Renee Salas, a physician at Massachusetts General Hospital, has been thinking about the connections between climate change and public health for a long time. In her emergency room in Boston, she sees the consequences of warming up close. Extreme heat, air pollution, vector-borne diseases, and longer allergy seasons are taking a toll on her patients. “My job as an emergency medicine doctor is to protect my patients and keep them healthy,” she said at a press briefing on Thursday. “Climate change increasingly threatens my ability to do that.”
Salas was at the briefing to weigh in on a new report that shows the health costs of climate change now far exceed $820 billion per year in the United States. One of her patients, she told reporters, a middle-aged man, visited her emergency department 30 times in one year with debilitating symptoms of Lyme disease — a tick-borne illness that can cause fever, headache, muscle pain, and neurological damage. She treated a four-year-old girl who had to seek emergency care for asthma attacks intensified by climate-driven pollen seasons and air pollution. The girl’s mother, a single parent, asked Salas for a doctor’s note so that she could show her boss that she was missing work for a legitimate reason — she had already been forced to miss three shifts that week and would likely have to miss more to care for her sick child.
Some of those health costs include premature death, medical care for physical and mental health after a major natural disaster, lost wages due to climate-related illnesses, and the price of filling prescription medications for those illnesses. The report found that the diagnosis, treatment, and management of new cases of Lyme disease nationwide costs between $860 million and $1.6 billion per year. The cost of mosquito-borne West Nile virus hospitalizations, premature deaths, ER visits, and other outpatient visits totals $1.1 billion annually.
By aggregating past climate-related public health costs, the report shows that myriad medical issues brought on by climate change are already taking a financial toll on taxpayers. And researchers estimate those costs will continue to rise as climate change accelerates. “These impacts are here and now,” said Vijay Limaye, an environmental health scientist at the NRDC and a co-author of the report. “They’re not just some distant threat.”

Robert Ren is a writer and teacher in New York. He has a BA from the University of Chicago and an MFA from Columbia University. Having escaped a corporate career, he currently tutors kids in standardized test prep. He managed to avoid the whole college admissions scandal, but that's only because his photoshop skills are terrible.

Bronwyn Scott-McCharen was born and raised in Jackson, Mississippi and graduated from Hendrix College in 2014 with a degree in Sociology and Anthropology. She then lived in Buenos Aires, Argentina for three years, where she immersed herself in the country's vibrant political culture under the guise of academic research. Her interests outside of writing fiction include travel, photography, international politics and history (especially Cold War history). She is currently hard at work on two novels in distinct stages of development--one completed manuscript in need of polish and another in the earliest phase of drafting and intensive research. She speaks Spanish and Brazilian Portuguese and hopes to soon add Bosnian/Croatian/Serbian to her budding repertoire of languages.

Mir Arif developed the idea of storytelling at an early age from strangers—astrologers, street magicians, herbal medicine sellers and other con-artists—frequenting the quiet alleys of his childhood neighborhood in Comilla, a small town in southern Bangladesh. He graduated from University of Dhaka with a degree in International Relations and worked as a staff writer for Arts & Letters. His short stories have appeared in various magazines and e-zines in the US, Singapore, India, Sri Lanka and Bangladesh. One of his short stories was longlisted for the Commonwealth Short Story Prize 2019. He likes to hike and spend time with parakeets.

Karen Gu's fiction has appeared in Paper Darts and The Margins and is forthcoming in McSweeney's Quarterly. She has been awarded fellowships from Kundiman, the Jack Jones Literary Arts Retreat, and the Loft Literary Center. After five years in Chicago and four years in Minneapolis, she is looking forward to the desert.

