Misoprostol is only FDA approved for preventing or treating gastric ulcers in patients who have prolonged exposure to the administration of non-steroidal anti-inflammatory drugs (NSAIDs).

Misoprostol causes uterine (womb) contractions and is used in several areas of female reproductive health:

Induce labor

Postpartum Hemorrhage (heavy bleeding after delivery (vaginal or C-section)

Misoprostol alone is used for induction of labor to terminate pregnancies in any trimester of pregnancy. It causes passage of the fetus and all gestational tissue

Cervical Priming prior to the following:

IUD insertion – Misoprostol / Cytotec tablets are inserted inside of the vagina or placed underneath the tongue 2 to 3 hours prior to IUD insertion.

This allows minimal to no pain or discomfort when inserting the IUD through the Cervix

Incomplete Abortions – partial passage of pregnancy tissue. Fetal tissue remains inside the cavity of the uterus.

Missed Abortion- non viable fetus inside uterus longer than 5 weeks

Hysteroscopy (scope inserted through the cervix to diagnose and treat various GYN issues inside the uterus

Last break!

My final (of my undergraduate) spring break has officially began!

I already received one grade for my final report and I got a 100%. It was bitch of a paper to write AND my least favorite course of the semester so I was surprised and happy AF when I saw my professor’s comments and the 100%.

We are heading to NC this week for mother’s day. It’ll be the first road trip in the new truck (M sold his Jeep and sold it for a great price). So that’s exciting. We plan to go to the beach and stay for a few days then head to Charlotte for the weekend.

Although I’ll be on break I do have a lot of things to do like pick a major for my graduate school (Social Work or School Counseling). I’ve been in a group for a few months now specifically for mental health and I’ve learned a lot from people who are in the same boat as I am or advanced.

Top Surgery, Part 1 - Pre-Op

As with many trans guy, I distinctly remember researching top surgery techniques and results when I was younger. When I first began developing my trans identity, I was fascinated by the ability to remove a part of my body that had always caused me to feel so much dysphoria. I was one of the unfortunate children that began puberty around the time I was 8 years old. Puberty is awful for almost any child but this was life shattering for my young, trans self. So, discovering that I could have surgery to remove my breasts changed my outlook on life in so many ways. But, in the back of my head, I never truly believed that a time would come when I could have this done. My future never seemed like something that was guaranteed when I was younger. 

This made my experience with top surgery all the more surreal. I was fourteen when I bought my first binder. I snuck and saved up lunch money for months before secretly having an underworks binder shipped to a friends house. Even after binding for years, it never became easier for me. I have weak lungs so I often developed lower respiratory infections. I've pulled my intercostals before and injured my back from repeated binding. Simply put, my body has never handled binding very well. Even after purchasing a GC2B binder (HIGHLY RECOMMEND), I was still experiencing problems. So I knew I had to make a huge decision. I either had to suck it up and continue to bind or begin to research top surgery option again. After a particular bad rib injury, I decided on the latter option. 

After that point, I was constantly looking into different surgeons and techniques. I knew that I would have to have a bilateral mastectomy due to my large breast size but there were so many different options even with just this one type of surgery. I struggled to decide whether I should fly to Florida or Colorado for a more well-known surgeon, both of which would be costly for stay and travel, or if I should pursue a closer surgeon that was not as popular. I cannot tell you how many different result pictures I critiqued throughout this period of time. During one of my readings through topsurgery.net, I saw a description of the button hole technique being popularized by Dr. Hope Sherie at the Cosmetic Concierge. I really liked the idea of retaining nipple sensation and keeping the primary nipple and nerves intact. This led me to research Dr. Sherie and her results. And, when I say research, I have to include the fact that a major portion of my current career is research. I went back and found out about her medical school and experience as a military surgeon and everything. Thinking back on it, it was probably a little creepy. BUT I wanted to make sure I made the right decision! 

