Hi hii! I’m just wondering, what were the struggles/how to get into forensic pathology because this is my 3rd year in med school and ide like to become a forensic later on so do you perhaps have any tips or things I should take note on?

Okay, that's a tricky question, since it's so region specific. So, caveat before I begin: this only really applies to US residencies/FP track. Anywhere else is going to have different systems, and you'd need to ask someone who went through that specific system.

So, first, congrats! You have gotten through the hardest part: you are IN MEDICAL SCHOOL, which is absolutely the choke point for all of this. I know it might not feel like it sometimes, but from here out you are way more in control than you were getting into med school.

As a third year, my main recommendation is to do general pathology rotations (surgical pathology is good, get some clinical pathology experience) and make sure you do a forensic or autopsy rotation, because you'll know really fast if it's really the route you want to go down. It can be a nasty, smelly, intense job. For some people, that's perfect, for others not so much, and you'll only know if you do a rotation. So once you start to get electives, focus in on those things.

This leads us to residency. If you do plenty of pathology rotations, you already have a massive jump on other people who apply and barely have any pathology experience. Having been pretty active in recruitment at my residency, I can tell you that an active interest and good base knowledge of what pathology is goes a long way. Another huge jump you can have on everyone else in trying for residency is to be friendly with everyone (especially techs or others you might talk to, because during your interview you are ALWAYS interviewing). Be interesting and be interested in their program. Know some good knowledge about what they're good at going in. And if you're convinced that FP is the way you want to go, focus on the residencies that have a good FP program attached to them, as it's much easier to get a fellowship if you come from their native program.

I went to a middling medical school and was only a slightly above average student. I certainly wasn't a superstar. My scores were fine. My grades were fine. But scores alone weren't going to help me. What helped was writing a killer personal statement, having REALLY good recommendations from pathologists and forensic pathologists, and being able to put on an outgoing and fun personality. Having an actual personality that is engaging and memorable is somewhat uncommon in pathology, so use what you have.

One of the keys I found in interviewing for a pathology residency was knowing that 90% of US medical school grads get their number 1 choice for residency in pathology (your mileage varies if you're shooting for the big name programs). It's a fairly small pool each year who are going into pathology, so YOUR ranking of residency programs generally matters a lot more than the residency's ranking of you. You are interviewing them just as much as they're interviewing you, so focus on the program that feels best for you. You are going to be in residency for four tough years, so you really want to be in a program that will support you well.

Once you're in residency, as I mentioned, it's a lot easier to get into a fellowship program that's already attached to your residency. I did this, so I can't really speak to the process if you're coming from an outside residency. Fellowship interviewing is somewhat like residency in that almost everyone trying to get an FP fellowship will match somewhere because very few people go into it, and there's an extremely high demand for forensic pathologists (which means you're basically guaranteed a job no matter where you want to go after fellowship! Just pick a location, and chances are good they're hiring), and more fellowship spots than applicants each year. That being said, getting into the top programs (I think the current top 3 are New York, Miami, and New Mexico) can be extremely competitive. Research the program. Make sure the fellows get their numbers, that they don't have to share, and that they get to see a wide variety of deaths.

I think confidence and kindness go a huge distance in getting you into both residency and fellowship. And knowing about pathology in general, having a good number of rotations in it, and being ready and willing to learn is going to get you the rest of the way. And even if you decide FP isn't for you, pathology is a very broad field, and you might suddenly realize you love blood banking or surgical pathology or hemepath or cytopathology or lab management. And your path residency can get you into any of those.

A specialized unit of the Armed Services was formed in 2018 to infiltrate and investigate targets as determined by high command. Because of the highly secretive nature of the operations, little information was given to each agent chosen for the particular mission. These men were recruited from the various divisions of the armed forces and given intensive physical training to fulfill each particular mission with each under an absolute command of silence if discovered.

These men were chosen based on a model developed by command. Each recruit was to be 5’10” to 6”, weighing 210 to 240lbs and have no known next of kin. It was imperative that there be no way of tracing these agents if captured. Each was informed of the likelihood of their capture, such was the nature of the assignments, and each given an opportunity to decline this service. Not surprisingly, none did.

…as you come to, you notice a rapid beeping-like a microwave, only much, much faster. 

 "well welcome back!" The interogator had entered the room and dismissed the technician. "That beeping? That's your heart. Pumping 185 beats a minute according to our monitoring equipment. You'll no doubt notice you're wired up to a bunch of machinery. Let me explain. Your refusal to cooperate intrigues us. Obviously, whoever sent you to gather information has prepared you well. Just how well we intend to find out. And you are older than the last couple of agents we’ve “hosted”. We are curious how this may have affected your heart.”

You remain unmoving. Straps across your chest and hips hold you fast to the exam table. A mask delivering oxygen prevents you from anything but grunting.

"We've attached electrodes to your chest and an IV in your upper arm. This gives us continuous readings on your heart as we continue our exploration. Our EKG indicates an enlarged left ventricle. We have altered our program to investigate this finding. " Glancing to the technician, "Prepare to inject the adrenalin into the prisoner's IV." now addressing you, "The adrenaline will be injected slowly as too much, too fast will stop your heart, no, slowly we'll bring your heartrate to 200, then after a few minutes, 220-230-240--our last guest took it up to 267 until he suffered a massive heart attack after four minutes. We opened his chest-and found his heart literally shredded from the attack. 

