History, guys.
In the Victorian era, underwear didn’t exist. Ladies wore bloomers, men wore… as far as I’ve read, nothing. So, I hope that helps you sleep at night - knowing no male in Black Butler was wearing underwear. Perhaps a thermal layer, but not what would be really considered underwear.