Sylvia Fox has too many interests and a wandering soul, which is why she writes fiction. Most recently, she spent the last two years in Baltimore, MD, surrounded and inspired by artists. So many aspects of her identity have led her to believe in the subversive power of showing up, taking up space, and creating space for others. Sarah Spaulding is a Tennessee native and a lover of the mountains that raised her. She graduated summa cum laude with her BA in psychology and English with an emphasis in creative writing from Carson-Newman University. There she discovered her penchant for digging around in people’s heads. She often writes poems to dig herself out of her own head. Her work appears in Tennessee’s Best Emerging Poets, Aletheia, Ampersand, The Sigma Tau Delta Rectangle, and soon-to-be a guide to Southwestern Iceland. When she’s not busy exploring the mire of humanity, Sarah enjoys dancing in the sunshine, petting other people’s dogs, and helping her father type his memoir.

Jo O’Lone-Hahn is from rural Pennsylvania, and is now on her way to Las Vegas, continuing on her lifelong mission to see the world. She has a B.A. in poetry, studio art, and religious studies from Hampshire College. She writes poems that focus on misunderstood people, naiveté, and the imagination inherent in remembering. Jo has held jobs such as: social worker, tattoo-shop-front-desk-chick, archivist, and tarot-reader-on-the-streets. She is also a member of the Departure Collective, a literary group which conducts workshops, organizes poetry readings, and creates chapbooks. When she’s not writing, she makes mixed-media artworks, wanders around, and befriends grumpy old men.

Kathryn McKenzie is a Las Vegas native with a BA in English. She drinks enough tea to match the annual consumption of the entire country of Ireland, and prefers snuggling up in her reading chair with a book, toast, and tea to almost anything in the world. Beyond her deep love of poetry and literature, her passions include: asking to pet every dog she sees, cracking her back after standing up in the movie theater, planning Halloween costumes years in advance, and talking about all the parties she is going to throw, but never actually throwing them. Female veterinarians say gender disparity remains an uphill battle

