that's it i'm quitting ooooooh i wanna quit so fucking bad
#she bork#they're making me switch positions w the other team lead so i'm losing the inbound team (which i hate bc 1. they hate the other team lead b#he sucks and ik they're going to suffer and 2. EYE built that fucking team from the ground up) and also losing my consistent schedule AND#losing my immunity from closing AND losing my extra $1 an hour for my overnight hours so essentially i'm taking a fucking pay cut!!!!!!!!!!#there are no benefits to this and that's it i have decided i'm fucking quitting this holds nothing beneficial or enjoyable to me anymore. i#applied for a remote writing job already so we'll see if they get back to me i have my fingers CROSSED.#lol yes i only applied to one but i'm hoping that if i only throw one dart at this dartboard it's a bullseye. if not i'll keep looking it'l#just take longer obviously bc i'm not quitting without something else lined up. i'm crazy and restless but i'm not an idiot#forgot to mention that i've known this was coming but i was under the illusion it was taking place mid-january but now have just been told#it starts NEXT WEEK. so the schedule i had for the next two weeks?? now incorrect and wrong. god this is going to make moving once we find a#place (again) so much more difficult bc i missed my chance to request time off for january already but w my old 3:30-11 schedule day in and#day out it would be fine to plan around bc it's consistent. now who fucking knows. not to mention the fucking traffic i'll get stuck in#every day going to and coming from work??? again will make moving more difficult bc that'll probably shave like 1.5-2 hours off my time#outside of work. god this sucks this sucks this sucks i hate it it feels like a punishment (even though my boss told me not to think that bc#he knew i would lol) and idk for what???? i feel like they think i'm inept in my current position and idk why. hate this hate this hate this

tips for flying about the usa
what to wear
comfy shoes you can take off and put back on easily/quickly, such as flip flops or velcro shoes/zipper boots. if going somewhere requiring a different kind of shoe, you'll need to decide whether you want to get through security faster, or have more room in your suitcase.
light jacket with large, secure(can close/things won't fall out easily) pockets for storing phone, wallet, and any other especially important items, so you can simply remove the entire jacket without having to empty pockets during screening, and don't risk losing said items if a bag needs checked or is otherwise lost.
comfy pants that don't require a belt(which would need to be removed for screening), such as sweatpants, and if they have pockets, don't use them so you don't have to empty them for security.
a face mask is recommended due to the high volume and density of both airports and planes, as well as the higher risk of illness from foreign bugs you may be newly exposed to and thus have no natural immunity for. just be aware you will probably be asked to remove the mask briefly during the initial security check to compare your face to your id.
choosing tickets
from my personal experience, southwest airlines suck (the layover they gave us wound up too short and our next plane left before we could even disembark the first one, and they did not reimburse us for the hotel we had to get while waiting for the next available flight), and united is good.
also just from my personal experience, la quinta inns are just nastier, shoddily put together versions of the nicer holiday express inns. (like they had the same exact showers but the ones at la quinta were put together wrong so the shower curtain couldn't close at all and stuff like that)
try to book tickets several months in advance, and use private browsing/clear your cache often, to get lower prices
early morning flights are best as they are less crowded, the staff are nicer, and you are less likely to be tempted to try to squeeze in more activities before departing, thus risking missing your flight entirely
plan to get to the airport no less than 2 hours early in case of traffic, issues getting through security, or other unforeseen delays
if possible, try to avoid any layovers that are less than 1 1/2 hours long, as they often result in either rushing or just straight up missing your next flight. if you can afford it, it may be worth manually booking individual flights, or just staying overnight somewhere rather than risking a tight layover.
try to only fly 2 hours or less per day if possible so that you can feasibly drive the distance in a day instead in case you miss a flight.
luggage
pack light. wear clothes multiple times and only bring what you are HIGHLY likely to need. do not bring extra entertainment items. your destination and/or phone apps should be plenty.
if you must pack an extra pair of shoes, maximize space efficiency by putting small clothing items like socks and underwear into plastic bags and then into the extra shoes.
miniature bottles of toothpaste and meds should be stored in clear plastic bags but likely won't need to be removed from suitcase during security check. you may want to pack them so they are easily accessible just in case though.
electronics larger than a phone may or may not need to be pulled out so pack them in a way that makes them quick to pull out and put back.
no knives. ever. some airports allow small ones but some don't, so you may get them through initially and then not be able to take them back home.
don't bring neck pillows, there's typically not enough room to use them on the plane anyway.
