T Y P E S O F C-S E C T I O N There are 2 types of c/s - planned (also called elective) and Emergency. They are broken into 4 categories and we’ll start from Category 4, working our way back. Category 4 - this is an elective section. These are planned ahead of time and would be used for reasons such as : having had a previous c/s and deciding to have a repeat, breech babies are commonly delivered by c/s, having a multiple pregnancy, some medical conditions may mean a c/s is safer (e.g. placenta praevia) or if for personal reasons you choose that a c/s is the right method of delivery for you. The other 3 are considered as emergency, with varying degrees of urgency. Category 3 - this is considered an emergency because it is not planned ahead of labour. This type of c/s is done when there is NOT an immediate risk to mum or baby e.g. You have already booked an elective c/s but go into labour before that time. The time between deciding on a Category 3 c/s and delivery varies because there is no immediate risk, but delivery is necessary. Category 2 - This is required when there is a risk to mum or baby but is not immediately life threatening e.g. infection, malpresentation (baby coming into the pelvis in an unusual position). From the decision being made to have a c/s to baby being born, NICE (2021) recommend delivery asap, and in most situations, within 75 mins of making the decision. Category 1 - this is the one with the most urgency for early delivery of baby. This is the category of c/s when there IS immediate threat to mum and/or baby. There are different reasons why this type of c/s may be required e.g. placental abruption, uterine rupture, cord prolapse (where the cord comes in from of the head). The delivery should be carried out asap and in most situations within 30mins. The RCOG recommend that ‘the delivery should be carried out with an urgency appropriate to the risk of the baby and safety of the mother.’ Hope this helps ❤️ 📸: @helloworld_birth #todandb #todeliveryandbeyond #gentlecaesarean #caesarean #caesareansection #csection #antenatal #birth #labour #hypnobirthing #onlineantenatalclasses #hypnobirthingclass #onlinehypnobirthing (at The Farm Midwifery Center) https://www.instagram.com/p/CNphCCcHzRj/?igshid=1g5messvls7o2

COVID19 & PREGNANCY Last nights news that there have been four preliminary reports of stillbirths potentially associated with Covid19 may have caused some worry. The name given to the condition is covid placentitis, which is an infection in the placenta and occurs in some women who have tested positive for Covid19. So far Dr. Ronan Glynn (Deputy Choef medical officer) has said that the reports need to be interpreted with caution as the coroners have not yet concluded their findings. However, a study carried out on over 4,000 pregnant women, combining UK and US data did show that there was - No increase in rates of Stillbirths, Low birth weight or early neonatal death. There was a small increase in the amount of premature births found. (Click the link in my bio to read this study) More research is needed before findings can be confirmed. What are the recommendations now from NPHET? •Pregnant women and people do not need to change their behaviour and continue to comply with the public health advice. •Take standard Covid19 precautions •Monitor baby’s movements - this has been shown to be an indication of your baby’s wellbeing. (I’m going to talk about monitoring your baby’s movements on my story or watch back in my highlights) Any questions? Leave them below⬇️ I’ll do my best to answer them #todandb #todeliveryandbeyond #pregnancy #pregnant #covidandpregnancy #irishmidwife #pregnantinapandemic #antenatal #antenataleducation #antenatalclass #onlineantenatalclasses #prenatal #labour #labor #postnatal #labouranddelivery #laboranddelivery #birth #birthphotography #newmums #mumtobe #due2021 #hypnobirth #hypnobirthing #hypnobirthingclass #onlinehypnobirthclass #newborn (at Ireland (country)) https://www.instagram.com/p/CMDHlXMnCas/?igshid=418ftaju0t0i

