 Needle Tract Recurrence of Renal Cell Carcinoma after Microwave Ablation by Tella David in Journal of Clinical and Medical Images, Case Reports

Abstract
A 21 years-old male was complaining from; decrease in vision, fever, blurring and inability to open the eyes in the sun. The patient attended the Eye hospital in Gaza where Fundus photo and serological tests were carried out in the eye hospital and Islamic University-Gaza laboratories. The patient complains from blurring and retinochoroditis. The ophthalmological examination revealed hypertensive non-granulomatous panuveitis, retinal vasculitis with focus of retinochoroiditis with lesioned central area suggestive of ocular toxoplasmosis. The serological tests proved that IgG was high in the patient serum and recorded 54.50 while IgM was negative. The fundus photo showed a clear lesion. Ocular toxoplasmosis is present among patients attending Eye hospital. The clinical cases could be detected among patients complaining from retinochoroditis. Ocular toxoplasmosis should be considered, and more investigations are needed. Keywords: Ocular; toxoplasmosis; fundus; eye; Gaza; infection.
Introduction
Thermal ablation is commonly used for management of small renal masses. Tumor seeding due to needle manipulation during renal mass biopsy or thermal ablative treatment is very rare. Seeding upstages a curable, organ-confined disease into a complicated recurrent cancer. This condition is described in few case reports and has not been reported after microwave ablation [3]. We report a patient that underwent transhepatic microwave ablation of a right renal cancer and developed a tumor along the ablation tract.
Case report
A 75-year-old male with complex medical history including heart disease, hypertension, chronic kidney disease, and sleep apnea presented to our clinic with a 4 cm right renal mass discovered incidentally following CT imaging for unrelated abdominal pain (Figure 1). A biopsy of the mass revealed a Fuhrman grade 2 clear cell renal cell carcinoma (RCC). Treatment options were discussed and the patient elected microwave ablation therapy. This procedure was performed by a urology and radiology collaborative team.

Three PR -15 (NeuWave Medical, Madison, WI) microwave probes were inserted transhepatically into the upper pole renal tumor. Correct positioning of the probes within the mass was confirmed with ultrasound and abdominal CT scan (Figure 2). Immediate post procedure films showed complete ablation of the tumor and the adjacent cyst. Significantly, the entire trans-hepatic tract was ablated during removal of the microwave probes, as is our protocol.

Six months later, the patient underwent a surveillance CT scan which demonstrated no tumor recurrence (Figure 3). MRI imaging at year 2 demonstrated stable post-treatment changes of right upper pole kidney ablation, without evidence of tumor recurrence or metastasis.

Five years following ablation, the patient presented with gross hematuria and right flank pain. A CT urogram demonstrated a filling defect in the proximal right ureter. Additionally, there was an ill-defined hypo-enhancing lesion at the posterior right hepatic lobe with an adjacent small perihepatic soft tissue nodule, not present on prior studies (Figure 4). The patient underwent right ureteroscopy. Upon entry into the bladder, blood was effluxing from the right ureteral orifice. Ureteroscopy revealed old clot in renal pelvis, but no tumors or active bleeding. Urine cytology was negative for malignancy.