This is the fourth in a four-part series, “On Call: Vets Under Stress”, that explores the pressures and overwhelming challenges facing those who choose a career in providing care for our animals.
In 2009, the American Veterinary Medical Association reported that female veterinarians outnumbered their male counterparts for the first time. While they make up more than half of the veterinarians in the industry today, a culture of masculinity still exists.
According to a study conducted by Lancaster University Management School and Open University Business School, female practitioners say gender discrimination remains an uphill battle.
“Many of the women we spoke to, especially those in their early career, reported experiences of clients – or even in their own practices – treating them as having limited competence and credibility..." co-author Professor David Knights of Lancaster University said.
Al Martens who has worked in a large animal practice in eastern Wisconsin for 43 years said many women starting out in the field with him faced many challenges.
"Women have really had a tough road. They had to be pioneers, in that they had to prove themselves more than the men," Martens said.
In areas of the state considered agriculturally dense, Martens says its common to have large animal practices staffed with more than one female veterinarian.
"If you get up in the rural areas where there's not a lot of farms, there may be a lot of farmers that haven't experienced a female veterinarian," he said.
Pulling their weight
Meg Mueller was fortunate that after graduating from the University of Wisconsin School of Veterinary Medicine in 2010, she returned to western Wisconsin where many of her clients already knew her.
But as the "new" vet at Osseo-Augusta Veterinary Clinic, she still had to make the grade out in the field.
"The tough part is you need to prove that you're going to be a good doctor. But you shouldn't have that connotation behind it that you're a 'female' and you have to work that much harder," Mueller says. "Thankfully my co-worker Dr. Linda Giese who has been practicing for over 20 years went through those trenches of gender disparity so that those of us that came after haven't had to fight as hard. But it's still out there."
Today three of the four veterinarians that staff Mueller's mixed animal practice are women.
Mueller says one of the biggest challenges that females face in large animal practices is the issue of working with very strong and powerful patients in - much of the time - nonclinical, less than ideal settings. And client's perception that they 'can't handle the job'.
"It's a very physically demanding job with some risk involved and some of those issues can come to light in those situations," Mueller said. "But sometimes you have to figure out what your strengths are both physical and mentally and be able to take those tasks at hand."
Gender discrimination
Although gender doesn't play a role in how well a veterinarian performs a surgery on a DA (twisted stomach) or diagnoses hypocalcemia, if left unchecked, it can factor how a client interacts or trusts the opinion of the veterinarian.
One female veterinarian told Lancaster University researchers that a client demanded a male veterinarian come out to her farm because the woman was unhappy with the treatment that the female veterinarian had delivered – even though the male vet did the same thing.
Veterinarian Katie Mrdutt says that while farmers in her clinic's territory up in northwest Wisconsin had already been exposed to female veterinarians, she sensed the hesitancy when she arrived on the farm of a new client.
"My favorite part of those experiences was showing how I could use my brain to tackle challenges that maybe the client thought only could be done with brawn," Mrdutt said. "One particular client had a beef cow having difficulty calving. He had been working on her for a few hours before he called me and had not been making any progress."
Within 10 minutes of her arrival the calf was on the ground and alive.
"He was shocked at how quickly I fixed the problem, and probably more so because I was 8 months pregnant at the time," said Mrdutt.
Dual roles
Among the 75 veterinarians interviewed in the Lancaster University study, many felt that women practitioners were automatically presumed to be potential mothers, a perception that could be viewed as problematic for long-term careers.
One female equine veterinary professional told dvm360, a veterinary media organization, that during her interview to get into veterinary school, she was asked how long she planned to practice before starting a family. An older veterinarian intimated to her that it wasn't worth her time to invest her time and money in veterinarian school because she would eventually leave the profession in order to have a family.
Many veterinarians like Mrdutt and Stacey Adams, who is the sole practitioner and owner of her Lake Geneva area practice, have found a way to balance the demands of motherhood and a taxing career. Mrdutt left private practice after three years to join a non-profit that focuses on responsible drug use education among farmers and veterinarians.
Adams, who runs a small beef operation at home and is the mother of two children under 4, including one with special needs, says over the past 12 years she has learned to set boundaries between her personal and work life. This includes reserving after hours and weekend calls for emergencies only.
"If this costs me a potential client, then they probably weren't a client I wanted in the first place," Adams said. "Some clients want to say that I only feel that way because I'm a woman, but I say it is because I value my family and my health."
Surprisingly the clients that have been most judgmental about her boundaries and family needs have been women. She recalls a time when her special needs son then 2, had to be admitted to a hospital 95 miles from their home for a severe respiratory infection.
During this health crisis, Adams received a call from a non-client who had a sick goat. Adams relayed she was unavailable and supplied the woman with two other clinics to call.
"She had the audacity to tell me that she had never had a veterinarian put a child before an animal and that I needed to re-evaluate my career choice if I felt my child was more important than her goat," Adams said. "I told her that any veterinarian can take care of her goat but only I can hold my son and comfort him when he is crying for his momma in the hospital."
Mrdutt says most farmers were welcoming when on more than one occasion she had to bring her child along on calls.
"Usually the farmer had more fun with him and forgot about assisting me," she laughed. "The beauty of working with some clients is that they get to know you and understand the challenges we face, especially balancing the demands of afterhours care."
Afterhours calls to an unfamiliar farm late at night may put a lone female veterinarian in a vulnerable situation. While Mueller has been fortunate to have a good working relationship with her clientele, some of her friends haven't been so lucky.
"I have classmates that have been in some pretty bad situations. No one deserves to go to work and be put in a position like that," Mueller said. "And it's hard because we're out on the road by ourselves, and then there's the call in the middle of the night for that client that you don't really know. That can be daunting."
Bridget Heilsberg, equine veterinarian, owner of Crown 3 Veterinary Services in Whitesboro, Texas, and president of the Women's Veterinary Leadership Development Initiative told dvm360 that safety on farm calls is a regular discussion for the women in a Facebook group she's part of.
Solutions range from traveling with a canine companion to calling friends, coworkers or loved ones before visiting a new client—even to carrying a firearm for protection.
"I personally not only have a firearm but I also have a German shepherd, and I travel with her to my farm calls," Heilsberg says. "I feel like it's an additional concern that women have because we have that concern in our everyday lives. Then it gets amplified when we go out on a farm call visit with someone we don't know."
A ways to go
Mueller says it's important for a veterinarian's colleagues and their clients to respect their need to find a healthy work-life balance.
"I think in the profession it still comes down to a respect factor if a woman chooses to both work and have a family or needs to pull back and not work as much," she said. "I'd like to think that we've come a long way, but we still have a long way to go in addressing those gender disparities across the industry, whether that be pay or the respect factor."
A study from Cornell University researchers that surveyed 2,760 veterinarians discovered a pronounced wage gap between male and female veterinarians, especially among recent graduates and top-earning practitioners.
The study, published in the March 15 issue of the Journal of the American Veterinary Medical Association, found that among the top quarter of earners, men were paid about $100,000 a year more.
While reasons for the wage gap were not identified, researchers noted that factors could include unconscious bias, size and type of practice and societal expectations.
Their analysis showed that type of ownership was instrumental in earnings. Partnerships were believed to be more beneficial for womens' income earning potential than sole proprietorships, while any form of ownership benefits men’s incomes. When it comes to the number of years worked, the study found that men move into higher income brackets at lower levels of experience than women.
Martens says female veterinarians on staff at Waupun Veterinary Service pull their weight the same as their male counterparts. He says that veterinarians - regardless of gender - that display a good attitude, strong work ethic and contribute to the practice are rewarded.
"This culture goes back to the guy that hired me. We try to make everyone a partner to make it economically feasible," he said.
Know your worth
Mrdutt says gender disparity and crushing student debt load, added to the strain on mental health from long hours and demanding clients and compassion fatigue, makes veterinarian medicine a hard profession for women to work in some days.
"I had my mind set on being a veterinarian since I could walk and talk, and I do love a challenge," Mrdutt said. "I'd tell any young woman considering this profession to know their worth. Know what you want and don't want, and that it's ok to take care of you first. Finally, know that you are more than capable."
Adams says she has never let being a woman hold her back in her profession.
"I have always been of the opinion that if you do your job and you do it well, people will respect you for it," she said. "Those who don't are people that you don't want for clients anyway. Those are the people who are going to take advantage of you and will never be happy no matter how much you do for them."

