Kenny outlines attacking attributes of new recruit Keane

Ireland manager Stephen Kenny has outlined the attributes of new recruit Will Keane ahead of next week's World Cup qualifier in Azerbaijan.
The Wigan Athletic striker was named in the squad, which was announced earlier today, ahead of the double-header, away in Baku and the home tie against World Cup hosts Qatar.
Keane's inclusion was the only real surprise in the 26-man squad, while Sheffield United left-back Enda Stevens, Derby County midfielder Jason Knight and Rotherham United winger Chiedozie Ogbene all return after injury ruled the trio out of the September matches.
Captain Seamus Coleman and defender Dara O'Shea are both ruled out through injury as Cyrus Christie retains his place in the squad, having been called up for the 1-1 draw against Serbia.
Keane came through the ranks at Manchester United alongside twin brother Michael, and while Will played underage international football for England, he changed allegiances and declared for the Republic of Ireland a number of years ago.
By contrast, Michael, initially played Under-17 and Under-19 levels with Ireland before going back to the country of his birth, and was capped at senior level with England.
Now 28, Will Keane moved from Manchester United to Hull, before taking residence at Wigan via Ipswich in 2020.

"Will has had a consistent run of games this year, he’s probably having one of his best seasons," said Kenny, speaking to the media at the FAI headquarters.
"I’ve been to see quite a bit of him, he links the play very well. He plays as a second striker at Wigan, leads the line quite well, has an appreciation of players around him, has a good weight of pass and good vision.
"He can score headed goals and different types of goals, so he’s having a good season so far."
Ireland have perennially struggled in the goalscoring department since the retirement of Robbie Keane, and while Kenny was not suggesting that the record goalscorer's namesake Will was the answer in that area of attack, he said that he was looking forward to working with him in camp this week and adds to the attacking unit in the squad.
"He’s a good option for us, so we’re looking forward to getting him in this week and working with him at close quarters," said Kenny.
"It’s his first introduction to the Irish international team and we’re looking forward to working with him."
Since the brothers Keane showed allegiances to both Ireland and England over a decade ago, others have travelled the same path, most notably Jack Grealish and Declan Rice in recent years.
And while Keane had already begun the process of declaring for Ireland before Kenny took over the national side, the manager said that he has spoken to the new addition and has enjoyed a good chat with him ahead of his inclusion in the squad.
"To be fair it is not something I initiated, Will has been qualified for the last number of years," said Kenny, when asked about the communication with the player in relation to players with dual nationality.
"I did have a conversation with him after one of the games. I met him recently and earlier in the season we had a good chat.
"His form has been consistent. I think he had injuries in the last couple of years and wasn’t playing consistently.
"He is good to watch, a very clever player, good awareness, good touch. He is an intelligent footballer and seems to be enjoying his football at Wigan and doing well.
"It is League One, admittedly, but every time we have gone to see him he has been impressive."

Sheriff Tiraspol's win over Real Madrid can be celebrated and lamented at the same time

Sometimes, two seemingly conflicting ideas can exist at once. Like how to feel about Moldova's Sheriff Tiraspol, and whether it's a "fairy tale" or a sad reflection of what it takes for a smaller, poorer nation to send a team to the Champions League.
Let's start with the good bit to cheer us up, shall we? This is a club that beat Real Madrid at the Santiago Bernabeu on Tuesday night to move to the top of their Champions League group, with six points out of six. Before beating the Spanish giants -- who have won more European Cups than anyone else, and who won four Champions League crowns in the past eight seasons -- in the group stage opener, they defeated Shakhtar Donetsk, who have qualified for the Champions League for 10 straight years.
To even get into this season's Champions League, Sheriff had to start their season in the first qualifying round way back on July 7, just five weeks after their 2020-21 campaign ended and at a time when the European football world was preoccupied with other matters, like the semifinals of the Euros. Along the way, they beat Dinamo Zagreb and former European champions Red Star Belgrade, both of whom were in the Champions League just two years ago.
Now consider the fact that Sheriff were only founded in 1997, that they play in a 12,746-seat stadium that they didn't fill even pre-pandemic -- their average domestic attendance in 2019 was under 1,000 per game -- and that in their history, they'd never advanced beyond the group stage. Oh, and their late winner was scored by somebody named Sebastian Thill, who himself made history as the first player from tiny Luxembourg to score a Champions League goal.

(Before you imagine Thill to be some kind of phenom who happens to have been born in a tiny country -- sort of a Benelux version of Liberia's legendary striker, George Weah -- well, he's not. He's 27, had only ever played in Luxembourg until last season and has been capped just 16 times, which means he doesn't even figure in the top 100 list of most-capped Luxembourg footballers.)
It's hard to make an apt analogy for Tuesday's win. Maybe if a tiny online college founded in the late 1990s beat Oregon, Maryland, Gonzaga and Duke in rapid succession? And if they defeated the latter at Cameron Field House in front of the Crazies, thanks to a buzzer-beating 3-pointer drained by a guy from Belize who isn't even among Belize's top 100 players?
When you then juxtapose it with the shelved (for now) Super League project driven by Real Madrid president Florentino Perez, who memorably said the Champions League group stage was "not interesting" and that football would be "dead by 2024" if not for the sort of Super League -- a structure that would de facto exclude not just the Sheriffs of the world, but all but a handful of Europe's professional clubs on a permanent basis -- the temptation to troll becomes hard to resist.
Was Real Madrid vs. Sheriff Tiraspol more interesting than a bog-standard group stage equivalent -- say, Real Madrid vs. Tottenham -- in Florentino's proposed Super League format, with two groups of 10 playing home and away and five of the 10 advancing? Probably. And when you consider the underdog twist, it becomes even more compelling.
Sheriff's starting XI had a grand total of 28 international caps: Thill's 16 for Luxembourg, Dimitris Kolovos' nine for Greece and Jasurbek Yakshiboev's three for Uzbekistan (I kid you not: Uzbekistan). They have an Adama Traore and a Cristiano, but it's not that Adama Traore (who's tearing it up in the Premier League), and it's obviously not that Cristiano, either. They have two guys -- one from Colombia (Frank Castaneda) and one from Ghana (Edmund Addo) -- they signed from a Slovakian club that are perennial relegation candidates. (If you've heard of Senica before right now, you're either very knowledgeable on football or Slovakian.)
Let it be a reminder that ordinary players, on an ordinary Tuesday night, can walk into the temple of football and knock it down, like Samson back in the day. That's part of the appeal of this sport. It's low-scoring, it's mano-a-mano, and the gap between superstars and extras may be huge over time, but on any given day, it can be tiny and anything can happen. Throw in the fact that this is the first Moldovan team to make it into the Champions League -- Moldova is, by some distance, the poorest country in Europe. That makes a difference in professional sports -- and that many in Tiraspol don't even consider themselves Moldovan, but Transnistrian or even Russian (here's a primer on the geopolitics), and you've got yourself a ready-made feature curio.
Except there's a little problem in viewing this strictly through the "fairy tale" lens. And no, it's not just the fact that there is very little Moldovan about this team: neither the coach, who is Ukrainian, nor any of the players who beat Madrid are Moldovan (or Transnistrian, for that matter). (Before Madrid fans -- or anyone else -- get high and mighty about national identity, please consider that at the final whistle, there were exactly zero Spaniards on the pitch.)
Rather, it's Sheriff itself and the circumstances behind their rise, as well as the people bankrolling them. This thread, by journalist Slava Malamud, who grew up a few miles away, gives some context from his perspective. At a time when football is often used for geopolitical or nationalistic purposes, when unsavoury or totalitarian figures play an increasingly large role in the success of many clubs, and when the gap between the rich superclubs and everyone else is as great as ever, it's tempting to see the Sheriff story as an antidote to do this. Except Sheriff are themselves a superclub, writ small.
They've been Moldovan champions in 19 of the past 21 years, they have the country's only modern stadium and they're bankrolled by the Sheriff corporation, a conglomerate that includes Transnistria's only supermarket chain, gas station chain, telephone network, TV channels, publishing house and distillery. The owners have close ties to the local government, which, in turn, is funded and protected by Russia. This isn't just a company team; it's a company town in the company enclave of Transnistria, and you can't shake the feeling that this is what it takes for "fairy tales" like this to take place in the modern game.
We can talk about dreams and the fact that on any given day, two Brazilians, two Colombians, two Greeks, a Peruvian, a Ghanaian, a Malian, an Uzbek and a Luxembourger -- virtually all of them journeymen and no-names -- can defeat Real Madrid, and that's the beauty of the sport. But dreams aren't enough. Without a club that's a local powerhouse, with a guaranteed annual shot at the Champions League qualifying rounds and owners who can do what they like (at least locally), you likely don't even get a chance to dream. Having a globalised, multinational squad of players isn't the only thing Sheriff have in common with some of the continent's elite.
So yes, both things are likely true. Sheriff's epic underdog victory at the Santiago Bernabeu is most definitely to be celebrated as an incredible achievement. Equally, the circumstances that made it possible are to be lamented, as is the fact that, without them, it's virtually impossible for 99% of Europe to compete.