J U S T B R E A T H E I have always said - never underestimate the power of your breath. When everything else seems out of your control, you can almost always control your breath. This can be especially helpful if, during labour and birth, your plan changes course. ‘Up Breathing’ is the name given to the type of breathing that works best during the active stage of labour. This is especially beneficial for helping you to relax and ease any tension (interrupting the fear-tension-pain cycle). ‘Down Breathing’ is the technique used in the second stage of labour. This is more about helping and gently nudging baby down through the pelvis. This type of breathing can be very helpful if the Fetal Ejection Reflex does not kick into action, helping you avoid Valsalva Pushing. (See my last few posts for more on the types of pushing). Have you tried any breathing techniques? Did you see my new Reel where I guide you through a breath? P U S H!! PT 2 Ever wondered how sometimes a baby comes so fast they’re born in the car on the way to hospital? How unplanned homebirths can happen? Or even so fast that they come along like in the picture shown here, just moments from the labour suite? I’m sure most of them mums tried not to push but their body had other plans. We have a reflex built into us, almost like a gag reflex, but it involves muscles in our pelvis. Think of it like throwing up - but throwing down the way instead. It’s called the Fetal Ejection Reflex or Ferguson’s Reflex. When you get to the pushing stage of labour (10cm) this reflex kicks in and causes involuntary pushing. It doesn’t always happen the minute your cervix is fully dilated! It might take a little longer. We talk about it more in-depth in my Antenatal and Hypnobirthing classes. We also look at how to give it the best chance to work (so you do the least amount of work), how an epidural might effect it and what you can do if you do have an epidural. Have you experienced this sensation if you’ve already had your baby? T H E B I R T H R O A R A primal noise you might find yourself surprised to be making. It is a great indicator for midwives during labour that you are getting closer to meeting your baby. Let. It. Out. It comes from deep within you, more from your belly than your throat. It is a release of the power inside of you. Birth can be intense and powerful, raw and extraordinary. It can be one of the first times you truly connect with your primal instincts. Don’t be afraid when you hear that roar - it is not always a bad thing. (Keep that in mind for the labour ward incase you hear someone in the room next door). Did you embrace it? Have you ever wondered how Hypnobirthing works? - Here’s a brief explanation of how it works and why we use it for birth. The very first thing to say is that Hypnobirthing is not that same as being hypnotised. The name can be a bit misleading, but it’s more like a form of hypnotherapy. And, despite the opinions of a well known obstetrician 😏, you are completely in control at all times (no finger-clicking-acting-like-a-duck stuff). We are constantly subjected to negative birth stories and ideas. From hearing other people tell theirs, to watching movies and telly. We are born without the fear of birth, it is learned from what we take in growing up. So to help us feel calmer, more excited and therefore interrupt the Fear-Tension-Pain Cycle (coming tomorrow) we can use Hypnobirthing to help change those embedded ideas of fear. By regularly listening to Hypnobirthing tracks during pregnancy, the message shared in them - usually along the lines of being safe, having nothing to fear and looking forward to seeing your baby, while harnessing the power of birth - can help reprogramme those thoughts. The added benefit of the tracks is they also offer relaxation due to the way they are read and can help you get into a hypnotic state. For more Hypnobirthing information join one of my upcoming classes. Contact me for details. Let me know if you have any questions⬇️ And I hope you find that useful, Laura x 📸: @lawrenrosephotography #todandb #todeliveryandbeyond #midwife #antenatal #prenatal #intrapartum #labour #labor #perinatal #postnatal #laboranddelivery #labouranddelivery #antenataleducation #antenatalclasses #onlineantenatalclasses #hypnobirth #hypnobirthing #hypnobirthingclass #onlinehypnobirthclass #newmum #mumsoninstagram #newborn #due2021 # (at Ireland (country)) https://www.instagram.com/p/CLZyuUMHeOG/?igshid=1vkpafllc0ltp

S H O U L D E R T I P P A I N This is a common complaint after having a c-section. Caused by the disturbances inside your body during the operation, your bowels can slow down, creating excess gas. This presses on the diaphraghm triggering referred pain in your shoulder. The pain is usually describes as a deep pain and can vary in severity. Some people may be concerned if they experience it, but there are ways to help. Generally analgesia, like paracetamol, is not as effective as something that will treat the production of the gas -like peppermint. You might be offered peppermint cordial, peppermint tea or Colperin (a tablet form) to ease this. Peppermint has been known to impact milk supply if you are breastfeeding and because I get asked about this I consulted with a lactation consultant to weigh up benefits/risks. The advice was that you would need to be drinking buckets of tea fo it to become an issue, so should be considered to help if you experience this discomfort. Have you felt this after a c-section? C A R E A F T E R A C-S E C T I O N Having a c-section means you are left with a wound on your skin, but there is also tissue and muscle healing deeper than that. Your doctor goes through 6 layers of skin, tissue and muscle during this surgery and all of this needs to heal aswel. So taking care of the wound is important but resting and taking care of yourself is just as important. Say yes to help. Eat well. Stay hydrated. See a Women’s Health Physio Think of how long you would allow yourself for recover after any other major surgery. This should be no different. 📸: @belleverdiglione #todandb #todeliveryandbeyond #caesarean #caesareansection #csectionawarenessmonth #antenatal #postnatal #labour #labor #birth #labouranddelivery #laboranddelivery #antenataleducation #antenatalclass #onlineantenatalclass #midwife #midwifeled #midwifeledantenatalclasses #hypnobirth #hypnobirthing #onlinehypnobirthingclass #hypnobirthingmidwife #newmum #newbaby #newborn #birthisbirth #bellybirth #due2021 #pregnant2021 #pregnant (at County Mayo) https://www.instagram.com/p/CN7isn7H1dc/?igshid=1mggqjn1fjzzt

