Hi, it’s Milky again (she/they). This time, everything’s been a little messy. Kokichi Ouma, help me out, please.
So what’s going on is that good things have happened recently (I got my learner’s permit, I bought my sister something and made her happy, and I even bought myself some Kokichi-themed items).
But other times, it feels like everything’s blowing up in my face. I got told off for not taking my medicine for three weeks, I got screamed at a lot, and I’m struggling with college stuff (keep in mind, I am technically a high school student taking college classes so I just feel like an outsider even on virtual classes—that doesn’t help either).
I just want some support and kind words from Kokichi. I feel like if I had my favorite character tell me something positive, then I’ll start to believe it!

There's a balance isn't there, good comes with bad and vice versa. Sorry to hear that the bad is hitting you pretty dang hard right now. I don't have the context, but typically taking medicine is good, unless it's not actually helping. So I guess my super brilliant advice on that part is to do what's best for your health, be it taking medicine or not taking medicine. Being screamed at is the worst, it's just painful on all levels, physical, emotional, yeah it sucks. I know this is an easy thing to say, but try to not let it get to you. Try to separate yourself from that hurt a little, and that might make it easier to heal and make a comeback.

Virtual learning sucks, and I get how it's easy to feel like an outsider. I don't think that virtual learning is exactly the best place ever to make friends, but you could always try. Just try to be social and try to join in conversations when you have the chance. Aside from that, you might have to try looking for friends outside of virtual learning, because let's face it, virtual learning doesn't offer up a whole lot of opportunities.