Republican bid to limit health officials could cause ‘preventable tragedies’ – experts

At least nine states have considered ‘emergency power limitation’ legislation proposed by conservative organization Alec
Republican lawmakers across more than a dozen states are working to limit the powers of local health departments in ways experts say is likely to lead to “preventable tragedies” during disease outbreaks, including the Covid-19 pandemic.
The attempts to limit the emergency powers of public health agencies comes alongside harassment of individual public health workers, renewed concern about the spread of the highly transmissible Covid-19 Delta variant, and a flagging US vaccination campaign.
“The supposed rationale is that a lot happened during Covid. People didn’t like being restricted so much,” said Lori Tremmel Freeman, CEO at the National Association of County and City Health Officials (Naccho).
“But if these laws and pieces of legislation stand beyond the pandemic we have a big problem,” she said. “They basically handcuff health officials from being able to use century-long mitigation measures.”
Montana barred local health authorities from quarantining people suspected of being exposed to or infected with a contagious disease. That ends an infection control measure used since at least the plague, known as the “Black Death”, hit Italian cities in the 14th century.
North Dakota banned state health authorities from requiring a face covering for any reason, a measure that could impact diseases such as tuberculosis. Florida gave the governor sweeping power to “invalidate” any local emergency order. Kansas removed the governor’s ability to close businesses during a public health emergency.
Together, at least 15 states are considering new limits to local public health authorities’ power in ways likely to diminish local expertise, the ability to locally tailor restrictions and lead to more politicized responses.
“I was a staff attorney at the Massachusetts department of public health for 10 years, and then I was the general counsel for the next 26,” said Donna Levin, national director of the Network for Public Health Law. Levin has since been with the network for an additional seven years of her career.
“I can safely say I have not in my experience seen the assault on public health officials, and threats and worse across the country, and I’ve never seen this wholesale attack on public health authority … in the context of emergency response,” Levin said.

Lawmakers across at least nine states have zeroed in on the emergency powers of health departments to limit how people gather in places such as churches, schools and businesses. Conservative model legislation has described these limitations as orders that “infringe on the rights of private parties”.
Many of the limits on public health measures have also come with the imprimatur of one of the most powerful and well-financed conservative legislative machines in the country – the American Legislative Exchange Council (Alec).
At least nine states have considered “emergency power limitation” legislation proposed by Alec that could directly limit public health response, a report by Naccho found. Those states – Arizona, Idaho, Kentucky, Montana, North Dakota, Pennsylvania, Utah, Virginia and Washington – lean conservative or purple.
Alec began circulating model legislation related to Covid-19 as early as last April. The emergency powers legislation was first published in 2020 December. Since then, experts said attempts to limit the emergency powers of health departments have become more concerted and widespread as the pandemic has marched on, with states such as Missouri curtailing the emergency powers of health departments in June.
Alongside a raft of new legislation pushed by Alec, a loose coalition of conservative groups have worked since April 2020 to drive attendance at anti-lockdown protests, fund lawsuits against public health orders and commissioned polls to support the appearance of a “grassroots” drive for reform.
Alec’s proposed legislation is meant to limit government spending, reduce regulation and impose “constitutional law”, according to principles laid out by Alec’s conservative economists early in the pandemic.
“As they should, every state will have a different solution and approach to reopening, and Alec will support legislators in these efforts,” said Alec’s CEO, Lisa Nelson, in April 2020.
Alec’s primary model for consolidating power in state capitals is through “pre-emption”, or banning any locality except the state from legislating on an issue. Alec has used this strategy across issues as wide-ranging as workers’ rights to environmental protections.
“This is an organization that is all about consolidating power in the state, particularly in conservative states,” said Kim Haddow, a strategic consultant for the Local Solutions Support Center (LSSC), a group that works to maintain the democratic rights of cities, counties and other municipalities.
That, Haddow said, allows Alec to deal directly with state legislators who tend to be “disproportionately representative of white, rural areas”, rather than thousands of localities. Pre-emption also denies urban centers, which tend to be more racially diverse and lean Democratic, the ability to enact progressive reform.
“Around the pandemic, [Alec] took advantage and exploited the ability to take away more local power,” said Haddow. “People are saying why vote” if states constantly pre-empt local legislation, she said. “Well, why indeed?”
Missouri is one of the states working to limit the power of public health authorities. In June, its Republican governor, Mike Parson, signed legislation to limit public health orders to 30 days in a statewide emergency, and 21 days when there is no state declaration. A local governing body then has to vote to renew such restrictions.
Parson was a speaker at Alec’s 2020 conference, which was forced to go digital after Florida, where they planned to hold the in-person conference, became the center of an outbreak.
“There is a longstanding discomfort with public health at its core, but Covid-19 has been immediately politicized in a way other health issues weren’t,” said Dr Elvin Geng, an infectious disease expert at Washington University in St Louis, Missouri.
Missouri is also among states now experiencing the most significant outbreak of Covid-19 since spring 2021, when a fast-moving vaccination campaign and social distancing led to the lowest rates of new infections since the pandemic began.
Though cases remain low overall compared with the peak of the pandemic, Missouri is among the least vaccinated states in the nation, with just 40% of people fully vaccinated.
“Local control is critical because epidemics are heterogenous in severity and timing,” said Geng. Missouri is currently experiencing one of the worst Covid-19 outbreaks in the nation.
Remarkably, Missouri was also the target of another well-financed conservative effort to avoid government spending that might affect taxes on the wealthy. The political advocacy group Americans for Prosperity, funded in part by the billionaire oil and gas magnate David Koch, worked to dissuade voters from expanding Medicaid.
The health insurance program for the poor and disabled would have provided health insurance to 230,000 low-income Missourians. Voters approved the ballot measure, which state Republicans refused to honor, and quickly took to court.
It ranks last in public health funding, and spent just $7 a person on public health measures in 2020, an analysis by the State Health Access Data and Assistance Center, a state-focused health research center found.

Is football's racism battle being lost on social media?

When it comes to online racist abuse, football appears to have had enough. Again.
Last week's series of incidents provoked a wave of condemnation from across the game and beyond.
And yet, this comes almost two years after top stars took part in a campaign called '#Enough', a 24-hour social media boycott in protest at a similar spate of abuse.
Now the same demands for tech platforms to do more are being made. So is the battle being lost? Why is it proving so difficult to tackle this issue? And where is this crisis heading?
Time to take responsibility The pressure on the social media platforms is intensifying. The appetite for more regulation is growing.
Last week the Premier League said the tech companies "need to do more", calling for "swifter removal of offensive messages and improved identification and banning of offenders".

