The Courage to Follow the Evidence on Transgender Care.
(WOW, the New York Times -- which a couple years ago had an ad about a qu**r girl who wished for a world in which J.K. Rowling wasn't the author of Harry Potter -- has published yet another opinion piece about trans, this one about the Cass Review. Personally, I think he's too lenient, but at least he's bringing attention to the review to Americans. )
(For those who can't read the NYT page, here's the text.)
Tumblr media
Opinion, David Brooks, April 18, 2024.
Hilary Cass is the kind of hero the world needs today. She has entered one of the most toxic debates in our culture: how the medical community should respond to the growing numbers of young people who seek gender transition through medical treatments, including puberty blockers and hormone therapies. This month, after more than three years of research, Cass, a pediatrician, produced a report, commissioned by the National Health Service in England, that is remarkable for its empathy for people on all sides of this issue, for its humility in the face of complex social trends we don’t understand and for its intellectual integrity as we try to figure out which treatments actually work to serve those patients who are in distress. With incredible courage, she shows that careful scholarship can cut through debates that have been marked by vituperation and intimidation and possibly reset them on more rational grounds.
Cass, a past president of Britain’s Royal College of Pediatrics and Child Health, is clear about the mission of her report: “This review is not about defining what it means to be trans, nor is it about undermining the validity of trans identities, challenging the right of people to express themselves or rolling back on people’s rights to health care. It is about what the health care approach should be, and how best to help the growing number of children and young people who are looking for support from the N.H.S. in relation to their gender identity.”
This issue begins with a mystery. For reasons that are not clear, the number of adolescents who have sought to medically change their sex has been skyrocketing in recent years, though the overall number remains very small. For reasons that are also not clear, adolescents who were assigned female at birth are driving this trend, whereas before the late 2000s, it was mostly adolescents who were assigned male at birth who sought these treatments.
Doctors and researchers have proposed various theories to try to explain these trends. One is that greater social acceptance of trans people has enabled people to seek these therapies. Another is that teenagers are being influenced by the popularity of searching and experimenting around identity. A third is that the rise of teen mental health issues may be contributing to gender dysphoria. In her report, Cass is skeptical of broad generalizations in the absence of clear evidence; these are individual children and adolescents who take their own routes to who they are.
Some activists and medical practitioners on the left have come to see the surge in requests for medical transitioning as a piece of the new civil rights issue of our time — offering recognition to people of all gender identities. Transition through medical interventions was embraced by providers in the United States and Europe after a pair of small Dutch studies showed that such treatment improved patients’ well-being. But a 2022 Reuters investigation found that some American clinics were quite aggressive with treatment: None of the 18 U.S. clinics that Reuters looked at performed long assessments on their patients, and some prescribed puberty blockers on the first visit.

