Would you write an Astarion x Ftm!reader? I really wish there were more fics/imagines with inclusive readers because gods that man is queer
Absolutely!
I’ve had an idea of a post top-surgery trans man for a hot minute, so here is that story!

“How Did You Get Those Battle Scars?”
Tumblr media
As the night fell into silence, and every one of your companions were asleep, you noticed that tonight was going to be one of those sleepless night. In the thick, hot, summer air, you tossed and turned in your tent on your cot. You wondered if anyone was able to sleep in such heat.
Not being able to bear it any longer, you decided to venture out into the night towards the river. Perhaps a dip in the water would cool you down enough to sleep better. At the river’s edge, you laid your clothes. Shirt, pants, underwear, all neatly laid together on rocks.
You dipped your toes in and walked in until the water was up to your waist. You closed your eyes, allowing the moonlight to shine onto your skin. Onto your face. The sounds of nature allowed you to relax. You play in the water, gently running your hands through it. The water felt nice for such a warm night. To completely rid of any heat, you dunked your head underwater. Shortly after, you headed out.
You looked back towards where you laid your clothes. The only thing there now were your underwear and pants. Why was your shirt gone? You looked around, but couldn’t find it. You noticed that Astarion’s tent was now lit up with a source of light. Candles, perhaps. You walk up to the tent, and made your presence known quietly, as not to disturb the others.
Quickly, you were drug into Astarion’s tent by Astarion himself. He reacted quickly, as if he was expecting someone else to be there. You noticed quickly that he was holding a blade to your throat, and you widened your eyes as you saw his sharp teeth, which could’ve been considered blades by themselves.
Astarion quickly realizes it you, and chuckles to himself. “Oh, haha, it’s you.” He sits across from you, and after shaking off any remaining fear, you ask, “You wouldn’t happen to know what happened to my shirt, would you?”
“Hmm, no, darling, in fact, I was in my tent all night,” he slyly smiles at you. You know he’s lying, so you start looking around for it yourself. You are in no mood to be played with, but Astarion likes seeing you frustrated. He notices the scars on your chest, and thinking nothing of it, he asks, “How did you manage to get those scars?”
“Do you pry this much,” you ask, annoyingly. Astarion gives out a chuckle, “Not every night, but I feel like it now. So, how did you get them? Hm?” As he asked the question, he stood up and took out a piece of cloth out his back pocket. It was your shirt. You noticed out the corner of your eye, and you tried to lunge for it. Astarion dodges your attempt at retrieving your shirt, and says “ah, ah, ah! Tell me more about those scars, and I’ll give it back.”
“If you think im embarrassed of my scars, I’m not. I just want my shirt.”
“If you’re not embarrassed, then you won’t mind me asking how you got your scars, my boy,” he said cunningly, making you blush. Your flushed cheeks make him giggle, and he sits back and relaxes while you stand in a frustrated fluster. Astarion doesn’t look away from you, and stares you down, barely even taking time to blink.
Eventually, your frustration subsides, and you sit down in front of Astarion. “I guess you could say they’re battle scars,” you begin, allowing Astarion to ask any questions he wants.
“Battle scars? That’s all?” He huffs, “I thought it would be more interesting.”
“Yes, we’ll, not just any battle scars. I guess this kind of battle isn’t fought often.”
“Go on.” Astarion’s attention was caught. You stretch out your torso, so the scars are more visible. You explain your story.
“From a young age, I knew I was different. As I was growing up, I was raised as a girl. My parents treated me as such, but deep down, I knew I wasn’t. Then I became a teenager, and soon, my body didn’t match how I felt about myself. It took a long time to fight that battle within myself, and for me, I finally won when I could take control of how my body looked.”
Astarion listened intently, grasping every word you said. “I see. Mental battles are sometimes harder to defeat than physical ones. I commend you for that.”
“Thank you.”
Astarion looks down at the shirt he holds in his hands, and slowly lifts his hands to give it to you. You look back at it and let out a small chuckle.
“Keep it.” You say as you stand up. He looks at you, shocked. “You want me to keep your shirt?”
“Yes. I have more. You can keep that one.”
“But.. what if the others question you?”
“Pft,” i scoffed, “If the others question me, I’ll give them answers. Remember, I’m not ashamed of my scars. I’m not ashamed of the battles I’ve won. Scars are trophies I wear to show that I could overcome. I’m not scared.” You leave Astarion’s tent, and go back to your own. You lay down in your cot, ready for sleep to take you under its wing.