Manchester United, with three of their squad subject to vile messages, urged the creation of verifiable and identifiable accounts so offenders could be caught, saying they and the players were "sick of it", making the point that no one should face such hate-filled abuse in their place of work.
The Duke of Cambridge, who is also the president of the Football Association, has praised those who have spoken out and called for an end to the "despicable" abuse.
Writing on Twitter, Prince William also cited the need for social media companies to raise their game.
"We all have a responsibility to create an environment where such abuse is not tolerated, and those who choose to spread hate and division are held accountable for their actions. That responsibility extends to the platforms where so much of this activity now takes place," he said.
The Professional Footballers' Association, which represents players, has asked the tech giants to prevent users from being able to send explicitly racist terms and emojis. "While racist abuse is allowed to continue on each platform, we can only conclude that this is a choice by the companies running the social networks," it said.
Putting such criticism to the platforms is no easy task. We asked for an interview with Twitter on this subject back in 2019. We are still waiting for one.
Our request last week for an interview with Facebook - the owner of Instagram - was met with a short statement saying: "There is no place for racism on Instagram and we are committed to removing it when we find it. We know there is more to do and we will continue to work closely with clubs, players and football authorities to investigate instances of discrimination and collectively tackle this issue."
Twitter too issued a statement: "Racist behaviour has no place on our service and when we identify accounts that violate any of the Twitter Rules, we take enforcement action.
"We have proactively engaged and continue to collaborate with our valued partners in football to identify ways to tackle this issue collectively and will continue to play our part in curbing this unacceptable behaviour - both online and offline."
New legislation The government meanwhile has vowed to press on with plans for "groundbreaking" new online harms legislation later this year to make tech companies legally responsible for the online safety of their users, and make them accountable to a regulator - Ofcom - over abusive content.
Racist messages would need to be removed "without delay". The proposals include the threat of massive fines of up to 10% of global turnover if the companies fail to meet their obligations, and there could even be criminal sanctions for senior executives.
Behind the scenes, I am told that while football authorities were encouraged by the recent meeting that Culture Secretary Oliver Dowden hosted with players to discuss online abuse, there is concern at how long it is taking to introduce new laws considered long overdue, and fears they may be watered down if the hugely powerful, US-based tech companies have their way and threaten to invest elsewhere.
But according to Kick It Out chief executive Sanjay Bhandari, the legislation - along with the promise of more regulation in the US from President Joe Biden - are grounds for optimism.
"The cavalry is on the way," the head of the anti-discrimination body told the BBC.
"That's the hope on the horizon, but that may not come quickly enough so there's things we need social media to do right now. The behavioural, cultural problems will take longer to solve. It's the technology problem we can do more quickly and all of the answers sit with Twitter and Facebook. Technology has been massively unregulated for 25 years and its time we closed that loophole."
Frustrations Some at the Premier League - which launched a dedicated, 'rapid-response' reporting system for players last year - say the social media platforms have improved in recent years, but not enough. There has been success with the removal of posts but officials want more effort to assist the authorities to find perpetrators so they can be banned by clubs (if they are a fan) or even prosecuted.
While there have been some successful prosecutions - an Irish teenager is due to be sentenced on Wednesday for sending racist abuse to former Arsenal and England striker Ian Wright - these are few and far between when the sheer scale of the abuse is considered.
One insider said league officials felt hampered by what they called impregnable and unresponsive tech companies using privacy, freedom of speech and the sheer weight of traffic as reasons for not handing over information that would help identify offenders. The fear is that the police - without the resources to pursue every possible crime - only intervene in the most serious cases, and abusers know they face little risk of sanctions. Court orders need to be obtained to force the platform to co-operate, evidence has to be gathered, and often offenders are overseas in different jurisdictions.
Another challenge is persuading players to take legal action, which can often seem a daunting prospect.
Big-tech's defence While admitting there is "more to do", Facebook does point to the fact that it is working with Kick It Out on a fans' education and reporting initiative, and has built tools designed to prevent unwanted contact on Instagram. It has also tripled the size of its safety team to 35,000 people, claiming it proactively took action on tens of million of pieces of hate speech last year. But considering the vast wealth of these companies, many believe more should be invested.
The tech companies argue that tackling this problem is not as straightforward as many assume. They have other areas of concern that must also be monitored across their vast, unregulated platforms; from suspected terrorism and paedophilia to the spread of misinformation.
Simply blocking certain discriminatory words or emojis from being used in the first place is problematic, because these can also be used in a non-offensive context, and across global platforms can have different meanings in different communities or territories. The companies can block devices for repeat offenders, but that person can then just use a different device.
Many are now calling for an end to the anonymity that provides a shield to many abusers with a mandatory verification process that means users must give passport details for instance before opening an account. Given the mistrust over social media platforms' use of personal data however, there are fears that this could pose a threat to activists, whistleblowers and persecuted minorities who need to protect their identity online.
Many also point out that in recent years, racially offensive comments have been published on social media by fully verified accounts.
Could football do more? There are also those who believe that the time has come for football's authorities and clubs to invest more, and take the burden of reporting away from the players.
"The problem is that the systems that they've got in place require a player to receive and then report the abuse themselves, and by that point the damage has already been done," according to Jonathan Hirshler, the CEO of Signify, a data science company that uses artificial intelligence to uncover the identities of abusers.
"The technology is there, using AI and open source data, to unmask vile and prolific abusers.
"It's also important that football moves from reactive to proactive, to pick up abuse before the players have to, and package evidence in a way that the social platforms and authorities have little choice but to act on. So we need a new approach, working constructively with the platforms, to ensure that abusers have nowhere to hide."
There is no time to waste. During just the final six weeks of last season, a joint PFA and Signify study identified more than 3,000 explicitly abusive messages aimed at Premier League players, 56% of which were racist. Of the players surveyed, 43% said they had experienced targeted racist abuse.
Football has become a lightning-rod for a wider, societal issue. The scale of the challenge facing the sport is significant, and seems to be getting worse. Finding a solution however, is proving far from simple.

7 Days Film Review Tribeca 2021
Karan Soni and Geraldine Viswanathan star in Roshan Sethi’s directorial debut about two strangers forced to shelter in place together after an awkward first date. It didn’t take long for coronavirus content to descend on networks and streaming services. From Connecting… to Love in the Time of Corona and beyond, these shows and films mined the calamitous virus like one would a new trend, attempting to extract meaning from the mayhem and reexamining everyday life under altered conditions. It’s hard to predict which of these projects will stand the test of time, but 7 Days, a charming romantic comedy about an Indian couple forced to shelter in place after their first date, deserves to be among those with at least some staying power.
Premiering at this year’s Tribeca Film Festival and directed by Roshan Sethi, 7 Days opens documentary-style, with clips of real-life Indian couples recounting how they met. The grainy, pixelated videos humorously recall the early pandemic days when, despite the existence of phones, Zoom became the preferred method of communication.
The testimonies tee up subsequent voiceovers by the mothers of the film’s fictional protagonists, Ravi (Karan Soni) and Rita (Geraldine Viswanathan). The two women dutifully recite details from their children’s online dating profiles: Ravi is endearing, slightly awkward and very funny. He spends most of his days in a lab and is eager to find the perfect “traditional” wife. Rita is ostensibly Ravi’s ideal match — she too is looking for a traditional marriage, will go fully vegetarian for the right man and her hobbies include cooking for others.
We meet Ravi and Rita picnicking under cursed conditions. It’s March 2020 and as news of the coronavirus spreads, the young adults fumble through the early moments of an uneasy first date in California (a city is never specified). The oppressive heat causes both parties to sweat profusely, the location — which Ravi picked based off outdated Google images — turns out to be more of an arid reservoir than a verdant pond, and the masks and gloves they both wear give the whole encounter dystopian vibes.
When the two begin receiving phone alerts about cancelled flights and shelter-in-place orders, they decide to call it. There’s only one problem: Ravi has no way of getting home. Rita reluctantly invites him to her cluttered apartment so he can order a taxi, and this is where the narrative unfurls, in obvious but funny fashion.
Unable to find a car or a hotel room, Ravi hangs out in Rita’s apartment and, within the first 24 hours, realizes the personality she presented online and during their first date is far from the real her. Beer bottles strewn across the kitchen reveal that she does, in fact, drink, and the leftover chicken drumsticks in her fridge mean she’s definitely not vegetarian. Thrown by the deception, Ravi, who now has to stay at Rita’s apartment for an indefinite period of time, abruptly ends the relationship that never really was.

The rest of the film follows the duo as they create a temporary makeshift life together. This premise — of two people with divergent personalities potentially falling in love — is not new, but 7 Days satisfyingly freshens up a stale formula, thanks in large part to the lead performances. Soni (who is known for his role as Dopinder in the Deadpool films and co-wrote the 7 Days screenplay) and Viswanathan (who broke out in Blockers) use their impressive physical comedy skills to imbue their characters with distinctive temperaments, creating some laugh-out-loud moments. Scrunching his eyes when in denial, giggling when nervous, Ravi has a lack of self-awareness that, as played by Soni, is amusing.
Viswanathan’s character is less fully realized on the page, but the actress does a considerable amount with what she’s given. Underneath the callous attitude and dark humor, Rita is a big softie, and Viswanathan intimates that vulnerable side with ease. Still, Rita is a caricature of a millennial woman à la Girls: She talks freely about using her vibrator, confesses to initiating a secret relationship with a married man and may or may not know how to operate a kitchen stove. Although these “traits” make for an entertaining contrast with Ravi, they’re not enough to give us a full sense of Rita’s character.
COVID-19 amps the film’s narrative stakes and steers its visual style. DP Jeremy Mackie favors shaky camera movements and close-ups, which give 7 Days a decisively realistic quality. Unable to leave the confines of Rita’s apartment, Ravi and Rita get real close, real fast. Part of this has to do with the fact that Ravi never stops talking, his monologues giving way to heartfelt confessions. Although Rita occasionally interjects or launches into her own monologue, hers register as less personal, almost perfunctory.
Enjoyable as it is, the romance in 7 Days can’t help but feel a bit contrived. I walked away from the film wondering about alternate possibilities that would still showcase the talents of the gifted leads — perhaps a movie in which Ravi and Rita redirect the awkwardness of that first date and commit to a whirlwind and chaotic friendship instead of aiming for the same old love story.

Calls to investigate Chinese laboratories have reached a fever pitch in the United States, as Republican leaders allege that the coronavirus causing the pandemic was leaked from one, and as some scientists argue that this ‘lab leak’ hypothesis requires a thorough, independent inquiry. But for many researchers, the tone of the growing demands is unsettling. They say the volatility of the debate could thwart efforts to study the virus’s origins.
Global-health researchers also warn that the growing demands are exacerbating tensions between the United States and China ahead of crucial meetings at which world leaders will make high-level decisions about how to curb the pandemic and prepare for future health emergencies. At the World Health Assembly this week, for example, health officials from nearly 200 countries are discussing strategies including ways to ramp up vaccine manufacturing and to reform the World Health Organization (WHO). But a US–China divide will make consensus on these issues harder to reach, says David Fidler, a global-health researcher at the Council on Foreign Relations, a think tank in Washington DC. “If there’s some turning down of the geopolitical heat between these two great powers, we could create some space to perhaps do some of the things that we need to do,” he says.
Others worry that the rhetoric around an alleged lab leak has grown so toxic that it’s fuelling online bullying of scientists and anti-Asian harassment in the United States, as well as offending researchers and authorities in China whose cooperation is needed.