Unfortunately, some researchers who questioned the Dutch approach were viciously attacked. This year, Sallie Baxendale, a professor of clinical neuropsychology at the University College London, published a review of studies looking at the impact of puberty blockers on brain development and concluded that “critical questions” about the therapy remain unanswered. She was immediately attacked. She recently told The Guardian, “I’ve been accused of being an anti-trans activist, and that now comes up on Google and is never going to go away.”
As Cass writes in her report, “The toxicity of the debate is exceptional.” She continues, “There are few other areas of health care where professionals are so afraid to openly discuss their views, where people are vilified on social media and where name-calling echoes the worst bullying behavior.”
Cass focused on Britain, but her description of the intellectual and political climate is just as applicable to the U.S., where brutality on the left has been matched by brutality on the right, with crude legislation that doesn’t acknowledge the well-being of the young people in question. In 24 states Republicans have passed laws banning these therapies, sometimes threatening doctors with prison time if they prescribe the treatment they think is best for their patients.
The battle lines on this issue are an extreme case, but they are not unfamiliar. On issue after issue, zealous minorities bully and intimidate the reasonable majority. Often, those who see nuance decide it’s best to just keep their heads down. The rage-filled minority rules.
Cass showed enormous courage in walking into this maelstrom. She did it in the face of practitioners who refused to cooperate and thus denied her information that could have helped inform her report. As an editorial in The BMJ puts it, “Despite encouragement from N.H.S. England,” the “necessary cooperation” was not forthcoming. “Professionals withholding data from a national inquiry seems hard to imagine, but it is what happened.”
Cass’s report does not contain even a hint of rancor, just a generous open-mindedness and empathy for all involved. Time and again in her report, she returns to the young people and the parents directly involved, on all sides of the issue. She clearly spent a lot of time meeting with them. She writes, “One of the great pleasures of the review has been getting to meet and talk to so many interesting people.”
The report’s greatest strength is its epistemic humility. Cass is continually asking, “What do we really know?” She is carefully examining the various studies — which are high quality, which are not. She is down in the academic weeds.
She notes that the quality of the research in this field is poor. The current treatments are “built on shaky foundations,” she writes in The BMJ. Practitioners have raced ahead with therapies when we don’t know what the effects will be. As Cass tells The BMJ, “I can’t think of another area of pediatric care where we give young people a potentially irreversible treatment and have no idea what happens to them in adulthood.”
She writes in her report, “The option to provide masculinizing/feminizing hormones from age 16 is available, but the review would recommend extreme caution.” She does not issue a blanket, one-size-fits-all recommendation, but her core conclusion is this: “For most young people, a medical pathway will not be the best way to manage their gender-related distress.” She realizes that this conclusion will not please many of the young people she has come to know, but this is where the evidence has taken her.
You can agree or disagree with this or that part of the report, and maybe the evidence will look different in 10 years, but I ask you to examine the integrity with which Cass did her work in such a treacherous environment.
In 1877 a British philosopher and mathematician named William Kingdon Clifford published an essay called “The Ethics of Belief.” In it he argued that if a shipowner ignored evidence that his craft had problems and sent the ship to sea having convinced himself it was safe, then of course we would blame him if the ship went down and all aboard were lost. To have a belief is to bear responsibility, and one thus has a moral responsibility to dig arduously into the evidence, avoid ideological thinking and take into account self-serving biases. “It is wrong always, everywhere, and for anyone, to believe anything upon insufficient evidence,” Clifford wrote. A belief, he continued, is a public possession. If too many people believe things without evidence, “the danger to society is not merely that it should believe wrong things, though that is great enough; but that it should become credulous, and lose the habit of testing things and inquiring into them; for then it must sink back into savagery.”
Since the Trump years, this habit of not consulting the evidence has become the underlying crisis in so many realms. People segregate into intellectually cohesive teams, which are always dumber than intellectually diverse teams. Issues are settled by intimidation, not evidence. Our natural human tendency is to be too confident in our knowledge, too quick to ignore contrary evidence. But these days it has become acceptable to luxuriate in those epistemic shortcomings, not to struggle against them. See, for example, the modern Republican Party.
Recently it’s been encouraging to see cases in which the evidence has won out. Many universities have acknowledged that the SAT is a better predictor of college success than high school grades and have reinstated it. Some corporations have come to understand that while diversity, equity and inclusion are essential goals, the current programs often empirically fail to serve those goals and need to be reformed. I’m hoping that Hilary Cass is modeling a kind of behavior that will be replicated across academia, in the other professions and across the body politic more generally and thus save us from spiraling into an epistemological doom loop.

#OTW 1960: FDA Approves "THE PILL"
By Miriam Kleiman, Public Affairs
The FDA's May 9, 1960 approval of oral contraception, aka "the pill", transformed reproductive health. Women's health advocate Margaret Sanger spearheaded and activist/philanthropist Katherine McCormick funded the R&D needed for this medical research breakthrough to improve women's lives through "birth control."
Margaret Sanger, a nurse, coined the term “birth control” and dedicated herself to educating women. Her own mother had 18 pregnancies in 22 years and died from ovarian cancer. In 1914, she started a newsletter, The Woman Rebel, to "advocate the prevention of conception."
Tumblr media
The Woman Rebel, No. 1; 3/1914.
Sanger was indicted repeatedly and even arrested on obscenity charges under the Comstock Laws (1873) which defined birth control as obscene and made it illegal to send contraceptive devices or even info about it through the mail.

