Health System began exploring telemedicine as a way to connect its physicians and specialists with patients
In 2005, Tift Regional Health System began exploring telemedicine as a way to connect its physicians and specialists with patients in the rural area surrounding its Tifton, Georgia, home base.
At that time, telehealth technology largely consisted of a hub-and-spoke network, based out of large tertiary care centers or academic medical centers.
"We understood [telehealth] was the future and we needed to be a part of this technology that could get our patients to the specialists that they needed to see 200 or more miles away," said Jeff Robbins, MD, director of telehealth and neurodiagnostics at Tift Regional Medical Center.
The virtual visits idea was starting to be discussed in rural parts of the country. The Internet was slow, but the tech was getting close to making distant encounters possible.
"In the early days, every encounter was basically a telehealth network within itself," Robbins said. "The technology only allowed us to connect to one endpoint at a time. The technology didn't allow us to network to a new endpoint or customer without a lot of IT involvement. Internet was slow and the devices used to conduct a patient-to-provider encounter were primitive compared to what we have today."
These issues prevented Tift Regional from achieving the outcomes it knew were possible but staff understood, given its track record at other hospitals, that telehealth could play a very important part in delivering healthcare in the near future.
Tift at that point partnered with the Global Partnership for Telehealth, a nonprofit with a 12-year track record in developing and implementing sustainable, cost-effective telehealth programs.
The Global Partnership for Telehealth markets telehealth systems to hospitals and other medical facilities in 11 states. There are a variety of telemedicine technology vendors with varied offerings on the market. These include American Well, Avizia, Cisco Systems, HealthTap, InTouch Health, MDLive, SnapMD, TeleHealth Services and Tellus -- many of those are in the Healthcare IT News Buyers Guide: Comparing 11 top telehealth platforms.
GPT's network of caregivers and its technology gave Tift Regional the ability to connect to nursing homes, school clinics, emergency rooms, stroke teams, specialized wound care teams and advanced critical care teams hundreds of miles away from its rural location in South Georgia.
"I like to say the miracle of telehealth is that it gives us the ability to erase time and distance," Robbins said. "Our patients benefit with virtually no travel time or expenses, decreased time waiting for an appointment, reduced medical costs, and extra value to the patient encounter and extended access to consultations with specialists not offered in their area and usually hundreds of miles away."
The partnership with GPT also allows Tift Regional's employed physicians to increase revenue because they can see patients outside their area, reducing missed appointments, and giving them the tools to treat more patients over time and have better patient follow-ups that improve outcomes, which also cuts down on readmissions, he added.
Telehealth carts generally include a monitor, camera, keyboard and remote control. Peripherals give physicians the ability to monitor vital signs, use a digital stethoscope, and use high-definition cameras for specific types of care such as dermatology or wound care.
Telehealth has become a critical component in Tift Regional's ability to deliver quality healthcare, and the healthcare organization has seen success in using the technology.
"Telehealth has increased access to healthcare within our organization by making it easier for our patients to obtain clinical services," Robbins said. "It also allows our hospital to provide emergency services that we cannot always provide like advanced/emergency stroke care. We have also seen an increase in improved health outcomes."
Telehealth allows Tift Regional to get its patients seen, diagnosed and treated earlier. This leads to improved outcomes and less costly treatments, Robbins explained.
"Telehealth has allowed us to have advanced ICU support and that has reduced mortality rates, reduced complications and subsequent hospital stays," he added. "We are seeing a reduction in healthcare costs through home monitoring, which is lowering costly hospital visits. Our stroke program is reducing the high cost of transferring stroke and other emergencies."
And Tift Regional has used telehealth to address the shortage in healthcare providers by allowing its patient population to see specialists outside Tift's area, also enabling Tift's own specialists to serve more patients, he said.
Before telemedicine, a virtual encounter meant both the presenter and the provider had to switch between many different programs. This presented issues when programs failed and data didn't link up correctly.
"The provider can now see who is waiting to be seen in the virtual waiting room, and data entry has been streamlined to allow patient data and notes to be uploaded into our existing EHR," Robbins said. "And maybe the best improvement is the ability to switch programs, going from Pathways to the stethoscope then the cameras within the same encounter."

Embattled sheriff got in way of son’s drug arrest

Embattled sheriff got in way of son’s drug arrest
A Georgia sheriff already under indictment for ordering a drug search involving nearly 900 high school students is back in trouble after allegedly interrupting a state investigator who was questioning his teenage son about dealing drugs.
Zachary Lewis Hobby, 17, was taken to his father’s jail and interrogated after he was arrested on Oct. 9. He faced charges of felony possession of marijuana with intent to distribute and criminal trespass, according to a document obtained by the Atlanta Journal Constitution.
That’s when his parents, Sheriff Jeff Hobby, 48, and Pam Hobby, burst onto the scene, according to the letter from Paul Bowden, Tifton judicial circuit district attorney, to Gov. Nathan Deal.
“Sheriff Hobby and his wife, who is also an employee of the Worth County Sheriff’s Office, barged into the room ostensibly to invoke the seventeen year old’s Fifth Amendment Rights for him (sic),” Bowden said.
Georgia sheriff indicted for conducting intrusive body searches
The younger Hobby had already been informed of his right against forced self-incrimination and had chosen to speak to the state investigator, Bowden said.
The investigator stopped interviewing Zachary Hobby after his father interrupted, Bowden said. Afterall, the state investigator was conducting the interrogation in Sheriff Hobby’s jail.
Bowden also noted that Hobby’s son was housed in his father’s jail instead of being transferred to another jail.
It is unclear how authorities will respond to Jeff Hobby’s interruption during his son’s interrogation.
Georgia deputies ‘cupped’ students’ genitals in drug sweep
The boy attended Worth County High School part of last year, but he wasn’t at the school at the time of the April search, Tommy Coleman, attorney for the Worth County Board of Education said.
On Oct. 3, Jeff Hobby was indicted on four counts because of his department’s drug search at the high school. Hobby was formally accused of one count of violation of oath by a public officer, one count of commission of crime with the offense of sexual battery, and two counts of false imprisonment, according to the charging documents.