Fever pitch The debate over the lab-leak hypothesis has been rumbling since last year. But it has grown louder in the past month — even without strong supporting evidence. On 14 May, 18 researchers published a letter in Science1 arguing that the idea of the coronavirus SARS-CoV-2 leaking from a lab in China must be explored more deeply. It points out that the first phase of a COVID-19 origins investigation sponsored by the WHO, which released a report in March, focused more on the virus coming from an animal than on its potential escape from a lab. For example, the report mapped a large market in Wuhan, China, and stated that most samples of SARS-CoV-2 recovered there by investigators were found around stalls that sold animals. Many virologists say that this focus is warranted, because most emerging infectious diseases begin with a spillover from nature, as seen with HIV, Zika and Ebola. Genomic evidence also suggests that a virus similar to SARS-CoV-2 originated in horseshoe bats (Rhinolophus spp.), before spreading to an unknown animal that then passed the pathogen to people.
The investigation concluded that an animal origin was much more likely than a lab leak. But since then, politicians, journalists, talk-show hosts and some scientists have put forward unsubstantiated claims linking the coronavirus to the Wuhan Institute of Virology (WIV), in the Chinese city where COVID-19 was first detected. Some members of US Congress and the media have gone further, alleging that the Chinese government is covering up a SARS-CoV-2 leak from the WIV, and even that Anthony Fauci, director of the US National Institute of Allergy and Infectious Diseases (NIAID), is involved, because NIAID funded some studies at the WIV. The WIV and Fauci have denied this, saying that they did not encounter SARS-CoV-2 until after the virus was isolated from patients in late December 20192.
Even if the letter in Science was well intentioned, its authors should have thought more about how it would feed into the divisive political environment surrounding this issue, says Angela Rasmussen, a virologist at the University of Saskatchewan in Saskatoon, Canada.
The lead author of the letter, David Relman, a microbiologist at Stanford University in California, still feels it’s important to voice his opinion — and says he can’t stop it from being misrepresented. “I am not saying I believe the virus came from a laboratory,” he says. Rather, he says that the authors of the WHO investigation report were too decisive in their conclusions. He suggests that the investigators might have called the natural-origins hypothesis “appealing” instead of “highly likely”, and that they should have written that they didn't have enough information to draw a conclusion about a leak. Investigators toured the WIV and questioned researchers there, but were not given primary data.
In the Science letter, the authors note that Asian people have been harassed by those who blame COVID-19 on China, and attempt to dissuade abuse. Nonetheless, some aggressive proponents of the lab-leak hypothesis interpreted the letter as supporting their ideas. For instance, a neuroscientist belonging to a group that claims to independently investigate COVID-19 tweeted that the letter is a diluted version of ideas his group posted online last year. The same week, on Twitter, the neuroscientist also lashed out at Rasmussen, who has tried to explain studies suggesting a natural origin of SARS-CoV-2 to the public. He called her fat, and then posted a derogatory comment about her sexual anatomy. Rasmussen says, “This debate has moved so far from the evidence that I don’t know if we can dial it back.”
Relman says he’s saddened by the abuse of his fellow scientists, but he stands his ground.
Scientists at odds Demands for laboratory investigations ramped up further as the World Health Assembly commenced on 24 May. The United States has since requested that the WHO conduct a “transparent, science-based” phase-two origins study, and US President Joe Biden announced that he has asked the US intelligence community, in addition to its national labs, to “press China to participate” in an investigation. The WHO, which does not have the authority to conduct an investigation in China without the country’s permission, is currently considering proposals for this next-phase origins study.
In the meantime, US headlines are exploding with revived interest in the lab-leak hypothesis, many of them related to two articles in The Wall Street Journal. One story refers to an undisclosed document from an anonymous official who was part of former US president Donald Trump’s administration, suggesting that three WIV researchers were sick in November 2019. And the second says that Chinese authorities stopped a journalist from entering an abandoned mine where WIV researchers recovered coronaviruses from bats in 2012. The researchers have long maintained that none of the viruses were SARS-CoV-2. Responding to The Wall Street Journal, China’s foreign ministry said: “The US keeps concocting inconsistent claims and clamoring to investigate labs in Wuhan.”
Kristian Andersen, a virologist at Scripps Research in La Jolla, California, maintains that no strong evidence supports a lab leak, and he worries that hostile demands for an investigation into the WIV will backfire, because they often sound like allegations. He says this could make Chinese scientists and officials less likely to share information. Other virologists suggest that such sentiments could lead to more scrutiny of US grants for research projects conducted in China. They point to a coronavirus project run by a US non-profit organization and the WIV that was abruptly suspended last year after the US National Institutes of Health pulled its funding. Without such collaborations, says Andersen, scientists will have difficulty discovering the source of the pandemic.
Diplomacy distraction More is at stake than the discovery of COVID-19’s origins, however. Global health-policy analysts argue that it’s crucial for countries to work together to curb the pandemic and prepare the world for future outbreaks. Actions needed, they say, include expanding the distribution of vaccines and reforming biosecurity rules, such as standards for reporting virus-surveillance data. But such measures require a broad consensus among powerful countries, says Amanda Glassman, a global-health specialist at the Center for Global Development in Washington DC. “We need to look at the big picture and focus on incentives that get us where we want to go,” she says. “A confrontational approach will make things worse.”
Fidler agrees. He says that the escalating demands and allegations are contributing to a geopolitical rift at a moment when solidarity is needed. “The United States continues to poke China in the eye on this issue of an investigation,” he says. Even if COVID-19 origin investigations move forward, Fidler doesn’t expect them to reveal the definitive data that scientists seek any time soon. The origins of most Ebola outbreaks remain mysterious, for example, and researchers spent 14 years nailing down evidence that the 2002–04 epidemic of severe acute respiratory syndrome (SARS) was caused by a virus transmitted from bats to civets to humans.
So, with a pressing need for biosecurity policies, Fidler thinks the United States should focus on fostering pandemic diplomacy through meetings between US and Chinese ambassadors, as happened with climate-change discussions in April. “Don’t we actually have some things we need to do to get ready for the next pandemic, given the debacle of this one?”