Katherine McCormick heard Sanger speak in 1917 and grew convinced that women could only fully control their lives if they could control if and when they chose to bear children. She redirected her advocacy to the cause of birth control, even smuggling in diaphragms from Europe to New York at Sanger's request.
When her husband Stanley died in 1947, Katherine inherited an estate estimated worth almost $40 million (more than $500 million today). Margaret Sanger introduced her to Gregory Pincus who was doing pioneering research on fertilization and hormones.
Katharine funneled to Dr. Pincus more than $2 million ($25 million today), nearly all of the money used to support his lab's research and development of the contraceptive pill.
Read the National Archives Prologue Magazine story Rich, Famous, and Questionably Sane to learn how McCormick, who was blamed her husband's inability to consummate his own marriage, became the catalyst for the sexual revolution.

Shannen Doherty Wants to 'Embrace Life' as Cancer Has Spread to Her Bones: 'My Greatest Memory Is Yet to Come' (Exclusive)
The 'Beverly Hills, 90210' star opens up in PEOPLE's latest cover story about her Stage 4 cancer diagnosis and how she hopes to inspire others by focusing on her future
ByDanielle Bacher
Updated on November 29, 2023 09:29AM EST
Shannen Doherty doesn’t mince words.
“I don’t want to die,” she asserts as a sliver of Los Angeles sunshine falls across her face on the set of her PEOPLE cover shoot four days before Thanksgiving.
The actress — who is best known for her roles on the hit ’90s TV show Beverly Hills, 90210 and later, Charmed — is characteristically candid, upbeat and dry-witted as she opens up about her Stage 4 breast cancer that has spread to her bones.
As she continues to receive treatment, the 52-year-old is more determined than ever to keep moving forward. “I’m not done with living. I’m not done with loving. I’m not done with creating. I’m not done with hopefully changing things for the better,” she says, cracking a smile. "I’m just not — I’m not done.”
Doherty’s eight-year journey with cancer has led her to reflect on the big picture of her life, a theme she will explore on her memoir-style podcast, Let’s Be Clear with Shannen Doherty, premiering Dec. 6 on iHeartRadio, where she plans to discuss everything from career highlights and past relationships to the numerous stages of her illness and health regimens.
After her initial breast cancer diagnosis in 2015 that she first shared with PEOPLE, Doherty had a mastectomy and underwent chemotherapy and radiation.
In April 2017, she revealed on Instagram she went into remission. But by 2019, the cancer returned — and she announced her diagnosis of metastatic stage 4 cancer the following year.
“When you ask yourself, ‘Why me? Why did I get cancer?’ and then ‘Why did my cancer come back? Why am I stage 4?,’ that leads you to look for the bigger purpose in life,” she explains.
She hopes to raise awareness and funds for cancer research — while showing that people with terminal cancer still have plenty to contribute to the world. “It’s insane to me [that] we still don’t have a cure,” she says.
In June, Doherty shared on Instagram that the cancer had spread to her brain and that she had undergone surgery five months earlier to remove a brain tumor. With dark humor, she named it Bob.
“He had to get removed and dissected to see his pathology,” she explains. “It was definitely one of the scariest things I’ve ever been through in my entire life.”
Right now, Doherty hopes to get into clinical trials as new treatments are developed. (It is estimated 168,000 people are living with metastatic breast cancer, according to the Susan G. Komen organization.) But she is most driven by her desire to prove that she can work despite her cancer diagnosis. And it's that deep-seated sense of discipline that helps her carry on.
“People just assume that it means you can’t walk, you can’t eat, you can’t work. They put you out to pasture at a very early age —‘You’re done, you’re retired,’ and we’re not,” she says. “We’re vibrant, and we have such a different outlook on life. We are people who want to work and embrace life and keep moving forward.”
Never miss a story — sign up for PEOPLE's free daily newsletter to stay up-to-date on the best of what PEOPLE has to offer.
Each morning, the actress feels grateful for another day with her friends, her family and her German shepherd Bowie.
"My greatest memory is yet to come," she says. "I pray. I wake up and go to bed thanking God, praying for the things that matter to me without asking for too much. It connects me to a higher power and spirituality. My faith is my mantra.”
As she explains, when you have cancer, everything is more poignant, and the sky is bluer.
“I know it sounds cheesy and crazy, but you’re just more aware of everything, and you feel so blessed. We’re the people who want to work the most, because we’re just so grateful for every second, every hour, every day we get to be here.”
For more on Shannen Doherty's cancer journey and how she's finding moments of joy amid her diagnosis, pick up the latest issue of PEOPLE, on newsstands Friday.