The patient was referred for ultrasound guided percutaneous biopsy of the liver mass. The biopsy revealed metastatic carcinoma, favored to be renal cell carcinoma. Metastatic workup with a chest CT demonstrated scattered (approximately 15) sub-centimeter pulmonary nodules suspicious for metastatic disease. A review of the CT showed 2 cm recurrence of the right upper pole renal mass. The patient has been evaluated by medical oncology and is currently on an active surveillance protocol for his asymptomatic metastatic disease.
Discussion
Percutaneous thermal ablation has been described to be effective in the management of appropriately selected cases of RCC. Few reports of metastasis in the ablation tract after treatment have been published [1, 2]. Metastasis following ablation is suspected to be caused by tumor spillage during cryoablation or during withdrawal of the probe at the end of ablation [3]. Accumulated anecdotal cases note that transhepatic access to the upper pole of the kidney can be performed safely in the rare cases where no other access is possible [4, 5].
Some reports suggest transhepatic radiofrequency ablation can be an advantageous approach for superior or anterior renal masses, especially in patients that are unable to undergo ablation in the prone position [4, 5]. The main disadvantage identified is the violation of the peritoneum, which portends increased risk of seeding. It is estimated that such seeding is a very rare, occurring in <0.01% of cases. These reports discuss tract ablation to mitigate this risk; however, the benefit of this approach is not well defined. To our knowledge, metastasis to the liver following a transhepatic approach has not been previously described after percutaneous microwave ablation of RCC. In the case of our patient, this was an anterior, superior mass. However, this recurrence occurred despite our careful ablation of the liver tract at the end of the procedure.
Conclusions
Seeding metastasis is an unfortunate and rare event, which can occur after percutaneous ablation of a renal mass. We caution all clinicians to avoid transhepatic access for percutaneous ablation of a renal mass, when feasible, to avoid possible hepatic seeding metastasis.

