Two Types of Mountaineer

A Fellow of the American College of Osteopathic Emergency Physicians, John J. “Joe” Fosbinder, D.O., serves as an emergency physician at Bakersfield Heart Hospital in Bakersfield, California. He is also an adjunct assistant professor of emergency medicine at A.T. Still University’s School of Osteopathic Medicine. An avid outdoorsman, Dr. John Fosbinder loves mountaineering and is currently pursuing the “seven summits of mountaineering,” climbing the highest peaks on all seven continents.
Mountaineering is far more than just walking to the top of a mountain. It can require the use of technical equipment to climb in difficult terrain. There are two styles of mountaineering: alpine and expedition.
Alpinism was developed by alpine guides in the 1970s and is extensively applied to low-level ascents at altitudes below 3.1 miles (5,000 meters). The idea behind alpine climbing is mobility. Individual climbers or members of a team conquer terrain with speed and competence. They keep their baggage light and carry only minimal protective gear. The guiding principle for every climb is “fast is safe.”
Expedition climbing is similar to the way the military conduct exploratory ventures. It is also referred to as high-altitude mountaineering, involving altitudes above 5,000 meters. The expedition comprises the establishment of a sequence of camps where the climbers spend time familiarizing themselves with the routes to take and getting acclimated to high-altitude conditions. Sometimes, the expedition team hires locals to serve as porters to carry some of their heavy baggage and equipment.

Compassion Fatigue and Burnout
Not everyone has a family member in the medical field to guide them on their career path.  Fortunate college students sometimes have eye opening opportunities shadowing healthcare professionals around the office, operating room, or hospital.  This experience can be like a catalyst in a chemical reaction.  First, the student is excited about seeing what direct patient care is all about.  Second, the student can ask the mentoring healthcare professional how he or she likes practicing medicine.  Sometimes the professionals respond with just the resonating words to motivate someone for a lifetime.  “Medicine is so rewarding.”  “I love what I do and I wouldn’t have it any other way.”  “I would do it all again.”  So what happens if you ask your mentor, “Should I go to medical school?” and the response is, “Nah, become a plumber.”  The answer is not what a bright-eyed, motivated student wants to hear.  
There is a fine line between the terms compassion fatigue (CF) and burnout. The medical community has members which combine compassion fatigue and burnout and those which divide them into two separate entities.  Merriam-Webster defines compassion fatigue as 1) the exhaustion and emotional withdrawal experienced by those who care for sick or traumatized people over an extended period of time, and 2) apathy or indifference towards the suffering of others as a result of overexposure to tragic news stories and images and subsequent appeals for assistance.  In contrast, they define burnout as exhaustion of physical or emotional strength or motivation usually as a result of prolonged stress or frustration.  According to the American Institute of Stress (http://www.stress.org), CF is “emotional residue or strain of exposure to working with those suffering from the consequences of traumatic events” while burnout is a “cumulative process marked by emotional exhaustion and withdrawal associated with increased workload and institutional stress.”  https://www.tendacademy.ca/what-is-compassion-fatigue/ says that CF is an “erosion” while burnout can be “fairly easily resolved” by changing jobs.  If only the differences were that easy to pinpoint.
After doing some research of my own, I found out that the main question is related to the reversibility of the condition.  Can CF be treated?  Is burnout curable?  
The average emergency room physician is said to “burn out” after 10 years of practice.  “Put simply, compassion fatigue happens when individuals become too physically, emotionally, and mentally exhausted due to the demands of the job.” (https://www.edumed.org/resources/compassion-fatigue-online-guide).  In my Welcome post, I introduced Dr. Mike Drummond, a family physician from a multigenerational medical family who practiced for 10 years and then had to leave medicine due to burnout.  In his case, it seems the condition of burnout was irreversible and he had to reinvent himself.  Fortunately for medical professionals, he has dedicated his career to helping others with reinvigorating their careers.
Even experts in the medical field tend to lump the terms CF and burnout together.  According to Dr. Mary Jane Esplen from the University Health Network (UHN) of Toronto, CF can be characterized by “guilt, loss of sleep, low energy, difficulty concentrating, and feelings of detachment.”
Other signs of burnout besides exhaustion can include cynicism like the doctor who would have rather become a plumber, lack of efficacy, desperation, internalization (feeling useless or meaningless), and overworking to name a few.  
Esplen explains that healthcare professionals are at a higher risk of developing CF due to lack of communication and support.  She says that education and awareness are the keys to prevention of CF.  Dr. Gary Rodin, also from the UHN, says the solution for CF is connecting emotionally with patients and showing emotion, closeness with patients that has been stigmatized in the past. Part of the terminology issue is a fear of judgment.  The term burnout could have negative connotations.  Well-being is a positive term that is now being used to highlight prevention of these potentially irreversible conditions.  The well-being and work-life balance of healthcare professionals is coming to the forefront of issues in today’s fast-paced medical environment.  Recently, the executive director of the Osteopathic Physicians and Surgeons of California (OPSC), Kathleen Creason, left her position of 15 years to lead the American Osteopathic Information Association (AOIA) in their taskforce on physician burnout prevention and treatment.
Speaking of prevention, http://www.stress.org says that even Mother Teresa, famous for her lifetime of nurturing the sick, mandated that nuns take a year of sabbatical every four to five years for self-care.  Do healthcare professionals need to do the same thing?  With the need to keep practice skills active and for so many to pay student loans, how can a healthcare professional follow Mother Teresa’s advice?  It could also be difficult to convince someone so used to working so hard that taking time off from a busy practice could lead to individual resiliency in the long run.  
I would not say that I am deterred from wanting to become a healthcare professional, but it is important for anyone with a desire to become a healthcare professional to be aware of all of these conditions.

Always a great time @mihavanacigars in the Antique Row and Arts Colony area of Pomona, California. Master Roller from Cuba, Raul Reyner is the best cigar roller you've yet to hear about. His 7K Cigar was rated 97 points and earned Boutique Cigar of the Year by @cigarandspiritsmagpublisher magazine. Raul, by the way, is in the @guinessbookofworldrecords for rolling the world's longest cigar! This is a hidden gem. Awesome peeps, a cigar lounge in the cut managed by my boy FONZI whose customer service and hospitality is second to none!