After some time, I felt satisfied by what I had found. I went ahead and sent an email to her practice inquiring about top surgery. No more than 30 minutes later, Dr. Sherie herself replied (I think I sent that email at around 10pm at night). After a few back and worth emails, I went ahead and scheduled a consultation. I had to put down a $100 deposit for the consultation appointment. This fee would be put towards my surgery fees if I chose to go with Dr. Sherie. For out-of-state patients' convenience, she also allows this consultation to happen over the phone. I live about four hours away from Charlotte so this was definitively something I wanted to take advantage of. 

The initial portion of the consultation call was done by the main office person (I'm not exactly sure what her title is!). She was awesome to talk to and full of information! She definitively helped me feel at ease and answered all of my questions. This portion of the call took maybe 30 minutes and included medical history, desired results, and surgery process. I was also told that their office could give me the contact information for other patients that volunteered their information for new patients and also lived out of state. It was good to know that I could contact other people that had similar experiences to my own. After this, I spoke directly with Dr. Sherie, though, at this point I had basically had all of my questions answered. She did suggest that I go with the free nipple grafts instead of button hole based on my desired results. I stated that the most important outcome was a flat chest with nipple sensation begin a close second. She let me know that I could change my mind right up until the actual procedure. The entire consultation probably took a little less than 40 minutes but I was satisfied with all of their information. I was told that their were a few surgery dates available during the summer and that I would be emailed a quote for both surgical techniques (button hole technique was quite a bit more expense than the standard nipple graft). 

After a week or so of more research and weighing of options, I called back and scheduled my surgery for June 15th 2016. This would give me enough time to get ready but would also allow me to heal properly before beginning classes in the Fall. After scheduling my actual surgery, everything just seemed to fall into place. Before booking my consultation, I received approval for Care Credit. This was actually one of my main reasons for proceeding with top surgery. I planned on getting my name changed prior to graduating in Spring 2017 so it would not impact my ability to get my counseling license. I was worried that changing my name would impact my credit and ability to receive credit lines so I thought it would be best to get the surgery done before this time. I went ahead and booked a hotel for three days in Charlotte, NC as well as arranged for a friend to act as a babysitter while up there. I had my pre-op phone call and received more detailed instructions for filling prescriptions and arriving at the surgery facility. Then, I just had to wait. 

It was also around this time that I lost my second internship site. I made the mistake of telling them I was going to have surgery and would have to miss a day. This led them to assume what type of surgery I was going to have and ask many intrusive questions that in no way were HIPAA compliant. I was treated as a lying, manipulative person that had withheld knowledge of my surgery and trans-ness from them. They attacked my integrity and professionalism. It made me feel like less than a person and pushed me into a bad bout of depression. I felt worthless. At this point, I had lost two different internship sites since coming out as trans. I felt like all of this was my fault and began to regret beginning to transition. It was not an easy time in my life. Despite that, I can clearly look back at that experience and say they were in the wrong. I handled everything as well as I possibly could. Keep in mind, I am a clinical counseling graduate intern. Our business is mental health. At this point, I had been counseling others for close to 6 months. You would think that these practices, out of anyone, could appreciate my coming out as trans and judge me based on my experience and skills as a therapist. But that was not the case. If anything, that whole experience showed me just how much of a need we have in the South for accepting therapists that have extensive experience with trans individuals. I hope that I can use all of this experience I've gained, the positive and the negative, to help others that are in similar situations to myself and create something positive out of this.

SUMMARY

This is a full-time position that will provide a centralized leadership over our internal ERP (Epicor) development team, management of our ERP application, and the management and implementation of ERP process improvements and training initiatives.

 ESSENTIAL DUTIES AND RESPONSIBILITIES

 Responsible for the technical and procedural direction of our Epicor development team.

 Responsible for transforming legacy processes to benefit from system functionality, improving the efficiency, effectiveness and data integrity of our Epicor  10 system.

 Interface with internal users throughout the discovery and scope definition process and develop business case justifications for the implementation of process improvement ideas.

 Develop and manage project timelines and resource schedules.

 Manage and resolve cases opened with software support for our Epicor ERP platform

 Manage the internal Epicor Team’s training curriculum and schedule.

 Manage End User Process (EUP) training design and implementation.

 Establish and maintain written and in-person communications within the organization’s executives, department heads and end users regarding pertinent Epicor activities.

 Keep current with the Epicor updates and revisions.