365 Day Movie Challenge (2017) - #228: Heat Lightning (1934) - dir. Mervyn LeRoy
Out of the depths of a sea of subpar Warner Brothers pre-Code films viewed by me this summer, Heat Lightning is an electric flash of brilliance. Contrary to the usual snappy tempo of stories made by the studio during this period, Mervyn LeRoy’s crime melodrama draws its narrative out slowly enough to match the oppressive weather of the setting, a gas station/diner in the middle of a scorching California desert. The business is run by sisters Olga (Aline MacMahon in a rare leading role) and Myra (Ann Dvorak), a pair that immediately establishes Olga as the older, levelheaded one - she’s a better mechanic than anyone else living in the area, a fact that all of the film’s male characters acknowledge - and Myra as the romantic who has just reached adulthood and wants nothing more than to escape and find some excitement with a man.
On a boiling hot afternoon, the sisters service a number of vehicles: first, an automobile belonging to a married couple, Gladys (Jane Darwell) and Herbert (Edgar Kennedy); next, a businessman (Harry C. Bradley) giving a lift to two would-be actresses (Muriel Evans and Jill Dennett); thirdly, Mrs. Tifton (Glenda Farrell) and Mrs. Ashton-Ashley (Ruth Donnelly), a couple of gals headed to Reno for divorces from absent husbands. Olga and Myra offer a night’s lodging to the two married women and their chauffeur, Frank (Frank McHugh), but the action really starts when the gas station is visited by two bank robbers on the run, George (Preston Foster, convincing as a sociopathic) and Jeff (Lyle Talbot). As luck would have it, George and Olga used to be an item back when they both lived in Oklahoma. All of a sudden, we realize that tough, seemingly sexless Olga is a woman with a past and feelings that go far deeper than what Myra has been led to believe.
Aline MacMahon carries the role of Olga beautifully, portraying each complicated facet of the character with masterful skill. Ann Dvorak also does excellent work as Myra, proof that she was another of Hollywood’s most underrated talents. In the supporting cast, I especially enjoyed Willard Robertson as Everett Marshall, a local law enforcer who is friends with Olga and whose affection for her may also be romantic (although that is just speculation on my part). Besides the tensions present in Heat Lightning’s various relationships, the story also serves as an intriguing precursor to the film noir genre, utilizing the desert climate, the isolated environment of the gas station and the sexual tension/longing/desperation felt by the film’s female characters to create a mood similar to another pre-noir, Archie Mayo’s The Petrified Forest (1936), as well as one of the later classics of the true noir era, Tay Garnett’s The Postman Always Rings Twice (1946). (Bonus: Heat Lightning’s cinematographer, Sidney Hickox, also shot such notable noirs and noir-adjacent dramas as To Have and Have Not, The Big Sleep, The Man I Love, Dark Passage and White Heat.) For fans of pre-Code cinema, and particularly for fans of Aline MacMahon, Heat Lightning is a gem that deserves reappraisal.

The Bleachers: The Characters
Missy – Demands to be Co-Team Mother. A tiny pistol. Wants ‘what’s right’ for her daughter. Tough exterior. Ex-wife of Tifton, mother to Lil Tiff’, answers phones at the gas company.  (40s)
Tifton – Team Mother. Short ‘n stout. Turned his life around 11 months ago, in hopes to win back the love of his life, Missy, after he disappeared five years ago. Always between jobs. (40s)
Ashley – Attends the local beauty college. Tifton’s girlfriend he met at a church event. Has a smartphone glued to her face. Not a sharp tool. (20)
Wanda – Runs Concessions. Tough. Mechanic. Deb’s wife & Missy’s best friend. When she says ‘Hey’, you’re unsure if she’s greeting you or about to kick your ass. Mom to Butch. (40s)
Deb – Runs Concessions. Pipsqueak. Keeps her mouth shut and stays busy so she doesn’t end up back in prison. Wanda’s wife. Mom to Butch. (late 30s)
Coach Jeff – Assistant Coach of the Honeybears. Okay at baseball in highschool, but when the one college offered him a scholarship, his mother talked him into staying home. Feels like it ‘just ain’t fair!”. The ballpark is his life. Stockboy at Walmart. Lives with his mother. (37)
Nubbin – X-ray Tech. Widowed. Nervous. Hesitates to speak up. His life is bringing his daughter to the ballpark and sipping a Dr. Wanda with Tooker at the Phone Pole. Natalie’s dad. (30s)
Tooker – Owns Tooker’s Towin’ & sponsors the Honeybears. Semi-good looking, with confidence for miles. Type that calls a cozy a beer titty. Missy’s side piece. (40)
Charlene – Cake Decorator. Her first season with the Honeybears. Observant. Thinks the ballpark patrons are toxic, but finds herself becoming the ‘therapist’ to everyone’s problems. Hank’s wife and Lil Lani’s grandmother. (late 50s-60s)
Hank – Insurance Salesman. Just wants to kick back and fit in. Feels Charlene nags too much, and he’s right. Can trail off into long, boring stories. Lil Lani’s grandfather. (late 50s-60s)
Janice – Arrogant. Goes head-to-head with Missy, but wants a piece of Tifton, or any other man. Awaits the next Newport News catalog in the mailbox. Knock off Coach bag. Jessica’s mom. (late 30s-40s)
