I like to think that the Hidden Circus is a place within the dreams of troubled people, its performers becoming trapped within it in exchange for a wish. Rui wished for Nene to be able to perform again, this desire being twisted to have Nene trapped in the circus in exchange for him. After fighting to take her place and allow for her to return to the real world, Nene then wished to stay with him in this mysterious world so he wouldn't be alone (especially since in her mind she caused this, while in his mind he did).

Rui is the ringmaster, thus others that become trapped in this world initially think he's the one behind it, even when he's stuck just like they are.
