Half-ass Book Review #005 - [Mental Health]
"Healing Back Pain - The Mind Body Connection"(1991) by John E. Sarno, MD
I was introduced to this book on an episode of Conan O'Brian's podcast, where he mentioned about this book. His sentiment on the book was very positive.
As I also felt frequent fatigue on my lower back, I thought maybe I would give it a try.
The main contribution of the book is the recognition of the link between mental health and physical pain, as well as trying to breakthrough a common stubbornness of modern medicine.
Dr. Sarno has seen many patients with chronic, even acute pain for some. Most if not all of the patients sought physical or chemical treatment such as massage, drugs or even surgery, and almost all doctors were the ones driving this consensus.
According to Dr. Sarno, over half of the cases the pain starts out gradually. Another thing he noticed is that many patients with the Tension Myositis Syndrome (TMS), as he calls, can easily recognize themselves with perfectionism, or was preoccupied with other stressful things in their life.
Stitching this information, Dr. Sarno came to a conclusion that the psychological strain the patients were going through has caused the pain.
The extreme stress a person goes through, either it's divorce, compulsion to achieve goal in work, friction with ones family, is a pill that is too hard to swallow. As response, our body tenses up certain muscle and induces pain, providing distraction for the patient.
The treatment for it? Dr. Sarno has tried combinations of psychotherapy, education, and physical treatments. But as far as he is concerned, providing only the education has helped the most. Just providing the notion that it is not a structural abnormality that is causing the pain, but it is the mental stress has helped the patient contemplate on it and ease the stress on their own.
One of the questions one could ask is then how does the pain outlive the actual problem it cause? The answer is that since the patient thinks the pain is caused by a triggering physical activity, the patient becomes hyper aware not to pain oneself again. This very factor, adds into the cause and will last even when the actual problem has gone away. The "fear of another attack" maintains the tension, and the fear often does come true.
This is because the main culprit of this syndrome is "anxiety". There are many ways anxiety can be started, even with the people who you absolutely love. For example, "the need to be loved, admire, respected; the drive to achieve" is something that isn't inherently a negative feeling or a harmful thinking. But it is quite easy to see how these can give stress to anyone.
The hardest part of the treatment is to let the patient accept that this is the correct diagnosis. Those who happened to improve on their situation on long term were the ones who could accept this knowledge therapy.
Dr. Sarno says he has also asked himself many times that "How do I know that what I am doing is not a placebo?" but it seems to work significantly well.

Literally all the shit rich people have turned into luxuries are stuff many disabled people need (or would need to manage their pain but can't afford it)
Comfy ergonomic chairs
Indoor pool/hot tub (therapy bath)
Massages on the regular
Aides (rich people call them servants)
Yea even a cook who makes you special meals (perfect for people with special dietary needs and for those with severe allergies, as well as people who are in too much pain or are otherwise unable to cook)
Elevators in your house (even small ones just for groceries, my rich aunt has one in her beach house!)
Rich people don't buy these for fun I hope but custom powerchairs are obscenely expensive. It pisses me off when I see another person invent "the wheelchair of the future!" Which then is literally never fucking used because none of us can afford it (and insurance definitely won't pay)
Indoor gyms or even personal exercise equipment. Hard to go out to a gym somewhere else when you're disabled, especially if you are immunocompromised
Outdoor spaces to relax in. It's literally vital for your mental health to at least see the outdoors. I'd rather be bedridden in a sunroom (with retractable blinds) than a shitty apartment with one tiny window.
There's even freaking health retreats these people go to regularly. There's a fibromyalgia retreat in new york where they basically take care of all your needs while trying different treatments and seeing which ones help. Either it's heaven or making money off of scamming desperate people who are able to scrape the money together to go.
Private planes, which I honestly think shouldn't exist, but one that specifically catered to people with disabilities (spaces for wheelchairs/other mobility devices, accessible handicapped airplane bathroom, anxiety reducing tools, trained medical personnel and care team)
Also customized cars, except instead of making gas guzzling racecars to joyride in while everyone else is trying to get to work, cars with electric ramps, lifts, doors, cars customized for someone with limb differences. Those cars where you can roll your wheelchair right up to the wheel. Fuck even self driving cars once they are no longer deathtraps.
Skincare products that are safe for sensitive skin like eczema but also actually work
Nice-looking clothes customized to fit limb differences, access points, look good in wheelchairs, colostomy bags, etc. while also being comfortable and not fast fashion.
Dental care!!! What the fuck why is this shit so expensive!! I don't want my teeth to fall out!! (Disabled people usually need more dental care bc we have a harder time keeping up maintenance)
Rich people go and splurge on all of these even though they don't need them while calling disabled people selfish for begging their insurance for even one of these.

