How are y'all? As my 3rd year of med school and my 3rd year of gyaru has started (galversery was August 16), I would like to set some goals for myself since if you want to achieve, it helps to clearly visualize what it is you want to achieve. If I’m being real, I was supposed to make this post a few weeks ago, but I’ve been so busy

I always say this but I just sorta started this blog as a way to catalogue the events in my life (bc I’m kinda forgetful) and I didn’t need anyone to read my content necessarily, just writing it was enough. I’m leaving gyaru officially.

Did I getcha www What I mean is that I am not going to refer to myself as explicitly a gyaru anymore. And I’m officially leaving the community. Kinda? The things going on in the community (not just recently if I’m being honest) since I’ve joined make me so embarrassed to just be in it despite me not even really being close with other foreign gyaru’s. I’m still going to do gyaru style, I just won’t be calling myself a gyaru if that makes sense. So more accurately I’m rebranding myself. I also hate just confining myself to one style so I’m gonna try and experiment with that abit this year as well. I’ve always kinda just been quiet and keeping to myself in this community, which really does make this decision and transition simple for me to execute.

Anyways without ado let's start with my goals:

Socail life goals

1. Talk only to my friends who have shown that they are good people. 

I made some friends within the past year, good and bad, but I choose as always to keep only positives in my life. If you have shown to me that you cannot be trusted, I understand you and I move on. If you were involved in any capacity with my ex-friend group drama in any capacity, it's done. We've had our good times, but as always it's time for me to move onto better things. Whether that be with my other friends or alone. I will be happy regardless.

2. Make sure to make time for the people that matter to me.

Despite how busy things may get, I should try and do something with my friends weekly or subweekly. Hanging out, even if it's to study together, shows you want to be close with that person and that you value that relationship. We all have our responsibilities too, so I will try my best for this one.

Career goals

I've already sorta planned/made steps for this, but regardless let's go!

1. No resits

I was lucky to not have any resits this time around however I have more responsibilities this year that I have taken on. Resits are exhausting and deplete your time and energy. Luckily the weighting is neutral, but this is to be avoided at all costs.

2. Be in a club/society

Because it’s good to have extracurriculars. I want to join the choir this year and am already the secretary for the nutrition society. I’m also in the innovations subcommittee for the surgical society atm. I’m gonna try my best to do it all.

3. Research 

I want to try and find a research project I’m interested and be guaranteed a summer project within this fall semester. It’s not mandatory but obviously it would be great to do one. God help me because am I gonna be busy this semester（ ;  ; ）

4. USMLE/ MCCQE preparation

I want to do step 1 before 4th year. I also would like  to start preparing from christmas but this will be the hardest thing I will do. I can do it tho. I have endless drive and willpower, so I just need to pace myself and to do well. And also. DO. NOT. PROCRASTINATE.

Fashion/subculture/SNS goals

1. Learn more parapara and post tiktok’s!

I know a few routines already but I’d like to learn even more this year! As a medical student I’m kinda busy but I want to learn some more this year as well.

2. Growing my TikTok 

Honestly, I slightly regret the time I devoted to growing my IG versus how that could have been used to grow my TikTok. Hopefully this is gonna be successful. At least by my own standards.

And even if I don’t end up completing my goals, I make some progress which is just as good to me. Hi! :D So sorry for the late response. Things got a bit hectic irl ;-; But I'm really glad to hear you're doing well!

I hope it's safe to assume you're from the West---I think majority of the folks on here are. The US system is the one I'm most familiar with, so I'll try to explain using that as the basis. So once you graduate from premed, you take the MCAT and go to medical school, right? Well, med school typically lasts for four years. Unless your program is MD/PhD or MD/MBA, where they sometimes stretch into six to eight years.

