"There Won't Always Be a Black Midwife to Overhear" Systemic Racism in Canada's Sexual Health and Reproductive Medical System

#8 Maternal medical mistreatment is not always presented as overt racism. It is also produced and reproduced by the lack of racialized representation in professional leadership positions that regulate healthcare, medical education, hiring, and promotion opportunities (Coen-Sanchez et al., 2022). The decisions for Canadian women's sexual health and reproductive rights are made at the Federal and/or Provincial levels where as of 2022: 1/4 of Federal Ministers of Health are women, one of which identifies as Indigenous and none are visible minorities (Coen-Sanchez et al., 2022). Institutions such as education and hospitals shape patient-clinician experiences but are built on a racial hierarchy that often prioritizes the care of white patients over racialized minorities (Dayo et al., 2022). Without the voices of Black and Indigenous women/birthing people involved, racial biases in our medical system continue undisrupted and cause health issues and disproportionate mortality rates for Black and Indigenous women (Coen/Sanchez et al., 2022). As exemplified in this video, without the intervention of a culturally informed, Black midwife, the patient would continue to be ignored which can lead to far more devastating consequences that are often preventable (CITE CTV?). Misinformed education that dismisses and stereotypes Black and Indigenous patients stems from deep-rooted racism in the Canadian medical system that persists in modern-day healthcare practices (Coen/Sanchez et al., 2022). Canadian medical education statistics report that leadership positions responsible for training doctors are 90% white, roughly 30% are women identifying, and 10% are racialized minorities (Coen/Sanchez et al,. 2022). As structural racism is carried from policy levels to healthcare institutions to individual experiences this demonstrates how the struggles and experiences of maternal health for Black and Indigenous patients are perpetucated by a lack of representation at various levels (Coen/Sanchez et al., 2022).
