The Lady is actually very pretty but she doesn’t see it, all the mirrors show her deformed! Not just her, it’s not all in her head otherwise we wouldn’t have seen it, so that’s why she wears the mask and smashes all the mirrors. 

However you can see from the side of her face (not the reflection, actually her) that she has a pointed, dainty face with a small nose but the reflection clearly shows differently, if her face was like that then her mask wouldn’t even fit her and her nose in the mirror is large and bulbous. 

Does she physically look that way and hides it with magic but mirror’s show her true self? It couldn’t have always been that way as she sat for a portrait once upon a time (shown above), if she truly thinks she’s ugly enough to wear a mask then she wouldn’t have bothered so what on Earth happened?