Food Healthy Kids, Food Healthy Adults
 “Food is medicine,” the old adage goes.
In medicine, it is good to remember that the most important difference between a medicine and a poison is the dosage. This applies to food. We’ve certainly seen the poor health outcomes directly and indirectly related to the US Cult of Food Overdose.
Cases in point:
US incidence of adult and childhood obesity: Epidemic. US incidence of colorectal cancer: Epidemic. US incidence of Type 2 Diabetes (T2D): Epidemic.
Each of these diseases is directly correlated to food overdose.  Worse, our sedentary lifestyles are co-conspirators. Imagine if a foreign power caused the disease and death that our sick food systems contribute to.
Bad food corporations spending millions on very effective marketing is one external villain. Our own willingness to trade our health for a mouthful of temporary feel-good is the internal villain. That means ultimately, the fix is in the mirror.
How did we get into this food-disease mess? In July 2018, the American Academy of Pediatricians (AAP) offered one strong answer: 
According to the AAP, the US Food and Drug Administration (FDA) got us into this mess. Wait. What?
Referring to a GAO assessment of FDA practices, the AAP supported the conclusion that “…the FDA is not able to ensure the safety of existing or new (food) additives through (their) approval mechanisms.” (emphasis added). Read the full statement here.
In the case of foodstuffs intended for (and heavily marketed toward) US kids, the FDA is doing a miserable job of protecting kids. Why would we expect otherwise? Regardless of political party in power, the FDA is led, managed, and run by the very food-for-profit and drug-for-profit businesses it is supposed to regulate. FDA appointees aren’t Moms or Dads looking out for kids, or kids looking out for their aging Moms or Dads, or everyday folks like you and me trying to make smart food choices. The FDA is run by a constant flow of people racing through the revolving door between government and industry, cashing in on your health and that of the nation with every turn of the door.
That sick mix of bad practice and corporate policy is killing us. It’s killing our kids and their parents.
America’s pediatricians are increasingly concerned for a reason. Not only are there bad chemical preservatives in foods marketed to kids, the packaging leaches harmful chemicals that leach into food. Keep in mind this is FDA permitted/approved packaging the stuff that agribusinesses use to convey convenient (aka “dead” and “processed”) food to us…and to our babies and kids.

