Nonagenarian survives rectal cancer with aplomb

Deploying short radiation well before surgical intervention helped cure him with minimal discomfort

Age is just a number for this nonagenarian who's just beaten cancer of the rectum with a meticulous procedure to return to his life as a columnist with a leading Marathi daily. The former chairman of Bank of Maharashtra, 92-year-old Vasant Patwardhan, did not let his spirits be buckled by his health condition and a careful approach to his treatment helped him resume his daily routine rather quickly.

An asthma patient, Patwardhan was passing blood with his stool over several months. Investigations diagnosed it as being caused by carcinoma of the rectum. After a few consultations, he was referred to Dr. Snita Sinukumar Cancer Specialist in Pune of Jehangir Hospital by another oncologist, Dr. Ananthbhushan Ranade

DrSinukumar advised Patwardhan and his family to opt for surgery as prolonged chemotherapy and radiation would not do him good at his age. His family went with the thought. "We too were not sure if chemotherapy and radiation would help him. He's been very active all his life and we did not wish to dampen his spirits. The doctors suggest that a short course of radiation followed by surgery could do the trick and it seemed ideal to us too, ex-plained Varsha Shree, Patwardhan's wife. Patwardhan too agreed to go along as long as it did not im- pact his commitment to his column.

Dr. Snita Sinukumar performed the surgery last month with a team of multidisciplinary doctors. Dr. Mansi Munshi who administered the radiation pointed out that the patient took it well. "Radiotherapy plays an important role in improving cure rates in rectal cancer. Administered as a precursor to the surgery it facilitates a clean surgery. The shrunk tumour may even allow anal sphincter preservation, dispensing with the need for a permanent colostomy. In Patwardhan's case, we did short radiotherapy over just five days, ensuring extreme precision and accuracy, instead of the regular five-week course," informed Dr. Munshi

Keeping in mind the patient's comfort the course of the treatment was designed. "Usually pursuant to short radiotherapy the surgery is done a week later. But we followed the evidence thrown up by a Swedish trial that recommended waiting between four and eight weeks after radiation allowing the tumour to shrink. Given his age, Patwardhan's surgery was done two months after radiation," Dr. Munshi added.

A thorough evaluation of fitness for anesthesia preceded the surgery. "We minimized the preoperative risk in Patwardhan's case with careful assessment of organ function and reserve, meticulous intraoperative management, and vigilant post-operative pain control," recounted Dr. Sameer Bhosle, consultant anaesthesiologist at Jehangir Hospital.

The time a patient is kept under anesthesia becomes very critical in geriatric cases, as prolonged anesthesia and excessive blood loss can lead to dire consequences. In Patwardhan's case, we completed the entire surgery in two and half hours, with minimal blood loss. He didn't receive any blood transfusion prior to the surgery," explained Dr. Sinukumar. "Early ambulation, good pain control, and careful attention to food and fluid requirement is key to successful outcomes. We were able to achieve that and successfully discharge Patwardhan from the hospital in five days," she added. The patient has gone home with a stoma bag to collect his stool, which will be removed in January.

On the duration prescribed for radiation, Dr. Sujay Hegde, a senior surgical oncologist with Ruby Hall Clinic, pointed out that while many doctors stick with long-term radiation for a good response, it is not possible to put all patients through it. "The short-term radiation is equally effective in many cases. Also in patients directly undergoing surgery the chances of reoccurrence is 40percent but with radiation preceding the procedure the risk can be brought down to 2-4 percent.
