Hypermobile Sacro Iliac Joint
A hypermobile SIJ is an unsupported SIJ. This is usually a result of excessive tension/fixation of the opposite SIJ. The muscles that support the integrity of the SIJ are The Quadratus Lumborum and Glut Min Med and lateral branches of the Glut Max. They intersect by way of a fascial continuing and muscle kinetic chain sequence. There is a front and a back to the muscles that have shortened to cause this condition to persist. Today we’ll show just the back portion.
My client had been engaging some injections to stabilize her left SIJ. The muscle co-contractions across her hips as a result of years of living with this instability were a sufficient barrier to her ability to respond positively to this intervention. The PT thought that the issue was weakness in the right Glut Medius, but the greatest fixation was medial in the area of the origins of the external hip rotators.
Here in this video is a typical neuromuscular facilitation/inhibition pattern of the posterior muscles that are a major part of this persistent left SIJ instability.
The Glut Med was indeed inhibited but so was the left lateral stabilization.The primary ‘compensation’ muscle was the right Piriformis. But the intense fixation of the Rt Piriformis compensating for a weak Rt Erector Spinae muscle caused a persistence of the lateral instability of both SIJ’s.
From an NMR perspective one sees the reduction over a series of sessions of the volume of the imbalance. There are layers of muscles involved.
Both the Rt QL and Rt Piriformis were inhibiting the Rt SIJ
But once that issue was addressed, the Rt Piriformis was STILL inhibiting the Rt AND Lt lateral stabilization. The lack of lateral stability causes the medially anchored external rotators to respond first when using the hips. One cannot simply consciously eliminate the stored and saved motor strategies programming into the Cerebellum. This is where NeuroMuscular Reprogramming has a big part to play.

HYPERMOBILE SACROILIAC JOINT PT 2 IN Part 1 We looked at the fixated muscles in the extensor compartment (particularly the right hip), that are contributing to the lack of functional support for the left SIJ. At the end of our short demo we saw no further inhibition in the right hip. The PT was focusing on conditioning the weak lateral muscles in the right hip, but it's the LEFT SIJ that's unstable. The next step in correcting this left SIJ pattern is to test the relationships between the hip flexors and back extensors and lateral stabilizers. Here's what we found... There is indeed a tightness on the right contributing to the weakness on the left. This pattern is exceedingly common. Either Iliacus can torque the pelvis and weaken the back extensors. Testing the back extensors, hip extensors, QL for side bending and Glut Med/Min Lateral stabilization and SIJ stabilization revealed and the right Iliacus muscle AND the Rect Femoris were contributing to the persistent weakness of the left SIJ. Whenever your client is exhibiting persistent weakness in the back muscles, ALWAYS LOOK TO EXCESSIVE SHORTNESS IN THE ILIACUS. Even if you don't know muscle testing if you notice a tight Quad and Hip Flexor, you can help the back by lengthening the front of the hip joints. Passive stretching is not as good as tissue lengthening followed by a regime of stretching. The challenge with stretching tight muscles is that they are short fo a reason. Transient synovitis is a diagnosis of exclusion for acute hip pain in children. It has largely been associated with viral and traumatic etiologies despite most of the cases being idiopathic in nature. The purpose of this study is to present a patient with transient synovitis that also demonstrates Sacroiliac Joint dysfunction. Established Sacroiliac Joint (SIJ) manipulation technique was used to resolve his symptoms. The patient in this study displayed physical and radiological finding consistent with transient synovitis of the hip associated with SIJ dysfunction. We theorize that mechanical joint dysfunction of the SIJ may lead to changes in the tension-length relationship of the iliopsoas muscle predisposing the hip joint to tendinitis and bursitis. This results in the clinical presentation of transient synovitis of the hip. Our patient was successfully treated with SIJ manipulation. This study emphasizes the importance of a thorough spinal examination in patients presenting with transient synovitis to rule out a mechanical pathology. Our case illustrates the efficacy of spinal manipulation in treating SIJ dysfunction. We propose that this evaluation and treatment option should be considered with patients diagnosed with transient synovitis of the hip.