The next morning, you awaken to the sun rising. You step outside, and you see Astarion, wearing your shirt. You smile to yourself, and find a folded up note and a shirt at your feet. You read the note, and it says:
“Dear, my boy, y/n. Your outlook on battles has inspired me, both mentally and physically. Scars do not define who you are, but they can define what you’re capable of. The pain and suffering you must’ve endured had to be far harder to heal than the scars you’ve obtained. I commend you for that. Take this offering of gratitude. It’s one of my shirts. I hope to inspire you the way you’ve inspired me.
- Astarion”
You fold the letter and put it with your belongings. You put on the shirt, which is a bit big for you, but you don’t mind. You step back outside and see Astarion look at you. You both smile at each other, and you now know that you can inspire almost anyone, including someone 200 years older than you.

The Brink on The River Bank
(Not Requested)
Tumblr media
Disclaimer: this story may have some lore not relevant to the lore of BG3 and/or D&D. I have not completed BG3, but figured this would be a fun story to write anyways! I hope you enjoy.
As my wound worsened by the hour, I could feel myself getting weaker, and with no one around, I knew death was certain. I clutched my wound with both my hands, holding onto my side where I had been cut deeply. I wasn’t doing anything wrong, but just happened to be at the wrong place at the wrong time.
The sound of a river nearby. I knew if I were to die, I’d want to die somewhere peaceful. Somewhere quiet. Somewhere where I could gaze upon the stars one last time. As I approached the river, I collapsed onto the ground, and couldn’t find the strength to get back up. I had to crawl the short distance to be beside the creek.
I laid on my back, allowing my hand to touch the water, washing off my bloodstained hands. I could feel my eyelids droop, I could feel death approaching me slowly, like an animal hunting it’s prey. Slowly, quietly, then it pounces. I was going in and out of consciousness, and soon, found myself in a world of dreams.
My dreamscape had been interrupted by a sharp pain in my throat. It was almost painful enough for my adrenaline to kick in so I could fight. But I couldn’t. The initial pain was the worst part. Now, I hardly felt a thing. I allowed myself to relax, believing that some animal was now eating me alive, but soon, everything would be over.
To my surprise, I woke up the next morning, under the shade inside a tent. In my confusion, I set out to discover where I was. As I went to open the tent’s door, I heard a voice, strictly telling me to not touch it.
“Ah, ah, ah!” The white haired man started.
“Where am I?” I stuttered, “Who are you?”
“Now now, let’s just take a seat and I’ll explain everything,” the man said. I took a seat the farthest away from this stranger I just met. He sat opposite of me, his eyes almost peering into my soul.
“First of all, my name is Astarion. Second of all, last night, I found you. Just by the riverside. You were almost dead.” That’s when I noticed I had no pain where my wound was. I had almost completely forgotten about it. I lifted my shirt to take a look, and it was gone. Healed.
I was amazed, and I looked at Astarion. “What happened to me? I thought I was as good as dead?”
“You would’ve been, had you not placed your hand in the water.”
“Wait, I don’t understand?” I said, confused.
“You placed a bloody hand in the river, and as it washed off, the blood went down the river. I found you that way, by following the blood trail in the river. It les me straight to you,” he let out a small chuckle,” you didn’t even flinch when I bit you! Ha!”
“Wait, that was… you? I felt it, but went numb shortly after.”
“Yes, we’ll, darling, you were almost dry. You were bleeding out so much, I’m surprised you even made it as far as you did.”
As he spoke his words, I took in all his features. His white hair, his red eyes, his pointy teeth. A vampire. This only triggered me to start exploring my own features, mostly by feeling my teeth with my tongue. Two sharpened canine teeth sat in the front, and I looked around for any reflective surface.
In almost a panic, I get up and try to find a mirror or spoon or anything I could see myself in. I was rummaging through anything Astarion had, when his footsteps kept getting closer. He held out a mirror to me, and I took it from his hands. I peered into it hesitantly, but was shocked when I could see everything behind me, but I couldn’t see me.
“Did you turn me into a vampire?” I asked hesitantly, my back facing him, my arms to my side. Slowly, I turned to face him.
“Yes, I did, darling, and I must say, you make a handsome vampire at that. Your eyes are the most astonishing shade of red I’ve ever seen.”
“Did you save me?”
“I guess in some ways, yes. But if you’re asking if I wanted to change you, the answer is also yes. When I smelled your blood, I traced it to you. I was hunting alongside the woods when the smell started coming down the stream. It was the most delicious smelling blood I have ever had the pleasure of smelling.”