R E A S O N S F O R A C-S E C T I O N There are lots of different reasons why someone may need to have a c-section. Here are a few: Prolonged labour (the true definition of prolonged labour and the cut off point in the hospital can vary by A LOT. We cover this in my class) Fetal distress - baby not managing well in labour. This can be caused by a variety of reasons. Baby’s are made for labour so quite often it happens when we intervene. Birth Defects - it may be decided that it would be safer for you and/or baby to have a c-section rather than a vaginal birth. Some health conditions may mean a c-section is safer. Obstetric Emergency’s - e.g cord prolapse, placental abruption Multiple births - this depends on how many babies and the position they are in. Repeat c-section - if you had a previous c-section and choose not to have a VBAC Placenta Issues - this can be anything from a low lying placenta to one that is embedded deep into the uterine tissue or a placental abruption. Breech birth - the majority of people with baby in a breech position choose to have a planned c-section. This list is not exhaustive and everyone’s journey is different. Would you like to share why you had one below?⬇️ 📸: @thefirsthello #todandb #todeliveryandbeyond #caesarean #caesareansection #csectionawarenessmonth #antenatal #postnatal #labour #labor #birth #labouranddelivery #laboranddelivery #antenataleducation #antenatalclass #onlineantenatalclass #midwife #midwifeled #midwifeledantenatalclasses #hypnobirth #hypnobirthing #onlinehypnobirthingclass #hypnobirthingmidwife #newmum #newbaby #newborn #birthisbirth #bellybirth #due2021 #pregnant2021 #pregnant (at Ireland (country)) https://www.instagram.com/p/CN7IMScHTGY/?igshid=1hiz1tavh0gdy

T I P S T O R E D U C E C/S 1️⃣Avoid an epidural, or at least delay it, if possible 2️⃣Remain in an upright position in labour and for birth (see my highlights - you can do this with an epidural too) 3️⃣Hire a doula - you decrease the chance of having a c/s by up to 25% 4️⃣Choose midwife-led care instead of private care 5️⃣If it is available and you are comfortable with it - choose to have a homebirth 6️⃣Do an independent antenatal education class like the one offered here at To Delivery And Beyond 7️⃣Try and stay at home until active labour 8️⃣Request Intermittent Auscultation of baby’s heartbeat rather than continuous monitoring 9️⃣If it’s a time issue and both you and baby are okay, request more time 🔟Avoid artificial rupture of membranes (when the doctor/midwife breaks your waters) unless indicated for a reason such as to monitor the heart-rate via the baby’s head (not common but sometimes used) 📸: @elliana_allon #todandb #todeliveryandbeyond #caesarean #caesareansection #csectionawarenessmonth #antenatal #postnatal #labour #labor #birth #labouranddelivery #laboranddelivery #antenataleducation #antenatalclass #onlineantenatalclass #midwife #midwifeled #midwifeledantenatalclasses #hypnobirth #hypnobirthing #onlinehypnobirthingclass #hypnobirthingmidwife #newmum #newbaby #newborn #birthisbirth #bellybirth #due2021 #pregnant2021 #pregnant (at London, Unιted Kingdom) https://www.instagram.com/p/CNvNeY1nZey/?igshid=u4ngjkatx0qo

This is ✨ M A G I C ✨ Following on from last nights post where we looked at what is involved in a Gentle C-Section (and so many of you loved it so hopefully more will be able to have them) - here is an example of a mum assisting the birth of her baby at a c/s Okay, this gowning up and putting on gloves might not be possible with every c-section, but it would be for a lot of them. Who would love to get their hands stuck in? 🙋🏻‍♀️ I know I would 😁 #todandb #todeliveryandbeyond #caesarean #caesareansection #csectionawarenessmonth #antenatal #postnatal #labour #labor #birth #labouranddelivery #laboranddelivery #antenataleducation #antenatalclass #onlineantenatalclass #midwife #midwifeled #midwifeledantenatalclasses #hypnobirth #hypnobirthing #onlinehypnobirthingclass #hypnobirthingmidwife #newmum #newbaby #newborn #birthisbirth #bellybirth #due2021 #pregnant2021 #pregnant (at The Farm Midwifery Center) https://www.instagram.com/p/CNumGkwnvuW/?igshid=9mhpwc7uf92h