try to leave some room in your main suitcase/carry-on if you can, and bring an additional purse or other smaller bag even if you don't need it, so you can fit potential souvenirs.
verify that all tags on luggage have up-to-date address and phone number, and make sure bag isn't too easy for others to mistake as theirs. (if you like plain black suitcases like i do, cover it in ribbons or keychains to make it easy to identify at a distance. i've had someone mistakenly take my bag and then turn it into security when they realized it wasn't theirs before)
other general tips
don't eat less than an hour before any flight, as you may become ill due to turbulence/gas fuel smell and the enclosed space of an airplane is pretty much the WORST place to be sick.
if you have time before a flight, go to the restroom even if you don't currently feel a need to. you REALLY don't want to have to go on the plane.
don't be afraid to ask to swap seats if your ticket doesn't get you where you want to be. people are generally nice about it, especially within the same row. (just be prepared to accept a no- they payed for that seat and aren't obligated to give it to you)
if you drive yourself to the airport, make note of exactly where you parked. the spots are usually numbered and lettered.

an incomplete list of the Bullshit ive gone through this year (2021 only), for personal edification:
I am in grad school trying to do research as well as TA a lab class during a global pandemic
My car is broken into in late February outside of my apartment. $1700+ of my backpacking/camping gear and personal items were stolen from it. Ironically they did not find the $20 cash I had. 
Car battery begins mysterious dying if left overnight and have to call for rescue from AAA 4 separate times over the course of March. I suspect it is related to being broken into but can’t prove it without a mechanic’s diagnostics.
First mechanic I bring my car to does nothing for the entire MONTH they have it, except break my air conditioning of all things. I live in a desert. It is now 90º every day. At one point they call me to say they can’t get the back doors to open. I walk 2 miles back to them from campus and demonstrate how the automatic doors work on a 2005 minivan. I begin to have regrets about my mechanic choice but the sunk fallacy cost keeps me there for several weeks.
Mid march I also wake up one day to severe jaw pain/a weird “loose” feeling, like my mouth is slanting sideways. It is midterms and I do not have time for this, so I take a lot of ibuprofen and eat soup for a week. After 3 days I shove pillows and blankets around my face one night to keep my jaw aligned and when I wake up the next morning it is severely tight instead of loose, and I have to carefully stretch it open whenever I leave my mouth closed for more than an hour. I guess I just have TMJ now.
At this point I am walking everywhere until bike supplies arrive to fix my flat tire since the bike store is too far away to walk to; including walking back and forth to campus since I can only bring 2-4 out of 8 students into the lab spaces at a time and so effectively have to run each weekly lab 2-4 times per week; as well as going back and forth for greenhouse experiment monitoring/helping undergrads on our NASA contest project
Early April I go to the dentist for a crown on one of my back molars, which I must pay for out of pocket because my new dental insurance purchased when I moved last September has a 1-year waiting period and so will not cover it ($1200). Stretching my jaw open so far for the procedure reignites my new TMJ back to high pain levels.
While still waiting on car in mid-April I have a severe averse reaction to the second dose of the Covid19 vaccine, resulting in painful ulceration of all the soft tissues in my body (mouth, stomach, genitals). It is a very bad time for 3 days and I book an urgent care appointment for the first time ever.
Urgent Care nurse-practitioner does not believe me when I describe what’s happening, and misdiagnoses me with herpes.
I am still biking everywhere but now I’m extra mad and in pain about it so take car back from mechanic so I can get groceries etc. I make an appointment with the dealership but it will be a week until they can take it. In the meanwhile I have to drive it every 8 hours so it won’t die which means getting up at 2am to drive it for 20 minutes in the middle of the night so it will still turn on in the morning. 
I have a terrible reaction to the numbing cream given to me for the painful open sores over my body, because of a lifelong mint sensitivity, resulting in an even greater amount of pain
The dealership can fix my car over the following week but its $1800 and now insurance isn’t sure they want to cover it after all
Herpes test comes back negative and nurse apologizes profusely and recommends a non-mint OTC numbing cream alternative that works (yay) and a numbing spray that does not work because it turned out to use an alcohol based propellant which should not be combined with open wounds esp on the genitals (ouch ouch ouch). I try to tell the nurse why I was right about my diagnosis and she was wrong but she still believes it was a latent virus of some other variety and and not an immune response alone, despite the published case studies I have brought to back me up. I decide I have bigger hills I need to die on right now and stop arguing. Sores persist into May but eventually do go down and numbing cream keeps me moderately functioning.