MAIDSTONE STROKE GROUP
AUGUST NEWSLETTER 2022
NEW 'SUPER CLINIC' TO OPEN AT CANCER CENTRE
A new “super clinic” has officially been opened at Maidstone Hospital. The new building is an addition to the Kent Oncology Centre the fourth largest provider of cancer care in the country which offers specialist treatment to around 1.9m people. On May 26, Maidstone and Weald MP Helen Grant was joined by MP for Chatham and Aylesford Tracey Crouch, who received care at the centre herself, to officially open the site. It has nine new clinic rooms which means patients can see consultants, registrars, cancer nurse specialists and radiograpers in a streamlined “super clinic” where multiple teams work together to provide smoother and quicker care. There are also two specialist rooms for patients to have quick blood tests and face-to-face consultations. The politicians were joined by new Maidstone mayor Cllr Derek Mortimer and received a tour of the new suite before the official unveiling of a commemorative plaque. The new suite is the lastest in a number of developments in cancer care at the Maidstone and Tunbridge Wells Trust, with the most recent developments including multi-million-pound investments for diagnostic equipment and facilities, recruiting more clinical staff and upskilling nurses to deliver chemotherapy treatment. Mrs Grant said: “My husband was diagnosed with prostate cancer six years ago and we entered a process that took about four years to complete, in multiple locations. “Navigating that journey was a challenge whilst also having to deal with the mental stress of the disease and its treatment. “Here at Maidstone Hospital, we are extrmely fortunate to have the Kent Oncology Centre with many services all co-located – making the process far easier”.

Vitamins 'a risk to cancer recovery
ONE in three cancer patients should bin vitamins and supplements in case they block recovery, scientists say. Herbal pills and remedies can worsen side effects or make treatment less effective for people having chemotherapy or radiotherapy. Ninet-one per cent of cancer patients take some kind of supplement, a study in th USA found. Some 35 per cent would be told to stop if their doctors knew what they were taking. Lead author Dr Stacy D'Andre, of Mayo Clinic in Minnesota, warned supplements or alternative therapies “can be dangerous in several ways.” She said: “They can be toxic, interact with other medications and may decrease the effectiveness of cancer treatments.” Brits should consult a doctor before using them, UK cancer experts say.

Heart joy with egg
EATING an egg a day could help stave off a heart attack or stroke, a study suggests. Scoffing the versatile shelled food staple was shown to boost metabolism and increase healthy proteins in the blood. That leads to more “good” cholesterol and strips away the “bad” type from arteries, protecting against clots. The team from Peking University, Beijing, said the study “highlights the need for more strategies to encourage moderate egg consumption”.

STATIN PILL IS CANCER FIGHT HOPE
Statins could help fight bowel cancer, experts say. The cheap pills already used by millions to lower cholesterol were found to stop tumour growth in laboratory tests. Researchers claim the breakthrough could help in a quarter of cases, benefitting around 10,000 UK patients each year. Queen Mary University of London scientists tested over 1,200 already approved drugs on cancer cells, journal Frontiers in Oncology reports. Though not a cure, experts hope the pills can be used to prolong life and aid treatment.

