Signs and Symptoms Changes in vital signs . Headache . Facial droop . Ataxia . Dysarthria . Dysphagia . Incontinence or retention . Cushing's Triad . KEY POINTS Increased Intracranial pressure is a medical emergency and needs to be treated immediately. These symptoms all relate to the lack of blood flow to the brain, the symptoms may be specific to the location of the stroke. Cushing's Triad: widening pulse pressure, decreased heart rate, irregular respirations. Nursing Assessment Make sure their airway is clear . Assess vital signs . Respiratory assessment . Full neurological assessment . Glasgow coma . Lab values : Clotting factors Glucose KEY POINTS FAST: Facial droop, Arm numbness or weakness, Speech impacted, Time of onset, and duration. tissue plasminogen activator would NEVER be given with hemorrhagic strokes. Nursing Interventions Act FAST! . Monitor respiratory status and maintain airway . Continue to assess Ensure proper positioning . Ensure safety during meals . Encourage mild activity . Help them re-learn to use their affected side . Frequent repositioning . Develop a way to communicate . Medication management . Antihypertensives . tissue plasminogen activator.