I listened to him tell me how he found me, how he saved me, and how he carried me back to his tent. He explained that he stayed by my side while I was turning, and he was preparing to tell me about everything once I woke up. He carefully sealed up the opening of the tent so no daylight could reach me, and so I wouldn’t accidentally burn myself when I awoken scared.
He explained how he had his mirror ready, so I could understand quicker, and he brought me back up to date. At this point, I felt a deepening burning in the back of my throat. I started to feel weak again, throughout the day. And as night came, Astarion helped me hunt for the first time.
He taught me how to be stealthy on my feet, as to not scare off prey, and how to be persuasive and confident. I hugged him after our first full day together, and thanked him for saving me. To my surprise, he hugged me back, and assured me that I would always be welcome if I needed a friend or any advice, although I don’t plan on leaving him any time soon. I’m glad that now, I have a second chance at life.

Fighting with Myself
Warnings!: Fluff, unsafe binding practices, readers chest is referred to as “chest.”
Tumblr media
I woke up steadily and slowly, trying not to rush through the morning. I woke up pretty early, so I figured everyone else was still asleep. I decided I would get ready and wait for everyone else. Although, I knew today would be a difficult one to get through. I could feel the dysphoria creeping in on me, making me clutch my chest closer to my skin.
I knew that before I even went out, I would have to bind my chest to alleviate any ounce of dysphoria, so I began looking, but they were nowhere to be found. I began tossing clothes, books, blankets, anything I could grab, I threw to the side. I began panicking. “Where the fuck are they?” I said in a frustrated whisper. Then, I saw a beam of light come from the front of my tent. Astarion was standing at the opening of my tent, looking down at me.
I held my chest closer to my body as my back faced him. He came in and sat down on my cot and smiled at me.
“Darling, what’s all this rummaging about for?” He asked, making gestures with his hands to represent all the noise I must’ve been making. I hung my head low, and felt the dysphoria hit me like a burlap sack filled full of rocks.
“It’s nothing, Astarion. I just gotta find something, but it’s not here. I don’t know where else to look…” I started moving things around again, trying to find my bindings.
“Try to retrace your steps darling, did you happen to go somewhere unusual last night? Somewhere you normally didn’t go?” Then the light came on in my head. I had actually taken a bath in the river, and took my bindings off there.
In my discomfort, I could hardly tell I was close to crying. I knew I couldn’t go out of my tent in these conditions.
“Will you please search the riverside for any thin strands of fabric? I need those. If you can’t find them, try looking again… I will not survive the day if I don’t have them…”
“Of course, darling, I will go look.” I sat on my cot, trying to calm myself down, trying to think of something else besides how heavy my chest felt on my body. How much of a burden it made me. Having to bind for hours, and it being difficult to breathe.
Eventually, the doorway of my tent opened, and Astarion held my bindings in his hands. I quickly grabbed them from him, face away from him, and wrapped them around my chest. I was struggling with them, as they kept getting twisted and they were knotted.
Astarion came up behind me, and with a gentle hand, began helping me get them on. I felt my cheeks blush towards his act of kindness. As I tucked in the last bit of wrap, I faced to turn Astarion.
“Sorry about the noise earlier… without these, today would’ve been a terrible day for me,” I tried explaining.
“So, what’s the bindings for, darling? Are you hurt?”
“Oh, no! There’s no easy way to say it,” I sat down and hesitated to let him know I was trans, but I explained the bindings truthfully anyway.
“Sometimes… Well, all the time, I don’t like how my skin feels. Especially around my chest. So I bind it, to make myself appear more masculine. I’m trans, Astarion. I understand you may not understand it, but that’s why.”
Without hesitation, Astarion said, “I’ve been on this mortal plane for 200 years, and you think I don’t know what trans is,” he let out a small giggle,” trust me, my dear. When you live 200 years, you meet a ton of people. Of course, I understand what being trans is. I may not understand what it entails, but I do know that your condition is highly uncomfortable. Plus, I knew from the moment I met you. Not that you were trans, but that you were a man.”
His last sentence hung with me for a moment. Not that I was trans, but that I was a man… Astarion saw me as a man. His words completely alleviated any amount of dysphoria I was feeling. I felt very confident after he spoke to me. I had hardly noticed the tears forming in my eyes, but Astarion held out his arms, and allowed me to hug him, crying into his chest.
“You’re a handsome man, darling. Everyone else thinks so too.” He stroked my hair, and from that moment on, I knew I could take on any fight or any uncomfortable feeling happening within my own mind, body, and soul.