1. "I HAVE A HARD TIME ORGASMING IN THIS POSITION. ARE THERE ANY TOYS THAT CAN HELP?" 3. "WHAT ARE THE EASIEST THINGS TO KEEP IN MIND TO MASTER WOMAN-ON-TOP?" Get out of your head and try not to overthink the process, suggests sexologist Malika O’Neill, LPC, and founder and CEO of The Pleasure Collective, LLC. Overthinking things might psych yourself out and O'Neill says this is typically where people will find themselves either going off rhythm or being so anxious that they aren't able to find pleasure in the experience. "Remember you're in control and allow that to make you feel powerful. Let your hips guide you and do most of the work," suggests O'Neill, adding that if you try to work out your entire body while on top, it'll tire you out faster and you'll likely lose stamina.
4. "IS THERE ANY PARTICULAR RHYTHM YOU SHOULD TRY TO MOVE TO DURING WOMAN-ON-TOP? I'VE HEARD (OR READ) THAT YOU SHOULD TRY TO SPELL 'COCONUT' WITH YOUR HIPS WHILE ON TOP. WHY DOES THIS TRICK WORK? ARE THERE ANY OTHER EASY TRICKS OR WORDS YOU CAN SPELL WHILE ON TOP TO MAKE IT SEEM LIKE YOU KNOW WHAT YOU'RE DOING?" The "coconut" trick is great, because it's all rounded letters that are easy to "spell" with your hips. O'Neill also suggests trying to spell the word "cowgirl." Go at a medium pace that allows you and your partner to feel each rhythm or letter — this not only allows you to slow down enough to make things pleasurable, but you're also able to gauge your partner's reaction and body language to see which letters they enjoy more than others.
Another easy trick O'Neill suggests for Riding 101 is to try to create the letters "W" or "M" with your hips as well. You can also have you or your partner try to create the letter "O" in slow motion, she suggests.
Get creative with it! If you or your partner find that you have a penchant for rounded letters or letters with lots of peaks and dips like "W" or "M," incorporate those into your hip rhthym. As for some very valid reasons you might not be into being on top, O'Neill says you might just prefer when your partner is in control, you might be afraid of heights, or you might experience sexual pain when on top for a variety of reasons.
Don't be afraid to voice your preferences and never feel obligated to get on top if you hate it just because you don't want to seem "lazy" — that's just not true. 7. "NO, BUT SERIOUSLY: HOW DO I GET OVER ANY INSECURITY ABOUT MY LOOKS WHILE ON TOP?" You can also try squatting with your feet flat. You’ll need more balance, but it opens your hips for deeper penetration.
9. "WHAT DO I HOLD ONTO DURING WOMAN-ON-TOP?" The answer: Build a sex fort. Okay not really, but it’s totally cool to call in for furniture backup if you need. “Don't be shy to use props especially if you have short legs or need extra support through the pelvic floor,” says Fiona Gilbert, sexual wellness consultant. Yes, in theory it makes sense that being on top lets your clit get more stimulation, but if that angle just isn't happening for you, you've got options. To find that sweet spot, Martin suggests moving your body forward and back very slowly, and left to right, to see if you can notice and slight differences in sensation. Basically, troubleshoot!
Too deep? No problem. "If you are uncomfortable because the penetration feels too deep, you can try lying forward and propping yourself up on your hands or forearms almost like you are in a missionary position," Martin offers. And don't get discouraged if you don't get there right away. "The key is to not get caught up thinking it's not working and you need to fix it, but to focus instead on sensually discovering the spot that holds the greatest pleasure for you."
12. "HOW DO YOU FIGURE OUT HOW TO MOVE YOUR BODY?" Once you've found the perfect angle, you can still get bogged down on how to move, especially if you think you have to be a dancer or someone with impeccable natural rhythm in order to nail it. But all you need is practice: Hall suggests starting by making small, slow adjustments to your hips or body until you hit that sweet spot.
"You can use your leg muscles and core to move up and down, then carefully lean forward or back to bring things into alignment," Hall says."Your hands can go on the bed for support and balance as you move into positions that feel great for you, and what feels good often changes, so you should feel comfortable adjusting as desired."
13. "WHAT CAN I TRY IF I JUST GET REALLY TIRED REALLY QUICKLY WHEN I’M ON TOP?" It's no surprise that being on top exerts a liiiiiiittle more energy than just lying there and noticing all the cracks in your ceiling. Martin counsels that you can always slow way down but adds, "The biggest thing is to remember to breathe. If you’ve ever done intensive workouts, you can go much farther and longer if you breathe deeply. This will seem totally natural since people tend to breathe much more intensively during sex." Deep inhales and exhales can help you focus on sensation and not become exhausted so quickly.
But you can also let your partner take over for a bit. "Sex is a dance," Halls says. And your partner is probably going to move their hips a little too, because it feels amazing. She also recommends leaning forward so you're lying on top of your partner in a sort-of missionary pose to ease the strain (and signal to them that they can contribute to the thrusting as well).
14. "ARE THERE ANY WORKOUTS YOU CAN DO TO MAKE ON-TOP SEX BETTER FOR YOU?" "Great sexual stamina comes from strength in your large muscle groups and a good cardio routine," Hall says. "Working on your legs and core will really help in the bedroom. I also always advise doing Kegels, which will help strengthen your pelvic muscles, which means bigger, better orgasms."
15. "ARE THERE ANY VARIATIONS OF THE TRADITIONAL 'ON TOP OF MY PARTNER, FACING THEM' POSITION? AND WTF ARE THEY?" You've got other choices! 1. Your bio is empty
Whether your profile is verified or not, you’ve got to give people something to go on; how will she know she wants to talk to you if there’s nothing to pique her curiosity? Plus, girls view empty bios as a red flag, so make sure to put something there.
2. You’re trying too hard I’m sorry but no one actually likes reading big words used incorrectly, or like at all. Keep your bio simple and just try not to type lyk dis. Just talk about some of your interests, what you’re looking for, and leave the rest to when you’re talking. As long as you come off genuine, you’ve got a chance.
3. Or you’re not trying at all Please don’t just write “Hookups” in your bio and go. Even if that’s all you want. 4. Try not to be condescending

5. Please, for the love of all that’s holy, stop with the shirtless pics
I get it, you’re proud of your body. You worked hard on it. But leave something to the imagination, right? Save it for when you’re actually chatting. Unless you’re on a beach or the pool in the picture, a shirtless pic just looks desperate.
6. You’re putting up pictures with Instagram or Snapchat filters and captions At least put up the original picture rather than something you got off your story or your friends’ story. Pictures with a little editing is different, but leave those weird Snapchat and Instagram filters to their intended platform please. And if you are using something you put up on your story, maybe try to make sure it’s relevant or at least funny.
7. You have less than three pictures uploaded Yeah, it’s not a huge problem if you’ve got other things in your profile to supplement a lack of enough pictures, but try to make sure you put up at least three decent pictures. Less pictures means less trust, right from the get-go.
8. Your first picture is not you
Yes, women do want to see your dog, or cat, or whatever animal you have or met, but the first thing they want to see is who you are. Not your pet, or your group of friends, or even that sick sunset you saw the other day. Make sure there’s enough pictures of you for them to know what you look like. This isn’t a team project.
9. Your bio is just a random quote
I’m sorry, these are worse than empty bios. In fact, skip the quote all together, unless it’s something super meaningful to you or earth-shattering in its poetry or wit or whatever. You can discuss your favourite quotes later; a quote in your bio is mostly just pretentious.
10 .You use too many emojis in your bio
Listen, we love emojis. They’re short, get the point across, and help indicate the tone of messages. But I’m pretty sure you don’t need to end every sentence with a 🏽 or a or even a . Stop 🏽. Just use your words, please and thank you.
11. You’re not smiling at all
Smiling makes people a thousand times more attractive. Fine, you put up a few pictures with you looking serious stuff, but that’s not all you are, right? Let your personality shine through your pictures; smile a little, upload pictures of you doing activities you enjoy.
In the end, the most important part about your bio is for others to get a taste of who you are as a person; don’t spoil the whole story, just give a teaser and be as true to your personality as you can. Believe me, it will save you a lot of grief later. And good luck!

7 types of vaginal discharge and what they mean
Clear and stretchy or lumpy like cottage cheese? We look at the 7 most common reasons for vaginal discharge, so you know when to relax - and when to worry.
Unexpected marks in your underwear? In most cases, vaginal discharge is completely normal and all women (and vagina-owners) will experience it. Vaginal discharge can occur at any age, but as it serves an important female reproductive function, most women first experience discharge in the run-up to puberty and continue to until after the menopause.
Discharge is your body’s way of cleaning the vagina, and it’s usually nothing to worry about. However, if your discharge smells or looks unusual, it can sometimes indicate something is amiss, so it’s good to get clued up on the contents of your knickers and nip any potential infections in the bud.
We spoke to Mr Narendra Pisal, consultant gynaecologist of London Gynaecology, Dr Karen Morton, consultant gynaecologist and founder of Dr Morton’s and Dr Sarah Welsh, co-founder at HANX and gynaecology doctor, about the most common causes of vaginal discharge, so you’ll know when to sit pretty and when to seek medical help:
What is vaginal discharge? Vaginal discharge is the result of secretions produced from small glands in the lining of the vagina and the cervix, and in most cases, it is nothing to worry about. The amount varies, but you may find you experience heavier discharge during pregnancy, when you ovulate and if you’re sexually active.