These chemicals, AAP says, “may interfere with the body’s natural hormones in ways that may affect long-term growth and development (NYT July 24, 2018, A14).
Cans, cardboard, even the “waxed” paper used—all leach chemicals into our foodstuffs.
Let’s give the pediatricians their say: “Our purposes with this policy statement and its accompanying technical report are to review and highlight emerging child health concerns related to the use of colorings, flavorings, and chemicals deliberately added to food during processing (direct food additives) as well as substances in food contact materials, including adhesives, dyes, coatings, paper, paperboard, plastic, and other polymers, which may contaminate food as part of packaging or manufacturing equipment (indirect food additives); to make reasonable recommendations that the pediatrician might be able to adopt into the guidance provided during pediatric visits; and to propose urgently needed reforms to the current regulatory process at the US Food and Drug Administration (FDA) for food additives.” These authors aren’t political hacks or corporate sharks. These are the applied scientists, the physicians, the men and women who spend 10 to 15 years of their lives just learning how to care for babies and kids…before they begin to invest the rest of their working lives doing more of that. Keywords: scientists, physicians, invest.

More from the Docs: “Substantial improvements to the food additives regulatory system are urgently needed, including greatly strengthening or replacing the “generally recognized as safe” (GRAS) determination process, updating the scientific foundation of the FDA’s safety assessment program, retesting all previously approved chemicals, and labeling direct additives with limited or no toxicity data.” What’s all this have to do with acupuncture, especially since Enerqi is not primarily a pediatric practice? Food as medicine, food as poison, including food that has been poisoned by food additives and toxic packaging.

Look at your plate, every single meal.
1.    About half of that plate (bowl, slab—whatever you eat out of or off of) should be covered with fresh vegetables, fruits, green leafy goodness, with small amounts of best quality fats—like organic olive oil, coconut oil, or butter. Yes, butter. Real food. Good fats are good.
2.    Of the other half, you should have a piece of protein about the size of your own palm. Fish, meat, tofu, beans—doesn’t really matter…with these exceptions:
a.    In a very large World Health Organization (WHO) study you can read here, processed meats (bacon, sausage, deli meats) were found to be correlated with increases in colorectal cancer at amounts greater than 4 ounces per week. A good analysis of the WHO study is here.
b.    Factory farm meats feed you antibiotics, growth hormones, and poo. Buy local and organic meats, preferably from farmers you can make eye contact with. Meat protein is not essential to health although it does strike the Happy Zone for many of us. There is no reason whatsoever to eat meat every day, certainly not at every meal.
3.    With rare exception, whatever the amount of carbohydrates you normally eat, start putting one spoonful back. Unfortunately, beer is a carb (talking to you, Wisconsin); mind your suds. 
4.    Anything you eat out of a paper bag, a can, or a cardboard box will be unnecessarily high in some combination of salt, sugar, carcinogenic additives, calories, and/or chemicals, including chemicals that the bought-and-paid-for FDA declares are “generally regarded as safe”—whatever the actual heck that toss-off bit of government-business codswollop means.
The pediatricians, bless them, are watching out for our kids, the next generation, the tiniest among us, whom we will all desperately need—every one of them—in 15 or 20 years. As it turns out, they’re watching out for all of us, too.
Let’s apply the same scientific rigor to our own grocery shopping, our own refrigerators, and our own plates, kids or no kids. Food is medicine. Good medicine. A necessary component to Your Radiant Good Health.

