The brain produces these waste substances as it expends energy and sucks up nutrients during the day. Taking out the trash is the job of the glymphatic system, however we don't yet have a full picture of how this network of drainage channels functions.

If you think about it, one of his main objectives is to put huge mental pressure to Yuuji so that his vessel “break”. Wouldn’t it add much more emotional strain to Yuuji if the first thing Yuuji see after he take back control is Megumi in the brink of death??? I still can’t fathom Sukuna’s particularly wild move of bringing Megumi to safety before Yuuji can take back.
Oh dear. Megumi is really the heroine and Sukuna is the rival of the protag.