‘We are mammals, and like all other mammals, we have fluid coming out of the vagina most of the time,’ explains Dr Morton. ‘When it is not literally coming out and making a mark on our underwear, the vagina should nonetheless be moist. The general moisture is due to fluid exuding through the walls of the vagina. This is called a transudate. It increases massively during sexual arousal and it is this that makes a woman wet.’
Vaginal discharge and your cycle You’ve probably noticed your vaginal discharge changes in appearance from time to time. In fact, the colour, consistency, and amount can differ from day to day, depending where you are in your menstrual cycle. Here’s a breakdown of what to expect:
Days 1–5: otherwise known as your period, discharge is usually red or bloody as your body sheds your uterine lining. Days 6–14: in the days following your period, you’ll notice there’s less vaginal discharge than usual. Then, as the egg starts to develop, it’ll become cloudy and may feel sticky. Days 14–25: right before ovulation, discharge will be clear, thin and slippery. After ovulation, it’ll go back to being cloudy and possibly milky. Days 25–28: again, you’ll notice a reduction in vaginal discharge just before starting your next period. Types of vaginal discharge Since ‘normal’ vaginal discharge varies so much, it can be tricky to know if something is up. Let’s take a look at the most common forms in more detail:
1. White vaginal discharge White discharge is totally normal, particularly towards the beginning and end of your menstrual cycle. It may also be cream in colour, or occasionally a very pale yellow. ‘After you have ovulated and produced eggs, there will be a release of the hormone progesterone in the blood stream, and this is the reason for the white discharge,’ says Dr Morton.
2. Clear vaginal discharge Clear, watery vaginal discharge is commonplace any time of the month, and is never anything to worry about. It may be especially prevalent after exercise, as intra-abdominal pressure forces your vagina to expel a little more than it normally would – this is totally normal.
When clear discharge becomes sticky or mucous-like, it’s a sign that you’re ovulating and your ovary is releasing a mature egg that will pass through the fallopian tube for fertilisation. ‘Two to three days before you start to ovulate, you will experience ovulation discharge, the texture being like raw egg white,’ says Dr Morton. This clear sticky fluid makes it easier for sperm to travel to the egg, which is also a great indicator for women who are wanting to conceive. 3. Thick gummy vaginal discharge If you are pregnant you may notice a heavier, gummy or thick discharge. This is because the cervix and vaginal walls get softer, and discharge increases to help prevent any infections travelling up from the vagina to the womb.
You can also experience discharge towards the end of your pregnancy as well. ‘Most women will have an increased amount of vaginal discharge during pregnancy, particularly in the later stages,’ says Dr Morton. ‘As long as the discharge isn't smelly, itchy or blood stained then nothing needs to be done. The most important thing to be aware of is if there's a lot of watery loss, get checked out by your midwife or the hospital.’
4. Clumpy cottage cheese vaginal discharge If your discharge has a thick, cottage cheese-like appearance, smells strongly, and is accompanied by itching or burning, it’s likely to be a yeast infection, otherwise known as thrush. You may also experience itching, soreness, burning and irritation.
‘Thrush usually occurs when there’s an overgrowth of yeast that lives normally in your gut. But if this yeast gets out of control, it can lead to unpleasant symptoms,’ says Dr Morton. ‘A typical reason is taking antibiotics, for something unrelated like earache or tonsillitis, which kills off healthy bacteria.’
5. Brown or red vaginal discharge Brown or red vaginal discharge typically occurs during or immediately after your period, which is perfectly normal and usually nothing to worry.
But a prolonged blood stained discharge can be more of a worry, so if you have any concerns get it checked out. ‘This could be a brown staining, slightly unpleasant smelling discharge,’ says Dr Morton. 'It should be investigated as it could be something more serious higher up on the cervix or in the uterus.’
Brown or red vaginal discharge could also be spotting or inter-menstrual bleeding:
Spotting: you might experience a small amount of blood between your period called spotting. If you’re on the pill, you may also experience spotting throughout your cycle. If you have spotting in place of your period, and you’ve recently had unprotected sex, it can also indicate pregnancy. Spotting early in pregnancy can also be a sign of miscarriage, so it’s worth speaking to a doctor. Inter-menstrual bleeding: while spotting can be normal and is common, regularly bleeding between your periods is called inter-menstrual bleeding. There are many causes of inter-menstrual bleeding which are not serious but rarely it can be due to a problem with the womb or cervix, including cervical cancer, so if it is happening to you then make an appointment with your doctor. 6. Grey, yellow or green vaginal discharge Vaginal discharge that is a dark shade of yellow or light green typically signals a bacterial or sexually transmitted infection – especially if it’s thick or clumpy and has a strong odour. Similarly, grey vaginal discharge can be a symptom of a common bacterial infection called bacterial vaginosis.
STIs can often can lie dormant with no symptoms – so if in doubt, always get it checked out. ‘All common infections, such as chlamydia and gonorrhoea, can cause vaginal discharge,’ says Dr Morton, ‘but they are also often silent and are causing much more serious problems higher up in the tubes’ – so if in doubt, it’s better to get checked out.
7. Smelly vaginal discharge If you have an unpleasant looking mucus discharge that is accompanied by a fishy odour and a burning sensation, it could be bacterial vaginosis. ‘This is due to a loss of balance of the normal bacteria of the vagina,’ says Dr Morton. ‘It’s also very common in older women when they don’t have oestrogen, which creates moisture in the vagina and keeps germs in the right balance.’
Vaginal discharge causes Most of the time, vaginal discharge is a healthy bodily function – but sometimes it can indicate thrush, bacterial vaginosis, or a sexually transmitted infection so it is worth getting checked out if you're not sure:
✔️ Bacterial vaginosis Bacterial vaginosis (BV) is a common infection of the vagina that can occur when the natural balance of the bacteria in the vagina changes, says Mr Pisal. ‘It’s an overgrowth of bacteria and is different to thrush which is an overgrowth of yeast,’ he explains. ‘This can cause intense vaginal irritation, a foul, fishy smell and sometimes itching. It may cause burning pain during intercourse.’
There’s no single cause for BV, but stress seems to trigger it, so make sure you take care of your mental health, says Mr Pisal. Chronic medical conditions such as diabetes also increase the likelihood of BV. Avoiding perfumed body wash and taking oral probiotics may help keep the condition at bay. ‘Making a diagnosis isn’t always straightforward so visit your GP or gynaecologist,’ he adds.
✔️ Trichomoniasis Trichomoniasis is a sexually transmitted infection caused by parasite Trichomonas vaginalis, says Mr Pisal, and is usually spread by having intercourse without using a condom and also sharing of sex toys.
‘Frothy green vaginal discharge with an unpleasant odour is typical of trichomoniasis,’ he says. ‘It can cause itching, irritation and painful sex. If it affects the urethra there may be associated pain when passing urine.’ ✔️ Yeast infection A yeast infection – or thrush as it is often known – is a common vaginal infection caused by an overgrowth of yeasts that usually live in the bowel, says Mr Pisal, most often Candida albicans, a yeast-like fungus. ‘Thrush occurs when the good bacteria in the vagina can't keep the fungus under control, creating a suitable environment for the overgrowth of this fungus,’ he says. It causes symptoms of itching and irritation with a thick, white, curd-like discharge.
Thrush is more likely to occur in pregnancy, while taking an oral contraceptive pill or antibiotics, or due to stress or uncontrolled diabetes, Mr Pisal continues. ‘There has been some association with low iron and vitamin D levels,’ he says. ‘Antibiotics can cause imbalance of the good bacteria in the vagina, thereby promoting overgrowth of the yeast – causing thrush.’
✔️ Chlamydia Chlamydia is among the most common sexually transmitted infections in the UK, and if left untreated, it can cause fertility issues. ‘Often chlamydia presents with no symptoms but in some cases there is discharge and irregular bleeding,’ says Mr Pisal. ‘Symptoms can increase with severity of the disease to include abdominal pain, fever and abscesses in the fallopian tube or ovary.’ If you're in any doubt, call your local sexual health clinic and get yourself tested as soon as possible.
✔️ Pelvic inflammatory disease (PID) When an infection is not diagnosed and treated promptly, it can spread to the fallopian tubes and the pelvis which can lead to pelvic inflammatory disease (PID). PID is an infection of the female upper genital tract, and can lead to blocked fallopian tubes and infertility.
‘PID is often characterised by severe pelvic pain, raised temperature and an increase in vaginal discharge, which may have a fishy smell,’ says Mr Pisal. ‘Admission to the hospital and intravenous antibiotics are sometimes needed.’
✔️ Human papillomavirus (HPV) HPV is the name given to a group of viruses that affect your skin and the moist membranes lining the body, says Mr Pisal. It’s extremely common – so common, it’s impossible to avoid unless you don’t have sex at all. Certain strains of the virus can cause cervical cancer, though this takes a very long time.
‘Early diagnosis through regular screening offers the best chance of stopping any irregularities in their tracks,’ he says. ‘If you have regular cervical screenings every three years, the abnormality will be detected before it becomes anything serious.’
HPV can cause genital warts and can affect the vaginal mucosa. While there are often no signs of early cervical cancer, there may be an increase in vaginal discharge, which may be pale, watery, pink, brown, bloody or unpleasant-smelling. How much vaginal discharge is normal? When it comes to discharge, there is no normal amount. Every woman is different, and the amounts will often fluctuate. ‘The exact nature of the fluid which comes out of the vagina will vary during the monthly cycle, and the amount will vary from woman-to-woman, in the same way as periods themselves will be variably heavy,’ explains Dr Morton. If you experience an unusual amount of discharge, it may be a warning sign. ‘The vagina is self-cleaning, so sometimes vaginal discharge could simply be telling you to stop messing around with it!’ says Dr Walsh.
When it comes to discharge, there is no normal amount and every woman is different.
‘The vaginal pH balance is very delicate, and the normal pH is between 3.8-4.5, making it moderately acidic. However, we can disrupt this balance by using perfumed soaps washes or wipes down there, or douching,’ she explains. ‘These products or excessive washing can disrupt the vaginal pH, making it more alkaline and easier for ‘bad’ bacteria to grow, causing abnormal discharge. This is what happens with BV for example.’ When to be concerned about vaginal discharge The colour of healthy discharge is usually clear, or milky white. If the colour, smell, or consistency of your discharge is different to usual, and especially if you also have vaginal itching, burning, pelvic pain or a high temperature, you could be dealing with an infection or other condition, so get checked out by a medical professional.
‘You should be concerned if it is smelly, itchy, sore or blood-stained,’ says Dr Morton. ‘On top of the physiological normal discharge there may be abnormal discharge which may be causing itching or soreness or have an unpleasant smell. Discharge should not be stained with blood, except mid-cycle, when there may be a tinge of blood in the ovulation mucus.’
If you notice any change from your ‘normal’ pattern – for example, a sudden increase in discharge that continues for a number of weeks, you should seek medical attention promptly, advises Mr Pisal. ‘This can be due to infections or retained tampon,’ he says. Meanwhile, an excessive amount of discharge ‘can be caused by some common and benign conditions, such as cervical polyps and cervical ectropion,’ he adds. What to expect at a doctor’s appointment When you see your doctor for abnormal vaginal discharge, they’ll likely conduct a physical exam. You can also expect to be asked questions about your symptoms, menstrual cycle, and recent sexual activity. ‘The examination may include a feel of your tummy, then look at your vulva and vagina,’ says Dr Walsh. ‘The doctor or nurse will have a look at your vulva to see if there are any rashes, or sores, and then use a speculum to examine the vaginal walls, look at the discharge, look at the cervix (the neck of the womb) and are likely to take swabs too. These swabs will be sent to the lab for testing to see whether any infection is present.’
If you notice a significant change from your ‘normal’ pattern, seek medical attention promptly.
The examination is usually quick and shouldn’t be painful. ‘You will be offered a chaperone, and the health professionals will support you through the examination, so if you’re feeling anxious, let them know,’ adds Dr Walsh. What if you have an STI or infection? If your discharge is typical of a particular infection, your doctor or health advisor may tell you there and then what the infection is and give treatment, explains Dr Walsh. ‘For example, with vaginal thrush, it has very typical signs and symptoms, so they may give you treatment for this if that’s the diagnosis. Other infections, such as herpes, may be diagnosed on taking a history and carrying out an examination.’
Where the doctor can’t immediately diagnose the problem, they may run further tests – for example, they may take some cell samples from your cervix to check for HPV, or examine your discharge under a microscope to determine the cause of the infection. If this occurs, they will likely send the swabs for testing and let you know as soon as they have the results, as well as letting you know if you may need any treatment. If the tests are taken at your GP surgery they may refer you to a sexual health clinic for treatment and follow up.