Headed Away to School? Here’s What Students With Health Issues Need to Know
College is a time of transition, but for those managing chronic medical conditions, it may also be the first time they will be wholly responsible for their own health: setting appointments, securing supplies and pharmaceuticals, and monitoring symptoms.
For those heading to schools far from home, it can be especially tricky navigating the complex world of insurance to pay for such crucial care. Coverage networks from back home might not cover a patient’s new doctors or a trip to an urgent care clinic. New plans may not cover the same medicines or procedures.
“Insurance doesn’t make sense to even people in health care,” said Jenna Riemenschneider, director of advocacy and special projects at the Asthma and Allergy Foundation of America.
Ensuring continuity of care and having medical support at school allow students to better pursue their studies and enjoy the college experience, health experts said. They recommend students and their families study up before heading to campus. Make calls to local or university health center providers and secure appropriate insurance to help make a smooth transition.
“You have to do some real due diligence to make sure you have the right protections,” said Erika Emerson, executive director of the Diabetes Leadership Council. “There are some things that are OK to whiff on, like if some things don’t happen and you learn a hard lesson, fine. Health care isn’t one of those things where there is a whole lot of forgiveness for mistakes that can be costly in terms of health outcomes and, certainly, financially.”
Income-Based Plans
Sam Grover was turning 26 around the time he headed from Utah to New York City for a medical school program. Per federal law, he would no longer be eligible to remain on his parents’ insurance after his birthday.
Grover, who has Type 1 diabetes, needed to be able to pay for his medical supplies, including a continuous glucose monitor, pump and insulin that keep his blood sugar levels in a safe range. After exploring his options, he enrolled in Medicaid, the federal-state health insurance program for low-income Americans.
“Growing up, I never saw myself as someone who would be in need, but then times changed, and I got diabetes, and I turned 26, and I didn’t have a job,” said Grover, who has one year left in medical school before beginning his residency. He said he hopes his work as a doctor will help make good on the help he received as a student from the government-sponsored health program.
Medicaid coverage varies by state. Grover found that Medicaid in Utah didn’t cover continuous glucose monitoring, for example, but he said New York’s program covers all his medical needs.
“It’s been the biggest blessing,” he said, adding that the cost of equipment and supplies to manage diabetes is expensive. “Just knowing that I am able to manage my diabetes while I am a student alleviates a lot of stress and anxiety that comes with having diabetes.”

