Tech is kind of all over the place in One Piece, but we do get a solid look into the medical field, between Chopper and Law’s backstories we can see the level of medicine available - which is a step up on several islands.

It’s not impossible to think that early 20th century medications existed. There’s refrigeration, microscopes, injections, and such that are canon, so it’s plausible.

Like Rosinante said of ope-ope no mi, it’s not magic. Law needed medical knowledge to use the fruit to its full potential. The same can be assumed of Iva. If the knowledge didn’t exist, then the steps beyond wouldn’t be available. What Iva could and couldn’t do would be limited.

The only difference between them and what’s available is a matter of time and convenience. Law can perform complicated surgeries in short order, and Iva can change what people’s bodies produce, but both effects can be had via other means.

So the short answer to my really long explanation is that yea, Marco can get you the hook up - he’d know the best places to get supplies. Hongo and Chopper too. Hell, Chopper probably has compounding recipes that would be tailored to his crew mates.