In south Africa, Primary Health Care (PHC) is defined as essential health care based on practical, scientifically sound and socially acceptable methods and technology, made universally accessible to individuals and families in the community through their full participation and at a cost that the community and the country can afford to maintain at every stage of their development. (University of Cape Town, 2019). It is accessible, affordable healthcare that will permit people in the community to live socially and economically productive lives. Within the context of communities, A and B, there are municipal clinics available to the public which offer essential services such as counselling, dispensing of medication, treat minor illnesses, provide advice on children’s growth and development, immunisation, family planning, antenatal care, voluntary counselling and HIV testing. (Municipal Clinic, 2019). In relation to the partnership that the University of Kwa-Zulu Natal has with the community there is occupational therapy services offered by fourth-year students in terms of providing the community with home-based intervention, health promotion, and prevention at schools in terms of substance abuse and promotion of adolescent and youth friendly services and overall implementation of community-based programmes that promote occupational engagement and improve the occupational performance levels of as many individuals as possible. A large part of an OT role in a primary health care setting is advocation for health promotion and prevention and they have various tools in which to do this. A vital tool is the media, the usage of media has promoted awareness to public health and is used to send brief messages to targeted populations as well as improve knowledge and increase adoption of healthy behaviors as we know there are many tools like such as the internet, pamphlets, word of mouth, posters and awareness groups but access remains a large barrier.
 Earlier this year a health professional made a statement that occupational therapists are not considered “essential personnel” and this conclusion lead me to believe that due to the economic status of our country it made perfect sense and there was some truth to this. This statement coupled with the lack of employment opportunities in governmental institutions made it clear that Occupational therapy (OT) is not a necessity. However, working in communities A and B I have come to realise the importance of occupational therapy at a primary health care level and the great need for it. These last four weeks have highlighted the fact that it is an essential service. In terms of rehabilitating communities, A and B through programmes such as women empowerment, educating the Gogo’s on different health promotion topics and educating the youth on the dangers of substance abuse to name a few. Overall occupational therapy is a tool in itself in the community as through intervention there is a reduction of stigma, better health outcomes in terms of a better quality of life and increased occupational engagement. However even though OT has a positive effect on the community, only some people know what OT is in terms of our role and the wide range of services we offer. This lack of knowledge is widespread and is not only present among the people in the community but is also amongst the health professionals working at the clinic.
Through health promotion speeches at the clinic and the conduction of an Inservice training with the staff there has been an increase in the number of referrals in community A and B. However, the effectiveness and sustainability of this can be questioned. Effective tools like the media need to be utilized in health promotion and prevention.
In the 21st century, the media plays a significant role in the lives of many people. Media in any form or tool has a wide reach and access that can influence the public by improving their health education level. The media plays a crucial role in disseminating health information and increasing awareness about health education. It does not only spread awareness but also informs and educates people over a period of time. This ultimately helps in the change of attitude & behavior in people in the community. In other words, the media is instrumental in bringing behavioural changes in knowledge, beliefs, and attitudes about health and healthy behaviours. (Unite for Sight, 2019) In community A, OT has promoted health through health promotion speeches at the clinic on different health topics, as well as promoted the adolescent and youth-friendly services (AYFS) offered at the clinic to the grade 6s at the primary school. In community B an in-service training was conducted with the nursing staff to educate them on the role of OT to allow appropriate referrals to be made. As well as AYFS was promoted and daily health promotion speeches. An OT referral box is also present at both community clinics to allow community members to request OT intervention and orientation to the box is done every clinic morning.
Similarly, in both communities, A and B substance abuse programmes are run at the primary and high school to prevent learners from using substances. This is done to educate learners on the dangers of substance abuse and the consequential negative effect it has on every aspect of life. In community A, a Gogo’s programme is run to educate the Gogo’s on different topics such as Arthritis and diabetes to prevent the decline of function and promote awareness on how to effectively manage their conditions. During screening at the clinics, mothers are educated on development and the milestones their children are meant to be meeting. Due to time being a great inhibiting factor, quick intervention is provided to community members sitting in clinic lines to promote better health outcomes such as mothers are educated on the importance of “tummy time” to improve head control and prepare their children for crawling. The importance of sending children to creche is highlighted and Gogo’s are educated on quick joint protection principles or energy conservation principles.
However, health promotion and prevention in both communities have been purely done through verbal advocation unless home visits were done, and specific individual therapy was conducted. At the clinic, there are posters available and there is the referral box that people can read to educate themselves or ask for assistance. However, most of the posters are typed in small font and lack simple and relevant information. Also, at clinics, it is busy and chaotic therefore many people don’t have time to read posters as they are waiting for their number to be called. Many don’t even want to use the “keep my place cards” to utilize OT services due to the fear of missing their turn. In terms of the internet being used as an effective media tool, it presents with many barriers as many do not have access to it to read about OT services or different conditions and how to manage it. In many households, there are televisions and radios available however the likelihood of health promotion and prevention is rare. I then asked myself where the people in this context get access to information to empower themselves? Due to the socioeconomic status of both communities there is a lack of resources, therefore, the internet is not an easily accessible media outlet. In this context, I found that the ‘media’ that could have the greatest impact was people themselves.
Community members utilize the clinic more than any other space in the community, therefore, it is vital that they are educated that if some do have access to mobile phones there is municipal WIFI available to search different conditions or things they want to know more about, there are also language settings available to read the information in a preferred language. If people don’t have access to cellular devices, then in order to break the significant stigma present amongst people of the community with regards to having children with developmental delays and people with disabilities daily advocation needs to be done at the clinic in terms of what services are offered and on different conditions. The people in the community need to be made more aware of what each health field offers and the importance of receiving the relevant help to promote better health outcomes. By identifying a nurse or member at the clinics this will allow for sustainability so when OT students aren’t there the referral box is being utilized. Posters that are visible, easy to read and relevant need to be put up so people waiting in the queues can read what services are offered. This will spread awareness as through daily health promotion, people sitting in the queue will be more aware and will be able to identify others in the community that requires services. The community caregivers (CCG’s) can be utilized and can conduct health promotion talks on different illnesses and diagnoses as well as can run campaigns during the month that create awareness and promote health services this will allow for carryover and sustainability.
Overall as much as the media in terms of technology plays a significant role in our lives, technology is not universally accessible to everyone in the world, more so the community. Taking into consideration that community A and B are considered low socioeconomic status which are very much tightly knit, word of mouth is the most effective media tool. As if more people are aware of what OT entails and what we have to offer they will be able to identify their neighbours and friends who require OT intervention. This was proven after a health promotion speech whereby a community member consulted with us about her neighbour who has a bedridden child with cerebral palsy (CP). This led to us doing a home visit, educating the mother on her child’s condition and getting the clinic to provide nutrition supplements. Whereas prior to this due to lack of education in terms of CP and the role OT the person who consulted us would not have. Therefore, daily health promotion and prevention speeches and proper posters can be used as an effective media tool.
