known about myself had changed. If that’s what you’re feeling, know that you’re not alone, and that you’re the same person you’ve always been, and that you just know more about yourself now. There’s a whole community that understands and has your back. Of course if you’re feeling totally comfortable about this discovery, that’s awesome and understandable too! Being gentle with yourself throughout the process of intersex diagnosis and discovery is so important, and letting yourself feel whatever it is your feeling and taking time to unpack and process however you need. The intersex community on tumblr is full of people who can listen and understand, and I’m also in some really supportive intersex discord servers that I would happy to give you a link to if you come off anon. 

Overall, I want to affirm that you are absolutely welcome to identify as intersex and that everything you’ve described is absolutely an intersex experience. I know this process can be hard and confusing, and navigating doctors can be difficult, so feel free to reach out if you have more questions or if you just need to vent.
