lmao not really, but according to my vet feline infectious peritonitis (FIP) is nearly unheard of in ireland, so when they sent away her liver biopsy and it came back with possible markers of FIP, it confused her until she remembered me telling her that she's my little vecchietta romana bc it's much more common on the continent.

so if this is the case, she would have contracted feline enteric coronavirus (FeCV) prob at birth from her mother, and it would have lain dormant until something triggered an inflammatory response, which in this case might have been the separation anxiety anorexia. the lab still has her liver sample, so they're doing an immunohistochemistry panel to confirm. the other possibility is that they saw slightly increased meiosis in her cells which could point towards cancer. so the test should confirm what we're looking at.

shit thing is that there really is no effective treatment for FIP (in Ireland at least). we'd be looking at steroids which could put it into some kind of remission but it's p much a fatal diagnosis bc once it develops into FIP, the virus aggressively compromises the internal organs. she seems to have a dry not wet expression, so she doesn't have fluid build-up in her abdomen, so at least there's that.

bc it's a relative of miss rona, some of the same treatments might work like remdesivir, but idk man it's about cost and quality of life at this point (just looked up a description of a treatment course and it could be €2.5-3.5K hhhhhhh). if she's on a time limit, i want it to be a good one and not just go for a treatment that's gonna distress her and make her feel unsafe with me if it's a shot in the dark anyway.

same goes for cancer - like what humans go through with chemo is traumatic, but people can make a fully informed choice to go through it. she can't and imo that's kind of low-level torture if it's gonna be a matter of giving her a few more months or smth.

my vet's been super up front and clear about the situation anyway and fucking hell i'm really glad i've been taking the time to emotionally confront her mortality in the past couple of years. she's my best little friend, but i'm not gonna do anything that's gonna put my emotional state over her quality of life.

What's your take on the FIP vaccine for cats? And how does FIP actually work? (As I side note, I came for the cat breed write-ups, but I stayed for the vet snark and evidence-based medicine - this is quite probably my favorite blog to read)

I don’t have a FIP vaccine for cats, and I’m fairly sure there isn’t one in Australia. I know it was under development, but the evidence to use it was not as solid as for the other vaccines we routinely use. My understanding was that it was best used in an outbreak situation, where one cat in a group has been diagnosed, or high risk situations where lots of young cats are living in close proximity, like a shelter or breeding cattery.

FIP is a weird condition (that I probably should do a video on) and it is fortunately rare, so bare with me as I go through the basics.

FIP (Feline Infectious Peritonitis) is not caused by ‘the FIP virus’, it is actually caused by a mutated Feline Enteric Coronavirus (FECV = the normal one, FCoV = the mutated one)

The non-mutated virus causes little more than diarrhea, similar to conditions in many other mammal species.

The non-mutated virus enters via the mouth (the faecal oral route), and remember cats lick themselves clean, then has a brief systemic phase (virus particles go all around the body) before it replicates in the gut to further transmission.

A normal cat gets over the non-mutated virus fairly promptly with little more than supportive care.

The normal Feline Enteric Coronavirus can mutate in any cat and become the variant that causes FIP. This is how we see single cats in a group come down with the disease.

The mutated virus which caused FIP is also transmissible and infectious, including in saliva, so mutual grooming is also a potential mode of transmission. This is probably what is happening when we see outbreaks.

Then the mutated virus which causes FIP goes on to have all sorts of nasty things happen, most of which can be explained as a massive inflammatory response which will ultimately cause either multiple organ failure, or fluid to accumulate in the abdomen, then thorax, until death. You can try to treat it with immunosuppressants to reduce that response, but generally it does not go well and FIP is basically doom in a can.

And it seems like most cats will only get an infection of either the normal, enteric virus or the mutated virus once, so a vaccine should be able to be developed but it’s not quite that simple. This is why both infections seem to mostly affect young cats - once they’ve had the infection once they don’t get it again. For the normal, enteric virus it’s because immunity is strong, for FIP it’s because they die.

(though some cats remain infected and continue to shed, but that’s another complication)

Especially when the vaccine is a cold-adapted attenuated virus. That means it’s been adapted to only replicate at a lower temperature than body temperature (okay) but is still a live virus (uh... wait).