Most people wake up feeling hot every once in a while and it's perfectly normal to sweat occasionally during the night. If the weather is hot or you're wearing too many clothes in bed, most people will feel a bit hot. But if you keep waking up bathed in sweat or you're struggling to sleep at all, you might be concerned.
Night sweats are a common symptom of the menopause and some hormonal conditions, but they can also be caused by medications and some medical conditions. The good news is most cases of night sweats aren’t a cause for concern and are easily fixed. Dr Roger Henderson looks at the possible causes of night sweats and how to treat them:
What are night sweats? The best definition of night sweats is excessive sweating during the night and there are many possible causes for this including simple ones such as sleeping in a bedroom that is too hot or wearing too many clothes in bed. Feeling warm on your face or body is different to a true night sweat where sweating wakes you up and drenches your clothes and sheets, and are not linked to the bedroom environment. What causes night sweats? There are many different possible causes of night sweats. The 10 most common night sweat causes usually seen by doctors include the following: Laser and peels: Laser and peels should only be considered when performed by an experienced medical clinician. There is a risk of scarring or irritation and worsening of any pigmentation if given by inexperienced practitioners. Cryotherapy: Cryotherapy is a freezing technique using liquid nitrogen to remove sunspots, carried out by a dermatologist. Proceed with caution as lesions may recur. Microdermabrasion: Microdermabrasion is a treatment that exfoliates your skin on a professional level, by removing the top layer of skin including the sunspot. Microneedling: Microneedling uses tiny needles to help trigger stimulation of natural collagen production so is used as an anti-aging treatment and has been seen to reduce sunspots. When to worry about sunspots Sunspots are harmless but if a pale brown spot seems to be enlarging, areas of darker pigmentation occur, or the skin surface changes make an appointment to see your doctor. Itching, bleeding or pain in a long term existing lesion should always be reported. If in any doubt whatsoever always get it checked out. 1. The menopause Over 80 per cent of women experience severe hot flushes both day and night during the menopause and the perimenopause, and these are triggered by the hormonal changes that occur during this time. Severe anxiety triggers our stress response, the consequences of which include sweating and a raised heart rate. Other common symptoms include nightmares, feelings of worry and dread, digestive problems, unexplained aches and pains, mood changes and feeling tired and generally unwell. 4. Low blood sugar If you are taking tablets or injecting insulin to treat diabetes, your sugar level may drop during the night causing significant sweating. Low sugar levels (hypoglycaemia) can be dangerous so always discuss this with your doctor or diabetes specialist. These can also occur as a result of drinking an excessive amount of alcohol before going to bed.
5. Infections The classical infection known to trigger night sweats is tuberculosis (TB) but almost any infection can cause them if it is severe enough, including viral infections such as influenza or Covid-19. Bacterial infections such as abscesses, severe chest infections, heart valve infection (endocarditis), abscesses and bone infections are also well known in triggering night sweats. When an infection is severe enough to cause night sweats, other symptoms such as fatigue, nausea and feeling very unwell are also usually present. 6. Idiopathic hyperhidrosis This is the term given to excessive sweating by the body both day and night and can be a condition in its own right, where the body produces too much sweat all the time without any identifiable medical cause being found. Specialist treatment can sometimes help with this but usually involves being referred to a specialist. 7. Cancer Although uncommon, night sweats can occasionally be an early symptom of some cancers, particularly lymphomas and leukaemia. Occasionally, undiagnosed cancers can trigger night sweats along with other symptoms such as weight loss and malaise.

8. Hormonal disorders Significant sweating both day and night can be experienced in certain hormonal conditions not linked to the menopause. These include common conditions such as an overactive thyroid gland (hyperthyroidism), and rarer ones such as carcinoid syndrome and pheochromocytomas.
9. Neurological disorders Although uncommon, disorders affecting the central nervous system can trigger night sweats including following a stroke, autonomic dysreflexia, and conditions causing autonomic neuropathy.

Lifestyle factors Eating hot and spicy food before going to bed, drinking too much alcohol, using recreational drugs such as cocaine, and having too much caffeine in your body are all potential triggers for sweating. When to see your doctor about night sweats It is always worth seeking a medical opinion if night sweats become a persistent problem, especially if accompanied by weight loss, fatigue, easy bruising, altered bowel function, pain, a persistent cough or a raised temperature.
The tests your doctor will carry out very much depend on the history of the sweats and the findings of a medical examination. Depending on these, test options include blood tests, X-rays and scans such as ultrasound scans or MRI scans. Night sweats treatment and prevention Night sweat treatment depends on the cause, but some causes can be prevented. For many people with mild night sweats, simple measures are often all that are required to treat them effectively. To lower your risk of experiencing night sweats try the following:
✔️ Always make sure your bedroom is a comfortable temperature and not too hot.
✔️ Avoid wearing excessive clothing at night.
✔️ Use lighter duvets, remove heavy blankets and open windows if necessary.

8 reasons why your hair is thinning and how to fix it

Shedding the odd hair is part of every day life and most people will notice a few strands in the shower or whenever you brush your hair. The scalp hair life cycle All of the hairs on our head have a normal life cycle, with most scalp hairs lasting for about three years before they fall out and are then replaced with a new hair. Hair grows from tiny pouches in the skin known as hair follicles, and at any one time all the scalp hair follicles are at different stages of their cycle. This means about one per cent of scalp hairs are always at the end of their natural life cycle and about to fall out – this is why we all see a few hairs in the bath or shower whenever we wash our hair. Most of us lose around 50-100 hairs every day in this way.
There are three stages to the life cycle of a hair:
Anagen: the anagen phase is the active growing phase when the hair strengthens and thickens. Catagen: in the catagen phase, there is a short resting period when the hair stops growing. Telogen: during the telogen phase the hair is gradually pushed upwards before being shed naturally and replaced with a new anagen cycle hair. What is thinning hair? Thinning hair is different to baldness (male or female pattern hair loss), and refers to only mild to moderate hair loss in areas rather than on a widespread basis.

If more than normal amounts of hair fall out, this condition is called telogen effluvium, and causes a general thinning of scalp hair but with a healthy scalp. This is the most common cause of thinning hair and typically occurs about 1-3 months after a significantly stressful life event such as after childbirth, an operation or a significant illness.
In most cases, no treatment is required and once the stressful event has passed the hair gradually thickens back up to normal with no long-term consequences. Hair loss diagnosis Doctors can usually give a quick diagnosis about the cause of thinning hair and hair loss with few if any tests being required. They usually ask questions about diet, stress, existing medical conditions and any family history of medical conditions causing thinning hair.