[I Have No Voice and I Must Scream]
Look at that cover!!! It’s so cute!!! The Panpandas are such an underutilized concept within Undead Unluck, I really hope we get to see more of them when Tatiana comes back. I feel a little bad for Chikara, though, he’s barely visible under Top and I had a heck of a time figuring out it was even him because I wasn’t sure what I was looking at until I realized we were seeing his bangs covering one of his eyes
I really hope that we’ll get to see the Under crew as Panpandas later too, it’s such a shame to exclude them. I imagine that we’d get that in a cover page with Bunny, though perhaps it could be Tatiana showing Billy her collection or Rip sewing them for Latla? There’s so many ways for this to go, it’d be such a missed opportunity not to! 
Man, I hope they make merch of these...
Oh, also, this is the third color page we’ve gotten in six chapters, currently averaging at one every other chapter for this year! I wonder if perhaps UU will be a bit closer to the front on the next shared cover...?
We cut to Southeast Asia in the country (?) of Buroja, which I had been unable to find when I was researching the South China Sea a few weeks back. I figured that perhaps it was an island nation that I just couldn’t find because it was so small or something, but no. The reason I couldn’t find it is because it isn’t real. Tozuka made it up, presumably to avoid stepping on any toes by referencing a real conflict, but in the process he stepped on mine by making me look a fool. Also, the name Buroja is apparently a reference to one of Tozuka’s favorite things, Gundam, as it’s an anagram for the Jaburo federation. According to David Evelyn, the wounded soldiers we see later even share designs with some pilots from Mobile Suit Zeta Gundam. David also realized that he had missed another Gundam reference much earlier in the series in Andy’s backstory, as the members of his gang were all named after  various Gundam characters! Man, I wonder if I’d be able to get away with making a bunch of Medaka Box references in my work...
We see Tella identifying an airdrop based on the sound, and he determines that they are under-armed. He reports this to Billy with the help of a map that seems to be made so as to allow Billy to read it through touch, confirming that Billy is already blind at this point in his history. I thought he would go blind during his tragedy, but I guess not. I wonder if that will be important, or if he was born that way? If he was born blind, that may explain why he’s such a capable fighter; rather than relearning how to operate without sight, he never learned how to operate with it and wasn’t hampered by the loss. Maybe, I guess we’ll see
We learn that this conflict isn’t exactly political in nature, rather it’s a treasure hunt, with various mercenary groups hired by presumably different nations to find an ancient superweapon: Disc.
As you may recall, Disc was the UFO that Gina was using to scan Lake Baikal to find a UMA back before Andy and Fuuko joined the Union, and ultimately the cause of her death when the on-board laser misfired as a source of Unluck. I had been under the impression that Disc, with its clear resemblance to Gina’s hat, was designed by Nico specifically for Gina’s use. When we later learned that Tatiana’s armor was simply called Sphere, I took that as confirmation that Nico just had terrible naming sense, but now it seems that Disc might actually be an Artifact. Perhaps it’s the ur example of a UFO
Tella worries about handing over Disc to whatever government their group, Horizon Balance, is working for, but Billy reveals that he intends to keep Disc and use it as a deterrent for future conflicts. The thing about mutually assured destruction, though, is that the world saw firsthand what nukes could do; if everyone believes Disc to be a myth, then they’d need to see proof before it can be a deterrent, so Billy’s belief that he’d never need to use it is perhaps a bit optimistic
Billy sends Tella back to camp and asks him to sing for the injured soldiers, as apparently he’s quite good at it, making his inevitable awakening as a Negator even more tragic. Also, I forgot to mention a couple of chapters ago that we got our first good look at Tella’s unmarred face, so I’ll mention it now: Tella’s a lot cuter than I would have guessed. It is 20 years earlier, so obviously he was going to look younger, but I didn’t expect him to have such a babyface. Hell, even Billy looks a lot softer, even though there’s very little different about his design. I think it’s that he doesn’t have that wrinkle under his eyes; what a small detail to much so much of a difference
So Tella sings for the soldiers and tells them he plans to keep all of them alive, but they chide him for being too softhearted to leave an injured soldier behind, which they cite as strategically unsound. Tella’s temper flares when they mention he got his softness from Billy, so I don’t think this is so much an insecurity of Tella’s, but rather a defense of Billy
The soldiers recite Horizon Balance’s oath, which states that their goal is to make the world fair and balanced (hence why Billy ends up becoming the vessel of Unfair), a goal that they are willing to die for if it means that their team can continue the fight. It’s really interesting that Billy was the leader of a group like that only to betray a similar group to lead a group that only has a concept of mutual benefit. It was already stated in the previous loop that Billy founded Under in order to force the Union to meet the conditions for Unfair, but this really drives home how much doing that must have gone against his beliefs and how difficult it must have been for him
Tella leaves the camp to keep watch, and Billy immediately identifies that he’s been crying. Tella accuses him of being able to see, but Billy states that it was clear from the sound of his breathing. I wonder if perhaps Billy’s superhuman hearing will somehow be relevant to Tella’s Rule, if Billy will be able to garner information from Tella that he can’t verbally share? Even if not, Billy and Tella really make an interesting pair: a blind man and a mute man. I’m sure that’ll make for some spicy fanfiction sooner or later
After taking over the watch, Tella realizes that the light airdrop he’d heard earlier was actually a decoy and much more heavily armed tanks are now on track to attack the camp. It is at this exact moment that Tella gains his Rule: Untell. I figured this would be the name after we met UMA Tell during the Ruin Arc, but I still feel like the name itself could use some work. Untold works well for the Rule, but Told doesn’t work as well for the UMA; Untellable also works, and I think that the UMA would probably still be Tell since I can’t imagine there’s a Believable and Avoidable when Belief and Avoid would make more sense
As I’m sure we all expected, Untell negates Tella’s ability to communicate with his body, hence why in ch. 120 we see him talking through a speaker hooked up to his throta and why in the modern day we would see him using speakers that plug into his forehead; presumably neither require any muscular movement, with the latter seeming to work on thought waves. I don’t know how the former would work, though, but the image of Tella with a tube jutting out of his bleeding neck illustrates a very painful process for learning how to circumvent his own “power”
We do see just after the fight with Summer that Tella can use his speakers to intercept attacks, so it’s clear that Tella 100 did figure out how to turn his obviously weak and restrictive ability into a strength and that the current Tella will undoubtedly do the same, likely without the same tribulations. My guess is that Tella interprets the oncoming attacks as incoming communications to him, negating them before they can hit his body. That might be a bit of a stretch, but I can’t think of a better way to describe that usage of his ability
Similar to Unrepair paralyzing a target that tries to stop their bloodflow, Untell paralyzes Tella whenever he tries to perform a communicative action such as pressing a button for Morse code or walking toward someone you want to talk to. It is only after Creed fires on the camp that any sound comes out of Tella’s mouth, the disbelieving and grieving whimpers not meant to reach anyone but only to react
On the subject of Creed, my roommate tells me that the tanks his troupe is using are Russian, implying that Creed may have been hired to find Disc by Russia, so I think there’s a good chance that Horizon Balance is working for the US, but that’s probably not an important detail right now
What is important is that the Union has arrived to defend Horizon Balance from Creed, deflecting enemy fire with Unchange and ready to return fire. I’m guessing that Void, who seems to be holding an attack stance, is meant to keep the enemy from scattering or retreating while Fuuko is going to fire Unluck bullets at the tanks. disarming them while not hurting the people inside. Also, Nico and Ichico can be seen retrieving the injured in the background, so they seem to have this situation pretty well under control
The question now is what role Isshin 12, Yusai and Sean are going to be playing in this battle. Perhaps Isshin made their body armor, Yusai will keep bullets from being fired and Sean will launch some kind of sneak attack? Again, we’ll see next week
Also, I know I pointed it out a few weeks ago, but the Union’s fatigues are so good; Gina in particular looks great in this shot. both because of her outfit and because of her stance. She looks so confident and cool, I love her so much
This chapter went a really long way for endearing me to Tella and continuing to endear me to Billy, I can’t wait to see how Fuuko convinces them to join her cause and how she gets Creed to become an ally rather than an enemy. Will Billy and Creed get their powers at the same time? Will Undecrease be the first victim of the new Unfair, or perhaps will spineless Sean end up viewing Billy as a threat and inadvertently give him Unseen?
This chapter was really good buildup for the coming fight, I can’t wait to see the payoff
Oh, you may also have noticed that I didn’t bring up 2003 again this time (not counting this paragraph). That’s because I’m starting to find it redundant; I’ve expressed my confusion plenty of times by now, and bringing it up every week is just as annoying for me to write as it probably is for you all to read it. For the time being, I’m going to have faith that it will be explained later as either a misunderstanding, events happening earlier like Sean’s birth, or as something more pressing than the majority of other preceding tragedies. Whatever the case, I don’t plan to mention it again until we know the truth for sure, but hopefully that will be sooner than later