"The airflow through the mask has your chest swelling up against the straps, the hair matted and dripping with sweat. While the enhanced exhalation sucked your belly hard against your ribs forcing your navel to protrude from the abdominal wall.

"We don't want a recurrence of that so after your heart reaches 250bpm, we will open your chest cavity and continue. We want to see the effect of the drug and our other procedures, on your heart first hand.”

With a nod to the tech, The stimulant was injected into the IV., the prisoners heart rate shot up to 210-his heart pounding so hard, the table shook with each beat. After 5 minutes, the order was given to increase the stimulant.

“220 bpm” the tech was also keeping track of the EKG readouts. After 5 minutes, an increase of stimulant and his heart rate was a thundering 230bpm. The prisoners chest was soaked with sweat. The mask was forcing air into his lungs ever faster.

“Take his heart up to 240, remove the chest strap and ready the thoracotomy tray. The EKG is showing some T-wave changes. I want to get him opened up when he reaches 240bpm. I hope this doesn’t end our investigation early”

The tray of surgical instruments was brought up along side of the prisoner. In addition to the scalpels, the tray also had a bone saw and rib spreader-all stainless steel. They flashed under the lights of the room.

This time the tech addressed the chief interrogator, ”the prisoners heart is beating 240bpm are you ready to proceed with the midline thoracotomy?”

“Just a moment, I want the prisoner to feel his heart pounding.” The interrogator picked up the prisoners hand and pressed it hard against his chest. “The next time you feel your heart it will be while it’s beating in your chest cavity?” Three more techs entered the room to assist the interrogator with the procedure. “We will begin with a midline incision about 14 inches long.” Tracing his finger down the prisoners chest and rigid abdomen. “Just above his navel. So let’s get started. Prepare to administer the anesthesia-we want to keep him conscious, remember. As soon as it takes effect, we’ll begin the procedure. ” 

One of the techs moved the prisoners hand off his chest as the interrogator began his incision. With one smooth movement the scalpel glided over the sternum and down the abdomen slicing through all layers of tissue while avoiding the abdominal muscle. 

“Heart rate?”

“242bpm”, was the reply.

“How long?”

“8 minutes, 27 seconds.”

The interrogator worked quickly-precisely. He’s done this before. “Bone saw! OK, prepare to open the chest cavity” The techs readied to rib spreader as the high-pitched whirr of the saw cut through the prisoner’s sternum. Once severed, the techs pulled the chest cavity open to insert the spreader.

“Alright, crank his chest open.” The crank spun around and soon the prisoners pericardial sac was exposed.

“Let’s get a good look at his pump.” Removing the sac released the heart from its last restraint and the straining muscle pushed up through the incision.

“Holding at 242?” He called out to the tech who nodded.

“Increase the stimulant gradually-bring his heart up to 260 and watch for any changes-we want to get this heart rate to 300bpm.

The tech, did as instructed and the prisoners heart rate climbed past 250 to 260. “This guys a machine! Look at the desending coronary artery standing out on the left ventricle, thick as your finger—a thrombus now would throw him into instant arrest.”

 In fact, the veins over his entire body stood out like rope. Those in his neck began constricting the flow of oxygen to the prisoner’s lungs. The anesthetist increased the pressure of air through the mask. The increasing pressure forced his lungs to swell with each breath, causing the heart to push up through the incision  Even at 260 beats per minute the prisoners heart pounded on seemingly impervious to the torture.

“This next stage will be critical,” to the tech alongside the IV drip, “increase heartrate to 270 beats per minute.”

“270″

It was at point, the EKG indicated a narrowing of the heart’s anterior coronary artery. Looking at the ventricle thrusting the entire heart out of the chest cavity, they could see the considerable narrowing of the artery. This could complicate the next part of the test.

“Increase to 300 bpm, ready the pacer wires and give me a syringe of alcohol. In order to attach the pacer, it is first necessary to inject a small amount of alcohol into the AV node. This will kill the part of the heart muscle that regulates the heartbeat.”

“300bpm!”

With the syringe in hand, the interrogator pressed his index finger on the quivering ventricle and pushed the tip of the needle into the prisoners heart.

“Now a careful injection of alcohol-” With the first drops of alcohol entering the AV node, the heart muscle immediately turned grey and the heart contracting at 300bps, faltered and stopped beating. “The injection way successful-the prisoner is now in asystole.’

Assistants readied the wires of the pacemaker and prepared to push them into the arrested heart of the prisoner. His heart rate would now be determined by the surgical team.

“…we have less than two minutes to get him hooked up to the pacer before we have to put him on bypass.” Working quickly, the two wires of the pacemaker were pushed into the heart muscle following the diagram projected on the screen. The techs couldn’t help but pick up the massive organ, now enlarged to twice the size it was at the beginning of the test. “ This heart is easily 1200 grams. How are we ever going to get it back in his chest?”

 ”When the wires are in place, begin charging the defibrillator. We’ll need a shock to prime the pump, as it were. Charge to 400, and set the pacemaker to 100 bpm”

The paddles were placed in the chest cavity, cradling the prisoners massive heart. “clear!” as the surgical team stepped back the paddles were energized. The heart muscle contracted fromm the shock but failed to beat. “charge to 600! again-clear!” This stronger jolt of electricity appllied directly to the surface of the pump caused the heart muscle to contract with such force that the prisoners chest bounced up off the table. After a few moments of silence, the heart began to beat at the preset rate of 100 beats per minute.
