The way they spend the WHOLE episode with The Doctor wondering about his place in River's life... she has other husbands... she had at least 2 wives?? She married a diamond. She said the easiest lie you can tell a man is that you love him bc they'll believe any story where they're the the hero.... he implied he wasnt important to her and she went along with it (with TEARS in her eyes).... he has the AUDACITY to question if she really loves him. And then when it comes down it. She professes her love for him when she thinks he's not around to benefit from it and he finds out that she thinks he doesn't love her. And that hurts him even more. To her it is the most obvious thing in the world that she loves him and to him it is the most obvious thing in the world that he loves her.