If tests are needed these include blood tests to check for anaemia, thyroid problems, vitamin levels and autoimmune conditions and less commonly may send hair or scalp samples for testing. 8 common hair loss causes There are a number of potential triggers for hair thinning and hair loss:
1. Hair treatments Without realising it, many people can treat their hair quite badly! Over-treating it with harsh hair products such as colour treatments, relaxers, bleach and perm solutions can all cause damage to the hair, as can extreme-hold hairsprays and gels. Pulling the hair into a very tight bun or ponytail can also put excessive strain onto the hair, breaking it away from the hair follicles in the scalp. Repeatedly pulling on the hair follicles (such as may occur when wearing braids) can cause the condition traction alopecia where hair thins in the area it is under most stress from being pulled. 2. Anaemia and iron deficiency In women, one of the most common causes of hair thinning and hair loss is iron deficiency, typically caused by menstrual blood loss but which may also occur due to dietary deficiency. Iron is essential for producing healthy hair and so low levels can impact on both the strength and quantity of scalp hair.
3. Vitamin B12 deficiency Vitamin B12, along with folic acid and other minerals, is very important for healthy hair since low levels of it affects the health of the oxygen-carrying red blood cells needed for healthy hair growth. B12 deficiency is most commonly seen in vegans, as the primary source of dietary B12 is through animal protein although it can also be obtained through fruit and vegetables.
4. Stress Along with the telogen effluvium hair loss linked to significantly stressful life events, general low-level uncontrolled stress causes high levels of hormones such as cortisol which can damage the growth of new hairs developing in scalp hair follicles, as well as raising male hormone levels that in turn can accelerate hair loss. Stress can also trigger conditions such as eating disorders, dandruff and digestive problems which themselves can then impact on healthy hair growth.
5. Pregnancy The normal hormonal changes that occur during pregnancy may also impact on hair growth, resulting in hair thinning during the later stages of pregnancy and for a few weeks after birth. This typically grows back normally without any treatment in the months and weeks afterwards.
6. A poor diet Because the body needs a full supply of nutrients to allow hair to grow normally and healthily, any deficiency in these can cause hair to thin. These include proteins and vitamins, and signs of poor nutrition include brittle hair that is dry and dull, sometimes with associated scalp dryness and dandruff.
7. The menopause Going into, or through the menopause can cause hair to thin because of falling levels of female hormones. Other symptoms of the menopause include hot flushes and sweats, altered periods, mood changes, vaginal dryness and altered libido.
8. Dramatic weight loss Crash dieting – especially if you have lost 20 pounds or more in a short period of time – can cause hair thinning to occur several weeks later. This can also occur with eating disorders. Hair loss home remedies Although simple home remedies are unable to reverse hair loss due to serious medical problems, they may be helpful in thickening hair when it has thinned because of lifestyle factors. These include:
✔️ Eating as healthy a diet as possible Because good nutrition is essential to hair growth, increasing your intake of hair-healthy food is very important. Such foods include:
Eggs - rich in protein and B vitamins. Oily fish and nuts - rich in omega-3. Red meat - rich in iron. Fortified foods such as milk and cereals - rich in vitamin D. ✔️ Massaging the scalp Scalp massage can help to promote blood flow to the scalp and so, in turn, hair growth. Using essential oils at the same time such as lavender or peppermint oil has also been reported to stimulate hair follicles although there is little scientific data to confirm this.
✔️ Aloe vera Aloe vera has long been used for treating thinning hair by reducing scalp inflammation and unblocking hair follicles. This can be applied in pure form as a gel to the scalp and hair or as a shampoo and conditioner that contain aloe vera.
✔️ Anti-thinning shampoos Although anti-thinning shampoos may not prevent hair loss, they can create the appearance of thicker hair by providing volume to individual hairs. They also contain vitamins and amino acids which can help improve scalp health over time but to achieve the best results they need to be used every day. Hair loss medical treatments For simple hair thinning rather than male or female pattern baldness, most doctors would not consider prescription treatment for hair loss unless it was causing major psychological distress. But if they were found to be necessary, available hair loss treatments include: When to see a doctor about hair loss In most people with thinning hair, this is self-limiting and the hair will normally grow back to normal thickness over time with no treatment being necessary. If you lose a lot of hair for no obvious reason – especially if you start to develop bald patches - see your doctor. If required, they can refer you to a dermatologist for further assessment or do some initial tests themselves.
For most people it is natural for hair to lose strength and volume as they get older. In many cases, simple home remedies and a healthy diet can help keep the hair as thick and healthy as possible.

Longer term risks associated with sun overexposure include skin cancer, eye problems – such as photokeratitis or cataracts, and premature ageing of the skin. Sunburn symptoms Sunburn can cause red, hot skin that feels painful and hot to touch and in severe cases blistering, swelling of the skin and fever. If you have heatstroke you may also experience symptoms such as dizziness, headaches or feeling sick.
It's important to remember that there is no level of 'good' sunburn. Any sunburnt skin, however mild, is a sign of skin damage and can increase the risk of premature skin ageing and skin cancer. Sunburn risk factors Anyone who is exposed to UV light is at risk of getting sunburn, although some people are more vulnerable than others. The following people should take extra care in the sun:

How long it takes for your skin to burn in the sun varies from person to person. If you're not sure, visit The Cancer Research UK website to find out your skin type and check if you are at an increased risk of burning. 10 ways to treat sunburn You can usually treat mild sunburn at home. If you're in pain and feeling uncomfortable, the following tips may help to relieve symptoms until your skin heals. If symptoms persist, you feel unwell or have any serious concerns about your sunburn, seek medical attention.
1. Stay out of the sun If you or your child has sunburn, get out of the sun as quickly as possible and stay indoors or in shady areas. Cover up the sunburnt skin and avoid all sunlight until the sunburn has fully healed. 2. Moisturise the skin Apply plenty of lotions or cream to moisturise the sunburnt skin, especially if it is hot to touch and when it starts peeling. Water based lotions and cream provide a better cooling effect on the skin. Avoid petroleum or oil-based creams, which can prevent heat and sweat from escaping and make the burn worse.
3. Drink plenty of water If you become dehydrated this can have a drying affect on your skin which may exacerbate your sunburn symptoms, so keep drinking lots of water to stay hydrated and cool you down.
4. Take a cool shower Cool down sunburnt skin by having a cold shower/bath and patting the skin with a cold wet sponge or flannel.
5. Apply a cold compress Applying a cold compress such as a wet flannel to the affected area may help cool the burn and ease discomfort.
6. Use aloe gel Lotions or gels containing aloe vera can also soothe sunburnt skin following over-exposure to the sun. Aloe vera moisturises and replenishes dry skin and has antibacterial properties which help to prevent peeling skin.
7. Try aqueous calamine cream Aqueous calamine cream may also help with itching or soreness. After application the calamine evaporates, which produces a cooling effect that helps distract you from the sunburn.
8. Don't pick or pop blisters Resist the urge to pick or pop any blisters that may form on sunburnt skin. A blister protects the skin underneath and reduces the risk of it getting infected.
9. Take painkillers If you are in pain and feeling uncomfortable or struggling to sleep at night, the pain of sunburn can be treated by taking painkillers such as paracetamol or ibuprofen which are available to buy from a pharmacy.
10. Apply a hydrocortisone cream If the skin is very inflamed and red using over-the-counter hydrocortisone cream for a couple of days may help. Hydrocortisone reduces inflammation and relieves redness and itchiness. If you need help, ask your pharmacist for advice. When to seek medical attention for sunburn If you feel unwell or have any concerns about your sunburn contact your GP or call the NHS on 111 for medical advice.
Signs and symptoms of severe sunburn include the following:
Burn over a large area of skin Blistering or swelling of the skin If you feel confused and unwell If you experience dizziness, headaches and nausea If you experience chills If you have a high temperature of 38C (100.4F) or above Sunburn prevention tips The best way to avoid sunburn is to minimise the amount of time you spend in the sun. The following tips can also help:
✔️ Wear suitable clothing To avoid sunburn opt for a wide-brimmed hat that shades the face, neck and ears, long-sleeved tops, trousers or long skirts and sunglasses with wraparound lenses. Alternatively carry a sun protective parasol.
✔️ Seek out shade Avoid direct sunlight and seek out shaded areas. However you can still suffer from the effects of the sun on the skin on a cloudy day, as ultraviolet rays can penetrate through cloud. This also means that even without sunburn, skin damage may still occur so always wear sunscreen.
✔️ Apply sunscreen Wear sunblock or sunscreen with a high sun protection factor (SPF). When buying sunscreen, make sure it's suitable for your skin and blocks both ultraviolet A (UVA) and ultraviolet B (UVB) radiation. The sunscreen label should include at least 4-star UVA protection and SPF15 sunscreen to protect against UVB.
✔️ Don't forget to reapply The average adult requires around 35ml or 6 to 8 teaspoons of sun lotion to provide adequate sun protection. If you plan to be out in the sun long enough to risk sunburn, sunscreen needs to be reapplied frequently and according to the manufacturer’s instructions. This includes straight after you've been in water and after towel drying, sweating or if it has rubbed off.
✔️ Avoid the midday sun In the UK, the risk of getting sunburnt is highest from March to October, particularly from 11am to 3pm, when the sun's rays are strongest. The ultraviolet index is a marker of how potentially damaging the sun may be that day. A high UV index means that unprotected skin will burn faster or more severely so avoid going into the sun when it is at its strongest (a good tip here is that this is when your shadow is shorter than you are) and always protect your skin when going outside.
✔️ Keep babies and small children in the shade Babies under six months of age should never be exposed to the sun. Babies older than six months should be protected from the sun, and wear UV-blocking sunglasses to protect their eyes.