✨ C-S E C T I O N T I P ✨ Sometimes it can be a calm experience, other times it can be chaotic. One thing I try to make clear in my classes when we cover c/s is that what’s happening in the room can help, or hinder, how you feel. (See my previous post for a breakdown of the different categories of c/s). If you are having an unplanned c/s (so category 1-3) the room can fill with people. Midwives, doctors, porters...all of this can be very overwhelming. What I like to point out is that - if the decision has been made for you to go to theatre for an emergency c/s - a category 2 (no immediate risk to mum or baby) is the most common type of emergency c/s. However, when the decision has been made, the staff usually kick into action and get you ready to go to theatre just as fast regardless of the situation. It’s very likely you’ll have someone telling you to: Drink something that helps settle your stomach contents Someone else is putting compression stockings on Someone may be putting a cannula in your arm and taking bloods A doctor could be gaining your consent A midwife could be going through a pre-op checklist An anaesthetist could be checking you to see what kind of pain relief you need. ‘All this happening at once!?’ I hear you say. Yes 🙃 So it’s no wonder the room and the experience can be a blur and chaotic. So my tip is: Don’t let the pace of what’s happening let you jump to a conclusion about the extent of the emergency. Bring yourself back to your breathing. Talk to your baby. Partners - it can be hard to know what to do but reminding you of the last 2 points is helpful. I hope you find these tips useful! #LabourLunchBox Easter edition 🐰🥚 I saw different variations of these around over the last few days and thought I’d make my own today 😊 It gave me a little throwback because a girl I was in school with had her birthday around Easter and we always had these at her party - but I think her mum made them with weetabix 🤔 All I did was: 1. Melt some chocolate (I used Cadbury’s to make it a bit richer but cooking chocolate is perfect) 2. Mix in @kelloggs.uki All-Bran (I think they looked the most like twigs) 3. Spoon into cupcake cases making a well in the centre. 4. Add @cadburyireland Mini-Eggs A nice treat for over the Easter and a great little snack for labour 🤍 #todandb #todeliveryandbeyond #labourlunchbox #easter #eastertreats #midwife #irishmidwife #antenatal #pregnant #pregnancy #postnatal #due2021 #dueapril2021 #antenatalclass #onlineantenatalclass #hypnobirthing #onlinehypnobirthingclass #labour #labor #birth #newmum #pregnant2021 (at Ireland (country)) https://www.instagram.com/p/CNPFrKFHoL9/?igshid=qgb8anjb95yo

W A I T F O R T H E W E I G H T ⚖️ A little tip I give the attendees of my class. What do I mean by it? During the first hour after birth (the Golden Hour) ideally mum and baby should not be separated. Skin to skin should remain uninterrupted during that time. There are so many amazing benefits from doing skin to skin e.g. temperature regulation, heart rate regulation, breathing regulation for baby. A huge rush of hormones for you to help with breastfeeding, bonding and to contract your uterus, helping to prevent bloodloss and deliver your placenta. If there is a reason why you can’t - don’t worry, your birth partner can do skin to skin instead and baby still gets all the benefits. *sidenote* I realise with restrictions at the minute this isn’t being facilitated after csections. It’s likely that during that time, your midwife might want to do a quick once-over check of the baby, their length, head circumference , weight and give vitamin K (if you choose to get it). But none of that is as important as your uninterrupted Golden Hour and most of it can be done with baby still against your chest! (actually the vitamin K is best given while breastfeeding as it helps soothe them). The weight however, can not be done while baby is on your chest. So a good way to maintain your space for that Golden hour is to delay the baby weight check. Make it clear in your birth plan ❌ I also tend to remind people that when you’re sending a message about the baby’s arrival, what does everyone want to know straight away? 😂 What you had and what was the weight? Swipe right ➡️ and tell me that’s not pretty much every message announcing a new baby? 😂 So if you don’t have it, it also prevents you wasting that hour (where it’s just you and your baby) before everyone starts finding out and the phones start hopping! 😁 Did you get to have the golden hour? 📸: @ambermay.birthphotography #todandb #todeliveryandbeyond #pregnant #birthphotographer #birth #labour #labor #antenatal #postnatal #newborn #newbaby #newmom #due2021 #pregnant2021 #antenatalclass #antenataleducation #onlineantenatalclass #irishmidwife #hypnobirthing #hypnobirthingclass #onlinehypnobirthingclass (at Dublin, Ireland) https://www.instagram.com/p/CNF6wAuH1DF/?igshid=8yvm3l964h5