i'm struggling to even put into words how i feel about these two scenes.
i honestly wonder how much of house's facial expressions in these moments were hugh laurie's choice and maybe weren't necessarily scripted, because the episode doesn't really offer any kind of conclusion on how jealous and, more importantly, how deeply fucking sad house looks after realizing wilson has been talking to amber.
first off - this happens a lot in this show, but this is one of those huge instances of "oh, how differently this situation would look if one of these characters was a woman." if that were the case, this whole thing would really be quite simple. if house was a proper Opposite Sex Love Interest, him standing at the door hearing the proof that his unrequited crush isn't over his last girlfriend...well, that would make perfect sense. The level of hurt and jealousy house seems to feel about wilson still being in love with amber is. well. it's Something.
so anyway - house gets that genuinely hurt look at three separate points: #1 is when he realizes wilson is talking to amber (and this moment is especially fascinating bc house's main emotion should arguably be relief at the knowledge he hasn't been hallucinating...but that appears to be taking a backseat to his jealousy for a woman who isn't even alive anymore)
#2 is when wilson tells amber that he wasn't able to go for a run tonight because house is having issues. now, this speaks to the broader problem that - even though house knows wilson feeds off neediness - house is still worried about being a burden to wilson and that eventually wilson will not want him anymore.
Then #3 - when wilson says to house's face that talking to amber makes him feel better when he misses her, and house doesn't. This moment is so painful and interesting to me because house inviting wilson to confide in him feels like a pretty big step in terms of growth! So for wilson to say he'd rather talk to his dead girlfriend than house...well, judging by house's face, the remark cuts pretty deep. (disclaimer: ofc wilson is completely valid for talking to amber; it just also makes sense that house would be hurt by this, especially in the context of him already feeling like a burden and trying so hard to be a better person)
anyway idk where else to go with this . . . i just feel like the episode sort of started to delve into this issue and then never really went deep enough or concluded this aspect - hence my theory that house's level of hurt may not have been scripted and it was just hugh laurie choosing to Do That with his face.

please be aware that disabled people are not "against exercising"
Some disabled people's symptoms worsen when exercising and they can't affort that while holding a job/studying/making an ssi application
some diasabled people cannot exercise unless in physical therapy, which they may not have acess to.
Some disabled people risk hurting themselves permanently by exercising
A lot of disabled people do exercise but abled people don't consider it exercise because it isn't normal enough.
Most disabled people have been told to "just exercise" or "try yoga". Most disabled people have been guilted into thinking it's their fault because they're not exercising or not enough. Most disabled people feel like shit when you bring up exercise because they'e learn it's followed up with either a lesson on how they can "cure their disability" or how they're "letting themselves go"
Some disabled people get trauma response by people telling them to exercise, or that execise is good for them even if that is true.
When someone tells me exercise is good for me i have to stop myself from sobbing on the spot. Because my dad and doctors told me i should just exercise no matter how many specialist's letters i have saying i risked permanent damage. Because my physical therapy is right there, i just can't acess it. Because all the ressources are a subway away and i'm being denied them. i'm being told to way for a year and a half while i watch my mobility and pain worsen, knowing it could be getting better. Because it reminds me how abandoned i am from people who could save me.
Because telling disabled people "exercise is good for you" or "have you tried exercising" is an insult to how much they want and are trying to get better. This is getting in the "i know better" position and getting mad when you do not know better.