Those four years are divided into pre-clinical (first two years of didactic a.k.a. MS1 and MS2) and clinical years (last two years, MS3 and MS4). After the first two years, you can take the first out of the three licensure exams, USMLE Step 1. MS3 and MS4 are your "clerkship" years. That's when you spend most of your time in hospitals and rotate under different specialties. After MS3 though there's another hurdle to overcome, and that's the second licensure exam, USMLE Step 2 CK, which tests your clinical knowledge. There used to be a USMLE Step 2 CS which was supposed to test your clinical skills but it's been discontinued since COVID.

Anyway, after you're done with that you continue MS4 with rotations in hospitals again. Around this time you'll also be applying for residency training in the specialty you want to be in. You submit your Step scores, med school grades, CV, letters of recommendation, etc. to an electronic portal which forwards them to all the residency programs you're considering, and if the program committee ~☆deems you worthy☆~, they'll extend you an invitation. Once interview season is over, you'll be able to rank the programs you like from your first choice to last, and the programs will do the same by ranking their favorite candidate to least. The electronic portal will compare the list you made with the programs', and this will determine if you guys "matched". During residency training you can take Step 3, but a few folks take it as soon as they finish med school just to get it out of the way.

Length of residency training varies per specialty and program. For comparison, Internal Medicine usually takes 3 years while Neurosurgery takes 7. Fellowship is when you do subspecialty training, and again the number of training years vary. There's also sub fellowship, where you'll be studying for the subspecialty of your chosen subspecialty (it's complicated ik the human body is a shitshow). Fellowship isn't necessary though, it really depends if you see yourself more of a specialist or a generalist. And after all that time and money and tears, you'll become an attending, which I guess is another brand of suffering on its own but at least the pay's better (???) ;-;

Anyway, that's how the US training goes. The names, years, and the cost of training definitely varies from country to country, but I find that med school -> residency -> fellowship/attending pipeline is essentially the same everywhere. Hope this gave you an idea! :D

today the nbme announced that they’re making usmle step 1 pass/fail starting 2022 and i am so fucking salty about it because

i was told since day 1 of medical school that this was the most important test of my life (verbatim. from multiple people). my score would determine what specialties i could go into, what kind of residency programs i could apply to - basically it would set the trajectory for the rest of my medical career. i set aside 7 weeks of my life after my second year to do nothing but study for this test. worrying about my score put a dent in my mental health and i remember feeling worthless and lost whenever i’d do poorly on practice tests. i put in so much hard work and discipline to study for this test to get the score i got, and for it to become a matter of pass/fail kind of renders all that hard work worthless (but this is a moot point because by the time pass/fail is in place, hopefully i’ll be in my intern year)

at first glance this policy change appears to be beneficial for all medical students because hey, you don’t need to study yourself sick for this test anymore because all you need to do is pass....... but when you think about it, the people who end up benefitting the most are people who attend elite, private medical schools that have prestige in the medical field. let’s say a student at harvard scored a 230 (an average passing score) and a student at a mid-tier state school scored a 260 (an exemplary passing score). when you take the numbers away, you’re left with a harvard student and a mid-tier state school student and let’s be honest, competitive specialties like ortho or derm are GOING to look at where someone went to med school it’s inevitable

who you know in the field has always been important, but changing step 1 to pass/fail also makes things like NETWORKING even more important to do in order to get your foot in the door for certain residency programs... which is hard for applicants who don’t have the advantage of being born into a family of doctors or the money to go to a school where the faculty have national/international recognition

i WILL give them this though - step 1 is arguably the least clinically relevant test (i.e. things that are tested are not always applicable to real-life patient care) so i’m glad that medical students can spend less time memorizing basic science facts and devote more time to learn how to be an empathetic healthcare provider

anyway i only see this really benefitting students who have the advantage of going to an elite institution or having an extensive network of physicians in the long run

if the nbme really cared about student wellness they could start by cutting down examination fees. step 1 alone costs $630.

Step 1: Appear for the National Medical Admission Test (NMAT), the national level admission test is required to be qualified to get admitted to MBBS in Philippines.

