(I've never done anything hanahaki-related before, so hopefully these are alright and are in line with stuff involving this trope/AU)
Killer's s/o
She'd be terrified the moment she saw the first snowdrop petals that she'd coughed up and realized she had Hanahaki Syndrome. She knew that pining after someone like this could be deadly, but she was torn between her feelings for Killer and how afraid she was of being rejected. Killer isn't exactly an expressive person, so she felt that confessing to him couldn't lead to anything but a rejection. 
Killer would notice the stray petals left around the deck of the ship, and eventually found out that they were coming from ____. He'd been keeping his feelings for her a secret, just because he thought that she'd never reciprocate his feelings; who would want to be with a man so repulsive that he had to hide his face/smile behind a mask to feel secure? Still, if there was the slightest possibility that she'd gotten Hanahaki because of him, he'd feel guilty for not telling her his feelings sooner.
He'd quietly pull them aside and tell her that he knows she has Hanahaki Syndrome, and that he doesn't know who she's in love with but...he's praying that it's him because he's been in love with her for so long. If he's the one who "caused" her to become sick, he wants to be the one to make her well again by confessing to her and (hopefully) hearing her confess to him. After they get together and she recovers, they both feel a little silly for not telling each other about their feelings until it was almost too late.
He's still a little anxious about showing his face to her, but with time and patience he would eventually do it. He still feels some (slightly misplaced) guilt over the fact that she almost died for him due to contracting Hanahaki, so he wants to show her that he respects her and trusts her enough to take off his mask when they're alone together.

Kid's s/o
Shit. Shit. Someone developing Hanahaki Syndrome over Kid is pretty dire, just because he really isn't the type to show signs of love or affection--a lot of people think a monster like him isn't really capable of love, but that isn't the case. He is, but how he shows it is hard for people who don't know him to recognize he cares about them. It also doesn't help that he's...kind of a dick, even/especially to the people he cares about.
So when ____ starts coughing up tulips, it's a sobering and terrifying moment of realization. She's in love with someone who she doubts could love her back, especially now that she'd contracted a deadly disease. Kid hates any form of "weakness", and she worries that he might reject her outright just because she had Hanahaki; not only was she weaker, but it was a disease that she'd caught because she couldn't keep her fucking "feelings" in check.
The thing is, Kid won't toss someone he knows/cares for just because they get sick or because they show like...feelings. He may make fun of them or give 'em shit for it, but that's only after he does everything he can to help them. So when he sees scattered and spit-up tulips on his ship and overhears ____ coughing one night, he immediately reacts by heading to the nearest island to get a doctor.
He refuses to listen to her protests, and it's only when he's about to anchor the ship and kidnap any "egghead" he finds on the island to help her that she finally breaks down a bit and says that it won't help. The only way for her to get better is to admit how she feels about him, and she worries that if the flowers don't kill her, the rejection just might do the trick anyway.
When Kid sees her start crying, he goes towards her and for a minute she's worried that he'd hate her: she's been swallowed up by her feelings, she's barely able to stand (much less fight alongside everyone), and now she's sobbing right in front of him. But that harsh, callous rejection never comes. He just carries her to his captain's quarters, downs a bit of whiskey, and says that if "manning up" and admitting he's liked her this whole time will make her get better and calm down, he'll bite the bullet and do what he's been afraid of doing this whole time.
Once she gets better, he's more...I won't say affectionate, but he's more willing to show how much he cares about ____ now; his words are a bit softer, he's gentle when he's touching her/near her, etc. He insists it's because he wants everyone to know that she's "his woman", but it's because he doesn't want to risk losing her to some damn flower disease ever again.