Car is fixed and I can drive again but it takes 2 hours of crying on the phone to my insurance company for them to agree to cover the cost of repair
I make a primary care appointment for the first time in years so I can have a doctor in this state if something like this happens to me again, in June I do intake/bloodwork/set up appointments to check out some other issues ive been having
Grad school finals happen which i wont get into but Yeah. Finals stress triggers another outbreak of canker sores, but mostly clustered in my mouth and only 2 on my vulva rather than 8-12. I eat only soup for another week. 
I get a referral to the local mental health clinic and call about setting up an appointment for an ADHD evaluation. They tell me to download and send in some paperwork and they will call when they have available appointments
I am supposed to be doing all my labwork over the summer but the committee member I need escapes my clutches and we don’t manage to set up a meeting to plan it out/for him to explain the protocols until late June
Bloodwork shows I am critically low in vitamin b12 and low in D, which may explain some of why I am so tired all the time
Ultrasound shows a 1.8cm mass in the adnexa near my left ovary. There are several options for what it can be (folicular cyst, other kind of cyst, tumor, ectopic pregnancy i nearly laugh at my Dr and reassure her the last one is not possible if nothing else). It may go away on its own or it may not. Follow up scan in 2 months
I remember I was supposed to email forms to the mental health clinic and finally send those in mid July. It seems cruel to make me be the one to remember this considering I am calling about a formal ADHD diagnosis.
I also finally pin everyone relating to my labwork down and have a follow up meeting + make a list of what we need to order, but the staff who place orders are on vacation and when they get back several reagents are backordered
I have my follow-up ultrasound. The tech takes lots of photos which indicates the mass is still present, but I won’t know any details until my next PCP appointment when they send over the analysis to her in mid-August
Beginning of August the reagents I need for the first steps of the process arrive exactly 1 day before I leave town for a wedding and the lab manager is about to leave town for the entire next week
After the wedding, severe thunderstorms and tornados trap me in Chicago for 4 extra days. I spend a lot of time at the airport or on my way between the airport and my parents house. A facebook friend gets video of the funnel clouds which at least gives me something to sadly email my advisor and committee members when I have to join our planning meeting from my gate at O’Hare
I lose my drivers license at the security checkpoint on my last trip through the airport and don’t realize until I am boarding the plane because of course that is happening to me now
On the shuttle from El Paso back to Las Cruces after this ordeal the driver stops and picks up a box labeled HUMAN BLOOD and puts it in the trunk and i am too tired to care anymore
I stay up all night making the world’s most pitiful r graphs for my meeting the next morning and everyone takes pity on me and does not call out how useless they are
I spend the weekend trying to motivate myself to actually go into the lab and start my procedures, and fail to leave my apartment. This reminds me it has now been a month (Aug 15th) since I sent in my paperwork and the mental health clinic has still not called me back about up an appointment
I get overwhelmed with Everything and make this list
So that’s where I’m at at the moment. And this doesn’t even include anything from 2020 thats just been continuous like, y’know, a global pandemic and having a bad breakup of a 4 year relationship and moving to a new city where I know no one for grad school etc. I feel like I’m falling apart/unable to do all the shit I need to right now but you know what? Actually its been a really bad time and maybe falling apart a little is justified ;_;

Repair and Reboot

I told you so.  I also told the POTUS in my open letter, but he did not read it. 
Who could honestly believe the nation would support dumping coverage for 22 million people?  As David Leonhard wrote recently op-ed in the New York Times: “They [Republicans and President Trump] had only one big weakness, in fact: They weren’t dealing in reality.”  When faced with reality, it is interesting what a few good Senators with a conscience will refuse to do. 
Success is never attained by taking shortcuts.  We do not need reform of health care; we need to reboot the entire system.  Special interests do not belong in the picture.  They are incompatible with developing innovative solutions that place profits on the back burner.   Congress is making this too difficult.  They need to roll up their sleeves, go back to the drawing board, and start again.  My suggestions:
Step 1:  Every member of Congress should participate in a mock hospital admission as a patient, starting with presentation to the ER, being poked and prodded, having surgery if necessary, and staying overnight to recuperate.  After your experience, you should be provided a “bill” on your way out the door and pay the balance by cash or check. 
Step 2:  Go see your own primary care physician for two reasons.  The first is to have an annual exam and to connect with your constituents in the waiting room, solicit their comments, thoughts, or suggestions, and converse with office staff to understand their perspective.  The second reason is to elicit feedback directly from your primary care physician.  Listen for groundbreaking solutions to the perplexing boondoggle of caring for greater numbers at a lower cost.