Medicaid benefits are typically valid only in the state where the plan originates, and not all states have expanded coverage to more adults under the Affordable Care Act. If Medicaid isn’t an option, experts said, individual coverage through ACA insurance exchanges can be good options for students, especially if they can use federal tax credits and subsidies to make those plans more affordable.
As with most insurance options, students must clearly understand whether coverage extends beyond the location of their university, and if it covers specialist care and required medications. They also need to be aware of which ongoing out-of-pocket costs they’re responsible for.
A Parent’s Insurance
For many students younger than 26, remaining on a parent’s or guardian’s insurance might be the best option. Experts said it is imperative to read the fine print to be certain coverage is comprehensive and extends to where they attend school.
Both of Kathy Przywara’s children have asthma, and one has food allergies. Both left California to attend schools in rural Pennsylvania. After doing research, the family kept both on their existing insurance policy, but they still had to find in-network providers near the two schools.
While they were able to transfer one student’s prescriptions to a national chain pharmacy, that was not an option for the other. Przywara, who is also senior community director for the Asthma and Allergy Foundation of America, worked with a local pharmacy to ensure prescriptions were filled. The pharmacy already had a delivery program in place to the school’s health center, making it easier for students to obtain medications.
Przywara said mail-order pharmaceuticals, if covered by insurance, might also be an option for students. Some insurance covers prescriptions for multiple months of medication.
“Know what’s in your policy and make sure that the things you are going to need are covered,” Przywara said. “It’s important to keep your condition managed. That means access to your physicians and to your medication, or else the financial burden gets worse.”
Despite one’s best efforts, Pryzwara and others cautioned, emergencies can happen. Therefore, it is important for students to determine whether urgent and emergency care centers nearest them are in their network. Under the Affordable Care Act, initial emergency care should be covered but other services might not be.
University-Sponsored Insurance
Many schools require students to have health insurance and offer university-sponsored plans, said Jake Baggott, a past president of the American College Health Association and an associate vice chancellor of student affairs at the University of Wisconsin-Madison. He said that while some university health programs are equipped to deal with more complex medical issues or diagnostics, others are not. Students need to be clear on the details, such as whether their policy covers off-campus care.
Shay Webb, 22, a University of North Carolina-Wilmington graduate student earning a master’s in clinical research and product development, thought she was covering her bases when she purchased a university-sponsored policy in 2017 as an undergrad. She got the policy to help offset the out-of-pocket expenses for her Type 1 diabetes not covered by a parent’s insurance policy.
After moving onto campus, Webb was diagnosed with rheumatoid arthritis. She believed the student policy would help cover her increasing medical expenses.
Several months later, she learned her claims weren’t being processed. The insurer told her it would not pay the claims because she was not attending class in person even though she had remained a full-time student, lived on campus and had no say in whether classes were online or in person. The online program was part of the university’s effort to simulate the real-world experience of professionals in her field.
“I was just in shock,” Webb said. “No one had ever told me.”
Webb and her family were left with thousands of dollars in unexpected medical bills.
“Unexpected health costs are a tremendous burden,” Baggott said, adding that for students who may be taking on tuition debt, unexpected health costs can make things much more difficult to manage. Baggott and others said students should seek assistance from campus staff members, advocacy organizations or other experts should any issues occur.

The Bottom Line: Know the Details Before Signing
Before someone commits to an insurance plan, it is important to understand the specific type of plan, which entity is offering it and exactly how coverage works. If problems arise, students should be sure they can switch plans as needed.
The Partnership to Protect Coverage, a consortium of patient advocacy organizations, issued a report stating that insurance rules implemented during the Trump administration have allowed for the proliferation of plans that do not comply with the Affordable Care Act. These plans, such as short-term insurance options and health care sharing ministries, expose enrollees to financial risk, especially people with preexisting and chronic medical conditions.
“Exhaustively look through all of your options. Look through benefit plans. Call providers. Call the insurance company,” said Erin Hemlin, director of health policy and advocacy at Young Invincibles, a group based in Washington, D.C., that focuses on improving the economic security of young adults. “Double- and triple-check to ensure your child is going to be in a plan that is going to be there when they need it.”

“Growing up, I never saw myself as someone who would be in need, but then times changed, and I got diabetes, and I turned 26, and I didn’t have a job,” said Grover, who has one year left in medical school before beginning his residency. He said he hopes his work as a doctor will help make good on the help he received as a student from the government-sponsored health program.
Medicaid coverage varies by state. Grover found that Medicaid in Utah didn’t cover continuous glucose monitoring, for example, but he said New York’s program covers all his medical needs.
“It’s been the biggest blessing,” he said, adding that the cost of equipment and supplies to manage diabetes is expensive. “Just knowing that I am able to manage my diabetes while I am a student alleviates a lot of stress and anxiety that comes with having diabetes.”

