i think for a long time dean genuinely doesn't know if cas even likes him. not likes likes him but enjoys his company, wants to spend time with him, etc. cas cares about him but he's not sure in what way. and based on the information available to dean and his specific combination of issues it's a reasonable question, and an active one at least through cas refusing to kill him in s9.

oh my god oh my god Cas is the person who only sees himself as valid in as far as he is serving someone else successfully. So he thinks he has to leave until he can bring Dean a trophy worthy of his affection. And DEAN is the person who only sees himself as valid in as far as he is caring for someone else successfully. So when Cas leaves all he sees is how he failed to care for him well enough for him to want to stay.