Usually I’d have no problems with an attenuated live virus vaccine. I use a bunch of them. But this is a virus that can spontaneously mutate into the 100% deadly version. Is it any wonder the vaccine is on the ‘only use this if you really have to, and not routinely’ list.

And the whole picture is probably not that simple. Some cat lines seem to have a predisposition, some virus strains are more virulent than others.

And research is continuing, and it’s made advances over the last ten years. In general practice the tests available to me to diagnose this condition are much better than they were 10 years ago, even if treatment isn’t really available yet and will likely be very expensive when it is.

I’ve mentioned before that I average seeing one FIP case a year, not really an ‘outbreak’ situation. I would like a vaccine to be available, it would be very useful in shelter situations, but FIP is a complex nightmare and better people than I are working on it.

My darling angel furbaby Sasha has effusive FIP, feline infectious peritonitis, causing pleural effusion in her chest cavity, or a filling of liquid that displaces her lungs so she cannot breathe properly. Her body is breaking down. FIP is a mutation of FECV and is fatal. There is no cure. We found out yesterday after taking her in to her regular vet after noticing shallow, elevated breathing, and getting an x-ray of her chest area. When our vet walked in, her expression turned my stomach into a cold hard rock. Sasha was immediately placed into an oxygen tank to help her breathing, and we were advised that FIP was suspected, and draining of her chest cavity would be the next step, along with blood work and diagnostic testing of the liquid pulled. Because of the sensitivity of the op, MASH was recommended as our treatment center. We rushed Sasha to MASH & they began sedation & oxygen therapy. After first draining 45mL, they did more x-rays & blood work. The viscosity of the liquid & the elevated globulins in her blood point to FIP. The effusion will return, her health will continue to decline, & euthanasia was recommended. The doc was able to drain another 45mL w/difficulty, & after more oxygen therapy we decided to bring her home for now. This is every furbaby parents’ worst nightmare. I even desperately emailed a doctor at UC Davis who has done trial studies on FIP, but there is no medicine for it now, at least not FDA approved. My heart was shredded earlier this year when Jinx died in my arms, & when I adopted Sasha I knew I would ultimately have to go through this again but this is beyond a cruel twist of fate. She’s barely 5 months old. 💔💔💔😭😭😭 My plan is to spoil her and love her as much as possible for her last few days, weeks, before her pleural effusion becomes serious again or her quality of life otherwise declines to that point where I will have to make the call. My heart is breaking in a million pieces all over again. This is so difficult. Hold your furbabies close. Feline Infectious Perotinitis (FIP) is a severe viral illness affecting cats caused by a mutation of the Feline Enteric Coronavirus (FECV). Most strains of FECV do not cause disease and infected cats are often asymptomatic. While infection with FECV is very common in shelters and multi cat households, the occurrence of the mutated FIPV is relatively low. In less than 10% of cats infected with FECV, the virus mutates into FIPV due to an abnormal reaction from the cat's immune system. This mutation is believed to occur independently within each cat. While FECV is highly contagious and found to be endemic in multi cat populations, FIPV is not thought to be transmitted between cats. However the possibility has been suggested to explain the rare occurrence of mini-outbreaks of FIP. The development of FIP is multifactorial. There is a genetic risk factor, which is estimated to account for about 50% of the risk. The other 50% are environmental factors, such as age, the level of exposure to FECV, not receiving passive maternal immunity, as well as stresses such as overcrowding, malnutrition, or spay/neuter surgery and any other factors that might reduce the cat's ability to combat the infection. Since there is a genetic component that contributes to the risk of developing FIP, littermates of kittens that have developed FIP are at increased risk. That scenario may explain why all 4 cats in this litter developed FIP. Although once considered 100% fatal, new anti-viral medications have demonstrated FIP remission in clinical trials. One drug, GS441524, is actually the precursor to synthesizing Remdesevir, the drug from Gilead Sciences being used to treat COVID19 in human COVID19 trials. The treatment period is a daily injection of GS441524, and supportive care daily for 84 days. If after 84 days the cat's FIP markers and labs have normalized, the cat enters the post treatment phase. This phase consists of monitoring the cat's behavior and their FIP markers and labs for signs of relapse.