HOSPITAL THEATRE TO CUT OP QUEUES
Plans have been put forward for a new multi-million pound medical facility which would create 181 jobs. The development at Maidstone Hospital in Hermitage Lane would cost around £30 million and represent part of the government's £8 billion Build Back Better NHS project. If approved, the standalone “Orthopaedic Barn” would have four theatres, a 22-bed inpatient ward with medical support and plant spaces. Maidstone Hospital deals with orthopaedic cases for all of Kent. If given the go-ahead, the two-storey building would help cut waiting times and backlogs, which have been worsened by the Covid-19 pandemic. The planning statement said: “there is a compelling need for the development arising from a demand for additional operating space to deal with existing backlogs in the system. “The improved functioning of hospital is critical in light of the pressures created by the Covid-19 pandemic which has placed immense strain on an already under-resourced service. “The application is a direct part of Maidstone NHS Trust's response to improve the efficiency of Maidstone Hospital in line with the Government's Build Back Better plan. “The proposed facility will have the direct ability to improve the lives both of those people currently awaiting procedures and those people who will need procedures in the future. “It will assist wider training and best practice and would link with the new Kent and Medway Medical School which has been set up to train the next generation of NHS doctors.” Construction would be”short” and the building screened by surrounding hospital buildings. It is also hoped the facility would help increase the amount of hip replacement procedures from five to 20 per day. The planning statement added: “Moreover, provision of this additional capacity will free up operating space in furthur theatres within the main hospital for more complex operations.”The development is predicted to significantly improve the efficiency of hospital functions to attend to backlogs in the waiting list.” A new patient car parking and drop-off area with six parking spaces including four blue badge spaces will be provided to the front of the building. Another 10 cycle storage spaces have also been proposed.

Influential right-wing group releases alarming policy proposals to restrict IVF
The Heritage Foundation – a right-wing, anti-LGBTQ+ think tank planning to play a large role in the next GOP presidential administration – recently released a set of preliminary policy recommendations for Congress on regulating in vitro fertilization (IVF). The eight recommendations were included in a March 19 blog post by Senior Research Associate Emma Waters, who praised the Alabama Supreme Court’s ruling that embryos created through IVF have the same legal rights as children and the fact that the decision means patients can sue for the wrongful death of a child if an embryo is mishandled or destroyed. Related: House Republicans are outraged that veterans in same-sex marriages may get access to IVF Four House Freedom Caucus members described IVF as “morally dubious” and said that it “should not be subsidized by the American taxpayer.” She outlined her beliefs in the “profound moral issues with the way IVF is practiced in the U.S.,” explaining that too many embryos are created per cycle and that the widely used pre-implantation genetic testing (PGT) to screen for genetic conditions is a form of eugenics. Stay connected to your community Connect with the issues and events that impact your community at home and beyond by subscribing to our newsletter. Daily * Weekly * Good News * Many LGBTQ+ people rely on services like IVF to build their families. It is the most common method of assisted reproduction. Operating under the notion that embryos are children, Waters’s first suggestion to Congress is to “impose a standard of care in IVF clinics to prevent the wanton or careless destruction of embryonic human beings.” Waters criticized the Alabama legislature for passing a law in response to the Supreme Court ruling that shields IVF providers from civil and criminal liability. The law did not, however, address the fundamental issue of whether frozen embryos should be considered children under Alabama law. Multiple embryos are produced for IVF and are necessary to ensure a pregnancy. Embryos often don’t survive the IVF process, don’t always have high reproductive potential, and often fail to implant in the uterus. Multiple embryos is necessary for IVF to be successful in producing a baby. Waters claimed the law does not protect the interests of the parents or the “embryonic life,” and in her policy recommendation, said, “the law should recognize the gravity of the harm and allow parents proper compensation under the wrongful death provision.” The other suggestions include limiting the number of embryos that can be created per round of IVF, only allowing one embryo to be transferred into an intended parent at a time, banning anonymous egg and sperm donations, banning PGT testing, requiring full informed consent from prospective parents, and funding more research into treating infertility until IVF becomes a true “last resort.” Media Matters described these proposals as “seemingly benign” but nonetheless “meant to heighten the barrier of entry to securing IVF procedures.” The progressive media watchdog group likened it to the TRAP Laws that impose harsh regulatory restrictions on abortion clinics to make it as difficult as possible for them to operate. The Heritage Foundation is behind Project 2025, a lengthy set of extreme far-right policy recommendations for the next GOP administration. The plan includes reinstating several Trump-era policies and stacking the administration with Trump loyalists. The 900-page policy book reportedly includes a recommendation to remove the terms “reproductive health,” “abortion,” and “reproductive rights” from all government laws, regulations, policies, rules, and grants. Project 2025 also contains a slew of suggestions for dismantling LGBTQ+ rights, especially trans rights. The proposals include removing all terms related to gender and sexual identity from all federal laws, regulations, contracts, grants, and… http://dlvr.it/T4ghf7