The CBSE iea practice exam for the United States Medical Licensing Examination (USMLE) Step 1 and is purchased directly from the NBME.e

Below are some key points to realize when thinking about these NBME practice exams:

Where to find them: To find the NBME practice exams log into their self assessment website and look under the “Comprehensive Basic Science Self-Assessment” where the practice tests for Step 1 will be listed.

Details of the exam: There are 6 exams offered as of now. Each one costs $50 to purchase (without any feedback on the questions) and 60$ to purchase (with feedback on the questions). Each exam is 200 questions in length (split into 4 sections that are each 50 questions in size).

Answer key/explanations: It is VERY IMPORTANT TO REALIZE THAT THESE EXAMS DO NOT COME WITH AN ANSWER KEY OR EXPLANATIONS. By paying the additional 10$ for feedback for each exam, students buy the “privilege” of being told which questions they answered incorrectly. This, while valuable, still leaves a lack of closure for students taking these practice tests. It is for this reason that many students gravitate away from using this resource. NOT TO WORRY much of the content that is tested on these exams can be found on Stepwards.com! Everything I know about the USMLE Step 1 as a non US IMG

Full disclaimer: i recently gave my USMLE step 1 and I’m waiting on my results, none of these tips can guarantee you a great score because I didn’t get my scores yet.

That being said, I’m going to try and include everything I can about the application process and the exam itself and things I could’ve used knowing when I was applying.

Info: My medical school is in India and I graduated this March.

1. Knowing if you want to go for the USMLE :

The USMLE is not an easier alternative like everyone thinks and it’s a major decision, do not go into it lightly, consider all the pros and cons, what you want to later in your life, how much it will cost, where you want to settle down eventually, what specialty you want, etc

It is expensive and time consuming especially for IMGs since a lot of our med school curriculum doesn’t give us breaks at the end of second year to give the step 1 like they do in the US medical schools.

You also need to talk to your parents because this exam and process, isn’t cheap. I’ve spent around 2000 dollars just for step 1 (application and study materials included).

2. Applying

The process for applying is pretty straightforward and easy to follow through the ECFMG website.

First, you make an ecfmg id and then you follow along.

Ideally, you should do this at least 6-7 months before you plan on giving the exam. Planning is everything. Try to have a rough plan of when you’re going to do what with all the  time periods factored in and all possible delays. The ECFMG itself is quite reliable but your medical school may take more time with your paperwork and that can cost you.

3. Form 183 and form 186

After you pay for and apply for your exam, for the offline part of your application to be accepted, your medical school will have to fill out either form 183 or form 186.

if you are still a student and you haven’t graduated, it’s form 183. If you’re doing your mandatory internship like in India, it’s considered that you HAVEN’T graduated and you still need to fill form 183.

If you have graduated, then you have to fill form 186.

The forms are generated by the ECFMG after you fill in your details and pay your fee.

Make sure your forms are filled perfectly by your medical school and not even the stamps are sideways! If a form is rejected you have to start a new application all over again which is a tedious process. And your college has to send the forms themselves. Mine tried to make me send mine aand it got rejected the first time around.

Some schools can even charge you some amount of money to do this (mine did) and depending on your student section, they can take time with it. Factor this in before you choose your eligibility period. The form took around 20 days just to reach ECFMG after being mailed and two weeks after that for it to be approved and  my scheduling permit to be generated.

Send your forms at least 2 months before the start of your eligibility period.

4. Eligibility Period

You can only give this exam once, you’d rather take as much time as you need rather than rush in and then do poorly. Initially when you plan, you can think that you need less time than you might need in reality, so choose a reasonable eligibility period. It costs a lot to change it later on as well and you’d much rather spend that money on good resources.

The same applies for choosing your date with prometric, If you change your date 31 days before the exam, you won’t be charged but after that, there is a charge on changing your appointment.

5. Prometric and Exam day

Do call and confirm with your center about your appointment and the rules. Cross check every little detail including what kind of ID you need to bring. I would suggest everyone to bring their passports even if you have another form of ID. I saw a lot of people struggle and have to go back to get their passports or reschedule in the last minute. Remember, your ID needs to have your photo and signature.
