Okay, hear me out. One of the things I enjoy most during clinic year is the thrill of having my labsheets approved. Before our forms are signed, we have to answer particular questions or perform procedures. Without the labsheet approval, you will not be able to dispense your patient's lenses.
You never really know what question or task will hit you and the uncertainty of it all challenges me to be on top of my active recall game. It allows me to revisit past lessons and gauge which areas I have yet to read more of.
It's also during this time that my past notes come in very handy. There are days I regret wasting my time from the past for not studying as productive as I hope to be. It was tiresome scanning all of my notes, backing up files, and never really opening them as much as I want to.
Earlier this month, I attended a youth summit geared towards research innovation. One of the instruments available in the fair was a weighing scale that also measured muscle and visceral fat. I haven't worked out in a long time. Hence, I was both surprised and happy to see that most of my weight still comes from muscle and that my visceral fat is at 1%.
The present version of myself is grateful for the hardwork I have put in the past. It may not be perfect or as seamless, but it is enough to give me a headstart for this tiring semester. These are things that are not directly specified on the checklist for clinic preparations. But these are things I could suggest to the next batch of clinicians. Stay physically healthy so you will have energy during and after clinic + store your notes (!!)

한국어 실력을 높이기 위해서 매일 한국어로 일기를 쓰고 있어요.
As I have mentioned, September was a jam-packed month. I continue to upskill in my various interests and hobbies and pursue opportunities that I see myself enjoying - which is true most of the time. There's just so much homework to do. 🙈
I started studying Korean again to pursue translation in the future. Korean Cultural Center in the Philippines offered free cultural and language classes for the final term this year, and I got a slot for one of their classes. Registration at KCC is competitive. I initially aimed for two classes, but I am still lucky that I got into one of them. With King Sejong Institute, students complete 60 hours of language learning to graduate. Classes are happy, free, confusing, and seemingly complex at the same time 😉 but our teacher helps us understand each lesson before moving to another topic.
Oh, and we get to eat together after class, too!
I'm also studying Italian. But that's a whole other, yet equally confusing story. Languages are already mixing up in my head. But I still think that I want to learn more!

it's our penultimate meeting for my retail trade laws class before prelims exams and i badly wanted to get called for recits this week bc i'll be going on a trip next week and i dont want my being on-deck looming over my head when im enjoying baguio. thankfully i got called ☃️🌬🌲🍃🍓
im a night person but studying in the morning makes me feel more productive. the soft light casted by the sun on things in my room sparks a particular kind of joy in me ☀️

Hello world
Hello world! This is R, an incoming 1st year medicine proper student in the Philippines. I made this blog to document my journey starting now and share learnings and resources.
The title of this blog refers to a quote from neurosurgeon and writer Paul Kalanithi’s book When Breath Becomes Air.
While discerning on whether I was ready to commit to applying for med school, I tried to read as much literature on and by medical practitioners as I could, but his book struck me the most. It was such an eloquent take on the humility of training so hard to save lives, only to be faced with your own mortality.
There were so many gems in the book but this quote struck me the most. In the book, this insight comes after one of his long time med school friends cannot come to terms with a difficult complication with one of his patient, and ends himself. Kalanithi could only imagine what his friend could have been going through, and in that moment, this is what he had to say.
“Our patients’ lives and identities may be in our hands, yet death always wins. Even if you are perfect, the world isn’t. The secret is to know that the deck is stacked, that you will lose, that your hands or judgment will slip, and yet still struggle to win for your patients. You can’t ever reach perfection, but you can believe in an asymptote toward which you are ceaselessly striving" — Paul Kalanithi, When Breath Becomes Air
As early as now, I concede I may not be able to save everyone (another author, Dr Ronnie E. Baticulon comes to mind.) I make no claim on perfection but I can commit to at least an attempt: to try, to strive, and to side with the good, whenever I can, and concede to limits and learnings, whenever I cannot.

According to the Philippine Clinical Practice Guidelines for CAP in ADULTS (2020):
Unstable Co Morbids include: -> Uncontrolled DM -> Active malignancies -> Neurologic disease in evolution -> CHF FC II-IV -> Unstable CAD -> Renal Failure on dialysis -> Uncompensated COPD -> Decompensated Liver disease
Legionella urine antigen test and Influenza test are conditional recommendation for CAP HR: -> Influenza test may be done during periods of high influenza activity (July - January) if CAP HR is preceded by inluenza-like illness symptoms such as body malaise, rhinorrhea, arthralgia, sorethroat plus risk factors such as >60y/o, pregnant, asthmatic.
DRUGS: PENICILLINS: amoxicillin MACROLIDES: clarithromycin, azithromycin BETALACTAMS: co-amoxiclav, cefuroxime TETRACYCLINES: doxycycline NON PSEUDOMONAL BETALACTAMS: ampisul, cefotaxime, ceftriaxone FLUOROQUINOLONES: levofloxacin, moxifloxacin
Atypical coverage for Aspiration Pneumonia is only recommended if with suspicion of lung abscess or empyema.
Antiviral treatment is recommended with antibiotic therapy in CAP HR with risk factors mentioned under influenza testing.
Treatment should be initiated within 4 hours regardless of risk.
Duration of treatment: -> CAP LR , CAP MR: 5 days if stable -> May extend duration if pneumonia is not resolving, complicated by sepsis, infected with less common pathogens or infected with drug-resistant pathogens.
CXR post-treatment is recommended after a minimum of 6-08 weeks to establish a baseline and exclude other conditions but it is not recommended for routine testing in stable improving patients.
Inadequate Response after 72 hours of Empiric treatment: -> Reassess for possible resistance -> Reassess for the presence of other pathogens such as M. tuberculosis, viruses, parasites, or fungi.