Medicaid benefits are typically valid only in the state where the plan originates, and not all states have expanded coverage to more adults under the Affordable Care Act. If Medicaid isn’t an option, experts said, individual coverage through ACA insurance exchanges can be good options for students, especially if they can use federal tax credits and subsidies to make those plans more affordable.
As with most insurance options, students must clearly understand whether coverage extends beyond the location of their university, and if it covers specialist care and required medications. They also need to be aware of which ongoing out-of-pocket costs they’re responsible for.
A Parent’s Insurance
For many students younger than 26, remaining on a parent’s or guardian’s insurance might be the best option. Experts said it is imperative to read the fine print to be certain coverage is comprehensive and extends to where they attend school.
Both of Kathy Przywara’s children have asthma, and one has food allergies. Both left California to attend schools in rural Pennsylvania. After doing research, the family kept both on their existing insurance policy, but they still had to find in-network providers near the two schools.
While they were able to transfer one student’s prescriptions to a national chain pharmacy, that was not an option for the other. Przywara, who is also senior community director for the Asthma and Allergy Foundation of America, worked with a local pharmacy to ensure prescriptions were filled. The pharmacy already had a delivery program in place to the school’s health center, making it easier for students to obtain medications.
Przywara said mail-order pharmaceuticals, if covered by insurance, might also be an option for students. Some insurance covers prescriptions for multiple months of medication.
“Know what’s in your policy and make sure that the things you are going to need are covered,” Przywara said. “It’s important to keep your condition managed. That means access to your physicians and to your medication, or else the financial burden gets worse.”
Despite one’s best efforts, Pryzwara and others cautioned, emergencies can happen. Therefore, it is important for students to determine whether urgent and emergency care centers nearest them are in their network. Under the Affordable Care Act, initial emergency care should be covered but other services might not be.
University-Sponsored Insurance
Many schools require students to have health insurance and offer university-sponsored plans, said Jake Baggott, a past president of the American College Health Association and an associate vice chancellor of student affairs at the University of Wisconsin-Madison. He said that while some university health programs are equipped to deal with more complex medical issues or diagnostics, others are not. Students need to be clear on the details, such as whether their policy covers off-campus care.
Shay Webb, 22, a University of North Carolina-Wilmington graduate student earning a master’s in clinical research and product development, thought she was covering her bases when she purchased a university-sponsored policy in 2017 as an undergrad. She got the policy to help offset the out-of-pocket expenses for her Type 1 diabetes not covered by a parent’s insurance policy.
After moving onto campus, Webb was diagnosed with rheumatoid arthritis. She believed the student policy would help cover her increasing medical expenses.
Several months later, she learned her claims weren’t being processed. The insurer told her it would not pay the claims because she was not attending class in person even though she had remained a full-time student, lived on campus and had no say in whether classes were online or in person. The online program was part of the university’s effort to simulate the real-world experience of professionals in her field.
“I was just in shock,” Webb said. “No one had ever told me.”
Webb and her family were left with thousands of dollars in unexpected medical bills.
“Unexpected health costs are a tremendous burden,” Baggott said, adding that for students who may be taking on tuition debt, unexpected health costs can make things much more difficult to manage. Baggott and others said students should seek assistance from campus staff members, advocacy organizations or other experts should any issues occur.

The Bottom Line: Know the Details Before Signing
Before someone commits to an insurance plan, it is important to understand the specific type of plan, which entity is offering it and exactly how coverage works. If problems arise, students should be sure they can switch plans as needed.
The Partnership to Protect Coverage, a consortium of patient advocacy organizations, issued a report stating that insurance rules implemented during the Trump administration have allowed for the proliferation of plans that do not comply with the Affordable Care Act. These plans, such as short-term insurance options and health care sharing ministries, expose enrollees to financial risk, especially people with preexisting and chronic medical conditions.
“Exhaustively look through all of your options. Look through benefit plans. Call providers. Call the insurance company,” said Erin Hemlin, director of health policy and advocacy at Young Invincibles, a group based in Washington, D.C., that focuses on improving the economic security of young adults. “Double- and triple-check to ensure your child is going to be in a plan that is going to be there when they need it.”
