A coworker wears rainbow carebear shades. I compliment her on them. A friend decks out his wheelchair in all sorts of punk stickers and patches and other decorations. I compliment him on them. Another friend has a purple sparkly cane, I compliment them on it.

But a former coworker had to wear an eyepatch for a relatively long amount of time following eye surgery, and it was just some gauze taped to his face. Another former coworker had a little scooter cart they rested their leg on as they puttputted around my workplace after a workplace accident left them temporarily unable to walk. A friend wore a huge orthopedic boot for weeks while her broken ankle heeled. I didn't talk to them about these things.

It's generally fairly obvious when someone wants you to compliment their assistance device. It's also the same for service dogs- often dyed dogs, dogs in very colorful or creative gear, dogs in clothes, etc are dogs that their handlers would welcome some sort of positive attention. Not always, but many times. Similarly, there's also dogs in fairly no-nonsense gear that aren't dyed or in creative costume and many of their handlers want to be left alone about the dog.