Extra credit:  Follow a primary care physician in a Health Professional Shortage Area (HPSA) for three days.  Listen, engage, clarify, empathize, and most importantly absorb how monumental this undertaking of reforming health care will be. 
Step 3: Return to Washington D.C. inspired and reboot, resolving to do it right this time. 
The nation has been having entirely the wrong conversation; that dialogue must change.  The biggest obstacle faced by lawmakers is maintaining access while reducing cost.  Providing coverage without coupling it to budgetary constraints is sheer lunacy.  However, reducing government involvement in coverage without ensuring the needy can afford health care will never garner widespread support.  Affordability has become an impossible dream and is currently our largest stumbling block. 
The U.S. spent $10,345 per person annually in 2016.  The average OECD country spends $3997 per person annually in comparison.  During the 1980’s Spain created a network of community health clinics within a 15 minute radius of every citizen, a system which was funded by the taxpayers.  In 1975, the average life expectancy from birth was equivalent in both nations, at 69 years of age.  Today, life expectancy in Spain is 83 years compared to 78.8 in the United States.  We are spending twice as much as Spain and our life expectancy is significantly lower. 
An appropriate policy goal would be to focus on developing a sustainable solution, implemented only after great deliberation.  Scaffolding already exists, in community clinics and Public Health departments; these facilities are cost-effective, yet grossly underfunded, underutilized, and unappreciated.  Every single man, woman, and child needs primary care services, a fact which in incompatible to the insurance model.  We must sever the connection between insurance and primary care.  Providing basic care universally is something we must accept as reality. As I have written before, investing in primary care as a solution is a no-brainer; increasing by one PCP/10,000 persons decreases mortality by 5.3%. 
Basic care will bring us all out from the shadows and into the light.  Provide immunizations, screenings, and annual exams to everyone in this country.  Those working in the community clinics will be employed by the government and salaried.  These clinics could have evening or weekend walk-in hours and handle urgent matters.  The electronic medical records system should be universal and patient-centric.  People will no longer live in fear of our government eliminating access for chronic conditions or emergencies.  Struggling families will not be one catastrophic illness away from losing their hopes and dreams. 
As we continue filling in the grid, specialty care should be added at the public health facilities or community clinics.  A specialist would cover a greater number of patients when overseeing or consulting on difficult cases with the primary care physicians.  These specialists would be employed by the government and salaried as well.  If an individual becomes severely ill or injured and requires very specialized treatment, hospitalization, or surgical management, either they have Medicaid, Medicare, or their catastrophic insurance plan kicks in to cover these needs. 
No discussion would be complete without including third party payers, who distance patients and physicians from being cognizant of cost.  For what we do in our offices, services could be far cheaper.  For example, a self-employed middle-aged patient with a $25,000 deductible sustained a 4cm laceration to the head and went to buy glue to repair it himself.  On this particular holiday weekend, the stores were already closed.  He inquired as to the cash price for repair after texting a picture. 
I had no answer, but primary care physicians love repairing lacerations and I am no exception to the rule.  He came to my office; I cleaned the wound and sutured it.  He handed me his credit card, similar to the cashier at a grocery or hardware store.  Supplies cost roughly $50; the laceration repair took 15 minutes.  I figured $150 seemed reasonable.  He paid $200 and was thrilled. 
While the lack of transparency hindered my research, I compared the cost to repair a 4 cm laceration in the emergency room.  The estimated charges were:  $1000 emergency room facility charge, physician cost $500, and the procedure bill was $200.  My hardworking patient would have coughed up $1700 at a minimum (some estimate as high as $1000 per stitch) and waited well over 15 minutes for the privilege. 
Allow the free market forces to remain a part of the infrastructure.  A great deal of the population fears a universal basic system because they are afraid of losing choice.  Direct Primary Care practices would flourish in a system with a basic care safety net for those in need.  Those who can afford choice would have options to patronize the private market, which absolutely should not be eliminated.   
Reviewing the events this week reminds me Rome was not built in a day. Repairing the tangled web of health care will take unconventional thinking and the tincture of time. Costs have spiraled out of control past the point of affordability.  The nation will only support reform once Congress overhauls our broken system prior to embarking on repealing anything.  Finally, everyone is profiting except the two most critical components: the physicians and their patients.  Repair, reboot, and rebuild from the ground up and when you do, start by putting patients ahead of profits.