I was tagged by @basmathgirl, thanks for thinking of me! :)
Relationships: Romantic? Nah.  But, I have the best found fam a person could ask for.
Break Ups: One LDR with a guy in high school.  It was amicable and we’re still friends.  Another in my 20′s that still aches.
Kids: No, thanks.
Brothers and sisters: 5 half brothers, 1 half sister, 1 stepbrother.
Pets: Guinan the tuxie cat and Mr. Handsome McMurderface the betta fish.
Surgeries: Carpal Tunnel in my right wrist, gallbladder removal.
Tattoos: One, a shark between my shoulderblades.  I want a lot more, but I’m too shy to go into tattoo shops...
Countries you’ve been to: I have never been outside the USA.  Maybe one of these days!
Been in an airplane: Yes, but only in the last few years.  I’ve been to Washington and North Carolina by plane!  I don’t like planes, but I like what’s on the other side of flights.  This body is not happy about long car rides anymore.
Been in an ambulance:  Yes, when I was little I had a trip in an ambulance after I had an allergic reaction to strawberries.  Another after a reaction to being force-fed orange juice all weekend.
Sing karaoke: Not technically, no.  I’m not against the idea, though!
Ice skating: No, my experience with rollerskates (not good lol) has given me the impression I should never be allowed on ice with nothing but blades on my feet.
Been on a cruise: No.  I would like to try it, maybe?  I worry, though, because I get sea sick.
Driven a motorcycle: No, that’s something I have no desire to do outside video games.
Ridden a horse: Yes, I did not enjoy it.  Horses are beautiful animals that I like observing with my feet on the ground.
Stayed in hospital: Yes, I stayed a couple nights around my gallbladder surgery.  It took every bit of my willpower (and some friendly convincing) not to rip out the IV and get outta there.  Hospitals freak me out.
Favourite fruit or berries: Honeycrisp Apples, Dark Cherries, Mangoes, Pineapple... Limes, Lemons... Most of which I have to be careful with because of a citrus allergy.
Favourite colours: Emerald green, dark purple, antique copper
Last text: “Woo!” lol
Coffee or tea: Oh, that depends on the circumstances... but, lately I’ve been drinking more coffee.
Favourite pie: Cherry streusel 
Favourite pizza: I am very fond of the standard pepperoni/red sauce/mozzarella ... but my digestive system is not.  So that’s a rare treat these days.  BBQ chicken is a close second.
Cat or dog: I am a cat person who also likes dogs.  I think if I had a house with a yard, I would adopt a dog.
Favourite time of the year: Autumn, when it’s just chilly enough for light layers.
Met a star: Yes!  In the past few years I’ve met: David Tennant, John Barrowman, LeVar Burton, John de Lancie, Jonathan Frakes, George Takei,  Nichelle Nichols, Wil Wheaton, and Brent Spiner.
Flown a helicopter: I have a memory of being in a helicopter as a kid.  I’m not sure if it was real.
Been on TV: Not that I know of!
Broken my leg: No, thank goodness.
Seen a ghost: Nah.  I was convinced as a kid that there was a ghost in the window of the house across from my grandmother’s house.  I would run like a maniac from the driveway to her house and back again, sure that if I didn’t look at it... it couldn’t get me.  In hindsight, it was probably a curtain. lol
Been sick in a taxi: I was very, very drunk.  But, I did not hurl.  I remember promising the driver I wouldn’t mess up his taxi.  That whole night was an interesting experience that I think I would be happy not to repeat. lol
Seen someone die: No.  I’ve had loved ones decline and die, but I was not there at the end.