I saw your post asking for BG3 imagines and I was hoping for a Transmasc reader who just got top surgery getting some love and support from Astarion. I'm hoping to get top surgery soon and am dying to see some sweet nurse Astarion. I'm fine with both SFW and NSFW, whatever you'd prefer!
Thank you in advance!
Oh. My. God. I love this trope 😭 here we go!

The medics were wheeling me back to my dedicated room, meant for my recovery. I felt cold, but that’s about all I felt. I was pretty numb, and I felt no pain. My head felt woozy, and my vision was distorted. Almost as if seeing double. I kept my eyes closed for the most part, due to the sun coming in through the windows and also being exhausted from the surgery I had undergone.
Astarion was waiting for me, and as I saw his white hair in my blurred vision, I gave him a small smile. I was wheeled into my room, and the nurses talked to Astarion for a little while. I couldn’t hear a word, as all my senses were distorted. The nurses left soon, and Astarion stood by my side.
“Hello, darling, how are you feeling?” He asked me, moving my hair from out of my face. I smiled and gave a small chuckle, and under a woozy influence, I replied, “I’m goooooood.” I heard Astarion’s giggle at my elongated word. “I feel funny,” I said. “I know, darling. It’ll go away soon,” Astarion spoke in a soft voice. Over the next couple hours, Astarion heard me talk nonsense, and I fell asleep for a few hours.
Once I woke up, the anesthetic wore off, and I could feel the pain in my chest. It was as if I had been hit by a boulder. I winced in pain, which caught Astarion’s attention from the book he was reading. “Oh dear,” he muttered to himself. He rushed over to me as quick as he could, and reassured me.
“Everything will be okay, y/n. Don’t try to move too much, now darling.” Astarion went to look for a nurse to tell them I was awake. Some nurses came in, and they explained that I would need to walk soon, just to get some movement in so bedsores wouldn’t develop. I expressed that I was in pain, and they gave me some medicine.
Astarion and a nurse helped me sit up, and get to my feet, which wasn’t exactly painless, but I expected more than what I was subject to. I was standing, Astarion and a nurse holding onto either of my arms. As I looked down, I noticed my toes. That’s never happened before. My chest had always blocked the view. This new view made me feel so euphoric and made me forget all the pain almost, then came the first step.
Left… Right… Left… Right… We walked down the hall slowly, my arms bent like a dragon’s or a dinosaur’s. My breathing was shaky, as each step caused a bit of discomfort. We did this every day for about a week, then the time came for the reveal.
The day of the reveal, I was able to get out of bed by myself, and I had been able to be more mobile, although my arms still stayed in the dinosaur position for most of the time. A nurse came into my room, and helped me get the bloody bandages off, and then, I was standing in front of the mirror.
I hesitated to look at myself, but I wasn’t sure why. Was I scared? Was I soaking in the moment? My life had been changed, and this will have a lasting impression on me for the rest of my life. I took my time to breathe, and then I opened my eyes. I saw the scars first, amazed at how straight they were with the bottom of my pec muscle. They were slightly coated with dried blood, but from what I could see, you could barely notice them.
I breathed a sigh of relief, amazed at the resources people were able to develop. I looked at Astarion through the mirror. He was smiling, almost crying from happiness for me. He had known I wanted this for a long time, and now it’s finally here. Him and I were both in shock, but in a positive light.
“Wow,” was the only thing I could muster. “This… this is me…” Astarion looked at me in the mirror, and placed his hands gently around my waist. “Such a handsome man,” Astarion said, making me blush. Tears welled up in my eyes, and I looked away from the mirror. I was so happy. I tried not to cry too hard, my scars still hurt. Astarion let go of me and watched me through the mirror, and I turned to the side to see how flat I actually was.
I started crying even more. All the dysphoria was gone. The only thing that was left was euphoria, and I was glad to share this with my best friend. My partner. Astarion. Eventually, I had to put the bandages back on, and I needed to rest. Throughout my healing period, Astarion helped me clean myself, he prepared me food to eat, and he helped me change my bandages.
Astarion helped me gain my strength back, and before we knew it, I was my normal self, able to do normal activities. I was able to walk outside without a shirt, and that was when I finally felt peace. I finally felt at home. That summer, Astarion and I swam almost every night. I could finally sleep without feeling dysphoria, and I was stronger than I ever was before. Nothing could hold me back now.
—
A/N: I love this imagine a lot, and I hope you do too! I’d love to have Astarion take care of me after top surgery 😌 I hope you can get top surgery soon!!! Let me know how it goes! :D anyways, I hope this was entertaining! I’m gonna try to work on more dialogue between characters in the future. ♥️ Thank you for this request!!’
