Phenylephrine is primarily used as a decongestant, to dilate the pupil, to increase blood pressure, and to relieve hemorrhoids.

Drug Interactions

Concomitant use of phenylephrine and non-selective monoamine oxidase inhibitors (MAOs) such as iproniazid and nialamid may result in paroxysmal hypertension, hyperthermia which may be fatal.

Due to the long duration of action of MAOIs, this interaction is still possible even after 15 days of discontinuation of the MAOI.

Dopaminergic ergot alkaloids such as bromocriptine, cabergoline, lisuride, pergolide may increase the risk of vasoconstriction and may cause a hypertensive crisis when administered with phenylephrine.

Vasoconstrictor ergot alkaloids such as dihydroergotamine, ergotamine, methylergometrine, methysergide may increase the risk of vasoconstriction and/or hypertensive crisis when co-administered with phenylephrine.

Tricyclic antidepressants such as imipramine when co-administered with phenylephrine may cause paroxysmal hypertension with the possibility of arrhythmias that is inhibition of adrenaline or noradrenaline entry in sympathetic fibers.

Noradrenergic-serotoninergic antidepressants such as milnacipran, venlafaxine may also cause paroxysmal hypertension with the possibility of arrhythmias with concomitant use of phenylephrine.

Selective type A monoamine oxidase inhibitors (MAOs) such as moclobemide, toloxatone may increase the risk of vasoconstriction and/or hypertensive crisis when administered with phenylephrine.

Linezolid on concomitant use with phenylephrine may increase the risk of vasoconstriction and/or a hypertensive crisis.

Guanethidine and related products may cause a substantial increase in blood pressure when co-administered with phenylephrine (hyper-reactivity linked to the reduction in sympathetic tone and /or to the inhibition of adrenaline or noradrenaline entry in sympathetic fibers). If the combination cannot be avoided, use with caution lower doses of sympathomimetic agents.

Cardiac glycosides such as quinidine may increase the risk of arrhythmias when co-administered with phenylephrine.

Halogenated volatile anesthetics such as desflurane, enflurane, halothane, isoflurane, methoxyflurane, sevoflurane may increase the risk of perioperative hypertensive crisis and arrhythmia when co-administered with phenylephrine.

The effect of presso-active sympathomimetic amines (Oxytocic agents) may be potentiated when co-administered with phenylephrine. Hence, few oxytocic agents may cause severe persistent hypertension and strokes during the post-partum period.

We all know that drugs should be taken strictly as prescribed, both in terms of dose and time, as well as nutrition during treatment. These are some combinations of foods and medicines that should not be taken together.