CHLOE X HALLE - DO IT

Do it.
Leah Isobel: No matter how much music I listen to or how old I get, it never stops being thrilling to see young, talented artists level the fuck up. Chloe and Halle Bailey's debut record showed so much promise, but was a little too precocious and heady for its own good. But like many a pop record about doing things before it, "Do It" works because it focuses on immediate, physical pleasures: looking good, dancing with your friends, curving dudes, and singing flawless harmonies. [9]
David Moore: The squishy bass plays well against the airiness of the vocals, which evoke an earlier era of feather-light performances that are nonetheless commanding -- Cassie, Natasha, Christina Milian. Like a lot of Chloe x Halle, it feels out of time, carved into a little tucked-away corner of the pop mainstream, ghost music for phantom clubs. [7]
Alfred Soto: Turns out whispery vocals over trap beats is cool as hell, and Chloe x Halle are the duo for it. Desperation as a wink-wink performance. A breathy kiss of a single. [7]
Tobi Tella: "Keeping it cute" really is the best way to describe this: more mature and fun without getting rid of the duo's best qualities. Is it a little bit basic? Sure, but just because a certain lane of music -- light R&B about nothing in particular -- is overdone doesn't make it inherently bad. [7]
Nina Lea: On first listen, I found "Do It" pretty underwhelming--it seemed to have slightly less of both the inventiveness of other tracks on Ungodly Hour and the youthful sparkle of "Happy Without Me." But it's slowly grown on me, radiating a sense of clear self-possession and a soaring openness usually absent from songs about going to the club. [6]
Nortey Dowuona: A smooth, frosted synth is sprayed over the loopy, lurching bass and shivering percussion, which is paved over with heavy bass synths as Chloe and Halle glide across it. They twirl together as they leap atop the bass synth mixer and lean back and float right up. Then slick drum programming slides down over the synths and melts them while Chloe and Halle float down and walk away. [8]
Wayne Weizhen Zhang: "Do," "crew," "cool," "boo", "you", "mood," "cute, "ooh": individually, they're short, punchy words, but together, they culminate in a fluffy cloud of assonance so dreamlike you could practically float away. The rest of the song is just as clever and aurally pleasing: "Do It" is one of the best "good vibes only" anthem released thus far in 2020. [7]
